As they evaluated the project they felt it was best to modify the clearing and grubbing phase of the project as follows. Arco proposes to clear the site in one phase with concurrent on-site stump grinding. In doing so the resultant stump grindings yield a bark mulch product which will serve to stabilize the site prior to any earth removal. The mulch will also be used as needed in conjunction with the construction of check dams and sedimentation basins and will aide with erosion control under severe weather conditions. It is their opinion that this method would have the least environmental impact on the site as it would not require trucking of stumps off site, while utilizing on-site material for stabilization. Initially, I was opposed to this approach as it would require more upfront expense to me, however, I can see the benefit of doing it all a once as the site would be more manageable. As currently approved,the two phase approach would cause the site to be unstable during both phases. (i.e. having the tree cutters come in every 6-8 weeks would create less ability to control excessive runoff and would call for multiple openings of the property). Fax (978) 686-5100 www.andoverconsultants,com September 29, 2014 North Andover Planning Board Town Offices 1600 Osgood Street North Andover, MA 01845 RE: As-Built Plan Land Disturbance Permit Map 96, Parcel 33-Stevens Street Dear Members of the Board: Please find attached a plan depicting the as-built conditions for the project titled"As-Built Plan, Stevens Street, North Andover, Mass.", prepared for Tom Patenaude and dated November 5, 2013. Andover Consultants, Inc. was not retained to conduct inspections during construction so the attached plan depicts site features that were visible, or accessible, at the time of the survey only. Based upon my review of the attached plan and site inspections conducted on November 13, 2013 and September 29, 2014, it is my opinion that the completed work is in reasonable compliance with the design plan with the following minor differences noted for your information. • The lot line between lots 2 and 3 was revised after approval and determined to be an insubstantial change by the Board on March 12, 2013. • The lot lines behind lots 4A, 5A and 6A were changed with the intent to convey approximately 20,620 square feet of the lots to abutter Evans and approved as an ANR plan by the Board on May 9, 2013. • The applicant was unable to obtain an easement for the replacement of the existing culvert in front of lot 5A, so the culvert the rip rap apron was not constructed per the plan dated September 1, 2011 depicting a new location. The replacement of the pipe, in place, was approved by the Planning Board as an insubstantial change on August 2, 2011. • There are retaining walls along the south side of the drive on Lots 4A and 5A and on the north side of the drive on Lot 313 that were not shown on the design plan. This is a minor change typical for residential drives. • The driveway swales along the drives on lots 4A, 5A and 6A were not constructed, but runoff from the driveways appears to be directed to the roadside swale as intended. • The retaining wall along the rear of lots 3B, 4A, 5A and 6A are contained within each lot rather than as shown on the design plan (as continuous along all 4 lots). This another minor change and lot grading is stable. • The drop inlet, in front of lot 5A, was constructed differently and includes a berm and pipe. The sump is 2.6' deep rather than the 4 feet depicted in the design. Civil Engineers • Land Surveyors ® Land Planners andover consultants inc. • The house and drive on lot 4A was reversed with the driveway constructed on the right side of the house. This is a minor change. • There are small flagstone patios behind lots 2B and 3B. There are small paver patios and walks to the driveways on Lots 4A and 5A that were not shown on the plan. These are typical for residential uses and are minor. • The limit of work extended onto the land of Stevens behind Lots 3B and 4A. It is my understanding that the Applicant obtained approval from abutter Stevens prior to commencing the grading work On behalf of the Applicant Tom Patenaude, we respectfully request that the Board release the performance bond posted for the project.
