16. Inspections by Others:
0 03/02/04—Postscript: Inspection by Ar•roiv Environmental. Items that were noted in
their report are listed below with our comments:
1 -Pre-construction requirements:
• Bird Boxes: This is a requirement for the Applicant to comply with rp for
to a certificate of compliance.
• Snow Stockpiling Plan: A draft plan has been prepared for review.
• No Disturbance Zone markers:Not completed at this time.
2 -Extra Haybales: It was noted in the report that haybales were present but were
fewer in number than the required 100,per-Item #48 of the Order of Conditions.
3-Erosion control is in need of repair. We are assuming this refers to the silt fence
and sedimentation barrier that needs repair. These are listed as items needin
attention in this report. The demolition ivork will create a lot of disturbance and final
landscaping has not been done, which poses a concern with the spring rains just
around the corner. Prior to demolition of the old school and regrading associated
with the new athletic fields the contractor will install sedimentation barrier, diversion
berms and two temporary sedimentation basins.
4- Trash pickup and trash within the resource area. This will be conveyed to the
contractor. It should be noted that trash at the south end of the site does not appear to
be construction related.
5-Need more specific information in the progress reports for•contractor work in and
around the f•esow-ce areas. SBNE can certainly include additional information
regarding work(past present and planned) at the resource areas in the weekly report
to comply with this if desired.
6- Wetland flags. Noted in Item 15.
0 03/16/04—Nothing to report at this time.
0 03/23/04—Nothing to report at this time.
0 03/30/04—The following is the status of above the comments.
0 Item 1-Meeting with school department and public works to be scheduled soon.
0 Item 2 and 3- Contractor completing new erosion control setup. Additional work is to be
completed. See discussion on these items in this report.
0 Item 4- Contractor appears to have picked up some trash in the northerly work site.
Additional work to be done plus southerly area still needs attention.

During the inspection it was sunny and clear. The outfalls of Cochichewick brook were
clear and no impacts to resource areas were observed. There was substantial tracking of
mud from the northern entrance from the trucking of the topsoil off site. The newly
constructed and upgraded erosion and sediment controls in the southerly work area
functioned properly. Minor maintenance and repair of erosion controls in other parts of
the site is necessary. Work on the demolition e f Contractor stated that a sweeper would

INSPECTION SUMMARY
1. Sediment control barriers around the perimeter of the current work area:
® 04/27/04—In addition to the areas mentioned above,(Several sections of sedimentation
barrier along the entrance off Osgood Street have been damaged from snowplowing
operations and will need repair when conditions allow.)some gaps in the perimeter
barriers in the southeast corner of the site have appeared as a result of the previous
heavy rains that are in need of repair. These include the stone filter berm at the
southeasterly outfall area and the haybale and silt fence near the walkway to the tennis
court. These are locations where substantial runoff drains to the wetlands,so repairs
need to be made immediately. Also, work will be starting soon in the northerly field area
and the perimeter barriers along the northeasterly side will need to be refreshed.
