Con Com. 10/9/03 Awaiting Con Com officer letter that items previously noted have been resolved. 11/13/03 Resolution is being finalized. 12/10/03 Replication resolution is in progress. 2. Millwork and casework— 12/10/03 Performance of the subcontractor(Henry Construction) has not been up to the requirements or expectations of Callahan. Meetings to expedite their performance will continue between all parties. Callahan has been asked to prepare alternative contingency plans. 1/08/03 Despite continuing problems with thier millwork supplier, Callahan confirmed that,with the exception of the auditorium and broadcast suite, millwork will be sufficiently complete for the target 2/01/04 occupancy permitdate. This progress will be accomplished with supplemental effort on Callahan's part, if necessary. The auditorium and broadcast suite occupancy will follow and complete ASAP. 2. Electrical- 12/10/03 Considerable work remains in the Tech/Ed and auditorium areas. Callahan has obtained the subcontractors commitment to maintain and adhere to the expedited schedules. The subcontractors president (Paul Palumbo) is actively involved in the management of the electrical work. 1/08/04 Callahan is continuing to expedite the electrical work with the President of Palumbo electric. Crew size is up to 20,with concentration in the educational and administrative areas, including exterior lighting ( necessary for night occupancy). Callahan indicated that effort will provide for occupancy of all areas, except the auditorium and broadcast suite, by the target date of 2/01/04. Auditorium and broadcast suite completion will follow ASAP. 3. Gym Floor- 12/10/03 Gym floor construction commenced on 12/3/03 and is scheduled to be complete approximately 1/21/03 (7 weeks). A meeting with the subcontractor was held on 12/2/03 to emphasize the schedule and quality requirements. 1/08/04 Gym floor is predicted to be completed by 1/16/04 with the bleacher instalation to follow immediately. 3. Change Proposals: 1/08/04 Proposed change orders (PCO's) continue to be solicited and reviewed. (See attached change order log). Athletic field up grade plans, Option#3, have been finalized and submitted to the contractor for pricing. 5. Schedule: Contract Completion Dates Phase I— new school 7/15/04 Phase II—Demo and site 5/5/05 NOTE: Contractor has furnished a letter dated 11/14/03 confirming their commitment to provide for final occupancy of the building by 2/1/04. (see attached). Since the referenced letter was received, considerable time has been expended analyzing a Anticipated Upcominiz Milestones: Target Substantial Completion Major construction (all but Auditorium & TV studio) 1/23/04 Major construction (Auditorium & TV studio) ASAP Inspections & certifications 1/30/04 Major punch list items 1/30/04 Occupancy permit 2/02/04 Contractor has submitted the required updated schedule dated October 2003. The proiect continues to maintain a pace consistent with the early delivery of the new building. 3 6. Technology cabling and telecommunications system 4/10/03 Technology wiring contract has been awarded to ESCOA with work onsite to commence shortly. 5/8/03 ESCOA commenced wiring work this week. 7/9/03 Work is in process. 8/7/03 Work continues in progress. 9/11/03 Work continues in progress. 10/9/03 A technology meeting was held on Friday 10/03 with ESCOA to expedite then progress to maintain Callahans overall schedule. Accommodations were made. 12/10/03 Work is in progress. 1/08/04 A meeting was conducted at the site on 1/06/04 to finalize coordination of the Electrical and Technology contractors. 7. Commissioning: 4/10/03 Commissioning contract has been awarded to R. D. Kimball Co., Inc. A pre commencement meeting occurred onsite 4/1/03 with work to commence immediately. 5/8/03 Integrated software has been procured and R. D. Kimball is actively involved in document review and action plan preparation. 7/9/03 Commissioning work is in process. A co-ordination meeting was held on 6/10/03 with an additional meeting scheduled for July 22, 200'3. 4 8/7/03 Additional co-ordination meeting was held on 7/22/03 with fiinctional test reports reviewed along with schedule. 10/09/03 Commissioning meeting was held on 9/30/03 and test forms, O & M manuals and test schedules reviewed. Testing and start ups witnessed to date: ejector pumps, generators and roof top air handling. 1 i/13/03 Commissioning meeting was held on 11/11/03 with testing schedules and submittals reviewed. (see attached). 12/10/03 Commissioning meeting was held on 12/2/03 (see attached information). 