Action Date A. SITE SAFETY AND SECURITY -- 07/16/02 A14. Fencing - JTC will confirm that open sections have been 07/30/02 tightened up, in particular the southwest corner. OWNER ISSUES DDP 05/07/02 B06.2 Revised Site Phasing Drawing - JTC provided a site plan showing the proposed water and road work, and the limited parking that will be available in the south lot when the new school opens. Topping Off Ceremony - NASD would like to have the students participate in a topping off ceremony when the last beam is erected. JTC stated the completion of the steel would be November 2002, which would allow NASD some time to plan. It was agreed that a piece of steel (in the gym, or the auditorium) could be signed by the students. HCI 06/27/02 B12.1 Keying (Postscript) - DDP and HCl met with NASD on 06/27/02. Based on discussions, HCI will prepare a keying schedule for review. HCI 06/27/02 B12.2 Access Control - HCI to furnish information on hand recognition systems that can be used in lieu of keypads or card control. B14. C. PERMITS & INSPECTIONS DDP 06/04/02 C01.2.6 Health Department- The Health Department must JTC sign off on the kitchen. Once the food service NASD submittals are approved, a meeting with the Health Department will be scheduled to review the kitchen. -- 07/09/02 C01.2.7 (Postscript) - Kitchen equipment drawing returned from engineer (with some revise and resubmit). Returned to JTC on 07/10/02. 03/26/02 CO3. DPW Permits -- 07/02/02 CO3.6 Water, sewer and highway all okay with DPW. 07/30/02 DPW will give criteria for testing. Record item. -- 07/30/02 CO3.6.1 DPW Testing - JTC reported that DPW has reviewed and is satisfied with site utilities testing and installation procedures to date. Record item. 03/26/02 C04. North Andover Fire Department Permits JTC 03/26/02 C04.1 The NAFD requires a propane permit. Should propane be required for construction purposes, JTC is to advise the NAFD of the total aggregate amount of propane on site. Propane tanks are to be secured and enclosed by a chainlink fence and must be located away from the existing school. DDP 04/02/02 C12. Environmental Reports - DDP will distribute the weekly and monthly reports prepared by SBNE to Bob Nicetta, NASD, JTC, MBC and the clerk at the weekly job meetings. -- 05/28/02 C14.2 Mass Highway Permits - JTC advised that the 07/30/02 traffic safety program that Mass Highway required is nearly complete, and the utility work permit/curb cut work permit extension is expected shortly. OBSERVATION REPORT NO. 19 North Andover High School Page 3 DPW 07/30/02 C14.4 Mass Highway Permit - JTC reported application, MHW including traffic safety plan, has first been submitted to NA DPW, per Mass Highway requirements. It will then be forwarded to Mass Highway. -- 07/09/02 C19.11 JTC to reshape swale at retaining wall above 07/30/02 existing school. JTC to monitor. Record item. -- 07/23/02 C26. Hay bales and stone berm to be repaired when excavation 07/30/02 complete. Record item. -- 07/30/02 C27. Siltation Controls - SBNE reported that these are no significant items outstanding that require attention at this time, and that ongoing maintenance of controls continue to be performed by JTC. Stormceptor Substitution - It was discussed that the site structure vendor has initiated proceedings with the NACC to have his product approved as an equal to the specified water quality structures. This will require a change in the Order of Conditions, and the review of the proposed change by the town's civil engineering consultant. The vendor is responsible for all submissions and fees to the Conservation Commission. JTC noted that the installation of these items is not on the critical path. C29. D. CONSTRUCTION SCHEDULE JTC 05/31/02 D02.2 Schedule of Values (Postscript) - DDP forwarded comments from MBC and DDP on the schedule of values, for JTC comment/revisions. -- 07/30/02 D21. Upcoming Activities - JTC distributed a schedule of activities through 08/11/02 (copy attached) with the following noted. U.S. Drywall confirmed all studs being installed are 16 gauge, as specified, which is the reason the previous drawings were returned "revise and resubmit." Because of R&R's substitution of the curtainwall system, it is agreed that only the studs at the first floor sill and third floor head are load bearing. ABP 07/18/02 F20.4 Lighting Submittal (Postscript) - DDP returned on 07/18/02 the lighting submittal. The entire submittal book shall be returned with the fixtures revised per comments inserted. Color selectives must be submitted with the resubmitted binders. JTC 07/23/02 F21.1 Steel Priorities (Postscript) - As of 07/25/02 all sheets of sequences 10 through 16 have been returned to JTC. Stairs - JTC noted stairs will be soon be resubmitted with structural calculations, and requested that F&B and DDP expedite their reviews. F26. G. COORDINATION DRAWINGS -- 05/21/02 G06.1.6 Rooftop Equipment - Steel at rooftop units was discussed at coordination meeting 05/21/02. It was agreed that structural steel will not be adjusted and supplemental framing for reaction loads will be installed by Sagamore. Coordination Drawings - JTC reported that work on the Subs coordination drawings has begun, and the process is expected to take 10-12 weeks. JTC 07/16/02 G12.2 Coordination/Academic Wing Center- JTC stated subs have worked out congested areas and coordination drawings will be submitted to DDP later this week. -- 07/23/02 G12.2.1 Coordination Academic Wing - (Postscript) Center Wing coordination drawings received by DDP on 07/24/02 and distributed to consultants for review. -- 07/30/02 G13. Coordination/Academic Wing East Third