Schofield Brothers of New England, Inc, has been designated as the "Environmental /
Erosion Control Monitor" for the North Andover High School Project. The purpose of the
Environmental /Erosion Control Monitor is to perform weekly inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and stormwater
management issues.
This is the fourth monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period for the month of December 2001 through
January 11, 2002. Attached are copies of the Inspection Reports (#13 and#14) relative to
the inspections performed on December 19, 2001 and January 9, 2002, plus information
relative to the completion of some minor work by the site contractor to address some minor
remaining site issues on January 11, 2002.
The latest routine Environmental /Erosion Control Inspections and Reports were
performed by Mr. Bert Corey, an Engineer for SBNE. During December through January
11, the site work under the site preparation contract was essentially complete. The Site
Contractor was no longer on-site except for work to address minor finish up items and the
site was in generally good condition. For that reason, there was no need to inspect the site
on a weekly basis and the inspections were reduced to two week intervals or following
significant rainfall or snow melt conditions, and as necessary to inspect new work by the
contractor to address erosion control issues. The site should remain stable through the
winter under frozen ground conditions and inspections should continue as described above
for that period. We recommend that weekly inspections resume during the late winter/

spring thaw (Late February/Early March) when erosion controls are more likely to need
routine attention.
Just prior to the December 19°i inspection, several days of significant rain gave the on-site
erosion controls the first good test and gave us the opportunity to witness the performance
of the various erosion control installations. In general, the erosion controls held up well
and erosion was contained on site, with no threat to wetland resource areas. Some minor
issues were found and the Contractor made corrections as necessary to address those issues.
Relative to the Wetland Replication Areas, the contractor applied straw mulch around the
newly planted shrubs to protect the plants through the winter as recommended. The
wetland replication areas will be periodically monitored beginning next spring to assure
survival of the plant materials and success of the wetland replication in conformance with
the Performance Standards under the Wetlands Protection Act Regulations.
As of the completion of work on the remaining erosion control issues by the Site
Contractor on January 11`1i, 2001, the erosion controls are complete, functioning properly
and the site is essentially "buttoned up"for the winter. It is our understanding that the
Contractor under the Site Preparation Contract is responsible for maintenance of the
erosion controls for 90 days following "substantial completion of the site work" unless the
subsequent contractor takes over the site work prior to that.
Based on our inspections, we have found that the work on the project has been performed
in compliance with the Special Permit and Order of Conditions. Please let us know if there
are any issues that we are not aware of.
The attached inspection reports provide detailed information as to our findings. If you have
any questions or if you have any comments or recommendations that would help us provide
information you need, we would be happy to hear from you.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:

INSPECTION SUMMARY
During this site visit, it was noted that the Site Preparation Work is essentially complete, there
was no site work being performed, and the contractor's equipment is no longer on site. Prior to
this site inspection, there were several heavy rainfalls over the previous few days. It was,
therefore, a good opportunity to observe the performance of the erosion and sediment controls, as
well as the general stability of the site.
1. Sediment control barriers around the perimeter of the current work area:
• Sediment control barriers remain in place and in good condition.
• In the southwest corner of the site,just south of the temporary retaining wall,
staked haybales should be placed to backup sediment fence at the location where
runoff concentrates near temporary DMH Tl.
2. Loam stockpile area in the northern portion of the site:
• The stockpile has been hydroseeded and only minor erosion and slumping of the
stockpile was observed.
• Sediment fence has been installed.
• Sediment fence, located in southwest corner needs minor repair.
3. Ordinary borrow stockpile area in the northern portion of the site:
• Some erosion of the stockpile was observed.
• The stockpile has been hydroseeded.
• Sediment fence has been installed.
• Erosion of the stockpile is generally being contained by the sediment fence
except at one location on the easterly side where the fence has been breached.
This location needs repair and added support from haybales is recommended, as
there seems to be a possibility for a heavy amount of silt here. Note that there is
substantial distance between the stockpiles and the perimeter erosion controls
discussed in Item 1 above, so there is no threat to the wetland resource areas.
4. Main entrance gate near trailers:
• New stone has been added to the construction entrance and this item now in new
condition.
5. Southern emergency entrance:
• A large puddle has formed in the existing paved parking area in front of the
entrance gate. The Contractor did create a shallow swale to direct water from
this area to the southeastern catch basin as recommended in Inspection Report
#12, but some additional minor grading needs to be done to allow runoff to enter
the swale from the haul road.

6. Catch basins:
• Silt boots and hay bales at the catch basins are in good condition except as noted
below.
• Temporary Catch Basin T1 and T2, located between the existing school and the
retaining wall need debris cleaned from the inlet silt boots.
7. Sediment basins and outlet structures:
• Sediment basins are completed and are functioning properly.
• With the recent rains, it is now evident that the grading along the west side of
the haul road, to the north of Sediment Basin#3, does not form a swale that
directs runoff to that Basin. As a result, runoff from the haul road and the
adjacent area is bypassing the basin and the trap rock chute. Sediment is being
carried down to the area at the easterly end of the temporary retaining wall.
This is a significant problem that needs to be corrected ASAP.
• Hydroseeding and matting has been completed.
• In the southwest corner of the site near Sediment Basin #2, the grading is now
complete. As expected, only a small area does not drain to the basin. Additional
erosion controls (silt fencing and hay bales) have been installed to control any erosion
at that small area, and appears satisfactory except as noted below.
• Just to the west of Sediment Basin#2, soil has washed away from area
immediately around the new 21" drain pipe, exposing the top and side surfaces.
The grade needs to be brought up in this area and the soil stabilized to prevent
continued erosion around this pipe that could damage the installation.
8. Other erosion control items.
• A gully has formed in the slope to the Stone Interceptor Sub-drain just to the
south of the Ordinary Borrow Stockpile. Silt is beginning to wash onto the stone
interceptor sub-drain and it appears that other gullies are beginning to form at
other locations on this slope. This needs to be addressed ASAP to minimize
damage to the Stone Interceptor Drain.
• Sediment fence,just north of CB#1 needs minor repair.
9 Other drainage structures:
• All major drainage structures have been installed and appear to be functioning.
• The stone outfall at the sub-drain outlet has been properly repaired and the sub-drain
appears to be functioning.
10. Wetland replication areas:
• Both wetland replication areas have been augmented with organic-rich loam,
appropriate plantings, peat moss, and seed.
• Plants could be mulched with hay to protect against freezing temperatures.

