INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:
• DEP File No. 242-1045: Issued 2/21/01
• Planning Board Notice of Decision: Issued 5/14/01

any questions regarding the information contained in this report.
The inspection took place with Bob Nicetta(North Andover Building Commissioner) and Tim
Callahan QT Callahan). No runoff was observed except for a small amount of seepage at the east
bank (Item 9). Items needing immediate attention are in bold.

2. Loam stockpile area in the northern portion of the site:
0 4/9/02 - The hydroseed application may have been washed down the side of the pile.
More erosion of the stockpile has occurred and observable deposits of silt are being
held back by the sedimentation fence.
® 4/18/02—No change from previous report noted above; grass is growing on the
stockpile although not uniformly distributed yet.
0 4/23/02—No change from previous report.

3. Ordinary borrow stockpile area in the northern portion of the site:
® Erosion continues to occur, striations can be seen on the slopes of the pile. A
significant amount of the hydroseed application appears to have been washed down
the side of the pile.
• 4/1/02: A section along the east side of the stockpile has slid down and covered four
sections of the silt fence. Sediment removal and silt fence repair is needed.
® 4/9/02 —No change since previous report.
« 4/18/02 -No change since previous report; there appears to be some grass growth on
the stockpile.
• 4/23/02 - No change since previous report. Item dated 4/1/02 was discussed at
construction meeting— a silt fence will be installed around the slide and then tied
back into existing silt fence.
® 5/1./02 — The silt fence will be installed around the slide. There is some grass
growth on the stockpile.
4. Main entrance gate near trailers:
® 4/9/02: Area in very good condition.

good condition.
• 4/23/02 -No change since previous report.
• 5/1/02—A.small puddle has formed at the gate but the haybales placed there are
preventing silt from depositing on the pavement. The silt boot at the southeast catch
basin is in very good condition—the haybales and silt fence surrounding the catch
basin are functioning properly.
• April 1: Gullies have formed along the sides of the haul road just north of the gate
and the wash out material is depositing at the bottom of the slope, which ultimately
interferes with the tendency of the puddle to drain towards the southeast catch basin.
If erosion continues to enlarge the gullies, regrading will be necessary.
• 4/5/02: ... A berm (similar to a speed bump) will be installed in the access road to
direct the flow to Sediment Basin#3. This will reduce the flow to the area at the
Southern Access Gate and thereby minimize the problem. The contractor will also
install a row of haybales along the gate and fence to control any remaining sediment.
• 4/9/02: The berm has been installed. Haybales to be installed as discussed at the
4/5/02 site walk.
• 4/18/02: Haybales were in place.
• 4/23/02: No change since previous report.
• 5/1/02: A.small amount of erosion continues to occur along the haul road but the
sediment is contained on site.
6. Catch basins:
• 4/9/02: Silt boots at catch basins appear to be functioning properly.
• 4/18/02: No change from previous report.

7. Sediment basins and outlet structures:
• Sediment basins are functioning properly.
• The area just south of Sediment Basin#3, an area designated to receive overflow
from Sediment Basin #3, is presently functioning as a secondary sedimentation basin.
• 4/1/02: The gully is getting larger and deeper and continues to make its way up the
haul road. If the gully begins to cut its way into the haul road, interfering with travel,
it will require repacking.
• 4/9/02: ... A berm (similar to a speed bump) has been installed in the access road to

• 4/18/02: The berm seems to be working perfectly and should prevent silt/fines, that
are now packed on the surface of the haul road, from getting beyond Sedimentation
Basin#3 during future runoff conditions.
• 4/23/02: A stockpile was created just north of SB #3, haybales will be placed to
prevent unnecessary amounts of silt from entering the basin. This stockpile will be
used for grade adjustment in the area and will not remain for extended periods of
time. More stone was added to stone swale leading into trap rock chute of SB #3.
• 5/l/02: Haybales have been placed to the south of the stockpile to protect SB #3.
• 5/1/02: The amount of silt deposits in the sediment basins do not warrant cleaning at
this time.
8. Other erosion control items.
• A deep gully (2' wide x 28" deep) travels down the slope towards CB#3. The gully
continues to get wider and deeper, the widest section occurring under the slope
stabilization fabric. There were no silt deposits on or around the stone interceptor
drain or catch basin #3 in the area below the gully. The General Contractor plans to
fill this gully with riprap stone.
• April 1: The gully continues to get larger and silt/fines have been washing out onto
the crushed stone covering the stone interceptor sub drain. The filter fabric at the
catch basin inlet is functioning properly,
• 4/18/02: Silt continues to wash out onto stone and is beginning to deposit around
catch basin inlet. Silt deposits should be removed from the top of stone and the filter
fabric replaced. Stone should be placed in gully.
• 4/23/02: Item above was discussed at construction meeting.
• 5/1/02: Silt deposits will be removed from the top of stone and the filter fabric
replaced at catch basin #3. Stone will be placed in the gully.
• April 1: At the southwest corner, some loose hay should be placed between the
sedimentation fence and the perimeter chainlink fence.
• 4/9/02: No change from previous report.
• 4/18/02: No change from previous report.
• 4/18/02: Grass is growing fairly evenly on the east bank. There are some ruts at the
bottom of the bank from vehicular traffic—the Contractor has installed a sign "All
Vehicles Stay on Road"
• 4/23/02: No change since previous report.
• 5/1/02: The berm installed at the southwest corner will be repaired,
• 5/1/02: Some type of a barrier will be placed along the east bank to prevent
vehicles from creating ruts in the embankment.

