Minicucci, Special Permit, Watershed District Dear Mr. Long: After public hearing duly advertised and held on April 7, 1980 and continued, the Planning Board unanimously voted to approve the Special Permit for Louis Priinicucci, Great Pond Rd. subject to the following conditions: Re application for special permit in the watershed district to construct a driveway, plan of land located in North Andover, Ma. for Louis Minicucci dated July 3, 1980 — 1) that as required in the Highway Surveyor's letter dated July 7, 1980: a. existing Catch Basin located on driveway shall be reconstructed as a standard catch basin and existing catch basin located on Great Pond Rd. shall be reconstructed as a standard catch basin as stated in his letter; b. existing 811 drain pipe shall be replaced with a 12" pipe with a minimum of 30" of cover over pipe; c. an open grate cover shall be installed across the driveway aligned with the driveway drain and shall be heavy duty pre painted steel with bars running parallel with driveway; reinforced rods shall be used in concrete trough; d. a permanent earth berm shall be constructed as shown on plan of land located in North Andover, Ma. prepared for Louis Minicucci and dated July 31 1980; e. a twenty foot wide repair and maintenance easement shall be conveyed to the Tom of North Andover as shown on said plan; f. new plans shall be submitted to the Planning Board which reflect the above changes recommended by the Highway Surveyor; g. 12" pipe to be extended 12 ft. into a stone—tapered and flared headwall to receive all drainage directed to Swale from the hill; and h. trough across the driveway shall be 1211 wide. 2) In accordance with the Board of Public Works letter dated May 19, 1980, the driveway shall be constructed to enter the left side of the proposed dwelling as shown on said plan. In accordance with the Board of Public Works letter dated April 17, 1980, all utility lines to be installed shall be installed along the existing driveway. 3) Hay bales shall be placed along the length of the existing and proposed driveway during the entire term of construction activities to prevent erosion. Minicucci Decision - 2 - 4. All trees located adjacent to out and fill areas shall be welled under the direction of the Tree Warden and any trees lost or damaged during construction which the Tree Warden feels should be replaced shall be replaced under his direction. 5. Contour lines 90, 92, and 94 on the northwesterly portion of property shall be re-designed to direct drainage toward proposed driveway. 6. Only trees necessary to be removed for construction of driveway shall be disturbed. The following members were present and voting: William Chepulis, Chairman; John J.
