1. Permits:
Mass. Highway permit for work on Route 125 has been received.
Building permit has been received.
DPW utility permits have been received and utility work is in progress.
A water ban waiver has been obtained from the North Andover DPW.
Health Department review of the kitchen is in progress with all required
information supplied.
2. Project Meetings:
Construction meetings are being held weekly at 1:00 p.m. on Tuesdays and are
being conducted in a professional and co-operative atmosphere.
Issues emphasized at recent meetings:
Enclosure of the Cafeteria, Art/Tech Ed, Auditorium and link to
maintain expedited schedule (see sketch). ( In Progress)
Ramping up the number of subcontractors and trades as additional
areas of the building become available. ( In Progress )
Activation of the Academic temporary heating system to allow
weather susceptible activity to commence. (Complete)
3. Work in Progress:
Work force is up to approximately 85 workers

in all previously noted areas.
5. Change Proposals:
Four change orders have been processed. Other proposed change orders (PCO's)

continue to be solicited and reviewed. (See attached change o log).
6. Schedule:
Phase I—new school 7/15/04
Phase II—Demo and site 5/5/05
Contractor has submitted the required updated schedule dated 12/02.
The project continues to maintain a pace consistent with the early delivery of the
new building. Enclosure of the auditorium, arts/link and cafeteria is in progress.
The temporary heating system has been activated to allow interior work to
continue in the above referenced areas along with the Academic building.
7. Construction Issues:
a. The mason contractor and General Contractor have agreed to mutually
acceptable terms to continue masonry operations under winter protection
procedures.
b. The contractor continues effectively maintain erosion control measures with
minimal migration of soil offsite.

C. One area of the upper parking lot adjacent to the east wing of the Academic
wing will require additional under drainage. Sketches have been prepared by
the Engineers and the G.C. is processing.
d. Topping in off ceremony and the student signing of steel beams was held the
week of 11/1.8/02.
e. Technology cabling and telecommunications systems are currently being
designed by SSS Inc. and will be procured through
ate Bid VOXList
Program. Recent correspondence addressing the protest
communication is attached.

Winter Heating Plan
ACADEMIC BUILDING:
The Building is currently 100%weather tight with the completion of the Curtain Wall
Work and installation of Clear Story Windows at the beginning of December. A
Temporary Closure Wall will be installed at the Connector and a Temp Roof at the
Coupla.
Heating will be accomplished utilizing two (2) 1,800,000 BTU Babfar natural gas
fired forced air heaters, operating 24 hours per day. One each will be located at the east
and west Building Faces, with ducting into the lst floor building hallways. 36 Inch Fans
will be setup in the Hallways to provided secondary air circulation as determined
necessary to provide a satisfactory Heat Distribution. Hot Air will be returned from the
3`d to the 1St Floor utilizing the Elevator Shaft and a Propeller Ventilation Fan.

Building Temperatures and heat distribution will be monitored utilizing local
thermometers.
ARTS BUILDING AND MECHANICAL PIT:
The Mechanical Pit will be sealed with Temporary Closures and the Arts Wing will
be isolated and sealed with Temporary Closures incrementally as Construction Progress
permits.
Heating of the Mechanical Pit will be accomplished utilizing a 450 BTU Babfar
natural Ras fired forced air heater, operating 24 hours per day. Heating of the Arts Wing
will be accomplished utilizing a 900 BTU Babfar natural gas fired forced air heater,
operating 24 hours per day. Secondary circulation will be utilized if and as necessary.
Building Temperatures will not be of concern until; and if Finish «'ork is initiated
after Structure and Rough In Activities.
Mason`- Construction:
Temporary Enclosure and Heat will be provided per the Specifications and Local
Codes with Propane Gas and small Forced Air Heaters. Propane Gas will not be utilized
for the Building Heating.