• 4/9/02: During the site walk on 4/5/02, it was determined that there was a problem
with the access road sub-drain (see attached memo of 4/9/02, referred to as"Site 2").
Water is seeping onto the access road from the drain indicating a blockage or some
other problem. This needs to be investigated.
• 4/18/02: This area continues to weep,
• 4/23/02: Item above discussed at construction meeting; a work order will be issued by
the DDP to allow inspection of the subdrain.
• 5/1/02: JT Callahan will notify this office when the inspection is to take place.
10. Wetland replication areas:
• No change from previous report.
• 4/18/02: The plantings are beginning to bud, except the blueberry.
• 4/23/02: No change since previous report.
• 5/1/02: Not observed.
11. Impermeable barrier and easterly embankment:
• April 1: Building Commissioner noticed a considerable amount of flow emanating
from the riprap covering the "blowout" area, At 10:35am the flow was approximately
1/3 of what was flowing a few hours earlier.
• April 2: No flaw coming from the blowout area. Fred King and geotechnical engineer
will investigate this area Friday (4/05/02)to try to determine source and method(s) to
repair it. They will also investigate the seepage flowing through the bank just north of
this area.
• 4/9/02: Blowout Area— This area was inspected on 4/5/02 by Richard Weber,
geotechnical consultant, SBNE, representatives of DiNisco Design Partnership and
the Contractor. See the attached memo dated 4/9/02 for additional information on
this area. No seepage was occurring during this 4/9/02 inspection. SBNE to prepare
a sketch plan for addressing this item.
• 4/18/02: No seepage was occurring.
• 4/23/02: No change from previous report. Item discussed at construction meeting and
SBNE to submit sketch plan.
• 5/1/02: No seepage was observed during this inspection. A sketch plan was submitted
to the Architect.
12. Dewatering:
• April 1: Effluent from dewatering operations should be directed overland towards the
sedimentation basins without creating an erosion problem or directed into a straw bale
and fabric bin. The effluent should not be directed towards the haul road.
• 4/9/02: No change from previous report.
• 4/18/02: Did not view any dewatering operations.

A deep gully (2' wide x 28" deep) travels down the slope towards CB 93. The gully
continues to get wider and deeper, the widest section occurring under the slope
stabilization fabric. There were no silt deposits on or around the stone interceptor
drain or catch basin 93 in the area below the gully. The General Contractor plans to
fill this gully with riprap stone.

General contract has been signed by all parties and bonds and insurance
certificates have been received.
Change Order#1 to adjust the tile subcontractor has been executed.
2. Permits:
Sitework and foundation permit has been transferred from Zoppo to Callahan.
Building permit has been applied for and awaits health Dept. review and approval.
DPW utility permits have been received and storm drainage is in progress.
A water ban waiver has been obtained from the North Andover DPW.
3. Project Meetings:
Preliminary meeting with Callahan, DDP, MBC, and School Administration was
held on 3/4/02 to review contract procedure.
Pre-construction meeting with the G.C., major subs, School Administration, town
Depts., DDP, and MBC was held on 3/26/02.
Construction meetings are being held weekly at 1:00 p.m. on Tuesdays.
Separate meeting have been held to review concrete, utilities and structural steel.

COMMENTS:
The purpose of this memo is to summarize our understanding of today's meeting on site.
1. Two areas of seepage along the cut adjacent to the wetlands were noted.
2. The seepage area closest to the existing school is located in an area where there is no membrane
and might be due to groundwater seeping along sandy zones in the till. We understand that
Schoefield Bros.Will prepare a sketch for an inverted filter to be constructed along the slope within
this area. The filter might consist of a combination freely draining sand overlain by a orotextile
fabric and riprap. This area would also be connected to the subsurface drain.
3. The second area of observed seepage might be related to a damaged or clogged subdrain. The
condition of the pipe will be assessed.
4. These repairs are intended to be the first step. Other refinements might be necessary.

