OWNER ISSUES DDP 05/07/02 B06.2 Revised Site Phasing Drawing - JTC provided a site plan showing the proposed water and road work, and the limited parking that will be available in the south lot when the new school opens. DDP will investigate available parking, possible temporary parking, and options for bus/parent drop off routes for review with NASD. DDP to prepare proposal request. -- 10/15/02 1316. Student Tour- It was discussed that 2 to 4 student representatives of the school paper would like to tour the site. The tour will be off hours, and would be guided by the clerk. NASD, COW and JTC to coordinate. JTC 10/18/02 1316.1 Photographs - JTC asked that they be given NASD prior approval of any photos taken of the site during the student tour. 1317. C. PERMITS & INSPECTIONS DDP 12/02/02 C01.3 Health Department (Post Script) - DDP submitted application to the Health Department 12/02/02. DDP will advise as to the date/time of any meeting. 03/26/02 CO3. DPW Permits 03/26/02 C04. North Andover Fire Department Permits Hampshire 10/29/02 C04.5.1 Sprinkler Permit- Hampshire has distributed copies of the sprinkler permit received from the fire department. JTC has been informed that the fire department takes no exception to the ongoing installation in the Academic Wing while Hampshire completes the hydraulic calculations for submission. -- 11/13/02 C04.5.2 Sprinkler Permit (Post Script) - Hydraulic 12/05/02 calculations received 11/13/02 and forwarded to RWS for review. Hampshire to submit calculations for remainder of building. DDP 04/02/02 C12. Environmental Reports - DDP will distribute the weekly and monthly reports prepared by SBNE to Bob Nicetta, NASD, JTC, MBC and the clerk at the weekly job meetings. JTC 08/20/02 C28.1 Stormceptor Substitution - The Conservation CDS Commission has approved the CDS Technologies equipment submitted by JTC. CDS is to submit exact details and shop drawings to the Conservation Commission's outside consultant for review (Postcript- after approval by Conservation Commission reviewer, shop drawing package shall be submitted to SBNE for final approval; initial submittal was marked "revise and resubmit"). JTC forwarded to DDP correspondence from CDS with regard to follow up submissions to con com. DDP forwarded to SBNE for review and comment. DDP 08/20/02 C33. Replication Area - SBNE advised that the replacement Zoppo blueberry plans were not well planted and have not survived. SBNE also advised that other wetland plants are fairing poorly with the dry weather. DDP will review with Zoppo. -- 11/19/02 C43.1 Erosion Controls - SBNE advised that the only 12/03/02 area of concern is at the southwest corner at the sewer line work, where the berm must be reformed. The only other items needing attention are silt boots requiring cleaning. Record item. JTC 12/03/02 C43.1.1 Erosion Controls - SBNE reported that while many items have been addressed, there remain some items that require attention. It was discussed that while the frozen site conditions at present work to stop erosion, JTC should address maintenance items before the spring. C46. D. CONSTRUCTION SCHEDULE -- 10/29/02 D37.1 Technology Cabling - JTC, DDP, and 3Si discussed cabling schedule. JTC advised ceiling grid is scheduled to start in the academic wing mid-February 2003. 3Si would prefer to cable before the grids are installed and will monitor project progress the next few weeks. 12/03/02 D43. Schedule Update - JTC reviewed the look ahead schedule through 12/15/02 (copy attached) with the following noted. Record item. -- 12/03/02 D43.1 Masonry - Mason staging and wrapping the Cafeteria/Arts Wing in preparation for start of brick veneer work next week. Coordination Drawings - JTC reported that work on the Subs coordination drawings has begun, and the process is expected to take 10-12 weeks. JTC 09/24/02 G17. Arts Wing AHU/Steel -JTC noted coordination issues at DDP openings for Rooftop Units. (Post script- Foley and Buhl visited the site 09/25/02. Report forwarded to JTC for discussion). DDP 11/26/02 G20. Roofing Gym AHU's - JTC noted that the solution discussed for flashing the auditorium RTU ductwork will be problematic at AHU-1 and AHU-2 because the ductwork is 96 inches wide. DDP will review and advise. DDP 12/03/02 G20.1 Roofing - JTC stated AHU7 will be set over TV JTC studio the week of 12/09/02. DDP to advise if JTC should raise curb