11. Impermeable barrier and easterly embankment:
• The embankment has been graded and covered with loam.
• The embankment slope has been hydroseeded and jute matting has been installed on
the steepest portions of the slopes. The work seems to be functioning well at this
time and no erosion of the embankment was observed.
12. General:
® The site is clean and orderly. As mentioned at the beginning of this report,
considerable rainfall occurred in the days prior to the inspection and the site generally
weathered the storm well. The rains did reveal a few problems that need to be
addressed and are discussed above. These are general internal site erosion conditions
and do not impact off-site areas or wetland resource areas. Timely correction of these
items will assure that no sediment leaves the work site.
® The emergency access way between the existing school and the retaining wall is
in a very muddy condition. The area is relatively flat and no erosion is
occurring,but consideration should be given to applying crushed stone or other
corrective method to allow easier access for emergency vehicles.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to

2002, I met Bob Johnson (R. Zoppo Corporation) at the site to discuss the three
outstanding items from Inspection Report No. 13. The contractor, R. Zoppo Corporation,
addressed those three items on Janumy 11, 2002 and postscripts have been added to the
appropriate sections in this report as to how those items were addressed.

INSPECTION SUMMARY
During this site visit, there was approximately one (1) inch of snow on the ground and standing
water in the sediment basins was frozen. The site is essentially "buttoned up" for the winter
months. The site is unchanged from the last inspection, except for the repairs completed by the
site contractor in response to the Inspection No. 13 report. In addition, I met with Bob Johnson
(R. Zoppo Corp.) to discuss and review three items from Inspection Report No. 13. The results of
those discussions are listed below in the relevant sections. Overall, the site remains stable; there
are no new issues that need to be addressed.
1. Sediment control barriers around the perimeter of the current work area:
• Sediment control barriers remain in place and in good condition.
2. Loam stockpile area in the northern onion of the site:
• The stockpile has been hydroseeded and only minor erosion and slumping of the
stockpile was observed.
• Sediment fence remains in place and in good condition.
3. Ordin ry borrow stockpile area in the northern portion of the site:
• Some erosion of the stockpile was observed.
• The stockpile has been hydroseeded.
• Sediment fence remains in place and in good condition.
• Sediment fence on the easterly side has been repaired including added support from
haybales at the location recommended. Note that there is substantial distance between
the stockpiles and the perimeter erosion controls discussed in Item 1 above, so there
is no threat to the wetland resource areas.
4. Main entrance gate near trailers:
• New stone had been added to the construction entrance and this item now in new
condition.

• A large puddle has formed in the existing paved parking area in front of the
entrance gate. The Contractor did create a shallow swale to direct water from

#12, but some additional minor grading needs to be done to allow runoff to enter
the swale from the haul road.
o Inspected with Bob Johnson; a portion along the side and at the base of the
haul road needs to be cut away to give runoff and puddling a chance to run
towards the swale.
1/11102 Postscript: A sliallow trench was cut and filled with 314" crushed stone along

slightly regraded. It was easy to see the large puddle at the entrance gate emptying
into the swale and should reduce the paddle significantly. This issue has now been
addressed to the extent feasible and is satisfactory.
6. Cate_ h basins:
• Silt boots and hay bales at the catch basins are in good condition.
7. Sediment basins and outlet structures:
• Sediment basins are completed and are functioning properly.
• With the recent rains, it is now evident that the grading along the west side of
the haul road, to the north of Sediment B #3, does not form a swale
the haul road and the
directs runoff to that Basin. As a result, no
adjacent area is bypassing the basin and the trap rock chute. Sediment is being
carried down to the area at the easterly end of the temporary retaining wall.
This is a significant problem that needs to be corrected ASAP.
o Inspected with Bob Johnson; the existing grading along the west side of the
haul road will be a cut and regraded ttIn the area of the chute,the grading ad
towards and into the trap rock
will be such that the runoff has the ability to enter the chute. Trap rock and
3/q" crushed stone will be placed in this area. The cut will start approximately
50 feet north of the chute, at the edge of the haul road and will extend to the
west as it approaches the chute.
1111102 Postscript: A shallow swale was cut alongside the haul road as discussed.
Trap rock was placed in front of the existing trap rock chute and 3/a" crushed stone
was placed along the entire length of th.e swale. A very shallow trench was cut across
the haul road to direct additional runoff towards the trap rock chute. The remaining
crushed stone was used to repair and repack the haul road in this area. It was rairriaug
during this work so it was easy to see that the majority of runoff traveling down the
haul road was directed into the sivale towards the trap rock chute, and ultimately into
sediment basin#3. This issue has now been addressed satisfactorily. tiVe will need to
keep an eye on this area to make sure it conntinues to drain properly.
• Hydroseeding and matting has been completed.
• In the southwest corner of the site near Sediment Basin #2, the grading is now
complete. As expected, only a small area does not drain to the basin. Additional
erosion controls, silt fencing and hay bales, has been installed to control any erosion
at that small area, and appears satisfactory.
® Just to the west of Sediment Basin#2, soil has washed away from area
immediately around the new 21" drain pipe, exposing the top and side surfaces.