1/08/04 commissioning plan and schedule dated 1/05/04 is attached. 8. FF&E: Delivery commenced November 20, 2003 Installation complete January 15, 2004 (approximately) 9. Athletic Field Modifications: 9/11/03 Site plan modifications to accommodate the proposed field revisions will not require technical review committee approval. Town department input will occur during the planning board process. (See DDP memo 8/4/03). 10/9/03 The Building Committee directed that the revised field layout includes a softball field located on the parcel adjacent to Prescott and that the lower athletic field adjacent to the entry road be re-graded to accommodate a future softball field. The drawing s are being finalized for submission to the Con Com for approval and the contractor pricing. 11/13/03 Conservation Commission meeting was held on 11/5/03. The Commission will handle the changes as an Administrative process, with no re-filing. Pricing of the site portion is in progress. 5 1/08/04 Planning Board meeting was held on 12/02/03. Planning Board requested that a wildlife study of the Habitat area be conducted,which is in progress. Pricing set has been issued to the contractor. 6 N. DEP File No. 242-1045 Planning Board Notice of Decision dated 5/14/01 Schofield Brothers of New England, Inc. has been designated as the"Environmental/Erosion Control Monitor"for the North Andover High School Project. The purpose of the Environmental/ Erosion Control Monitor is to perform periodic inspections of the project site to insure compliance with the Order of Conditions issued by the Conservation Commission and the Special Permit issued by the Planning Board with respect to sediment and erosion controls, limit of work compliance, wetland replication and stormwater management issues. This is the twenty-six monthly inspection report prepared and submitted in compliance with the above referenced permits. It covers the period October 29, 2003 through October 25, 2003. Attached are copies of the Inspection Reports (#99, #100, #101, and#102) relative to the inspections performed on November 4`h 1 ph 18" and November 25`h 2003. During this timeframe, the site remained in stable condition and there were no new issues with regards to erosion and sedimentation,.During all site inspections the water at the two outfalls in the Cochichewick Brook area were clean. Small amounts of erosion continued to occur at the ordinary stockpile and at a couple of the areas adjacent to the new school building that were receiving roof runoff. Overall, the site was in good condition, and there were no adverse affects to the wetland resource areas. The Eastern Red Cedar trees planted in the buffer zone planting area appeared to be in poor health going into winter. The contractor and subcontractor were made aware of their condition. An inspection of the buffer zone planting area will be conducted in the spring to determine the success of the plantings. SBNE is to provide more detailed contour information for Wetland Replication#2 and to propose a planting plan for Wetland Replication#I and Wetland Replication#2 that is more suitable to the existing conditions. )SCHOFIELD BROTHERS ENGINEERING - SLR EYING • PLANNING it 20217 North Andover High School Monthly Inspection Report— October 29, 2003 through November 25, 2003 Page 2 Items needing attention that have been carried forward include: • Sediment at the outfall (near the tennis courts)to Cochichewick Brook needs to be removed. • The loam stockpile area needs to be re-secured with sediment barrier. • Several sections of silt fence at the ordinary stockpile have been pushed over and need to be repaired immediately. • The haybale barrier south of the spillway at Sediment Basin#3 needs to be supplemented with additional haybales and the silt fence on the western side of the southeast stockpile needs to be repaired. Based on our inspections, we have found that the controls have functioned properly and there have been no adverse affects to the resource areas. Prior to the winter conditions, it is expected that the contractor will address the items needing attention to bring the project into full compliance with the Special Permit and Order of Conditions. Postscript: An inspection of the site was per formed on by Arrow Environmental, a consultant to the North Andover Conservation Commission. At this time of year,the sediment barriers are relatively idle because the ground is frozen and the surrounding areas are stable and not subject to disturbance from contractor work. ® 02/03/04—The sediment barrier along the access road has been pushed slightly from the snow plowing