A. SITE SAFETY AND SECURITY
-- 06/22/04 A48.3.4 Hand Readers - JTC reported IR has the correct
06/29/04 cable and will instruct Greg Roberts in the use of
06/30/04 all system features in two weeks, after the end of
07/06/04 the school year.
(06/22/04) Greg Roberts reported that IR has
scheduled training for 06/29/04.
(06/29/04) Greg Roberts reported that IR
provided additional instruction. IR will deliver
cable for handheld unit on 06/30/04.
(07/06/04) Greg Roberts reported cable has been
delivered. Record item.
ABP 06/10/04 A48.4.06 Security System (Postscript) - JTC, DDP and TE
met with ABP and Simplex on 06/10/04. ABP
confirmed that in addition to the three gym area
doors that are not monitored due to wiring
problems, another ten doors (mostly in the
cafeteria, tech ed and art areas) now have wiring
problems. ABP shall troubleshoot and correct the
wiring problems.

Simplex 06/10/04 A48.4.08 Security System (Postscript) - Simplex will
NAHS through Greg Roberts, obtain a printout from
American Alarm so that the "supervisory"trouble
can be traced. It was discussed that Simplex
needs to advise Greg Roberts as to what has
been done with the system after each of their
service calls.
ABP 06/22/04 A48.4.09 Security System - Greg Roberts stated that the
Simplex security system has not been armed since the
06/10/04 meeting with Simplex, ABP, DDP and
JTC and neither Simplex nor ABP have been in
contact with Greg Roberts since that date. JTC
stated that this is in conflict with JTC's explicit
instructions to Simplex and ABP. JTC will
address this issue with ABP immediately.
Simplex 06/29/04 A48.4.10 Security System - Greg Roberts stated that
07/06/04 Simplex has not yet come to the site and the
system remains unarmed,which is more critical
now that school is over. Both JTC and NASD
will contact Simplex to come and block out the
doors which are a problem, so that the system
may be armed.
(07/06/04) Greg Roberts reported system still
cannot be armed, but Simplex was on site
07/01/04 and is to come back 07/06/04 to delete
16 to 17 problematic door contacts so that the
school may arm the system.

B. OWNER ISSUES
JTC 04/27/04 B33.2.4 Studio Door Bell - JTC stated that they may have
ABP 05/04/04 another electrical sub install the door bell, given
Clerk ABP's failure to do so.
(05/04/04) ABP stated doorbell will be installed,
stating that they had difficulty pricing the flashing
light required for the studio. Clerk shall monitor time
for ABP to install doorbell.
DDP 03/16/04 B47. Tech Ed Compressor- In response to NASD question, DDP
advised that the compressor for the shop air system is an
F&E item.
ABP 05/25/04 B47.1.1 Tech Ed Equipment- DDP confirmed that tech ed
equipment will be delivered and installed in June
(Postscript: vendor confirmed week of 06/07/04).
ABP to remove stored materials from the
Fabrication Shop.
DDP 06/01/04 B48.1.7 Bleacher Crawler- JTC will obtain from C-H the
NASD 06/22/04 quote for a crawler for the portable bleachers as a
condition for payment.
(06/22/04) Proposal received by DDP on
06/22/04. DDP will review with NASD.

JTC 05/18/04 B49.2.1 Admin/Arts Windows -JTC reported that the
R&R replacement crank being installed by R&R only
allows the vent to open a few inches. DDP
stated this does not meet specification and is
unacceptable. JTC will so advise R&R.
DDP 03/30/04 B50. Gymnasium Switches - Greg Roberts requested that DDP
TE investigate substituting pushbutton switches for gym
lighting, curtain, and backboard counters. GR noted that the
key switches installed are all keyed differently and require a
good deal of time to operate individually. The pushbuttons
could be secured from unauthorized use by a surface
mounted locking cabinet. Also, GR requested that the gym
lighting only be operated by the center control point. DDP
and TE to review.
3S1 04/20/04 B52. Intercom - NASD advised that the intercoms cannot be heard
ESCOA in rooms 3004 and 3006. 3S1 has been advised, and it is
thought that this is a cabling issue to be addressed by
ESCOA.
JTC 05/17/04 B54.1.1 Coat Room (Postscript) - Note on Drawing
3.14.12 specifies closet pole and hat shelf by
06200 - Finish Carpentry in Coats 1193A.

-- 07/01/03 C69.4.1 Wetland Markers - Paul Szymanski confirmed that
wetland markers had been installed, but have
apparently been removed.
JTC 06/01/04 C86.3.3 Replication Areas 1 & 2 - DDP to issue proposal
06/08/04 request to JTC to add the supplemental
06/22/04 plantings.
06/29/04 (05/04/04)PR #54 issued to JTC for additional
wetland replication plantings on 05/04/04.
(05/25/04) JTC will submit price as soon as
possible, and stated that planting can be
completed by 06/15/04.
(06/29/04) JTC has been informed by Emanouil
that additional plants will be in place by 07/02/04.