o Inspected with Bob Johnson; grading at the exposed drain pipe will be such
that runoff from the haul road will have a tendency to move towards the

basin. Approximately 75 feet north of this area, there may be a tendency for
water to run off the haul road, stream along its edge, and cross over the
drain pipe as it heads toward Sediment Basin #2. New grading will slightly
elevate the side of the haul road along this length. In addition, two rows of 2
or 3 haybales will be positioned along the path where runoff seems to be
occurring, to slow down any runoff prior to crossing the drain pipe.
1/11102 Postscript: The soil excavated to construct the swale along the haul road was
used to cover the exposed pipe and to create a berm along the 75' length of road.
Because of the working conditions, I was told it would be difficult to smooth the berm.
Ha>>bales were placed as discussed. As expected, noticeable runoff flow was
following the berm towards sediment basin#2. This issn.te has noiv been addressed
satisfactorily. We will need to keep an eye on this area to make sure it continues to
drain.properly.
8. Other erosion control items.
® A few gullies had formed in the slope to the Stone Interceptor Sub-drain just to the
south of the Ordinary Borrow Stockpile. In the area where the largest gully had
formed, haybales have been placed to provide backup to the silt fence at the top of the
slope and a 30-foot silt fence with haybales has been placed at the bottom of the
slope, immediately north of the stone interceptor sub-drain. The area appears in
satisfactory condition at this time.
9 Other drainage structures:
• All major drainage structures have been installed and appear- to be functioning.
• The stone outfall at the sub-drain outlet has been properly repaired and the sub-drain
is functioning.
10. Wetland replication areas:
® Plants have been mulched with hay to protect against freezing temperatures.

Schofield Brothers of New England, Inc. has been designated as the"Environmental/
Erosion Control Monitor" for the North Andover High School Project. The purpose of the
Environmental / Erosion Control Monitor is to perform periodic inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and storrnwater
management issues.
This is the fifth monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period January 12, 2002 through February 8, 2002.
Attached are copies of the Inspection Reports (#15 and #16) relative to the inspections
performed on January 25, 2002 and February 8, 2002.
During this time frame, the site has remained stable and in good condition. The combination
of winter- conditions and absence of any site work has lead to an uneventful period. At both
inspections, there was observable runoff. The January 25 inspection followed a day of rain
and the February 8 inspection occurred during warmer weather when snowmelt was
occurring. During both inspections, erosion controls were in good condition and were
functioning properly, including the repairs made by the site contractor on January 11.
Observations made during the two (2) visits were as follows:
® Runoff from the haul road at the emergency entrance that usually formed a large and
deep puddle at the gate as it accurnulated at the base of the slope was visibly flowing
into the Swale and catch basin, considerably reducing its contribution to that area.

• Runoff that typically followed the haul road and bypassed sedimentation basin#3 was
flowing into the trap rock chute leading to sediment basin #3. Repairs made to this area
are working properly to direct the majority of runoff, especially the flow occurring on
the west side of the haul road, towards the trap rock chute.
• On the west side of the site, runoff along the haul road has been following the berm and
into sediment basin#2. No erosion around the drain pipe has occurred.
• At the southern end of the borrow area, runoff continues to flow down the slope in
gullies, the largest flow heading towards a silt fence installed to protect the stone
interceptor drain. Some of the silt is beginning to wash around the sides and onto the
stone, Although not a problem at this time, attention may be needed in the future to
prevent large amounts of silt from washing onto the stone.
• No breaches of the exterior sedimentation fence have occurred.
• The only minor attention needed is to repair a torn silt fence at the north end of the
borrow area.
Overall, the site remains in a very good and stable condition. In addition to repairs
previously made, the site seems to be benefiting from the relatively low amount of
precipitation experienced to date.
Based on our inspections, we have found that the work on the project has been performed in
compliance with the Special Permit and Order of Conditions. Please let us know if there are
any issues that we are not aware of.

INSPECTION SUMMARY
During this site inspection, most of the site was covered with snow and ice but there was
observable runoff due to the relatively warm weather. Similar to the last inspection, the majority
of runoff was flowing towards the appropriate structures. Other than a minor repair that is needed
to fix a torn silt fence (located at the loam stockpile), the site essentially remains unchanged and
in stable condition.
1. Sediment control barriers around the perimeter of the current work area:
• Sediment control barriers remain in place and in good condition.
2. Loam stockpile area in the northern portion of the site:
• The stockpile has been hydroseeded and only minor erosion and slumping of the
stockpile was observed.
• Sediment fence remains in place and in good condition.
• The silt fence, at the north side, has been torn from a stake.
3. Ordinary borrow stockpile area in the northern portion of the site:
• Some erosion of the stockpile was observed.
• The stockpile has been hydroseeded.

• 1/11/02 PostScript: A shallow trench was cut and filled with 3/4" crushed stone along
the base of the haul road. Along the east side of the haul road, near the swale, was
slightly regraded. It was easy to see the large puddle at the entrance gate emptying
into the swale and should reduce the puddle significantly. This issue has now been
addressed to the extent feasible and is satisfactory.
• The repairs to this area seem to be working correctly. There was noticeable runoff
flowing to the catch basin. The puddle that usually formed at the entrance gate was
almost non-existent.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:

NEEMPECTION SUMMARY
This site visit was conducted after a day of rainy weather. During this site visit, there was very
little snow on the ground, some areas were iced over, and runoff was visible. The site remains
stable, no new issues need to be corrected but some conditions are listed that require careful
monitoring so that they do not get out of hand.
1. Sediment control barriers around the perimeter of the current work area:
• Sediment control barriers remain in place and in good condition.
2. Loam stockpile area in the northern portion of the site:
• The stockpile has been hydroseeded and only minor erosion and slumping of the
stockpile was observed.
• Sediment fence remains in place and in good condition.
3. Ordinary borrow stockpile area in the northern portion of the site:
• Some erosion of the stockpile was observed.
• The stockpile has been hydroseeded.
• Sediment fence and additional haybale support mentioned in the previous report
remains in place and in good condition.
• The sediment fence is working well; there is a noticeable amount of silt being held
back.

considering the proximity of the construction site and the existing
school. The high school will be occupied during the construction
period. The safe, continuous operation of the school is of prime

areas, with prior coordination with owner required. See phasing

03/26/02 A03. Air Quality- Air quality will need to be carefully monitored to keep

H. PAYROLL REPORTS

certified weekly payroll records to the Clerk of Works. The Clerk
shall verify the records. Two (2) copies of the payroll records
shall be maintained. One (1) copy shall be kept by the Clerk as
submitted, accessible only to the Building Committee, architect,
and attorney general's office. The second copy will have
sensitive information blackened out. This edited copy shall be
accessible to the public upon the receipt of a written request to the
Building Committee. Record item.
03/26/02 H02. The architect stated that any contractor that falls more than two (2)
weeks behind on the weekly payroll records will not be paid any
monies on their requisition until rectified. Record item.