operations. Due to the frozen conditions,this barrier, as well as other sediment barrier, will need to be reevaluated in the spring. ® 02/17/04—The condition of the sediment barriers was the same as previous report. Major Item SuMMAiY: A) Existing Building Demolition and New Fields: Portion of the existing structure (Field House and adjacent structure) has been demolished with the remaining portion undergoing haz mat abatement. B) HVAC and Commissioning: 4/ A meeting to address final commissioning, chiller start up and open issues was held on 4/6/04 with all team members. Action list is attached and is targeted for completion by the end of April. C) Building Water Intrusion: 4/8/04 The unusual torrential 6 to 8" rainfall of 3/30 through 4/1/04 caused water to enter the structure in certain locations. Inspections and repairs commenced immediately on the morning of 4/2/04. The majority have been addressed and those requiring intrusive testing and investigation will be addressed during April vacation. D) Auditorium Substantial Completion 4/8/04 The substantial completion date for the auditorium is scheduled to occur on or before April 23, 2004 (the last day of April vacation). E) Punch list The contractor is working a 2"d shift to minimize disruption in completing the punch list. An updated list is being prepared. 3. Change Proposals: 1/08/04 Proposed change orders (PCO's) continue to be solicited and reviewed. (See attached change order log). Athletic field up grade plans, Option#3, have been finalized and submitted to the contractor for pricing. 2/12/04 Athletic field pricing was received from the contractor on 12/10/04 and is being compared with the estimate received from Fogarty on 2/9/04. 4/8/04 Change Order 411 for the Athletic Field upgrades has been executed in the amount of$4,099,808. Information Purpose of this meeting was to review the status of mechanical contractors' items for completion, status of JCI/McQuay control interface issues, readiness for commissioning, and to plan resumption of contractor demonstration of system operation. 12.2.0 Richard Villani (SPH) is taking over the HVAC project Information management for Sa amore to finish out the project. 12.3.0 Peter Foss (SD) stated that McQuay technician, Leo Norton Information had met at the site with Dan W (JCI) approximately 3 weeks ago and had worked with JCI to made some adjustments to correct the JCI communication problems identified during the late February testing. Subsequent to Leo Norton's site visit, it was noted that the Return Air Temperature sensor was not viewable at the JCI control for the two constant volume units (RTU-4 & 5 and AHU-1 & 2). Peter F. said he thought it could be a point mapping difference between the CV units and the VAV units. McQuay will provide tech support to correct the problem before resumption of demonstration. 12.5.0 It was agreed that the McQuay technician would join the Information team to demonstrate the McQuay control unit operation as part of the resumption of commissioning demonstrations. A thorough explanation of the unit controls features and how it interfaces with the JCI system will benefit the RDK test team and the NAHS operating personnel. The date to resume the RTU demonstration with the McQuay tech was scheduled for April 14, 2004. JCI will participate on 4/14 and will continue the demonstration of additional units the following week when school is not in session. 12.5.1 The JCl/McQuay control interface point summary read/write Information matrix RDK has been using was discussed. The matrix has been revised per the meeting discussion and a copy is attached. 12.5.2 Peter Foss (SID) and Ed Estabrook (F&T)will meet 4/7/2004 Information to review the sequences that are provided by the unit controller and how they map to the contract document sequence designs. The McQuay tech will ensure that the units are set up as agreed. 12.5.3 Bill S. (RDK)to provide a copy of the Functional BS 4/6/2004 Performance Test procedures for a typical roof top unit FPT for to McQuay. RTU-1,2,3,6 Provided by FAX Complete 12.6.0 Mike Pramik (JCI) stated that JCI will be at the site BS 4/13/2004 4/20/2004 and will continue demonstration with RDK on 4/21 and 4/22. RDK expects to continue FCU and VAV testing starting 4/23. 