for flashing as at auditorium. JTC noted a roof drain will be under the ductwork. DDP 12/03/02 G21. Coordination Drawings - Drawings CO-2-8, CO-2-SEC, and Consult 5216-1-9 received by DDP 12/05/02. G22. PAYROLL REPORTS -- 11/26/02 H11.1 Payroll Reports - V&G has submitted some 12/03/02 reports to the clerk; the clerk will review and update status. Record item. -- 12/03/02 H11.2 Payroll Reports - The clerk reported that V&G reports are up to date. Record item. H12. I. REQUISITIONS -- 04/30/02 107.1.1 Cash Flow - JTC has furnished DDP with a cash 12/03/02 flow for the first eight months of the project. DDP will add project costs and forward to town. Record item. JTC 11/26/02 117. Pencil Requisition - JTC will prepare pencil requisition for review at 12/03/02 job meeting. -- 12/03/02 117.1 Pencil Requisition - JTC distributed for review and comment on 12/03/02 the pencil requisition (Post Script- DDP returned comments to JTC 12/09/02.) CHANGE ORDERS DDP 08/13/02 J04.1 Steel Change Order Proposals - Cape and Island Steel (C&I) explained their position that they intend to be compensated for their redetailing time resulting from revisions issued by DDP (Field order No. 32, 34 and 41) even though there are no "direct" (material and labor) costs involved with these changes. DDP noted that while the detailing expense is understandable, the hours claimed seem high, and that for the overall project it is DDP's belief that reduction in steel material, fabrication and erection makes up for addition time to redraw. C&I did not agree with DDP assessment. DDP will consider the discussion and issue a formal response. C 1 12/03/02 J04.1.1 Steel Change Order Proposals - JTC will see JTC that C&I submits any outstanding Change Order DDP Proposals for structural steel. JTC requested that C&I meet with JTC and DDP on 12/17/02. DDP 11/12/02 J08.2 Mechanical PCO's -All outstanding mechanical/plumbing have been approved by DDP and work orders have been issued. DDP will include these in the next change order. JTC 11/19/02 J10. PCO Log - JTC distributed a log showing status of Proposed Change Orders as of 11/18/02. JTC will forward to MBC copies of issued PCO's not received, and will forward PCO's for several of the open items. OBSERVATION REPORT NO. 37 North Andover High School Page 7 ABP 11/19/02 J11. Electrical PCO's - It was agreed that Thompson Engineering TE and AB & Palumbo shall attend the 12/03/02 job meeting. The purpose will be for TE and AB&P to review the pencil requisition, as well as for TE and AB&P to discuss electrical change order review and approval issues with JTC, DDP and MBC. Record item. -- 12/03/02 J11.1 Electrical PCO's - JTC, MBC, DDP and TE met with ABP to discuss Electrical PCO's. The following was discussed. ABP 12/03/02 J11.1.1 Mark-ups - JTC to confirm mark-ups of hourly wages and bond costs. -- 12/03/02 J11.1.2 Language Lab PCO's (W.O. #07) - ABP to formally submitt revised cost proposal in the (approximate) amount of$23,000.00. TE 12/03/02 J11.1.3 HVAC Power PCO (W.O. #19) - TE to formally ABP respond to PCO, noting credits offered are unbalanced in comparison to chanrges for additional work. Credit for starters discussed. ABP to submit for consideration wiring of three pieces of HVAC coordination items which may be a `wash' with credit for starters. TE 12/03/02 J11.1.4 Panel CP 1-9 PCO (PCO No. 34) - TE will respond to ABP claim, TE noted while the feeds got longer as a result of the relocation of the panel, all the branch circuits got shorter. ABP 12/03/02 J11.1.5 Credits - ABP to submit credit proposals for exterior light fixture revisions (W.O. #16), telephone man hole deletion (W.O. #11), service change (F.O. #28). -- 12/03/02 J11.1.6 Future Cost Proposals - DDP and MBC stated that if ABP cost proposals for additional work continue to be unjustifiably high, DDP will issue unilateral cost work orders in order to move the work forward. Work Order No. 16 (postscript) - WO No. 16 deleting light fixtures (credit) issued on 09/06/02. K08. OBSERVATION REPORT NO. 37 North Andover High School Page 8 L. FIELD ORDERS -- 11/26/02 L16. Field Order No. 101 - Following the meeting, DDP explained 12/03/02 F.O. No. 101 to JTC and the mason, PCM, noting that the joist seat dimensions caused DDP to lower the top of the window opening at the top of the brick one inch at cafeteria. DDP stated that it is visually preferable to cut