03/26/02 101. Application for Payment Procedures- A pencil draft of the
requisition from the contractor shall be submitted on the first
Tuesday of the month. The final copy shall be submitted on the

conditions, other Owner provided information, Contractor prepared
Coordination Drawings, and prior project correspondence and
documentation to determine that the information to be requested is

Planning Board Notice of Decision dated 5/14/01
Schofield Brothers of New England, Inc. has been designated as the"Environmental/
Erosion Control Monitor" for the North Andover High School Project. The purpose of the
Environmental /Erosion Control Monitor is to perform periodic inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and stormwater
management issues.
This is the sixth monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period February 9, 2002 through March 15, 2002.
Attached are copies of the Inspection Reports (#17, 4118, and #19) relative to the
inspections performed on February 22, 2002, March 4, 2002, and March 15, 2002.
During this time frame, the site experienced the largest amount of runoff during the winter
season. Snowmelt and "frost-out" conditions contributed to the formation of larger gullies
and erosion. The site contractor was back on site, March 7, to make the necessary repairs
outlined in the inspection reports. It should be noted that all erosion issues were contained
within the site and there were no impacts to any of the resource areas. Overall, the site
remains in very good condition. A meeting with the General Contractor, JT Callahan, and
Architect, DiNisco Design Partnership took place on March 15, 2002 at the site to review
the condition of the site and the expectations going forward.
Observations made during the three (3) visits were as follows:
The haul road at the southern emergency entrance experienced a large amount of
erosion. Several large gullies were formed and deposits accumulated at the entrance gate
upsetting the tendency for runoff to flow towards the southeast catch basin. Silt was

deposited on the pavement immediately outside the gate. This area was fully restored by
the site contractor and remains in good condition.
• Sedimentation Basins 41, 92, and #3 continued to function properly, collecting the
majority of runoff from their respective areas.
• A gully that began to get too large,just south of Sediment Basin 43 and along the haul
road, was repacked with medium dense crushed stone. The gully has begun to reform
but the patch along the haul road remains in very good condition. The area that this
gully feeds has been acting as another sedimentation basin and the sedimentation fence/
hay bales are collecting silt that would flow towards the southern entrance and school
parking area.
Erosion continued to occur down the slope above the stone interceptor drain. Silt
deposits the stone interceptor drain were removed, the silt fence was extended, and no
new deposits have occurred.
® A large gully has formed at the top of the slope above catch basin#3. The General
Contractor plans to fill it with riprap stone. Silt/fines that deposited on the on the
crushed stone covering the stone interceptor drain and around the catch basin have been
removed and the catch basin has been protected with filter fabric. No new deposits have
occurred.
• No breaches of the exterior sedimentation fence have occurred.
• Several repairs have been made to the silt fence surrounding the stockpile. One section
of fence, along the east side where the heaviest amount of silt had been depositing, was
pushed down and covered. This area was repaired and heavy silt deposits held back by
the fence were removed.
® A"blowout" along the east bank occurred and has been filled with riprap. The source of
seepage that created this area where the soil has slumped away from the bank is
unknown at this time and further exploration is ongoing.
The response from the site contractor to make repairs in a timely fashion, and as stated in
previous reports, the relatively low amount of precipitation experienced during the winter
season has helped to keep the site in very good and stable condition. JT Callahan is taking
over the site as the General Contractor and is about to begin the next phase of construction.
Weekly site inspections by SBNE will begin as soon as construction resumes.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:
• DEP File No. 242-1045: Issued 2/21/01
• Planning Board Notice of Decision: Issued 5/14/01
A meeting was held at the site with the general contractor to review the conditions of the
site and to determine if any site issues needed to be resolved prior to the general contractor
taking control of the site. Attendance included: Rick Rice of DiNisco Design Partnership;
Pat Saitta of Municipal Building Consultants; Tim Callahan, Mike Callahan, and Phil
Briselli of JT Callahan; Fred King and myself of Schofield Brothers of New England, Inc.
Two items that need further discussion include: l.) the previous hydroseed application to
stabilize the stockpiles, and 2.) the area at the east bank where the "blowout" occurred. See
Item 11 below. The "blowout" is a localized area where heavy seepage has caused the soil to
slump away from the bank.

INSPECTION SUMMARY
The site inspection was performed because it coincided with the meeting mentioned above. The
site was relatively dry and no runoff was observed except for a small amount of seepage
occurring along a short section at the northern portion of the east bank. This small amount of
seepage may be occurring from the wetland in the immediate area. The site remains in stable
condition. Previous reports have been pared down and new comments have been placed in
italics.
1. Sediment control barriers around the perimeter of the current work area:
® Sediment control barriers remain in place and in good condition.
2. Loam stockpile area in the northern portion of the site:
• The hydroseed application may have been washed down the side of the pile. More
erosion of the stockpile has occurred and observable deposits of silt are being held
back by the sedimentation fence.
• At the meeting, it was concluded that there is a high likelihood grass will grow on the
loam pile to stabilize it even though a major portion of the hydroseed application had
been washed down the sides.
3. Ordinary borrow stockpile area in the northern portion of the site:
• Erosion continues to occur, striations can be seen on the slopes of the pile. A
significant amount of the hydroseed application appears to have been washed down
the side of the pile.
• Sediment fence and additional haybale support on the easterly side, mentioned in the
previous report, remains in place and in good condition; it is working well; there is a
noticeable amount of silt being held back.

stockpiles.
• At the meeting, it could not be determined whether grass will not become established
on the ordinary borrow stockpile. Further monitoring will be necessary.