Floor(Postscript) - Reviewed drawings returned by DDP 07/30/02. Record item. G14. H. PAYROLL REPORTS -- 05/14/02 H05, Foundation Subcontractor-The clerk noted that the iron 07/30/02 workers have not been included in the foundation sub's reports. JTC will forward the required reports to the clerk. Record item. -- 06/25/02 H05.3 Foundations/Iron Workers - Clerk advised that 07/30/02 records are up to date, but questioned number of iron worker apprentices. JTC to review with Valle Concrete. Record item. -- 07/02/02 H05.3.1 Iron Workers - JTC to discuss with union the number/ratio of apprentices currently at the site. -- 07/30/02 H05.4 Meeting with Subcontractor-Prior to the 07/30/02 meeting, Premier(the reinforcing subcontractor) met with all in attendance to review questions with their submitted payroll reports, as follows: - Hours on payrolls do not correspond with clerk's observations and the actual volume of work accomplished during certain periods. -The state mandated ratio of journeymen to apprentices' is not being met, and apprentices levels are not marked on payrolls. - The total hourly wage does not appear to meet the minimum prevailing wage. Premier 07/30/02 H05.5 After discussion, it was agreed that the clerk will furnish JTC with a summary of his manpower observations to date. It was agreed that from now on, Premier's workers will sign in and out each day using wage forms posted in JTC's trailer. Premier will make sure that workers get the required rate, and will review and address any other discrepancies. H08. OBSERVATION REPORT NO. 19 North Andover High School Page 7 I. REQUISITIONS JTC 03/26/02 106. Schedule of Values shall be submitted immediately for the architect's and consultants' review and approval. -- 06/04/02 106.3 Masonry Schedule of Values - Noting that 07/30/02 brickwork is schedule to begin in July, MBC noted mason should submit their schedule of values ASAP. Record item. -- 07/22/02 106.3.1 Masonry Schedule of Values (Postscript) - 07/30/02 Breakdown received 07/22/02. DDP and MBC to review and comment. Record item. JTC 07/30/02 106.3.2 Masonry Schedule of Values - MBC and DDP PCM noted that labor and material for most items, such a brick veneer, are more balanced than is typical (US 2:1 labor to materials). This will only be an issue if there are to be stored material payment requests. JTC will review the comments with PCM. DDP 04/30/02 107.1.1 Cash Flow - JTC has furnished DDP with a cash flow for the first eight months of the project. DDP will add project costs and forward to town. 112. J. CHANGE ORDERS DDP 06/25/02 J03. Change Order No. 3 - DDP will review potential change orders COW submitted 06/25/02 with clerk and MBC, and will issue MBC approved COP's with Change Order No. 3. DDP 07/02/02 J03.1 Change orders to be reviewed for NASBC COW meeting for August. MBC J04. K. WORK ORDERS JTC 06/11/02 K04.1 Work Order No. 8 - JTC will submit "ballpark" pricing from subcontractor before numbering existing parking spaces. JTC and NASD agree that work is to be done in August, before start of school. -- 07/23/02 K04.1.1 Work Order No. 8 - Submitted to DDP on 07/30/02 07/23/02. Proposal Request No. 1 - DDP issued PR No.1 for revisions to site utilities resulting from site utiltiy coordination by RWS. Sagamore 05/28/02 M09. Proposal Request No. 4 - DDP issued PR No. 4 addressing miscellaneous HVAC items to JTC for pricing by Sagamore. Coordination shall progress according to the change in advance of agreeing on a cost. ABP' 06/04/02 M10. Proposal Request No. 3 - DDP noted PR No. 3, for electrical power to exhaust fan EF-60 (added under PR No. 4) was issued 05/07/02. -- 06/05/02 M11. Proposal Request No. 5 (Postscript) - On 06/05/02 DDP issued PR No. 5 for revisions to language lab power and conduits. -- 07/15/02 M11.3 PR No. 5 (Postscript) - TE, MBC and DDP 07/30/02 comments on language lab PCO returned to JTC 07/15/02. JTC will advise Tim Callahan of this comment. -- 07/30/02 M12.1.1 PCO No. 014 - JTC has revised PCO No. 014 following review with the Clerk, and it will be incorporated into Change Order No. 3. -- 07/18/02 M12.2 PCO No. 015 - (Postscript) PCO No. 015 for unsuitable soils removed approved as submitted. RFI Status - DDP and JTC exchanged logs showing status as of this date. JTC noted no critical items are open at this time. Record item. N22. O. DEMOLITION 03/26/02 001 . Building/Abatement Demolition is scheduled for August 2004 and prerequisites will be reviewed as that date approaches. NAFD 03/26/02 001.1 Public safety training will take place in the vacated NAPD existing high school for two weeks after the school is vacated by the owner. 002. P. SITE WORK P09. Q. ARCHITECTURAL JTC 03/26/02 Q01. Color Selection Submittals - DDP advised that materials Subs requiring color selection shall be submitted as soon as possible so the architect can prepare recommendations for the owner. DDP 07/16/02 Q01.1 Glazed Masonry Color Selection - PCM stated that due to lead times and because glazed masonry units are used for wall back up construction (in locker wing) color selection should be expedited, and noted that DDP can pick from either Spectra Glaze or Astra Glaze selections. -- 07/02/02 Q03. Metal Siding and Windows finish to match R&R to coordinate 07/30/02 with panel supplier. Record item. -- 07/16/02 Q03.1 Metal Panel Colors - DDP noted windows and 07/30/02 preformed cladding are specified to match a Citadel color. JTC will contact BASS to determine intended panel supplier, so that DDP can advise as to color selection for windows. Masonry Sample Panel - PCM will construct panel on stud 07/30/02 back up, for review and approval 