NASD 10/15/02 B12.1.1 Keying (Post script) - Keying schedule based on

Environmental /Erosion Control Monitor is to perform periodic inspections of the project
site to insure compliance with the Order of Conditions issued by the Conservation
Commission and the Special Permit issued by the Planning Board with respect to sediment
and erosion controls, limit of work compliance, wetland replication and stormwater
management issues.
This is the sixteenth monthly inspection report prepared and submitted in compliance with
the above referenced permits. It covers the period November 20, 2002 through December
20, 2002. Attached are copies of the Inspection Reports (455, 456, 957, and #58) relative
to the inspections performed on November 26, December 3, December 10, and December
17, and a follow-up inspection on December 20, 2002, respectively.
Winter conditions were experienced during this time period. Two of the inspections were
performed when the site was blanketed with snow. Due to the frozen conditions, erosion
was kept to a minimum. There was a significant rainfall event that occurred Saturday 12/14,
which created a significant amount of runoff, especially at the southeastern work area where
a combination of rainfall, snowmelt, and stormwater exiting a drainage pipe overwhelmed
the area. Runoff ponded in the school parking lot in the immediate area and the finer
particles contained in the runoff passed through the filter fabric protected catch basins and
created a dirty, yellowish-brown discharge at the outfall. Some of the outstanding items
carried forward from previous inspections were completed immediately at Bob Nicetta's
request. A follow-up inspection was completed on 12/20 during a rainfall event to review
the status of repairs and adjustments to the erosion and sedimentation controls. The controls
that were recently repaired and added were fi>nctioning properly.

The following is a list of outstanding items and new items completed.
Southeast work area:
• Stone and haybale checkdams were installed along the drainage path where stormwater nmoff
exits the drainage pipe.
• Haybales were installed along the outside of the security fence for additional filtering protection.
• Silt boots were cleaned/replaced at the catch basins in the parking lot.
• A silt fence was installed at the south and east sides of the stockpile.
Southwest work area:
• Steel mesh was installed at the shoulder of Chickering Road to hold the straw mulch in place.
• Haybales were added to the"reset" silt fence to hold the bottom of the silt fence to the ground.
• Silt fence was added to the haybales at the southwest parking lot fence to contain the
sedimentation.
Sediment Basin #2:
• Sediment was removed from the swale leading to Sediment Basin 42 and the berm along the side
of the swale was re-established.
• Excess sediment was removed from Sediment Basin 42.
Sediment Basin #3:
Stockpiled materials in the small secondary, sedimentation area immediately south of the basin
were consolidated.
The entrance to the trap rock chute was re-established.
The berm ruining across the haul road was re-established.
Repairs that were underway during the 12/20 follow-up inspection:
Installation of silt fence barrier between Sediment Basin 93 and the stockpile.
Sediment removal from the parking lot at the southeast work area was scheduled for the
following week when school was out of session.
Based on our inspections, we have found that the work on the project has been performed in
compliance with the Special Permit and Order of Conditions. Please let us know if there are

Southwest work area: The installation of the sewer manhole and connection to
Chickering Road was underway during the inspection. It is our understanding that
this is a short-term alteration that is beyond the limit of the perimeter erosion controls
and required the temporary removal of the perimeter haybales and silt fence.
Immediately upon completion of the installation, the disturbed area at the
shoulder of Chickering Road must be stabilized and the perimeter silt barrier
must be re-set. Also, the swale/berm that diverts the site runoff to Sediment
Basin #2 must be re-established. For stabilization of the disturbed area beyond
the perimeter erosion controls, the contractor stated that they would apply straw
mulch. Covering the mulch with a jute netting will be needed to hold the mulch to wash
in place against the wind, and from f meltAlsosnsno bbanksWVill make it difficult to

Southwest work area: A silt fence was installed along Chickering Road and is
reinforced with haybales at two locations. The catch basin, located at the edge of
pavement, was protected with haybales. At the perimeter barrier, the silt fence was
re-set but the bottom of the silt fence needs to be secured into the ground.
Although straw mulch has been added to the shoulder of Chickering road, jute
netting should be installed. Some erosion has occurred and the sediment has
collected on the shoulder of the existing high school parking lot. Haybales were
installed to prevent any additional sedimentation in that area. The berm and swale