• Silt boots and hay bales at the catch basins are in good condition

• Sediment basins are functioning properly.
• The area just south of Sediment Basin#3, an area designated to receive overflow
from Sediment Basin#3, is presently functioning as a secondary sedimentation basin.
8. Other erosion control items.
• A few gullies remain in the slope above the Stone Interceptor Sub-drain,just south of
the Ordinary Borrow Stockpile. The largest gully has haybales placed at its
beginning and a silt fence is installed at the toe of the slope to protect silt from
washing onto the pea-stone covering the sub-drain.
• A deep gully (2' wide x 28" deep)travels down the slope towards CB#3. Repairs
previously noted were the removal of the silt covering the stone interceptor sub-drain,
removal of the silt at the beehive grate, and covering the grate with filter fabric.
o The general contractor will take control of this area and intends to fill the gully
ivith riprap stone to stabilize that it.
9. Other drainage structures:
• All major drainage structures appear to be functioning.
• The stone outfall at the sub-drain outlet to the east of the haul road is flowing and
appears to be functioning properly.
10. Wetland replication areas:
• No change from previous report.
11. Impermeable barrier and easterly embankment:
• 3/4/02: This (blowout) area is approximately 5' wide and 1-1/2' deep at its deepest
point. A 7" diameter hole is located at the back wall, bottom center of the gully. At
this point, I am wondering if subsurface flow is concentrating in this area and caused
the blowout. Repaired — leftover riprap/soil has been used to fill this area
• This blowout is a concern; although no observable runoff was occurring at the time
of the inspection, small deltas of silt were extending onto the haul road from the
repaired area. It is likely that this condition may continue long term and that the
source of seepage creating the hole and blowwout should be found and corrected. In
addition,just north of this area, a small amount of seepage is flowing through the
bank and towards the haul road This may be coming from the wetland in the
immediate area. The General Contractor, JT Callahan, will investigate these areas.
12. General:
• The site is continues to be clean and orderly.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:
• DEP File No. 242-1045: Issued 2/21/01
• Planning Board Notice of Decision: Issued 5/14/01
Please note that R. Zoppo Corporation was on site March 7, 2002 to address items needing
attention outlined on previous inspection reports. Observations during the March 4
inspection along with comments on the repairs are included in this report (italics). At this
time, all items have been addressed and there are no new issues.

INSPECTION SUMMARY
The site inspection followed a period of rain on Sunday, 3/3/02; no snow or ice was present,
there were small ponds of water in the sediment basins, and no runoff was observed. No new
issues have been found; the gullies and silt deposits mentioned in the previous inspection report
continue to get wider and deeper. Following the repairs made on 3/7/02, the site is in stable
condition.
1. Sediment control barriers around the perimeter of the current work are
Sediment control barriers remain in place and in good condition.
2. Loam stockpile area in the northern portion of the site:
® The stockpile has been hydroseeded and only minor erosion and slumping of the
stockpile was observed. The hydroseed application may have been washed down the
side of the pile. More erosion of the stockpile has occurred and observable deposits of
silt are being held back by the sedimentation fence.
® 317102 Repairs to items mentioned in previous reports include:
o The sediment fence, at the north side, has been torn from a stake. Repaired
o The sediment fence, at the southwest corner is sagging and needs maintenance.

area can be found where a small amount of silt has run down the side of the haul

o In addition, a broken fence stake ivas repaired and all locations along the silt

grate. The opening where the two ends meet should be closed, reinforced,
and a silt boot should be installed at the inlet grate.

• The stone construction entrance is in very good condition.
5. Southern emergency entrance:
e During this inspection, there was a shallow puddle at the entrance gate. By observing
the high water mark around the catch basin and the noticeable amount of silt covering
the parking lot immediately outside the gate, a pond must have formed at the catch
basin and extended beyond the gate area. This area is functioning as a sedimentation
trap during periods of higher runoff. Maintenance required at this time should be to
remove silt from the paved area and to install a new silt boot at the catch basin. Both
items repaired
® A large, double gully (-1-1/2' deep by—40' long) has formed along the east side of
the haul road and a smaller gully (-1' deep by —15' long) has formed along the west
side of the haul road. Most likely, these were the largest contributors of the silt found
on the pavement. We recommend repairs of this area be made including installation
of haybale checkdams positioned in a way to slow runoff flow, but not to impede
possible future construction traffic. Repaired, the gullies have been filled 1vith
existing washed out material along with 3/t" medium dense crushed stone fill. There is
a slight pitch in the haul road to direct runoff to the srvale. After repairs were made
the area was in excellent condition.
® 3141102: The.stone, gravel, and soils (from the gullies) have deposited at the toe of the
slope and have eliminated the pitch allowing runoff to flow towards the sivale. In
addition, the runoff from the gully is noiw forming a channel within the deposits and

• With respect to Sediment Basin 42, repairs seem to be working properly. The
adjacent drain pipe remains covered and no noticeable gullies have formed alongside
the berm except an insignificant one at the end of the haul road/berm where the flow
enters Sediment Basin 92. The only difference from the previous inspection is that the
gullies entering the basin are getting larger.
o 314102: The 3/d"stone surrounding the stz•uctum has settled at the west face of the
concrete structure, exposing 2' of fabric. Repaired
• The area just south of Sediment Basin#3, an area designated to receive overflow

stabilization fabric. Captured silt is beginning to pull the fence down. Silt
has washed onto stone interceptor drain from each side of the silt fence,
each washout covers an area— 12' long. Repaired- the silt fence has been
re,paired and the silt has been removed from the top sinface of the stone
interceptor drain.
• Silt is starting to get by the sedimentation fence, installed just north of the stone
interceptor drain where it begins on the west side of the site, and is beginning to cover
the crushed stone as it travels towards the beehive inlet grates. Silt boots should be
installed at these two (2) inlets. Repaired—minor amount of silt has been removed
from around the beehive grates, which could not be removed to install a silt boot.