the top course of brick one inch to maintain a consistent fascia. PCM to review with their office. Record item. -- 12/03/02 L16.1 Field Order#101 - JTC reported that PCM will be cutting down the top brick. record item. L17. M. PROPOSAL REQUESTS JTC 05/07/02 M04. Proposal Request No. 1 - DDP issued PR No.1 for revisions to site utilities resulting from site utiltiy coordination by RWS. ABP' 06/04/02 M10. Proposal Request No. 3 - DDP noted PR No. 3, for electrical power to exhaust fan EF-60 (added under PR No. 4) was issued 05/07/02. JTC 11/19/02 M15. Proposal Request No. 12 - DDP has forwarded to JTC for pricing P.R. No. 12, which substitutes painted GWB ceilings for lay-in tile ceilings in student accessed toilet rooms (not staff toilet rooms). In discussing with JTC, DDP advised that the cost for access panels needed by the respective trades should be included. JTC 11/19/02 M16. Proposal Request No. 10 - DDP has forwarded to JTC for pricing P.R. No. 10 for additional gym "drop curtains" requested by the athletic department to completely separate the wood basketball courts from the track. JTC 12/02/02 M17. Proposal Request No. 15 (Post Script) - DDP forwarded to JTC for pricing P.R. No. 15 for revising condensate drainage for arts wing units AC-2 and AC-3. JTC 12/02/02 M18. Proposal Request No. 16 (Post Script) - DDP forwarded to JTC for pricing P.R. No. 16 for revisions to Trainer's Room 1141 requested by athletic department. M19. N. REQUEST FOR INFORMATION DDP 09/24/02 N29.1 RFI #153 - DDP stated third floor slab projection at upper end of Main Street will not be cut back (will remain flush with brick). DDP will formally respond to RFI #153 once architectural details are revised. -- 12/03/02 N38. RFI Log - DDP distributed log showing status as of 12/03/02. No critical BFI's outstanding. Record item. Building/Abatement Demolition is scheduled for August 2004 and prerequisites will be reviewed as that date approaches. NAFD 03/26/02 001 .1 Public safety training will take place in the vacated NAPD existing high school for two weeks after the school is vacated by the owner. 002. P. SITE WORK DDP 10/15/02 P11.2 Art Wing Storm Drainage - JTC advised that it would be significantly less work to core manhole 2B for piping entering from the southwest then to relocate manhole 2A. DDP to issue directive. JTC 10/22/02 P17. Road Subsoils - The clerk reported that a strip of the recently paved east driveway is exhibiting subgrade failure, and will need to be repaired. P18. Q. ARCHITECTURAL JTC 03/26/02 Q01 . Color Selection Submittals - DDP advised that materials Subs requiring color selection shall be submitted as soon as possible so the architect can prepare recommendations for the owner. DDP 10/08/02 Q01.3 Color Selection - JTC noted that, with drywall to begin in three weeks, ceramic tile colors will be needed soon thereafter. JTC 11/19/02 Q20. Sample Classroom - DDP and JTC discussed and agreed that DDP it would be beneficial to complete one east wing classroom as a `sample' for workmanship, etc. DDP and JTC will further discuss as work progresses. -- 11/19/02 Q21. Toilet Room Doorframes - Clerk advised that door frame 12/09/02 throats are not deep enough for plywood underlayment and wall board. U.S. Drywall proposes to install plywood flush with and between studs. DDP requested USD install at a sample door frame. Record item. -- 12/09/02 Q21.1 Toilet Room Door Frames - Sample approved 12/09/02. Record item. -- 11/19/02 Q22. Auditorium Masonry - MBC advised JTC that cavity at top of 12/03/02 brick veneer should be protected from water entering and freezing. Record item. -- 12/03/02 Q22.1 Auditorium Masonry - Top of cavity covered. Record item. OBSERVATION REPORT NO. 37 North Andover High School Page 10 JTC 11/26/02 Q23. Access Panel Review - MBC stated that access panel Clerk placement and size must be carefully coordinated so that Subs required access to above-ceiling items is achieved. DDP noted that this will be critical at hard toilet room ceilings (Proposal Request No. 12). The clerk will review ceiling openings as they are framed. Q26. R. STRUCTURAL JTC 05/21/02 R02.1.1 HVAC Screen - JTC, after review with metal panel sub and C&I, reported that panel will span verticals without the second TS 5X5 girt. DDP and F&B take no exception, and requested submittal of detail proposed. Bass 11/26/02 R02.1.2 HVAC Screen - DDP returned