DEP File No. 242-1045 Planning Board Notice of Decision dated 5/14/01 Schofield Brothers of New England, Inc, has been designated as the "Environmental Erosion Control Monitor" for the North Andover High School Project. The purpose of the Environmental /Erosion Control Monitor is to perform periodic inspections of the project site to insure compliance with the Order of Conditions issued by the Conservation Commission and the Special Permit issued by the Planning Board with respect to sediment and erosion controls, limit of work compliance, wetland replication and stormwater management issues. This is the fourteenth monthly inspection report prepared and submitted in compliance with the above referenced permits. It covers the period September 25, 2002 through October 22, 2002. Attached are copies of the Inspection Reports (#47, #48, #49, and #50) relative to the inspections performed on October 1, October 8, October 16, and October 22, 2002, respectively, During this timefrarne, the site continued to be in good condition. Items carried forward from previous reports were completed prior to the most significant rainfall event (1-1/2"), which occurred just before Inspection #49, The site was well prepared for the large amount of rain. As expected from such a rainfall event, cloudy water was observed passing through several layers of sediment controls which can not filter the finest silt and clay materials. Although the controls were working as effectively as possible, cloudy water was observed discharging from the two drainage Outlets to Cochichewick Brook following that event. There were no other observations of cloudy water discharging from the drainage outlets during the other inspections. Overall, the contractor has maintained the level of the controls to adequately handle the conditions at the site. Based on our inspections, we have found that the work on the project has been performed in compliance with the Special Permit and Order of Conditions. Overall, the site and perimeter controls were in good condition. The contractor installed a pavement binder course at the north parking areas and along the east side haul road to Sedimentation Basin #3, which reduces the amount of exposed soils. Except for a small amount of sediment that needs to be removed from the parking lot adjacent to the southeast work area, there were no new issues. Sediment control barriers around the perimeter of the current work area: • 10/16/02—Southeast work area: The perimeter haybales and silt fences were working properly. Runoff from the site was observed to pass through the barriers and exit with a cloudy appearance, which seemed to be the case in several areas where the haybales, silt fence, filter fabric, and sedimentation basins filtered the water as much as possible. The water, which passed through these controls, appears to be source of the cloudy water that discharges to the brook. Southwest work area: This area was in good condition. • 10/24/02—Southeast work area: Sediment needs to be removed from the parking lot adjacent to the southeast work area. It is recommended that the southeast area be stabilized as much as possible prior to the winter conditions. 2. Loam stockpile area in the northern portion of the site: • 10/16/02 —The loam stockpile has good grass and weed cover and appears to be stabilized. The perimeter silt fence is in good condition. • 10/24/02 —The area was in good condition. '.. 3. Ordinary borrow stockpile area in the northern portion of the site: • 10/16/02 —The perimeter fence was in good condition. • 10/24/02 —The area was in good condition. '.... 4. Main entrance gate near trailers: • 10/16/02 —More stone was placed at the entrance and there was a stockpile in the area, apparently to be used after the rainfall event. • 10/24/02 —The area was in good condition. 5. Southern emergency entrance: • 10/16/02 —The haybale and silt fence controls at the gate and surrounding the catch basin were working properly. Only a small amount of erosion was occurring along the haul road. A puddle had formed on the parking lot at the gate. • 10/24/02 —The area was in good condition. There were no heavy deposits of sediment where the puddle had formed. 6. Sediment basins and outlet structures: • 10/8/02 —The areas were in good condition. As mentioned above, the contractor plans to slightly deepen the diversion swale along the retaining wall to remove sediment that has collected in the swale and to prepare for future storm events. • 10/16/02 —The diversion swale mentioned above was completed and runoff was observed to be flowing towards the two sedimentation basins. • 10/24/02— Grading and rolling operations along the east haul road required that the diversion berm be removed. It appears that subgrade material will be placed and compacted in this area. 8. Other erosion control items. • 10/24/02 —Nothing to report. 