07/23/02. PCM requested mortar color selection (Postscript - DDP confirmed 07/18/02 that mortar shall be as noted on sample panel sketch). Record item. OBSERVATION REPORT NO. 19 North Andover High School Page 10 -- 07/23/02 Q04.2 Masonry Sample Panel - Panel was reviewed 07/30/02 and noted that mortar was not what was desired. FO No. 44R1 was revised with correct mortar type (Type I Portland Cement). Record item. -- 07/30/02 Q04.3 Masonry Sample Panel - DDP noted that the panel with the revised mortar color was reviewed by DDP 07/29/02 and approved. Record item. DDP 07/16/02 Q06. Slab Depressions - DDP advised that the owner wishes to upgrade main street/cafeteria floor finishes, and the substitute flooring will require a slab depression of up to 1 inch for material thickness. DDP must advise as to extent before center wing slab on grade is poured by the end of July. DDP 07/30/02 Q06.1 Slab Depressions - JTC advised DDP that the basement slab will be poured 08/09/02, and the center wing west floor is scheduled to be poured with it. DDP to advise as to slab depression for proposed flooring substitution. If need be, JTC will not pour center wing west on 08/09/02. -- 07/23/02 007. Metal Panel Supplier- Bass wants to discuss Alucobond 07/30/02 panels at meeting next week prior to weekly meeting. Record item. -- 07/23/02 Q07.1 DDP bring samples of Citadel panels, etc. Record 07/30/02 item. -- 07/30/02 007.2 Metal Panels - Before the job meeting DDP and 08/02/02 JTC met with Bill Spates/Bass Assoc. and Eric Edwards/Alucobond. Bass proposed substituting the Alucobond system for the specified Citadel panel. The respective systems were reviewed and installation and aesthetic issues discussed. DDP to advise. Record item. Bass 08/02/02 007.3 Metal Panels (Postscript) - DDP advised JTC on 08/02/02 that it will be acceptable to substitute the Alucobond panel. DDP advised that Bass shop drawings must thoroughly address the one inch greater assembly thickness of the Alucobond panel. All panels and trim shall match custom PPG off white color selected for windows. Bass to meet with DDP and JTC 08/06/02 to finalize substitution issues. -- 07/30/02 Q08. Roof Fasteners/Main Street - With regard to DDP's concern that fasteners will be visible through the Main Street roof deck, JTC has confirmed with the roofer that there will be no screws used this area. Record item. Q09. OBSERVATION REPORT NO. 19 North Andover High School Page 11 R. STRUCTURAL JTC 05/21/02 R02.1.1 HVAC Screen - JTC, after review with metal panel sub and C&I, reported that panel will span verticals without the second TS 5X5 girt. DDP and F&B take no exception, and requested submittal of detail proposed. DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 06/18/02 R06.1.3 Bolts - All bolted connection will use TC bolts. T&L will test representative samples. C&I 06/18/02 R06.1.5 Plant Inspection - The C&I plant in Mass. only T&L fabricates the bolted members. T&L will want to visit the plant where the gym trusses are fabricated, which will be an out of state C&I facility. C&I to advise as to when truss production is to begin. -- 07/16/02 R07.1 Unsuitable Soils - The unsuitable soils have 07/30/02 been completely removed from the south wing main street area, as well as pocket found in the auditorium area. The known unsuitable soils yet to be removed are in the gym floor area (to be cut an additional foot and proofrolled) and at the water line/driveway east of the exiting school (to be completely removed). Record item. JTC 07/02/02 R09.1 Wall at receiving area in Gym was poured too high; need opening for overhead door. JTC to repair. -- 07/23/02 R11. F&B to visit site to review exterior stud work prior to brick work beginning. Record item. JTC 07/30/02 R11.1 Stud Installation - It was noted that F&B visited the site to review studs 07/23/02. F&B noted that some stud track fasteners, etc. are not yet installed. JTC advised that areas remain accessible so that fasteners can be installed. JTC 07/30/02 R12. Unsuitable Soils - JTC and COW reported that all the "black" organic soils have been removed at the water line. The only unsuitables remaining to be removed will be done when the retaining wall work begins. The week of 08/11/02, when the slab prep begins, the underlying organic soils will be cut and proof rolled as previously discussed. R13. OBSERVATION REPORT NO. 19 North Andover High School Page 12 S. FIRE PROTECTION Hampshire 03/26/02 S01. Fire Department Approval - RWS stated that drawings, specifications and calculations shall be sent to the NAFD before submittal to architect. RWS 06/11/02 SO4. Header Sample - In accordance with the specification 07/30/02 requirements, Hampshire has submitted a sample of the header construction for approval. The sample will be in the clerk's trailer for review by Robert W. Sullivan the next time they visit the site. Record item. -- 07/30/02 SO4.1 Header Sample - DDP noted sample was approved by RWS during their last site visit. Clerk to keep sample in trailer for reference. Record item. -- 07/30/02 S06. Sprinkler Service - JTC noted line into academic wing is to be installed next week. Record item. S07. T. PLUMBING T05. U. HVAC U04. V. ELECTRICAL NASD 06/11/02 VO4.3.2 Service Right-of-Way - Because the new pole DDP 3997-1 is not on school properly but is in the road right-of-way, MEC needs a letter on town letterhead stating: "The town of North Andover is aware that MEC is setting pole 3997-1 in the public way to serve the new High School." MEC 06/11/02 VO4.3.3 New Service Pole - MEC will set the new pole in approximately two weeks. ABP 06/11/02 VO4.3.4 Service Backcharge -The increase in backcharge JTC will be issued as a change order from ABP through JTC. MEC 07/16/02 VO4.3.5 Service Installation - JTC advised that electrical service installation is awaiting "reengi nee ring" by Mass Electric. ABP 05/21/02 V06. Switchgear- Submittal is critical for approval because ABP intends to install gear prior to work overhead in mech. room area is complete. MBC noted it will be important for switchgear to be fully protected if installed prior to building being tight. PAYROLL REPORTS -- 07/02/02 H05.3.1 Iron Workers - JTC to discuss with union the 07/30/02 number/ratio of apprentices currently at the site. Record item. -- 07/30/02 H05.4 Meeting with Subcontractor-Prior to the 07/30/02 meeting, Premier(the reinforcing subcontractor) met with all in attendance to review questions with their submitted payroll reports, as follows: - Hours on payrolls do not correspond with clerk's observations and the actual volume of work accomplished during certain periods. - The state mandated ratio of journeymen to apprentices' is not being met, and apprentices levels are not marked on payrolls. - The total hourly wage does not appear to meet the minimum prevailing wage. Premier 07/30/02 H05.5 After discussion, it was agreed that the clerk will furnish JTC with a summary of his manpower observations to date. It was agreed that from now on, Premier's workers will sign in and out each day using wage forms posted in JTC's trailer. Slab Depressions - DDP advised that the owner wishes to 08/09/02 upgrade main street/cafeteria floor finishes, and the substitute flooring will require a slab depression of up to 1 inch for material thickness. All panels and trim shall match custom PPG off white color selected for windows. Bass to meet with DDP and JTC 08/06/02 to finalize substitution issues. Bass 08/06/02 Q07.4 Metal Panels - DDP, JTC and Bass met prior to 08/06/02 job meeting to review the Alucobond panel to be substituted on the job. DDP distributed contract drawings with details of panel/wall and panel/window construction interfaces that must be revised by Bass's shop drawings to address the 1 3/4 inch thicker panel system. The Alucobond panels (and windows) would then be painted to match the running trim. DDP to review samples provided by Bass and advise. JTC stated a decision must be made ASAP, so that R&R can be notified of any change. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 06/18/02 R06.1.3 Bolts - All bolted connection will use TC bolts. T&L will test representative samples. Sediment control barriers around the perimeter of the current work area: • 8/6/02—Southeast work area: The newly installed sedimentation barrier and silt barrier along the southeast side has been functioning properly. The small amount of sediment generated from the earthmoving and regrading activities has been contained. The haybale barrier in the most southeastern corner has been reinforced. The parking lot area needs to be cleaned and swept in the vicinity where the water line was installed, where the remaining bituminous patch is to be placed, and along the security fence towards the southeast entrance gate. • 8/13/02 —The perimeter sediment control barriers, and recently repaired/reinforced sections were in good condition. Southeast work area: The parking lot areas were swept. A loam stockpile, located in the area undergoing regrading, will either be spread in the area or moved to the approved loam stockpile area. A swale was constructed along the east side, between the sedimentation barrier and the roadway grading, to channel seepage to a lower area. • 8/20/02 —The perimeter sediment controls were in good condition. Southeast work '.... area: See Item#6 (new silt boots are needed at the two catch basins). 2. Loam stockpile area in the northern portion of the site: • 8/6/02—The loam stockpile has good grass and weed cover and appears to be stabilized. The perimeter silt fence is in good condition. • 8/13/02 — Same as previous report. • 8/20/02 — Same as previous report. 3. Ordinary borrow stockpile area in the northern portion of the site: • 8/06/02— Continued from 7/09/02 - A portion of the perimeter silt fence was overtopped with sediment near the truck washing area. The area needs to be cleaned up and the fence repaired. • 8/13/02 —Repairs mentioned above were completed. • 8/20/02—A minor repair to a silt fence tear is needed on the east side. As stated in earlier reports, the stockpile entrance on the east side should be closed if hauling operations to and from the stockpile area are completed, or prior to a rainfall event. 4. Main entrance gate near trailers: • 8/6/02 — Continued from 7/26/02 - Noted some tracking of sediment onto adjacent roadway. The construction entrance is in need of maintenance. • 8/13/02 —Repairs mentioned above were completed. • 8/20/02 — Some tracking of dirt onto Prescott Street was observed. Catch basins: • 8/06/02 — Continued from 7/16/02 - The silt boot and haybales need to be replaced at the catch basin located between the retaining wall and the existing school. • 8/13/02—Repairs mentioned above were completed. • 8/20/02— At the southeast parking lot area, near the water tie-in: new silt boots are needed at the two catch basins. 7. Sediment basins and outlet structures: • 8/6/02 — Sediment Basin#2 and #3 are in good condition and functioning properly. • 8/13/02 — Sediment Basin#2 and #3 are in good condition. • 8/20/02 - Sediment Basin #2 and #3 are in good condition. There has been some gully formation in the area close to SB 92, apparently from flow tests performed at the nearby hydrant. The sediment load in SB #2 has increased but not to a depth that requires removal. 