The tree replacement resolution you have suggested seems quite fair to us, and we will await the fall planting
season for the work to be completed.
Thank you, also, for the update on the construction hours. I don't want to leave you with the impression that
the construction noise has been totally unbearable. Rather, except for the previously mentioned
abnormalities to the hours and day,we have really been quite surprised (and pleased) by the lack of
intrusiveness the construction has had to date.

TREE REPLACEMENT-The two new trees that have died are under warranty,and we will notify Zoppo to arrange with you replacement in kind.As for the
transplanted tree,as we had previously discussed with you,transplanted trees are not covered by a warranty from Zoppo. However,it was agreed that the success of
the screening would be reevaluated later in the project,and in
keeping with that agreement we will recommend to the SBC that the dead transplanted tree be replaced.Since it is not possible to replace it with a new tree of equal
size,we would recommend replacing it with two new evergreens,most likely during the fall planting season.
WORK HOURS-We have confirmed with the town that by ordinance,work hours are 7:00 a.m.-6:00 p.m.Monday through Saturday. This means that the
contractor cannot start the equipment before 7:00 am.,and we will remind him of this requirement.As for weekend work,we did not intend to leave you with the
impression that there would be no work perfomed weekends;Sunday work is not allowed without receiving a police department permit beforehand(and you should
feel free to call the police if you hear work on a Sunday).Saturday work,since it costs the contractor overtime,should rarely take place,and was likely done this
past weekend to make up for time lost due to the rain.
We apologize for any misunderstanding with regard to the weekend work,and would be happy to meet with you to review the above,or any other questions you may
have.I am on site every Tuesday afternoon from 12:30 to 4:00 p.m.;please feel free to call to schedule a time to get together.

JTC 04/02/02 C01.2 Building Permit- Bob Nicetta reported that the Health
Department must sign off on the building permit, and
it may be helpful for JTC to meet with the Health
Department Administrator.
JTC 04/16/02 C01.2.1 Health Department- JTC has called Health
Department and will follow up.
JTC 05/07/02 C01.2.2 Building Permit- A full building permit will be needed
NABC before the steel erection begins 06/03/02. JTC will
fill out forms and Bob Nicetta will convert foundation

the HVAC subcontractor has bought out the job, and
the rooftop equipment submittals should be soon

general, are as follows:

work progresses in the south wing, and where

west end of the gym has been removed. The
corrugated steel pipe under the gym slab, already
partially clogged, will be filled as much as possible

pipe under footings, will be removed.
-- 05/14/02 R03.3.4 E12 Line - The unsuitable soils underlying E12 line
will be removed and replaced with structural fill. Filter
fabric and stone subbase is not required where
subgrades consist of new structural fill.
-- 05/14/02 R03.3.5 Unsuitable Soils - Unsuitable soils under footings
will be removed and replaced with new structural fill
as per item R03.3.4 above. The black topsoil under
the gym slab will be excavated to a sub-subgrade
of 2 feet below bottom of slab, proofrolled, and
geotextile fabric and structural fill placed over the top
soil.
-- 05/14/02 R03.3.6 Academic Wing - Piers will be removed (Postscript-
all five to be removed) and the existing pipe will be
removed, including under the walls. The foundation
walls will be underpinned in a single length using
concrete vibrated to the underside of footing.
-- 05/14/02 R03.3.7 Payment - Where applicable, unit prices apply. JTC
intends to assign one operator to perform all the
"extra"work described above in sequence, which
will help track related costs.
DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP
F&B with regard to having a lift point on the roof for bringing up items
that are too heavy or too large to go up the roof hatch. DDP noted
that if one point was designated to lift a load of a few thousand
pounds (i.e. HVAC equipment), the entire roof would need to be
designed for that live load. DDP will investigate means of lifting
items like a snow blower to the roof.

Schofield Brothers of New England, Inc. has been designated as the"Environmental/
Erosion Control Monitor" for the North Andover High School Project. The purpose of the
Environmental /Erosion Control Monitor is to perform periodic inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and stormwater
management issues.
This is the eighth monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period April 10, 2002 through May 7, 2002.
Attached are copies of the Inspection Reports (#23, #24, #25, and #26) relative to the
inspections performed on April 18, April 23, May 1, and May 7, 2002.
There was a considerable amount of construction activity during this time frame, which was
directly related to earthmoving activities required for building footing and foundation wall
installations along with some underground utility installations and grade adjustments. Given
the amount of activity, the site continued to remain in stable condition, and erosion and
sedimentation issues were contained on site and there were no impacts to any of the
resource areas.
Items that needed attention were addressed by JT Callahan and included:
• the silt fence surrounding the ordinary stockpile was extended around a slide that
occurred at its east side
• the large gully that formed in the slope above catch basin#3 was filled with stone
and the area surrounding catch basin#3 was cleaned and repacked with stone.
• a barrier fence was installed along a portion of the east bank to prevent vehicles from
creating ruts in the embankment