9. Other drainage structures: • 10/16/02— A puddle formed around the building subdr-ain outlet. The catch basin, was protected with a silt boot and crushed stone and was in good condition. No flow was observed at the discharge end of the subdi-ain at the easterly embankment. • 10/24/02 — Same as previous report. 10. Wetland replication areas: • 10/16/02 —The condition of the wetland replication plants that"dried up" has '.... remained the same. • 10/24/02 — Same as previous report. The architect mentioned that the previous site contractor would be contacted. 11. Impermeable barrier and easterly embankment: • 9/10/02 —The contractor is beginning the process to install the seepage inlet, (at the "blowout" area), which should be installed and functioning prior to winter. • 10/8/02 —No seepage was observed. • 10/16/02— A small amount of seepage was observed at the "blowout" area. • 10/24/02 —No seepage was observed. ate '.. 12. This inspection took place during a rainfall event, which produced approximately 1-1/2 inches of rain and was a good test of the systems. Items carried forward from the previous inspection had been completed. The contractor has been responsive in maintaining the existing controls and had the site well prepared for this rainfall event. It was clearly evident that sediment is being captured as effectively as possible by the various erosion controls. As expected from such a rainfall event, cloudy water was observed discharging �SCH0F1LLD BROTHERS ENGINEERING • SURN EN ING - PLANNING I from the two drain pipe outfalls. This is caused by relatively low concentrations of the fine silt and clay material that can not be completely filtered by the control systems. INSPECTION SUMMARY 1. Sediment control barriers around the perimeter of the current work are 9/17/02 - Southeast work area: runoff flowed along the security fence and earth berm until it reached a low spot, where it flowed onto the parking lot and traveled for approximately 90 feet until it reached a catch basin protected with a silt boot. Some sediment was observed on the parking lot close to the security fence and should be removed. The silt boot was in good condition. Southwest area: This area was in good condition, runoff flowed towards Sediment Basin#2. ® 9/24/02 —Southeast work area: As mentioned above, sediment was removed from the parking area, the silt boots were cleaned, and a subcontractor was brought in to install the sediment barrier, consisting of a silt fence and haybale combination. Southtivest area: This area was in good condition but the contractor will place some haybales at points along the security fence where there could be a possibility of silt getting beyond the fence during larger rainfall events. (Did not inspect - 10/1/02) 10/1/02 —Southeast work area: The area was in good condition. There is a small amount of runoff water (clear appearance) from the site that seeps through the sedimentation fence and onto the parking lot. Some stockpiles remain in the area. Southwest area: The area was in good condition. 10/08/02 —Southeast work area: The area was in good condition, there was some sediment on the pavement. The silt boots need replacement— see Item # 6. Southwest area: The area was in good condition but the contractor will place some haybales at points along the security fence where there could be a possibility of silt getting beyond the fence during larger rainfall events. 2. In addition, the contractor will rebuild the diversion berm at the trap rock chute leading to Sediment Basin #3. • 10/1/02—The above item has not been completed. A berm was reinstalled at the trap rock chute of Sediment Basin#3 to divert runoff from the haul road area. The Sediment Basins were in good condition. • 10/8/02 —The areas were in good condition. As mentioned above, the contractor plans to slightly deepen the diversion swale along the retaining wall to remove sediment that has collected in the swale and to prepare for future storm events. 8. Bob Nicetta observed on his Monday inspection that there were no issues with Cochichewick Brook following the rainfall over the weekend. The contractor plans to slightly deepen the diversion swale along the retaining wall to remove sediment that has collected in the swale and to prepare for future storm events. In addition, the contractor will rebuild the diversion berm at the trap rock chute leading to Sediment Basin #3. • 10/1/02—The above item has not been completed. A berm was reinstalled at the '... trap rock chute of Sediment Basin #3 to divert runoff from the haul road area. The Sediment Basins were in good condition. 