8. Other erosion control items. • 8/6/02 — Continued from 7/02/02 & 7/26/02 - Some of the stone that was used to fill the large gully that had formed in the embankment just north of catch basin #3 has been washed out and should be placed back in the gully. In addition, the silt boot should be replaced at Catch Basin#3. • 8/13/02—Repairs mentioned above were completed. • 8/20/02—Nothing to report. 9. Other drainage structures: • 8/06/02 -Building subdrain outlet: sediment covers only a small area around the catch basin; condition is satisfactory. • 8/13/02 — Same as previous report. DEP File No. 242-1045 Planning Board Notice of Decision dated 5/14/01 Schofield Brothers of New England, Inc. has been designated as the `Environmental/ Erosion Control Monitor" for the North Andover High School Project. The purpose of the Environmental /Erosion Control Monitor is to perform periodic inspections of the project site to insure compliance with the Order of Conditions issued by the Conservation Commission and the Special Permit issued by the Planning Board with respect to sediment and erosion controls, limit of work compliance, wetland replication and stormwater management issues. This is the eleventh monthly inspection report prepared and submitted in compliance with the above referenced permits. It covers the period July 3, 2002 through July 30, 2002. Attached are copies of the Inspection Reports (#35, #36, #37, and#38) relative to the inspections performed on July 9, July 16, July 26, and July 30, 2002. A significant rainfall event proceeded the July 9 inspection (05). The only problem observed was that a small amount of material got past the perimeter sedimentation barrier located at the southwestern corner and was deposited on the grass strip adjacent to Chickering Road. It was surmised that runoff overtopped a swale and an interior silt fence then eroded a hole underneath a haybale; the sediment observed in the grassed area beyond the haybale did not appear to impact the roadway. This area was repaired and reinforced with a stone berm. There were no other significant rainfall events that occurred for the next three inspections and the site remained dry and stable. During that time, there were no observations of any impacts to the resource areas, weather related or construction related. There were several maintenance items listed on the inspection reports that were carried forward, such as replacing silt boots, replenishing crushed stone, replacing a catch basin grate, and repairing the silt fence around the ordinary stockpile. Nevertheless, these items were reviewed with site personnel and the maintenance is expected to occur. Observations during the last four inspections included: • Other than the small breach at the southwest corner,the perimeter sedimentation fence remained in good condition. • In the southeast area, where the water tie-in and regrading work was being performed,the perimeter controls and temporary controls functioned properly; no impacts to the wetland area or stream in the vicinity of the work was observed. • Sediment on the pavement near the building subdrain and southeast entrance was swept and removed. Since that time the areas have remained in good condition. • Runoff from the significant rainfall event overtopped the retaining wall. To prevent this from happening in the future, the Swale above the retaining wall was slightly regraded to better direct runoff towards the sedimentation basins. • Due to the dry conditions,the sediment basins have been sitting idle. • The stockpiles remained in good, stable condition; repair of the perimeter silt fence is necessary for the added protection during a rainfall event. • The southeast entrance remained in good, stable condition;the catch basin controls were re- worked. • When dewatering operations were observed, discharge was directed towards the sedimentation basins. • The truck washing area was used as intended. • Although not observed at each inspection, site personnel have informed us that the site was watered when necessary to control the dusty conditions. • The wetland replication plantings appeared to be in good condition with exception to the blueberry plants. Based on our inspections, we have found that the work on the project has been performed in compliance with the Special Permit and Order of Conditions. This site inspection was performed following a significant rainfall (just under 1 inch) that occurred the previous evening. The contractor's representative, Tim Callahan, was present for part of the inspection and several items were reviewed as described in the report. °oSCHOFIELD BROTHERS ESGINEERING • SLR%E.1-ING - PLANNING INSPECTION SUMMARY 1. Sediment control barriers around the perimeter of the current work area: • 6/25/02 - Sediment control barriers remain in place and in good condition. • 7/02/02—Same as previous report. It was re-iterated in the construction meeting, that although the new sediment barriers along the southeast side have been installed correctly and offer protection to the brook running down the east side, attention should be given to protecting runoff from freely traveling across the paved areas and towards the south as work commences in the southeast area. • 7/9/02—In general, the perimeter erosion control barriers were in good condition with the exception of one location at the southwest corner of the work site (to the west of Sediment Basin#2). At that location, the temporary diversion berm that directs flow from the haul road to Basin#2 broke through during the previous rain and overtopped the adjacent