material submittal "not approved" noting steel panels were submitted, rather than aluminum as specified. Shop drawings with calculations have yet to be submitted. DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. T&L 11/05/02 R06.1.9 Plant Inspection -After no problems were found at the first plant inspection of the gym trusses, a follow up inspection revealed several defects. All gym trusses will be inspected at the plant, and the auditorium and gym trusses will be further inspected by T&L in the field. DDP 10/08/02 R17. Headroom at Stair#8 -JTC has confirmed that headroom has JTC been reduced at Stair#8 (Boiler Room), noting that the next higher tread falls under the beam. JTC and DDP to develop corrective action. JTC 11/05/02 R17.1 Headroom at Stair No. 8 - Following the meeting, V&G JTC and DDP inspected conditions in field. It apears that the stair has been shifted south 3-4 inches, which resulted in headroom problem. DDP also noted discrepancy at upper landing. JTC to review with V&G. V&G 11/12/02 R17.1.1 Headroom at Stair#8 - JTC advises that stair can be shifted to the north enough to provide the required headroom. OBSERVATION REPORT NO. 37 North Andover High School Page 11 NABC 11/12/02 R17.1.2 Stair#8 Top Landing - V&G has not constructed the top landing with the required pull side door clearance. Bob Nicetta will observe conditions, given that this door is at the top of a stair. V&G 10/22/02 R18.1 Stair Structural Meeting - JTC, DDP, F&B, and V&G met 10/22/02 to review structural stair issues. (F&B memo of meeting has been distributed). V&G will resubmit shop drawings and calculations. V&G 11/19/02 R18.1.2 Stairs (Post Script) - Revised calculations returned to JTC 11/19/02. Submission of shop drawings revised to reflect V&G's engineer's calculations must be submitted. V&G 12/03/02 R18.1.3 Meeting (Post Script) - JTC, DDP, F&B, and F&B V&G to meet 9:00a.m. Tuesday 12/10/02. V&G JTC will have revised shop drawings and calculations. DDP JTC 11/19/02 R22. Auditorium Ceilings (Post Script) - In response to questions posed by JTC at the 11/19/02 job meeting, DDP advised JTC that layout drawings and engineering calculations for suspended stud auditorium ceilings shall be submitted for review (see 09250-1.05.13). Clerk 11/20/02 R23. Auditorium Trusses (Post Script) - In response to discussions JTC at 11/19/02 job meeting, DDP contacted Foley & Buhl, who clarified that T&L should do a visual inspection of shop welds of the auditorium trusses. The clerk and JTC will coordinate an expedited inspection by T&L when conditions allow, and most likely on a Saturday. -- 11/26/02 R24. Masonry Grout - It was discussed that the 28 day grout 12/03/02 breaks for the lower north section of the auditorium CMU walls was 80% of structural design strength (1700psi vs. the specified 2000psi). Foley & Buhl feels this is adequate. The seven day breaks for other areas will be compared to this area once they are available from T&L. JTC will have the supplier review and respond to the low test results. Record item. -- 12/03/02 R24.1 Masonry Grout - Subsequent grout breaks have all been in excess of design strength. No further action necessary. Record item. DDP 12/03/02 R25. Expansion Joint - It was noted by JTC that C&I had F&B mistakenly added a steel plate along D7 line, closing the gap at the auditorium wing expansion joint. JTC asked if this is a problem because it will be difficult to cut the plate off. DDP will review with F&B. R26. S. FIRE PROTECTION S08. OBSERVATION REPORT NO. 37 North Andover High School Page 12 T. PLUMBING -- 12/03/02 T09. Gas Service - JTC noted gas will be turned on 12/06/02. Record item. T10. U. HVAC -- 11/12/02 U06. Insulation - JTC advised that ductwork must be insulated 12/03/02 where tight to beam flanges, with particular care to be taken at rated chases. Record item. -- 12/03/02 006.1 Insulation - The clerk reported that Sagamore is insulating between beam flanges and the top of ductwork. Record item. JTC 11/19/02 U08. Fan Coil Covers - Fan coil units have been delivered to the SP&H site in advance of receiving color selection from DDP. Units do not match selected color or selected color for fin tube radiation. JTC to review with Sagamore. U09. V. ELECTRICAL MEC 06/11/02 VO4.3.3 New Service Pole - MEC will set the new pole in approximately two weeks. MEC 10/08/02 VO4.4.1 New Service (Post script) - The