12.7.0 BS (RDK) inquired as to the readiness of AB Palumbo to Information complete the electrical demonstration. ABP has advised JTC that Simplex will be back on 03/30/04 to correct wiring problems with 3 gym doors (area also protected by motion detectors), and with 7 motion detectors on the academic wing second floor. (04/13/04) ABP has been given given a list additional motion detectors that are malfunctioning. ABP 04/20/04 A48.4.4 Security System - It was discussed that there Simplex have been no direct conversations with Simplex to indicate that the security system is complete. It was noted that three doors in the gym still are not on the system. Greg Roberts advised that American Alarm is monitoring the system, and that there has been only one false alarm reported thus far. -- 04/20/04 A49. OWNER ISSUES ABP 01/20/04 B33.2.3 Studio Door Bell - DDP stated PR #35 was Clerk 02/10/04 issued on 08/04/03 for ABP to install a door bell at 03/23/04 the Comcast studio entrance. JTC to expedite a response from ABP. (02/10/04) JTC will press ABP to respond to the Proposal Request to add a door bell. Tech Ed Compressor- In response to NASD question, DDP advised that the compressor for the shop air system is an F&E item. -- 04/20/04 B47.1 Tech Ed Equipment- DDP advised that the furnishings for the Fabrication Shop will be delivered the last week in June. C-H 04/13/04 B48.1.3 Portable Bleachers Lockdown/Crawler- DDP has 04/14/04 contacted Corbin-Hufcor, asking them to advise 04/20/04 regarding NASD's lockdown (Item B48.1.1) and Crawler (Item B48.1.2) questions. Administration Windows - Greg Roberts reported that administration area windows will not crank all the way closed. JTC to advise R&R to correct. EFCO 04/13/04 B49.1 Administration Windows - JTC advised EFCO is looking into problem, which appears to affect the smaller vents in the Admin and Arts wings, noting that the handle provided may not be compatible with the crank assembly. DDP 03/30/04 B50. Gymnasium Switches - Greg Roberts requested that DDP TE investigate substituting pushbutton switches for gym lighting, curtain, and backboard counters. GR noted that the key switches installed are all keyed differently and require a good deal of time to operate individually. The pushbuttons could be secured from unauthorized use by a surface mounted locking cabinet. Also, GR requested that the gym lighting only be operated by the center control point. DDP and TE to review. 3S1 04/20/04 B52. Intercom - NASD advised that the intercoms cannot be heard ESCOA in rooms 3004 and 3006. 3S1 has been advised, and it is thought that this is a cabling issue to be addressed by ESCOA. B53. C. PERMITS & INSPECTIONS 03/26/02 CO3. DPW Permits NADPW 03/16/04 C65.1.1 Loam Removal - JTC stated that there is 03/30/04 approximately 6000 yards of loam to be revised 04/13/04 by the town. 04/20/04 (03/16/04) JTC has determined that there are 5000 yards of excess loam that need to be fully removed from the upper site by June 2004. JTC stated the town may begin taking the loam now. (03/30/04) DPW will begin removing loam on 04/05/04. (03/22/04) Postscript - Bob Nicetta advised DDP that DPW can only remove 1000 yards of the excess loam, and will do so on 04/01/04-04/08/04. DDP advised JTC on 03/22/04 that JTC will be responsible for removing the remainder of the excess loam. (03/30/04) DPW will begin removing loam on 04/05/04. (04/13/04) Bob Nicetta advised DDP that he met with DPW on 04/13/04. DPW advised that site to which loam will be delivered is too wet due to recent rains, and removal will commence when weather conditions allow. Gym Backboard Switches - NASD advised that the key switches are breaking because the switch plate is improperly punched, and the switch rotates with the turn of the keys. ABP to correct. EFCO 04/13/04 B49.1 Administration Windows - JTC advised EFCO is looking into problem, which appears to affect the smaller vents in the Admin and Arts wings, noting that the handle provided may not be compatible with the crank assembly. DDP 03/30/04 B50. Gymnasium Switches - Greg Roberts requested that DDP TE investigate substituting pushbutton switches for gym lighting, curtain, and backboard counters. GR noted that the key switches installed are all keyed differently and require a good deal of time to operate individually. The pushbuttons could be secured from unauthorized use by a surface mounted locking cabinet. Also, GR requested that the gym lighting only be operated by the center control point. JTC stated the town may begin taking the loam now. (03/30/04) DPW will begin removing loam on 04/05/04. (03/22/04) Postscript - Bob Nicetta advised DDP that DPW can only remove 1000 yards of the excess loam, and will do so on 04/01/04-04/08/04. (04/13/04) Bob Nicetta advised DDP that he met with DPW on 04/13/04. (04/27/04) JTC has obtained a quote from a loam hauler who will screen the loam on site and haul excess away for $5/yd.