• All major drainage structures have been installed and appear to be functioning.
• The stone outfall at the sub-drain outlet to the east of the haul road is flowing and
appears to be functioning properly.

• A small gully has formed alongside the northerly edge of the jute matting from the
top of the slope to the bottom of the slope and a small silt delta (3'x4') has formed on
the haul road. Not a problem at this time, but this may need some attention in the
future. We will need to keep an eye on this area.
• Although the small gully mentioned above remains the same, another area
approximately 28' north of this spot has washed away from the east bank. The area is
approximately 4' wide where the bank meets the haul road, travels 9' up the bank,
and is 1' deep. The silt from this area has washed onto the haul road. Repair and
maintenance of the slope is recommended as soon as field conditions permit.

INSPECTION SUMMARY
During this site inspection, no snow or ice was present, and no runoff was observed. From the
observable conditions, it seems that the site experienced a large amount of runoff due to
snowmelt and "frost-out" conditions. New gullies have been formed, existing ones have been
enlarged and silt deposits are increasing as expected. Erosion control features are certainly being
put to the test. It should be noted that the erosion occurring is being held on site and has not
breached any of the perimeter erosion control features. All erosion control issues at this time are
internal to the site and are not an immediate threat to wetland resource areas. To make sure that
erosion remains under control, we recommend that the items needing attention receive repairs or
maintenance as soon as possible in anticipation of spring rainfall and other typical seasonal
conditions.

• With respect to Sediment Basin#3, the swale constructed alongside the haul road and
into the trap rock chute seems to be working properly. It was easy to see that almost
all the runoff from this area was directed into the trap rock chute and into the basin.
Although the water held in the basin receded to a silt layer, the high water mark was
approximately 2-% 'from the top of the concrete structure.
• With respect to Sediment Basin#2, repairs seem to be working properly. The
adjacent drain pipe remains covered and no noticeable gullies have formed alongside
the berm except an insignificant one at the end of the haul road/berm where the flow
enters Sediment Basin #2. The only difference from the previous inspection is that the
gullies entering the basin are getting larger.
• The southwest corner in this area is holding up well. It cart be observed that a small
amount of runoff has gone beyond the haybale/sedimentation fence hart no deposits of
silt can be seen.

JTC 04/02/02 C01.2 Building Permit - Bob Nicetta reported that the Health
Department must sign off on the building permit, and
it may be helpful for JTC to meet with the Health
Department Administrator.
03/26/02 CO3.DPW Permits
JTC 03/26/02 CO3.1 Driveway Permit - The DPW requires a driveway
permit (no fee). JTC is to go to the DPW to get a
permit. Prescott Street at the construction entrance
shall be kept clean. This could be daily manual,
weekly power sweeping or as directed by DPW,
whatever is required to keep Prescott Street clean.
JTC 03/26/02 CO3.2 Sewer Permit-The DPW requires a sewer permit.
JTC 03/26/02 CO3.4 Water for Construction - JTC to obtain hydrant
permit for water use. JTC to advise DPW of the
approximate amount of expected water use. A
backflow preventer and meter will be attached to
hydrant. A water use ban is going into effect. JTC
shall apply for a waiver as soon as possible.
03/26/02 C04.North Andover Fire Department Permits
JTC 03/26/02 C04.1 The NAM requires a propane permit. Should
propane be required for construction purposes, JTC
is to advise the NAFD of the total aggregate amount
of propane on site. Propane tanks are to be secured
and enclosed by a chainlink fence and must be
located away from the existing school.
03/26/02 C06.Conservation Commission Issues.
03/26/02 C06.1 Deposit - JTC to submit$30,000 check as required
by ConCom orders. Record Item.
NABC 04/02/02 C06.2 Deposit - JTC has submitted a $30,000 deposit as
required by the Conservation Commission. DDP
inquired whether the Conservation Commission has
released the deposit submitted by Zoppo now that
JTC's deposit is in hand.
04/02/02 C09.Conservation Commission Agent- Bob Nicetta advised that he
will continue to serve as the Conservation Commission Agent for

JTC 04/02/02 C10.Washing Concrete Trucks - It was discussed that Order of
NABC Condition Item No. 67 requires approval of any on site washing of
concrete trucks. JTC will work with Bob Nicetta to construct a
suitable wash area; the area shown on the phasing drawings as a
potential trailer location is proposed by JTC.
DDP 04/02/02 C11.Water Flow at Bank- The flow observed after the recent rains
SBNE along the east haul road was discussed. Schofield Brothers of
N.E. and Richard Weber will be on site Friday 04/05/02 to observe
conditions and develop a strategy for investigation and repair.

portion of the driveway work out of phase, installing

be unaffected. No exception was taken to this

RWS 04/09/02 G08.3 Utility coordination - JTC noted areas where storm
SBNE drainage and water/sewer work are apparently in
conflict. RWS at SBNE will review and develope

deep below wall the piping must be before footing
haunches are not needed.
DDP 04/09/02 R01.2.1 JTC noted that at one location where pipe is well
under footing, sleeves were placed in stone backfill,
then footings poured over the stone. Once pipes