perimeter silt fence and eroded a small hole through the haybales. A small amount of sediment reached the grass area adjacent to Chickering Road but did not appear to impact the roadway. The berm and the sediment barrier needs to be repaired. • 7/9/02—The area of the proposed water main connection at the southeast corner of the site was reviewed with Tim Callahan. That work is scheduled to occur in the next few days. The connection will involve cutting a trench and installing the pipe through the existing parking area to the existing watermain in the driveway. The entire alteration is a narrow area and is estimated to take 3 days including pavement patching. The area of the trench cut drains to three existing catch basins. There is no direct overland flow to the wetlands or stream. Due to the nature of the work, it is not necessary to install the complete perimeter erosion controls around the subject area at this time. To protect the wetland areas, the three catch basins are to be protected with silt boots and haybales. The obvious location where stormwater would overflow to the stream, in the event of a severe storm or blockage of the catch basin inlet, is at a low point over the lawn at the southeast corner of the parking lot. At that location, a section of erosion control barrier (haybale/silt fence) should be installed in the lawn area to protect the adjacent stream. This was discussed with Tim Callahan on site and at the weekly meeting following the inspection. It is likely that de-watering of the trench will be necessary and Tim Callahan stated that they will set up a temporary "haybale settling basin" as shown on the erosion control plan. (Note that the basin should be at least 100 feet from wetlands as required by the Order of Condtions). 2. Loam stockpile area in the northern portion of the site: • 7/09/02—The loam stockpile has good grass and weed cover and appears to be stabilized. Ordinary borrow stockpile area in the northern portion of the site: • 6/11/02—The ordinary stockpile has some grass cover. The perimeter fence is in good condition, however two sections have been left open for hauling operations and should be closed when completed or prior to a rainfall event. • 6/20/02 —Same as previous report. • 6/25/02—Same as previous report. • 7/02/02—Hauling operations have been completed and the openings in the perimeter fence will be closed.Erosion was evident at the ordinary stockpile. • 7/09/02—A portion of the perimeter silt fence was overtopped with sediment near the truck washing area. The area needs to be cleaned up and the fence repaired. 4. Main entrance gate near trailers: • 7/02/02—Stone (3/a" — 1 1/2") was used to replenish the surface at the entrance. • 7/09/02 —The area is in good condition. 5. Southern emergency entrance: • 7/02/02—Area is still in good condition; haybales have been placed in front of the gate. However,based on the light covering of silt on the paved area outside the gate, there must have been a large puddle that formed. The silt on the pavement should be removed. In addition, the area around the catch basin should be reworked which would include: replacing the silt boot, replacing the silt fence surrounding the catch basin and removing the silt that has built up at the silt fence, and surrounding the silt fence with only one row of haybales. • 7/09/02—The silt on the pavement has been removed and small berm has been installed across the haul road to help direct runoff to the catch basin. Other work remains to be completed but the catch basin protection is functional. 6. Catch basins: • 6/11/02—Silt boots will be re-checked and cleaned/replaced if necessary. • 6/20/02—Silt boots were in good condition. • 6/25/02—Silt boots were in good condition. • 7/02/02—The silt boot and haybales need to be replaced at the catch basin located between the retaining wall and the existing school. • 7/09/02— Other than as mentioned in Item 5 above, the silt boots and haybales are functional. 7. Sediment basins and outlet structures: • 7/02/02—The diversion berm needs to be relocated approximately 15 feet to the south so that runoff will be directed into the trap rock chute. The haul road diversion berm at Basin #3 was repaired and is functioning properly. The Swale above the temporary retaining wall that directs flow to Basins#2 and #3 is overflowing to the wall at two locations. The swale needs to be cut deeper to prevent this. 8. Other erosion control items. • 6/11/02 —The southwest corner of the site remains in good condition. The area around CB#3 is in good condition—no gullies have formed and sediment washing onto stone interceptor drain has been minimized. • 6/20/02—A small Swale has been installed along a portion of the west perimeter security fence and appears to be directing flow toward SB#2. The subdrain has been extended through the retaining wall. Although the end of the subdrain pipe has yet to be extended to the catch basin, flow is traveling towards the catch basin. • 6/25/02—No new issues. • 7/02/02 —The southwest corner is in good condition; the swale along the west side is functioning properly to direct runoff toward SB#2. Some of the stone that was used to fill the large gully that had formed in the embankment just north of catch basin#3 has been washed out and should be placed back in the gully. • 7/09/02—See Item 1 above relative to the southwest corner. 