During the 12/17 inspection, the contractor was working on the outstanding items and requests
from previous inspections. The water at the two outfalls to Cochichewick Brook was clear. There
was a thick covering of sediment on portions of the southeast parking lot. An inspection was also
conducted with Bob Nicetta and Richard Warner (Clerk of the Works) following the regularly
scheduled construction meeting to review what was observed on Saturday and what repairs were
requested. Outstanding items and requests were reviewed with the site contractor. The site
contractor requested that a follow-up inspection be performed on Friday, 12/20 to view the
progress made on the outstanding items.
The follow-up inspection was performed 12/20 during a rainfall event and there were muddy
conditions on site. The brook running along the east side was clear. The two outfalls were
discharging a relatively small amount of cloudy water. The contractor was making good progress
towards completing repairs and adjustments to the erosion and sedimentation controls.

curtainwall. The east welded connection is visibly higher than
the west connection, and the girt is bent near its center. DDP

F&B take no exception, and requested submittal

"not approved" noting steel panels were
submitted, rather than aluminum as specified.
Shop drawings with calculations have yet to be
submitted.

submitting a letter stated screen panel will meet

DDP 04/30/02 R04. Roof Rigging - NASD noted previous conversation with DDP
F&B with regard to having a lift point on the roof for bringing up
items that are too heavy or too large to go up the roof hatch.
DDP noted that if one point was designated to lift a load of a
few thousand pounds (i.e. HVAC equipment), the entire roof
would need to be designed for that live load. DDP will
investigate means of lifting items like a snow blower to the roof.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:

Several inches of snow blanketed the site during this inspection. Repairs to the erosion and
sedimentation controls that were underway on 12/20/02 appeared to have been completed. There
were no observed runoff conditions and there were no new issues.

1. Sediment control barriers around the perimeter of the current work area:
• 12/20/02 —The outstanding items listed above were completed except "Sediment
needs to be removed from parking area and catch basins adjacent to the work area",
which was scheduled for 12/23 when school was out of session and the parking lot
would be empty. Some notes regarding the above repairs: wire mesh was installed at
the shoulder of Chickering Road to hold the straw mulch in place, haybales were
added to the"reset" silt fence to hold the bottom of the silt fence to the ground, silt
fence was added to the haybales at the southwest parking lot fence to contain
sedimentation. In addition, in the southeast work area, silt fence was installed at the
west side and south side of the stockpiled materials in the area. The site foreman was
informed that the silt fence needs to be "toed" into the ground and needs to be
extended along the east side of the pile.
• 12/30/02 — Because of the snow banks, it could not be determined if the sediment was
removed from the parking area. However, the exposed pavement area was in good
condition.
2. Loam stockpile area in the northern portion of the site:
• 12/20/02 —The site foreman was informed that a portion of the silt fence was lying
on the ground at the north end of the stockpile area and needs to be repaired.
• 12/30/02—A portion of the silt fence was lying on the ground at the north end of
the stockpile area and needs to be repaired.
3. Ordinary borrow stockpile area in the northern portion of the site:
• 12/20/02 —The repairs to the silt fence were underway during the inspection.
• 12/30/02 —The repairs to the silt fence were completed.
4. Main entrance gate near trailers:
• 12/20/02 —The site foreman was informed that the stone entrance needs maintenance.
• 12/30/02 —The constriction entrance was in good condition.
5. Southern emergency entrance:
• 12/20/02 —The area was in good condition.
• 12/30/02 —The area was in good condition.
6. Catch basins:
• 12/20/02 — Several catch basin silt boots were replaced.

stockpiled materials in the secondary basin were consolidated to the side of the basin.
The berm was completed. Sediment Basin 92 was cleaned of excess sediment and the
drainage swale leading to it was cleaned and reformed. The Swale running along the
south, leading to both basins, was cleaned in several areas.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to
compliance with the Orders of Conditions issued by the North Andover- Conservation
Commission and the Special Permit issued by the North Andover Planning Board with respect to
sediment and erosion controls, limit of work compliance, and stormwater management issues.
For reference, the Orders of Conditions issued for the project include:
• DEP File No. 242-1045: Issued 2/21/01
• Planning Board Notice of Decision: Issued 5/14/01
Please contact Bert Corey or Fred King at Schofield Brothers of New England, Inc. if you have
any questions regarding the information contained in this report.
The site was not inspected the previous week, week ending 01/25/03, because of the frozen and
unchanged conditions, which was confirmed with Bob Nicetta.
The site continues to remain in stable condition. Snow and snow banks cover the sedimentation
and erosion controls on the site and around the perimeter. No runoff channels were observed.
There were no new issues.