Schofield Brothers of New England, Inc. has been designated as the"Environmental /
Erosion Control Monitor" for the North Andover High School Project. The purpose of the
Environmental /Erosion Control Monitor is to perform periodic inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and stormwater
management issues.
This is the seventh monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period March 16, 2002 through April 9, 2002.
Attached are copies of the Inspection Reports (#20, #21, and 922) relative to the
inspections performed on March 26, 2002, April 1, 2002, and April 9, 2002.
During this time frame the site continued to remain in very good condition, the ongoing
erosion and sedimentation issues were contained within the site and there were no impacts
to any of the resource areas. Gullies that were previously packed and regraded continued to
form along the haul road at the southern emergency entrance and just south of
Sedimentation Basin #3. A berm was installed across the haul road to prevent runoff from
bypassing Sedimentation Basin#3 and contributing to the gully formations and flow to the
southern gate area. Two areas along the east bank required further investigation, the
"blowout" area and a small area just north of it, where seepage was seen to be flowing from
the bank. The"blowout" seems to have been caused by flow following a seam of sandy soil
in the compact glacial till. Solutions to resolve this issue are being developed. The seepage
flowing through the bank was found to be coming from the sub-drain system indicating a
probable blockage or break in the pipe, which needs to be investigated and corrected.
Weekly inspections have been reinstated.

Observations made during the three (3) visits were as follows:
• The exterior sedimentation fence, haybales and fence, remain in good condition and no
breaches occurred.
• Sedimentation Basins #1, #2, and #3 continued to function properly, collecting the
majority of runoff from their respective areas. The stone filters surrounding the inlet
structures are in very good condition.
• Although the previous gullies were repaired, new gullies continue to form and enlarge
along the sides of the haul road at the southern emergency entrance and along the haul
road just below Sediment Basin#3. A berm, similar to a speed bump, has been installed
to reduce the amount of surface runoff from reaching these areas and it has been
suggested that haybales be placed at the southern entrance fence and gate to prevent silt
from washing onto the pavement just outside the gate.
The large gully that formed in the slope above catch basin #3 remains but erosion seems
to be occurring at a slower rate. The Contractor plans to fill this gully with riprap stone.
There has been some deposit of silt/fines onto the stone interceptor drain but no deposits
have occurred around the catch basin inlet and it remains in very good condition,
protected by filter fabric.
• With the exception of one area at the ordinary borrow stockpile, where a slide occurred
and covered four sections of silt fence, the silt fence surrounding the stockpile remains in
good condition. The resulting deposit from the slide will have to be removed and the silt
fence repaired.
• An investigation of the "blowout" area took place with a geotechnical engineer and
others. It has been surmised that the flow causing this blowout is from seepage traveling
through a seam of sandy soil in the compact glacial till. The flow seems to only occur in
response to precipitation. SBNE is working on a resolution to correct this localized
flow.
• A small area of seepage at the east bank that has been observed during dry periods has
been found to be coming from the sub-drain system indicating a possible blockage or
breakage. A method to assess the condition of the sub-drain pipe has been
recommended.
• Dewatering operations have begun, coinciding with the building footing installation. It
has been noted that effluent should be directed overland towards the sedimentation

basins without creating an erosion problem or passed thought a straw bale and filter
fabric bin. Effluent should not be directed towards the haul road.
® An area for truck washing was established and washings were contained. -
Dust control procedures are noted in the Erosion & Sediment Control Narrative -
Attachment 3, dated October 4, 2000. Since a water ban is in place for the Town of North
Andover, calcium chloride may be used as a dust control method. Granules or flakes may be
applied at a rate that will keep the surface moist but not cause pollution.
Based on our inspections, we have found that the work on the project has been performed in
compliance with the Special Permit and Order of Conditions. Please let us know if there are
any issues that we are not aware of.
The attached inspection reports provide detailed information as to our findings. If you have

2. Loam stockpile area in the northern portion of the site:
• The hydroseed application may have been washed down the side of the pile. More
erosion of the stockpile has occurred and observable deposits of silt are being held
back by the sedimentation fence.
3. Ordinary borrow stockpile area in the northern portion of the site:
• Erosion continues to occur, striations can be seen on the slopes of the pile. A
significant amount of the hydroseed application appears to have been washed down
the side of the pile.
• The sediment fence remains in good condition around the perimeter of the stockpile
area.
4. Main entrance Rate near trailers:
• The stone construction entrance is in very good condition.
5. Southern emergency entrance:
• Puddling was occuring at the gate but was draining into the Swale and catch basin.

the road near the Swale, not significant at this time.

• Silt boots and hay bales at the catch basins are in good condition
7. Sediment basins and outlet structures:
• Sediment basins are functioning properly.
• The area just south of Sediment Basin#3, an area designated to receive overflow
from Sediment Basin#3, is presently functioning as a secondary sedimentation basin.
The previously repaired gully feeding this area is beginning to form again but is
insignificant at this time.
8. Other erosion control items.
• A few gullies remain in the slope above the Stone Interceptor Sub-drain,just south of
the Ordinary Borrow Stockpile. The largest gully has haybales placed at its
beginning and a silt fence has been exended at the toe of the slope to protect silt from
washing onto the pea-stone covering the sub-drain.

11. Impermeable barrier and easterly embankment:
• 3/4/02: This (blowout) area is approximately 5' wide and 1-1/2' deep at its deepest
point. A 7" diameter hole is located at the back wall, bottom center of the gully. At
this point, I am wondering if subsurface flow is concentrating in this area and caused
the blowout. Repaired — leftover riprap/soil has been used to fill this area
This blowout is a concern; although no observable runoff was occurring at the time of
the inspection, small deltas of silt were extending onto the haul road from the repaired
area. It is likely that this condition may continue long term and that the source of
seepage creating the hole and blowout should be found and corrected.
• Just north of this area, a small amount of seepage is flowing through the bank and
towards the haul road. This may be coming from the wetland in the immediate area.
• The blowout area is in the same condition as when it was repaired There was no
noticeable concentrated flow low coming from this area.