Observations made during the four 4) visits were as follows:
• The exterior sedimentation fence, haybales and fence remained in good condition
and no breaches occurred.
• Grass cover began to grow on the loam stockpile, the ordinary stockpile (to a lesser
extent), along the east bank, and along the sides of SB #2 and SB #3.
• The diversion berm crossing the haul road at SB #3 continued to work well and it
appears to be minimizing erosion along the haul road just south of it and at the
southeast entrance. Gullies that were already formed remain in place.
• On days when no runoff is observed, a small amount of seepage continued at the
east bank where a problem with the subdrain is suspected.
• A temporary stockpile was created just north of SB #3 and used for grade
adjustments in the general area between SB #2 and SB #3. Haybales were placed to
protect SB #3 from unnecessary amounts of silt.
• No dewatering operations were observed.
• The truck washing area was used as intended; all washings were contained.
• Wetland replication plantings were in good condition except for blueberry plants.
• Dust control was not an issue for this time period.
Other items:
• Sedimentation Basin 91 was taken out of service, as expected, for building
construction to continue in that area.
• A sketch plan depicting a method to resolve the concentrated seepage at the
"blowout" area was submitted.
• A work order was issued to inspect the subdrain; scheduling a subcontractor is in
process.
Based on our inspections, we have found that the work on the project has been performed in
compliance with the Special Permit and Order of Conditions. Please let us know if there are
any issues that we are not aware of

Schofield Brothers of New England, Inc, has been designated as the "Environmental /
Erosion Control Monitor" for the North Andover High School Project. The purpose of the
Environmental / Erosion Control Monitor is to perform periodic inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and stormwater
management issues.
This is the ninth monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period May 8, 2002 through June 4, 2002.
Attached are copies of the Inspection Reports (##27, 428, #29, and 430) relative to the
inspections performed on May 14, May 21, May 28, and June 4, 2002.
The site experienced its largest single rainfall event of the year, and in combination with the
site construction activities, put a strain on the erosion controls, The water level in
Sedimentation Basin #3 got its highest level, overflowing the spillway and passing into the
secondary sedimentation basin. Excess flow then continued to erode the already formed
gullies at the southeast emergency entrance. Once again, the southeast entrance was
repaired. A TV camera inspection of the subdrain was completed and some silt and gravel
were found but not enough to create a blockage. Interestingly, while excavating to find the
end of the subdrain (in the area where the chronic seepage has been occurring at the east
embankment) a, trench drain was found. Stone was added to the excavation to link the
trench drain to the subdrain and backfilled. Since then, no weeping has been occurring.
Grade adjustment operations at the southern portion of the construction site have not
contributed any erosion or sedimentation issues. Along with maintenance provided by the
general contractor, the erosion and sedimentation controls have been working properly to
keep silt and fines on site and there were no impacts to the resource areas.

runoff and the sediment is conictined on site, it is preferable to leave the features

approximately perpendicular to the subdrain. This drain is believed to have been part
of a previous subdrainage system to lower high groundwater experienced on the
former athletic fields. The PVC pipe in the immediate area was removed and a trench
was dug and lined with stone between trench drain and the subdrain inlet. The trench
was then backfilled. It is anticipated that this work will eliminate the seepage onto the
haul road. Monitoring will continue.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:

any questions regarding the information contained in this report.
This inspection followed 1-1/2 days of rain. The total amount of rainfall, approximately 3 inches,
coupled with the amount of construction activity, certainly put an uncommon load on the site
erosion and sedimentation controls. Items needing attention were discussed with the contractor
following the inspection and repairs are underway. The TV inspection of the subdrain was
performed on 5/15; the results are posted in Item 9.

expected. Sediment Basin#3 received so much runoff that the high water level
exceeded the elevation of the secondary spillway. The resulting flow through the
spillway undermined some of the riprap structure. The additional sediment load was
contained in the secondary sedimentation basin. (It is also believed that the excess
flow was a major contributor to the gully enlargement at the southeast emergency
exit). The erosion that occurred at the riprap should be repaired.
The high water level mark observed on the inlet structure indicates that the spillway is
at a lower elevation than the beehive inlet. However, the intent of the design is to
allow a high water level condition to discharge through the beehive inlet prior to
overflowing the spillway. SBNE will evaluate this with the project architect
and the general contractor.