8. Heating will be accomplished utilizing two (2) 1,800,000 BTU Babfar natural gas fired forced air heaters, operating 24 hours per day. One each will be located at the east and west Building Faces, with ducting into the 1St floor building hallways. 36 Inch Fans will be setup in the Hallways to provided secondary air circulation as determined necessary to provide a satisfactory Heat Distribution. Hot Air will be returned from the 3rd to the 1St Floor utilizing the Elevator Shaft and a Propeller Ventilation Fan. Building Temperatures and heat distribution will be monitored utilizing local thermometers. ARTS BUILDING AND MECHANICAL PIT: The Mechanical Pit will be sealed with Temporary Closures and the Arts Wing will be isolated and sealed with Temporary Closures incrementally as Construction Progress permits. Heating of the Mechanical Pit will be accomplished utilizing a 450 BTU Babfar natural gas fired forced air heater, operating 24 hours per day. As you know,we are finishing up the area by area review with North Andover High School personnel. Once the review is completed at our next meeting on November 12th, I will be able to complete the cabling specifications. My target is to have the specifications and drawings complete by November 30,2002. This should give us enough time to have a cabling contractor on-board by mid to end of January, 2003. As discussed previously, although the exact process for procuring voice,video and data cabling services for the North Andover High School project has not been finalized,the intent is to follow the Owner's concept of utilizing a State OIT-09 Cabling Contractor for the cabling work. Our role in the process will be as it typically is, as the design consultant. Hopefully we will be able to discuss the Owner's desires on the procurement process at our Nov 12th meeting. If we do not have time to do so, we will need to finalize the process at meeting on Nov 19"'or at our following bi-weekly meeting which would be on Nov 2e'. My suggestion is that the Owner solicit pricing based on the specifications and drawings from four or more State Contractors. We should discuss whether there should be one conference/site visit or multiple conference/site visits. Color Selection Submittals - DDP advised that materials Subs requiring color selection shall be submitted as soon as possible so the architect can prepare recommendations for the owner. Flooring Upgrade - DDP advised that it has been decided to pursue the substitution of thin set poured epoxy terrazzo, which will not require a slab depression. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 11/05/02 R06.1.9 Plant Inspection - After no problems were found at the first plant inspection of the gym trusses, a follow up inspection revealed several defects. V&G 11/05/02 C44.3 Stair#1 - Workmanship (welds, rusty iron at clip angles) not good in stair#1. -- 11/19/02 C45. Drainage Revision - SBNE and JTC discussed possibly installing the detention galleys "out of phase", which would benefit both the school operations (increase available parking) and the progress of the work. The following items were noted. Record item. -- 11/19/02 C45.1 Relocations of Galleys - SBNE stated the galleys can be shifted as an administrative charge, not requiring a ConCom hearing. Relocation would reduce existing parking, which is not workable for the school. DDP noted that while the detailing expense is understandable, the hours claimed seem high, and that for the overall project it is DDP's belief that reduction in steel material, fabrication and erection makes up for addition time to redraw. C&I did not agree with DDP assessment. DDP will consider the discussion and issue a formal response. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 11/05/02 R06.1.9 Plant Inspection - After no problems were found at the first plant inspection of the gym trusses, a follow up inspection revealed several defects. The project continues to maintain a pace consistent with the early delivery of the new building. Enclosure of the auditorium, arts/link and cafeteria is in progress. The temporary heating system is scheduled for activation in early December to allow interior work to continue in the above referenced areas along with the Academic building. 7. Construction Issues: a. The mason contractor has expressed interest in working continuously all winter. The G.C. and sub are finalizing an arrangement to cover and heat with propane certain areas of exterior masonry. b. The contractor continues effectively maintain erosion control measures with minimal migration of soil offsite. 