9. Other drainage structures: • 7/02/02—The end of the subdrain at east embankment was covered with 3/a" — 1 '/z" stone. The temporary elbow and extension has been installed at the end of the building subdrain, which protrudes from the retaining wall. Additional lengths of pipe are needed to extent the subdrain outfall closer to the catch basin. Although the silt boot and stone covering (3/a"— 1 '/a" stone) prevent silt and fines from entering the catch basin, the slow draining characteristics of these filters along with an intense rainfall event have allowed a large puddle to form in the area. A healthy deposit of silt covering the pavement remains and should be removed. • 7/09/02—The building subdrain pipe has been extended to the catch basin and the silt at the catch basin has been removed. 10. Wetland replication areas: • 6/25/02—Did not inspect wetland replication areas. We will inspect these closely at the next regular inspection. • 7/02/02—It appears that the blueberry plants were replaced, however, they were planted poorly. All other plantings are in good condition. • 7/09/02—The new blueberry plants appear to be wilting and are in need of attention. It was re-iterated in the construction meeting, that although the new sediment barriers along the southeast side have been installed correctly and offer protection to the brook running down the east side, attention should be given to protecting runoff from freely traveling across the paved areas and towards the south as work commences in the southeast area. 7/9/02 —In general, the perimeter erosion control barriers were in good condition with the exception of one location at the southwest corner of the work site (to the west of Sediment Basin #2). At that location, the temporary diversion berm that directs flow from the haul road to Basin #2 broke through during the previous rain and overtopped the adjacent perimeter silt fence and eroded a small hole through the haybales. A small amount of sediment reached the grass area adjacent to Chickering Road but did not appear to impact the roadway. The berm and the sediment barrier needs to be repaired. ® 7/9/02 —The area of the proposed water main connection at the southeast corner of the site was reviewed with Tim Callahan. That work is scheduled to occur in the next few days. The connection will involve cutting a trench and installing the pipe through the existing parking area to the existing watermain in the driveway. The entire alteration is a narrow area and is estimated to take 3 days including pavement patching. The area of the trench cut drains to three existing catch basins. There is no direct overland flow to the wetlands or stream. Due to the nature of the work, it is not necessary to install the complete perimeter erosion controls around the subject area at this time. To protect the wetland areas, the three catch basins are to be protected with silt boots and haybales. The obvious location where stormwater would overflow to the stream, in the event of a severe storm or blockage of the catch basin inlet, is at a low point over the lawn at the southeast corner of the parking lot. At that location, a section of erosion control barrier (haybale/silt fence) should be installed in the lawn area to protect the adjacent stream. Although the silt boot and stone covering (3/4" — 1 t/2" stone) prevent silt and fines from entering the catch basin, the slow draining characteristics of these filters along with an intense rainfall event have allowed a large puddle to form in the area. This will require a change in the Order of Conditions, and the review of the proposed change by the town's civil engineering consultant. PAYROLL REPORTS -- 07/30/02 H05.4 Meeting with Subcontractor-Prior to the 07/30/02 meeting, Premier(the reinforcing subcontractor) met with all in attendance to review questions with their submitted payroll reports, as follows: - Hours on payrolls do not correspond with clerk's observations and the actual volume of work accomplished during certain periods. -The state mandated ratio of journeymen to apprentices' is not being met, and apprentices levels are not marked on payrolls. - The total hourly wage does not appear to meet the minimum prevailing wage. Premier 07/30/02 H05.5 After discussion, it was agreed that the clerk will furnish JTC with a summary of his manpower observations to date. It was agreed that from now on, Premier's workers will sign in and out each day using wage forms posted in JTC's trailer. Premier will make sure that workers get the required rate, and will review and address any other discrepancies. H08. I. REQUISITIONS JTC 03/26/02 106. Steel Change Order Proposals - DDP and JTC discussed 08/13/02 proposed change orders issued by C&I steel with regard to detailing changes. DDP acknowledges that charge for engineering may be applicable for change in scope (i.e. more steel), but DDP is opposed to be approval of detailing charges for shop drawings comments for which there are no direct costs. DDP and JTC agreed to review the scope of the changes once all the shop drawings have been reviewed and approved. Record item. DDP 08/13/02 J04.1 Steel Change Order Proposals - Cape and Island Steel (C&I) explained their position that they intend to be compensated for their redetailing time resulting from revisions issued by DDP (Field order No. 32, 34 and 41) even though there are no "direct" (material and labor) costs involved with these changes. DDP noted that while the detailing expense is understandable, the hours claimed seem high, and that for the overall project it is DDP's belief that reduction in steel material, fabrication and erection makes up for addition time to redraw. Bass 08/02/02 Q07.3 Metal Panels (Postscript) - DDP advised JTC on 08/02/02 that it will be acceptable to substitute the Alucobond panel. DDP advised that Bass shop drawings must thoroughly address the