Building Committee approved direct payment in full of MEC backcharge at 10/10/02 meeting. MEC expected to install service once payment received. JTC 10/22/02 VO4.4.2 New Service - After MEC sign-offs on trench, etc. MEC on 10/23/02, service installation will start in two weeks, and be completed two weeks thereafter. JTCwill protect transformers as required by MEC. ABP 07/15/02 V07.1.1 Communications Manhole (Postscript) - AT&T JTC advised that manhole can be deleted. W.O. No. 11 (credit) issued 07/15/02. ABP 08/06/02 V07.2.4 Verizon Pole - Verizon representative was Verizon present at 08/06/02 job meeting. Verizon's position remains that ABP staff staked the location for pole on site. Pole must be relocated to right of way, as shown on the drawings. ABP will re-stake the location for Verizon, and will run conduits up to where pole will be relocated. After obtaining the right of way agreement, Verizon will reset the pole. The purpose of the Environmental /Erosion Control Monitor is to perform periodic inspections of the project site to insure compliance with the Order of Conditions issued by the Conservation Commission and the Special Permit issued by the Planning Board with respect to sediment and erosion controls, limit of work compliance, wetland replication and stormwater management issues. This is the fifteenth monthly inspection report prepared and submitted in compliance with the above referenced permits. It covers the period October 23, 2002 through November 19, 2002. Attached are copies of the Inspection Reports (#51, #52, #53, and 954) relative to the inspections performed on October 29, November 5, November 13, and November 19, 2002, respectively. During this timeframe, the site experienced several rainfall events and three of the four inspections followed a significant amount of rain. Muddy conditions followed each rainfall event and the sedimentation controls were observed to be functioning properly. The cloudy discharge at the two drainage outfalls observed in the past is being successfully minimized by management of the site and maintenance of the erosion controls. The southeast work area seemed to experience the largest amount of erosion. The perimeter sedimentation controls and catch basin protection worked well to minimize the amount of sediment collecting on the parking lot and getting into the catch basins with filter fabric silt boots. During the installation of the retaining wall in this area, effluent from a dewatering system was observed to be too cloudy and was shut down. No other installation work is scheduled in this area during the winter. Additional haybale checkdams are planned for this area to slow runoff and to reduce erosion through the winter months. ING • PLANNING 20217 MEN= North Andover High School— Monthly Inspection Report—October 23, 2002 through November 19, 2002 Page 2 The seepage inlet was installed at the east embankment to control the seepage coming from the"blowout" area observed earlier in the year. It appears to be functioning properly. A sewer connection and manhole were installed at the southwest and required a short-term disruption outside the perimeter controls. A combination of haybales, silt fence, catch basin protection, and mulch have been installed to stabilize the area. We have recommended that jute netting be applied to the disturbed area at the shoulder of Chickering Road to hold the mulch in place through the winter months. There were no major upsets to the controls and only the typical maintenance requirements were necessary. This included sediment removal from the parking lot next to the southeast work area and silt boot replacement. The stone construction entrance was maintained in reasonably good condition given the amount of rain and muddy conditions. Dirt and mud tracked onto Prescott Street was kept to minimum. Additional observations included: • The sedimentation basins remained in good condition and appeared to be functioning properly. There were visible indications that runoff was flowing towards them. • Maintenance to the swale and berm at the southwest corner was completed to make sure that runoff from that area travels toward Sedimentation Basin #2. • The area where the building subdrain exits the retaining wall remained in good condition. • The southeast entrance gate and the catch basin remained in good condition and there were no erosion