INSPECTION SUMMARY
SBNE responded to a call initiated by Bob Nicetta (North Andover Building Commissioner), who
witnessed a large amount of flow coming from the "blowout"area. There was a rain shower
earlier in the morning prior to this inspection. The site inspection followed a meeting with Bob

to review the "blowout"area. The southern entrance gate area was also reviewed with Bob and
Richard. Gullies continue to form and enlarge along the haul road at the southern entrance gate
and just south of sediment basin#3. Silt continues to deposit on the stone interceptor drain. It
should be noted that although erosion and sedimentation continues to take place, it remains
internal to the site with no impacts to resource areas.

material is depositing at the bottom of the slope, which ultimately interferes with the
tendency of the puddle to drain towards the southeast catch basin. If erosion
continues to enlarge the gullies, regrading will be necessary.
6. Catch basins:
• April 1: The silt boot at the catch basin in the emergency egress road that runs
between the school and retaining wall should be replaced. Silt boots at other catch
basins are in good condition.

• April 2: There still seems to be some weeping in this area and should be carefully
examined with geotechnical engineer Friday (4/05/02)
• The blowout area is in the same condition as when it was repaired. There was no
noticeable concentrated flow coming from this area.
• April 1:Building Commissioner noticed a considerable amount of flow emanating
from the riprap covering the "blowout" area. At 10:35am the flow was
approximately 113 of what was flowing a few hours earlier.
• April 2:No flow coming from the blowout area. Fred King and geotechnical engineer
will investigate this area Friday (4105102) to try to determine source and methods) to
repair it. They will also investigate the seepage flowing through the bank just north of
this area.
12. Dewatering:
• April 1: Effluent from dewatering operations should be directed overland
towards the sedimentation basins without creating an erosion problem or
directed into a straw bale and fabric bin. The effluent should not be directed
towards the haul road.
13. Truck Washing Area:
• April 1:An area, approximately 20'x20'has been established for truck washing
operations on the northern portion of the site, west of the ordinary stockpile.
Haybales have been placed on the west and south sides and stone has been placed for
driving. It will be important that the washings do not flow towards the stone
interceptor drain.
14. General:
• April 1: Construction is underway.

interferes with the tendency of the puddle to drain towards the southeast catch basin.
If erosion continues to enlarge the gullies, regrading will be necessary.
• 4/5/02: This specific item was discussed on site with representatives of DiNisco
Design Partnership, the Contractor and others (see Items 2 and 3 of the attached
memo of April 9, 2002). The problem appears to be partly a result of some runoff by-
passing Sediment Basin#3. A berm (similar to a speed bump) will be installed in the
access road to direct the flow to Sediment Basin#3. This will reduce the flow to the

will also install a row of haybales along the gate and fence to control any
remaining sediment.

• The area just south of Sediment Basin#3, an area designated to receive overflow
from Sediment Basin #3, is presently functioning as a secondary sedimentation basin.
The previously repaired gully feeding this area is beginning to form again but is
insignificant at this time.
• 4/1/02: The gully is getting larger and deeper and continues to make its way up the
haul road. If the gully begins to cut its way into the haul road, interfering with travel,
it will require repacking.
• 4/9/02: This specific item was discussed on site with representatives of DiNisco
Design Partnership, the Contractor and others during the site walk on 4/5/02 (see
Items 2 and 3 of the attached memo of April 9, 2002). The problem appears to be
partly a result of some runoff by-passing Sediment Basin#3. A berm (similar to a
speed bump) has been installed in the access road to direct the flow to Sediment
Basin#3 as discussed. This will reduce the flow to this area.
8. Other erosion control items.
• A deep gully (2' wide x 28" deep) travels down the slope towards CB#3. The gully
continues to get wider and deeper, the widest section occurring under the slope
stabilization fabric. There were no silt deposits on or around the stone interceptor
drain or catch basin#3 in the area below the gully. The General Contractor plans to
fill this gully with riprap stone.
• April 1: The gully continues to get larger and silt/fines have been washing out onto
the crushed stone covering the stone interceptor sub drain. The filter fabric at the
catch basin inlet is functioning properly.
• April 1: At the southwest corner, some loose hay should be placed between the
sedimentation fence and the perimeter chainlink fence.
• 4/9/02: No change from previous report.
9. Other drainage structures:
• 4/9/02: During the site walk on 4/5/02, it was determined that there was a
problem with the access road sub-drain (see attached memo of 4/9/02, referred
to as "Site 2"). Water is seeping onto the access road from the drain indicating a
blockage or some other problem. This needs to be investigated.
10. Wetland replication areas:
• No change from previous report.
11. Impermeable barrier and easterly embankment:
• April 1: Building Commissioner noticed a considerable amount of flow emanating
from the riprap covering the "blowout" area. At 10:35am the flow was approximately
1/3 of what was flowing a few hours earlier.
• April 2: No flow coming from the blowout area. Fred King and geotechnical engineer
will investigate this area Friday (4/05/02) to try to determine source and method(s) to

• 4/9/02: Blowout Area—This area was inspected on 4/5/02 by Richard Weber,

the Contractor. See the attached memo dated 4/9/02 for additional information on
this area. No seepage was occuring during this 4/9/02 inspection. SBNE to prepare
a sketch plan for addressing this item.
12. Dewatering:
• April 1: Effluent from dewatering operations should be directed overland towards the
sedimentation basins without creating an erosion problem or directed into a straw bale
and fabric bin. The effluent should not be directed towards the haul road.

13. Truck washing Area:
• April 1: An area, approximately 20'x20' has been established for truck washing
operations on the northern portion of the site, west of the ordinary stockpile. Haybales
have been placed on the west and south sides and stone has been placed for driving. It

DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP
F&B with regard to having a lift point on the roof for bringing up items
that are too heavy or too large to go up the roof hatch. DDP noted
that if one point was designated to lift a load of a few thousand
pounds (i.e. HVAC equipment), the entire roof would need to be
designed for that live load. DDP will investigate means of lifting
items like a snow blower to the roof.