one inch greater assembly thickness of the Alucobond panel. All panels and trim shall match custom PPG off white color selected for windows. Bass to meet with DDP and JTC 08/06/02 to finalize substitution issues. Bass 08/06/02 Q07.4 Metal Panels - DDP, JTC and Bass met prior to 08/06/02 job meeting to review the Alucobond panel to be substituted on the job. DDP distributed contract drawings with details of panel/wall and panel/window construction interfaces that must be revised by Bass's shop drawings to address the 1 3/4 inch thicker panel system. Masonry - DDP observed masonry work in place, noting that weep tubes must be cut flush with backside of brick to function properly. It was observed that PCM is keeping.cavities clear of mortar. DDP observed that even after cleaning, some brick has excessive mortar on its face. JTC will advise PCM of the above. DDP 08/13/02 Q10. Flooring Upgrade - DDP advised that it has been decided to pursue the substitution of thin set poured epoxy terrazzo, which will not require a slab depression. It was discussed that the slab finish may be different for the poured epoxy. DDP to review. Q11. R. STRUCTURAL JTC 05/21/02 R02.1.1 HVAC Screen - JTC, after review with metal panel sub and C&I, reported that panel will span verticals without the second TS 5X5 girt. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 06/18/02 R06.1.3 Bolts - All bolted connection will use TC bolts. T&L will test representative samples. C&I 06/18/02 R06.1.5 Plant Inspection - The C&I plant in Mass. only T&L fabricates the bolted members. T&L will want to visit the plant where the gym trusses are fabricated, which will be an out of state C&I facility. C&I to advise as to when truss production is to begin. JTC 07/02/02 R09.1 Wall at receiving area in Gym was poured too high; need opening for overhead door. JTC to repair. JTC 07/30/02 R11.1 Stud Installation - It was noted that F&B visited the site to review studs 07/23/02. F&B noted that some stud track fasteners, etc. are not yet installed. JTC advised that areas remain accessible so that fasteners can be installed. JTC 07/30/02 R12. Unsuitable Soils - JTC and COW reported that all the "black" organic soils have been removed at the water line. The only unsuitables remaining to be removed will be done when the retaining wall work begins. The week of 08/11/02, when the slab prep begins, the underlying organic soils will be cut and proof rolled as previously discussed. R13. S. FIRE PROTECTION Hampshire 03/26/02 S01. MEC 06/11/02 VO4.3.3 New Service Pole - MEC will set the new pole in approximately two weeks. For reference, the Orders of Conditions issued for the project include: • DEP File No. 242-1045: Issued 2/21/01 • Planning Board Notice of Decision: Issued 5/14/01 Please contact Bert Corey or Fred King at Schofield Brothers of New England, Inc. if you have any questions regarding the information contained in this report. Overall, the site was in good condition. The weekend rainfall did not create any adverse erosion or sedimentation issues; there were visible indications that runoff was flowing towards the sedimentation basins. SCHOFIELD BROTHERS ENGINEERING • SURVEYING • PLANNING II INSPECTION SUMMARY 1. Sediment control barriers around the perimeter of the current work area: • 8/6/02 —Southeast work area.- The newly installed sedimentation barrier and silt barrier along the southeast side has been functioning properly. The small amount of sediment generated from the earthmoving and regrading activities has been contained. The haybale barrier in the most southeastern corner has been reinforced. The parking lot area needs to be cleaned and swept in the vicinity where the water line was installed, where the remaining bituminous patch is to be placed, and along the security fence towards the southeast entrance gate. • 8/13/02 —The perimeter sediment control barriers, and recently repaired/reinforced sections were in good condition. Southeast work area: The parking lot areas were swept. A loam stockpile, located in the area undergoing regrading, will either be spread in the area or moved to the approved loam stockpile area. A swale was constructed along the east side, between the sedimentation barrier and the roadway grading, to channel seepage to a lower area. • 8/20/02 —The perimeter sediment controls were in good condition. Southeast work area: See Item #6 (new silt boots are needed at the two catch basins). • 8/27/02 — Same as previous report. See Item #6. In addition, the southeast work area at the parking lot was cleaned and will be swept within the next couple of days. 2. Loam stockpile area in the northern portion of the site: • 8/6/02 —The loam stockpile has good grass and weed cover and appears to be stabilized. The perimeter silt fence is in good condition. • 8/13/02 — Same as previous report. • 8/20/02— Same as previous report. • 8/27/02 — Same as previous report 3. Ordinary borrow stockpile area in the northern portion of the site: • 8/06/02 — Continued from 7/09/02 - A portion of the perimeter silt fence was overtopped with sediment near the truck washing area. The area needs to be cleaned up and the fence repaired. • 8/13/02 —Repairs mentioned above were completed. • 8/20/02 —A minor repair to a silt fence tear is needed on the east side. As stated in earlier reports, the stockpile entrance on the east side should be closed if hauling operations to and from the stockpile area are completed, or prior to a rainfall event. • 8/27/02— Same as previous report. 4. Main entrance gate near trailers: • 8/6/02 — Continued from 7/26/02 - Noted some tracking of sediment onto adjacent roadway.