issues. • Items carried forward: • Jute netting needs to be installed at the disturbed area where the sewer connection was completed. • In the southeast corner, haybale checkdams should be placed along the channel where stormwater runoff exits the drainage pipe. • The haybales separating the stockpile from Sedimentation Basin 93 should be replaced. • It is now the time for the excess sediment in Sedimentation Basin 92 to be removed. • The stockpiled materials in the small secondary sedimentation area immediately south of Sedimentation Basin #3 should be removed. Based on our inspections, we have found that the work on the project has been performed in compliance with the Special Permit and Order of Conditions. This inspection followed significant rainfall over the previous weekend. Conditions internal to the site were muddy but the erosion and sedimentation controls were functioning properly and no adverse conditions to the resource areas were observed. Cloudy water discharging from the outfalls was minimal and Cochichewick Brook was relatively clear. The installation of a Sewer Connection and manhole at the southwest corner of the work area at Chickering Road was underway. This work is a short-term alteration that is beyond the limit of the erosion controls and will need to be stabilized immediately upon completion of the installation. Sediment control barriers around the perimeter of the current work area: • 10/29/02 —Southeast work area: Sediment needs to be removed from the parking lot adjacent to the southeast work area and the two silt boots need to be cleaned or replaced. Southwest work area: This area was in good condition. • 11/05/02 - Southeast work area: Items mentioned above were carried forward. The retaining wall installation is complete. Other than stabilizing this area for the winter, no other work is scheduled. Southwest work area: This area was in good condition. The silt boot at the corner of the parking lot needs to be cleaned or replaced. Haybale checkdams should be placed along the channel where stormwater runoff exits a drainage pipe and travels towards the perimeter sediment barrier where it passes through to the parking lot. Southwest work area: The installation of the sewer manhole and connection to Chickering Road was underway during the inspection. It is our understanding that this is a short-term alteration that is beyond the limit of the perimeter erosion controls and required the temporary removal of the perimeter haybales and silt fence. Immediately upon completion of the installation, the disturbed area at the shoulder of Chickering Road must be stabilized and the perimeter silt barrier must be re-set. Also, the swale/berm that diverts the site runoff to Sediment Basin #2 must be re-established. For stabilization of the disturbed area beyond the perimeter erosion controls, the contractor stated that they would apply straw mulch. Covering the mulch with a jute netting will be needed to hold the mulch in place against the wind, and from melting snow banks, which tend to wash away the mulch and erode the surface. Also, snow banks will make it difficult to maintain the mulch through the winter. 2. Catch basins: • 10/29/02 —The two silt boots covering the catch basins in the parking lot near the southeast work area should be cleaned or replaced (mentioned in Item #1). Sample Classroom - DDP and JTC discussed and agreed that DDP it would be beneficial to complete one east wing classroom as a `sample'for workmanship, etc. DDP and JTC will further discuss as work progresses. Access Panel Review - MBC stated that access panel Clerk placement and size must be carefully coordinated so that Subs required access to above-ceiling items is achieved. The clerk will review ceiling openings as they are framed. 