2 c. The contractor has acknowledged the rigorous requirement to protect the gypsum sheathing during construction(in and out)from moisture, and has agreed to use a considerably more durable and moisture resistant material (Dens Glass) for the balance of the project. This is a no cost improvement to the project. d. One area of the upper parking lot adjacent to the east wing of the Academic wing will require additional under drainage. Sketches have been prepared by the Engineers and the G.C. is processing. e. Topping off ceremony and the student signing of steel beams is scheduled for the week of 11/18/02. f. A Temporary Closure Wall will be installed at the Connector and a Temp Roof at the Coupla. Heating will be accomplished utilizing two (2) 1,800,000 BTU Babfar natural gas fired forced air heaters, operating 24 hours per day. One each will be located at the east and west Building Faces, with ducting into the 1st floor building hallways. 36 Inch Fans will be setup in the Hallways to provided secondary air circulation as determined necessary to provide a satisfactory Heat Distribution. Hot Air will be returned from the 3rd to the 1st Floor utilizing the Elevator Shaft and a Propeller Ventilation Fan. Building Temperatures and heat distribution will be monitored utilizing local thermometers. ARTS BUILDING AND MECHANICAL PIT: The Mechanical Pit will be sealed with Temporary Closures and the Arts Wing will be isolated and sealed with Temporary Closures incrementally as Construction Progress permits. Heating of the Mechanical Pit will be accomplished utilizing a 450 BTU Babfar natural gas fired forced air heater, operating 24 hours per day. Heating of the Arts Wing will be accomplished utilizing a 900 BTU Babfar natural gas fired forced air heater, operating 24 hours per day. Secondary circulation will be utilized if and as necessary. As you know,we are finishing up the area by area review with North Andover High School personnel. Once the review is completed at our next meeting on November 12th, I will be able to complete the cabling specifications. My target is to have the specifications and drawings complete by November 30,2002. This should give us enough time to have a cabling contractor on-board by mid to end of January,2003. As discussed previously, although the exact process for procuring voice,video and data cabling services for the North Andover High School project has not been finalized, the intent is to follow the Owner's concept of utilizing a State OIT-09 Cabling Contractor for the cabling work. Our role in the process will be as it typically is, as the design consultant. Hopefully we will be able to discuss the Owner's desires on the procurement process at our Nov 121h meeting. If we do not have time to do so,we will need to finalize the process at meeting on Nov 19th or at our following bi-weekly meeting which would be on Nov 26th. My suggestion is that the Owner solicit pricing based on the specifications and drawings from four or more State Contractors. We should discuss whether there should be one conferencelsite visit or multiple conference/site visits. The specifications will include contractor qualifications and requirements,and will require at least five references from the potential contractors. The Owner then can select the contractor based on price, references, and experience. The Owner may wish to interview some or all respondents, and visit any or all reference sites, so we should be prepared to look at schedules.at the next meeting. Reveal at Fascia - Following the meeting, DDP and JTC reviewed conditions at fascia for academic wing. Bass 11/26/02 R02.1.2 HVAC Screen - DDP returned material submittal "not approved" noting steel panels were submitted, rather than aluminum as specified. Shop drawings with calculations have yet to be submitted. DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 11/05/02 R06.1.9 Plant Inspection -After no problems were found at the first plant inspection of the gym trusses, a follow up inspection revealed several defects. All gym trusses will be inspected at the plant, and the auditorium and gym trusses will be further inspected by T&L in the field. DDP 10/08/02 R17. Headroom at Stair#8 -JTC has confirmed that headroom has JTC been reduced at Stair#8 (Boiler Room), noting that the next higher tread falls under the beam. JTC and DDP to develop corrective action. Auditorium Trusses (Post Script) - In response to discussions JTC at 11/19/02 job meeting, DDP contacted Foley & Buhl, who clarified that T&L should do a visual inspection of shop welds of the auditorium trusses. The clerk and JTC will coordinate an expedited inspection by T&L when conditions allow, and most likely on a Saturday. F&B 11/26/02 R24. Masonry Grout - It was discussed that the 28 day grout JTC breaks for the lower north section of the auditorium CMU walls was 80% of structural design strength (1700psi vs. the specified 2000psi). Foley & Buhl feels this is adequate. The seven day breaks for other areas will be compared to this area once they are available from T&L.