12/10/02 Q23.1 Access Panels/Toilet Rooms -JTC advised that there will need to be access panels to reach junction boxes in toilet rooms (PR#12) which were installed before proposed ceiling change. Record item. Q26. R. STRUCTURAL JTC 05/21/02 R02.1.1 HVAC Screen - JTC, after review with metal panel sub and C&I, reported that panel will span verticals without the second TS 5X5 girt. DDP and F&B take no exception, and requested submittal of detail proposed. Bass 11/26/02 802.1.2 HVAC Screen - DDP returned material submittal "not approved" noting steel panels were submitted, rather than aluminum as specified. Shop drawings with calculations have yet to be submitted. DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. -- 11/05/02 R06.1.9 Plant Inspection - After no problems were found 12/10/02 at the first plant inspection of the gym trusses, a follow up inspection revealed several defects. All gym trusses will be inspected at the plant, and the auditorium and gym trusses will be further inspected by T&L in the field. Record item. 10/08/02 R17. Headroom at Stair#8 - JTC has confirmed that headroom has 12/10/02 been reduced at Stair#8 (Boiler Room), noting that the next higher tread falls under the beam. Auditorium Trusses (Post Script) - In response to discussions 12/10/02 at 11/19/02 job meeting, DDP contacted Foley & Buhl, who clarified that T&L should do a visual inspection of shop welds of the auditorium trusses. The clerk and JTC will coordinate an expedited inspection by T&L when conditions allow, and most likely on a Saturday. Record item. After obtaining the right of way agreement, Verizon will reset the pole. The project continues to maintain a pace consistent with the early delivery of the new building. Enclosure of the auditorium, arts/link and cafeteria is in progress. The temporary�eating system is scheduled for activation in early December to allow interior work to continue in the above referenced areas along with the Academic building. 7. Construction Issues: a. The mason contractor and General Contractor have agreed to mutually acceptable terms to continue masonry operations under winter protection procedures. b. The contractor continues effectively maintain erosion control measures with minimal migration of soil offsite. 2 c. One area of the upper parking lot adjacent to the east wing of the Academic wing will require additional under drainage. Sketches have been prepared by the Engineers and the G.C. is processing. d. Topping off ceremony and the student signing of steel beams was held the week of 11/18/02. e. Technology cabling and telecommunications systems are currently being designed by SSS Inc. and will be procured through the State Bid List Program. Heating will be accomplished utilizing two (2) 1,800,000 BTU Babfar natural gas fired forced air heaters, operating 24 hours per day. One each will be located at the east and west Building Faces, with ducting into the I"floor building hallways. 36 Inch Fans will be setup in the Hallways to provided secondary air circulation as determined necessary to provide a satisfactory Heat Distribution. Hot Air will be returned from the 3rd to the 1"Floor utilizing the Elevator Shaft and a Propeller Ventilation Fan. Building Temperatures and heat distribution will be monitored utilizing local thennometers. ARTS BUILDING AND MECHANICAL PIT: The Mechanical Pit will be sealed with Temporary Closures and the Arts Wing will be isolated and sealed with Temporary Closures incrementally as Construction Progress permits. Heating of the Mechanical Pit will be accomplished utilizing a 450 BTU Babfar natural gas fired forced air heater, operating 24 hours per day. Heating of the Arts Wing will be accomplished utilizing a 900 BTU Babfar natural gas fired forced air heater, operating 24 hours per day. Secondary circulation will be utilized if and as necessary. Town of North Andover Dear Ms. MacEvitt: After our recent phone conversation regarding the above referenced project, we forwarded to OSD a copy of our 22 November 2002 submission to the Attorney General's office in regard to VOX Communication's protest of the applicability of OSD statewide contracts ITC 09 and ITT 11. While the AG's office has yet to formally respond, they have forwarded to Urbelis & Fieldsteel, town counsel for North Andover, a copy of a 23 September 1997 AG decision in the matter of data and video system cabling for the Dedham Middle School. This matter is similar to that of our North Andover project, in that a bidder protested the use of the "State Blanket Contract" procured by the Bureau of Information Technology Acquisitions of the Department of Procurement and General Services. In the Dedham decision, the Attorney General found that technology wiring must be bid pursuant to.MGL 149, not procured via the "Blanket Contract" in force at that time. This decision brings into question the very existence of the "Blanket Contract" (and by extension, ITC 09 and ITT 11). The reasoning being: • Cable "installation" constitutes construction work subject to MGL c149. • All construction work having an estimated cost of more than $25,000.00 must be bid subject to MGL c149. • For contracts less than $25,000, quotes may be solicited from any three contractors in accordance with Section 30B of the Uniform Procurement Act. Therefore, noting that there is no differentiation in the law between public schools (which happen to be the building type for which the referenced protests have been filed) and any other public building, there would seem to be no practical and/or legal application of ITC 09 and ITT 11. DDP noted that while the detailing expense is understandable, the hours claimed seem high, and that for the overall project it is DDP's belief that reduction in steel material, fabrication and erection makes up for addition time to redraw. SITE WORK DDP 10/15/02 P11.2 Art Wing Storm Drainage - JTC advised that it would be significantly less work to core manhole 2B for piping entering from the southwest then to relocate manhole 2A. DDP to issue directive. JTC 10/22/02 P17. Road Subsoils - The clerk reported that a strip of the recently paved east driveway is exhibiting subgrade failure, and will need to be repaired. P18. Q. Color Selection Submittals - DDP advised that materials Subs requiring color selection shall be submitted as soon as possible so the architect can prepare recommendations for the owner. DDP 12/10/02 Q01.4 Color Selection - DDP has forwarded ceramic tile colors. JTC has requested prime/paint colors and vinyl flooring colors for academic wing. JTC 11/19/02 Q20. Sample Classroom - DDP and JTC discussed and agreed that DDP it would be beneficial to complete one east wing classroom as a `sample' for workmanship, etc. DDP and JTC will further discuss as work progresses. DDP 12/17/02 Q20.1 Sample Classroom - JTC advised DDP that color selection is needed for marker board, tack board, floor tile and wall paint in order to prepare a sample classroom. JTC 11/26/02 Q23. Access Panel Review - MBC stated that access panel Clerk placement and size must be carefully coordinated so that Subs required access to above-ceiling items is achieved. DDP noted that this will be critical at hard toilet room ceilings (Proposal Request No. 12). Issues include placement of frame for roof drain that necessitates leader passing under a beam, sprinkler lines not installed tight to structure, and at Stair 4, revised roof drain connection to conductor that is lower than 7'-6" ceiling height. DDP will review and advise. -- 12/30/02 Q27. Library Tube Girt - Following the meeting, DDP observed conditions at the tube steel girt bracing the tall central curtainwall. The east welded connection is visibly higher than the west connection, and the girt is bent near its center. DDP will consider cladding the girt and will advise. Q28. R. STRUCTURAL JTC 05/21/02 R02.1.1 HVAC Screen - JTC, after review with metal panel sub and C&I, reported that panel will span verticals without the second TS 5X5 girt. DDP and F&B take no exception, and requested submittal of detail proposed. Bass 11/26/02 R02.1.2 HVAC Screen - DDP returned material submittal "not approved" noting steel panels were submitted, rather than aluminum as specified. Shop drawings with calculations have yet to be submitted. DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP F&B with regard to having a lift point on the roof for bringing up items that are too heavy or too large to go up the roof hatch. DDP noted that if one point was designated to lift a load of a few thousand pounds (i.e. HVAC equipment), the entire roof would need to be designed for that live load. DDP will investigate means of lifting items like a snow blower to the roof. NABC 11/12/02 R17.1.2 Stair#8 Top Landing - V&G has not constructed the top landing with the required pull side door clearance. Bob Nicetta will observe conditions, given that this door is at the top of a stair. JTC 11/19/02 R22. Auditorium Ceilings (Post Script) - In response to questions posed by JTC at the 11/19/02 job meeting, DDP advised JTC that layout drawings and engineering calculations for suspended stud auditorium ceilings shall be submitted for review (see 09250-1.05.13), C&I 12/10/02 R26. Damaged Roof Deck - Following the meeting, JTC was advised that Main Street roof deck is exposed to view and damaged sections must be replaced/repaired.