(06/01/04) DDP has approved verbal pricing from
JTC and JTC has directed the landscaper to
install the additional plants (Postscript- PCO 174
received by DDP on 06/02/04 and Work Order

(06/08/04) JTC stated additional plants will be
planted the week of 06/14/04.
(06/22/04) JTC stated Emanouil will be on site
this week.
(06/29/04) JTC has been informed by Emauoil
that additional plants will be in place by 07/02/04.

JTC 11/18/03 C89.2.2 Buffer Zone Replication Plants-JTC reported
05/25/04 that the landscaper has examined the condition of
06/22/04 plants and has stated that any plants that are
06/29/04 found to be dead this spring will be replaced
under warrantee.
(05/25/04) JTC stated dead (not eaten) plants
will be replaced by 06/15/04.
(06/08/04) JTC stated dead plants will be
replaced the week of 06/14/04.
(06/22/04) JTC stated Emanouil will be on site
this weekend.
(06/29/04) JTC has been advised by Emanouil
that plants will be replaced by 07/02/04.
JTC 06/01/04 C99.07.6 MEP Punchlists - JTC to provide DDP with
copies of the plumbing, HVAC and Electrical
(South Wing) punchlists with completed items
crossed off for verification by DDP's consultants.
C-H 06/08/04 C99.12.9 Bleachers -JTC stated 'p' aisle section will be
06/29104 installed after the end of the school year, due to
the welding required.
(06/29/04) Greg Roberts reported rails are in
place with some paint touch up required.
-- 04/06/04 C106.4 North Swale - It was observed that runoff was
overtopping the swale on the north side of the
driveway during the heavy rains. This will be
monitored as the north site is completed.
JTC 07/06/04 C119. Erosion Control - SBNE advised that there are a number of
long standing issues that have yet to be addressed
(perimeter sediment fence at north field, CB's 38 &44, trash
in resource areas). In addition the haul road and Prescott
Street must be swept, and CB's 12, 20, & 23 cleaned. JTC
stated these would be addressed this week.

D. CONSTRUCTION SCHEDULE
JTC 08/12/03 D82.1.2 MEP Submittals - DDP has not received any of
ABP 09/11/03 the O&M and startup info requested by RDK at
Hampshire 10/28/03 the pervious commissioning meetings. JTC will
11/05/03 again request submission by the subs before the
08/26/03 commissioning meeting.
(08/19/03) - HVAC manual (one copy) was
returned "approved as noted" by F&T. DDP
requested final three copies be submitted for
RDK/Owner's use and information.
(08/26/03) - HVAC and Plumbing start-up/test
procedures received; electrical information is
outstanding. Final HVAC manuals to be
submitted. Plumbing manual to be forwarded to
DDP by JTC. Electrical O&M manual in process.
(09/11/03) Final HVAC manuals submitted.
(09/30/03) Plumbing and electrical O&M's yet to
be submittal to DDP.
(10/28/03) Plumbing O&M received. Electrical and
Fire Protection O&Ms yet to be submitted.
(11/05/03) Electrical and fire protection O&M
Manuals received and returned "Revise and
Resubmit."
JTC 03/02/04 D82.1.3 MEP Manuals - JTC reported that they have the
ABP 05/25/04 HVAC and Plumbing Manuals from the
subcontractors, and expect to have the electrical
O&M manuals soon.
(05/25/04) JTC noted Greg Roberts has a
preliminary copy of plumbing, fire protection and
HVAC manuals for reference.
JTC 05/18/04 D82.1.6 Kitchen Manuals - Returned to JTC on 05118/04
BS for minor revisions by Boston Showcase (BS).
JTC will keep in trailer on site and insert/delete
materials noted by Crabtree McGrath in their
review.
JTC 07/06/04 D137. Punchlist Schedule - MBC requested that JTC put together
a schedule for the completion of the punchlist work, advising
that it would show the subcontractors that there is a deadline
for completing their work.
-- 07/06/04 D138. Construction Progress - JTC gave an overview of
upcoming activities, as follows. Record item.
-- 07/06/04 D138.1 South Field - The last of the polished stone will be
placed and spread, and fine grading will start this
week. Ballfield lights to be delivered today, and
conduit runs for ballfield lights will begin.The
mason has run out of glazed block, and has
ordered units needed to complete the ticket
booths. Record item.
-- 07/06/04 D138.2 North Fields - 75% of subdrain has been
completed and 50% of the track pavement drain
has been completed. Record item.

-- 07/06/04 D138.3 Site Lights - JTC reported that they will soon be
ready to complete the site lights in the south
parking lot area. JTC is in discussions with ABP's
bonding company before proceeding with another
electrician. It was discussed that the lights must
be completed by the start of the school year. It
was noted that JTC has relatively little exposure
on this item with the bonding company should
JTC proceed without the bonding company's

CTI 04/13/04 F85.3.3 Tile in Cooler/Freezers -CTI must confirm if cooler
freezers were installed using SP-100 or Spectra
Loc.
DDP 04/13/04 F85.3.4 Laticrete Recommendation- PM recommends that
06/28/04 given the use and cleaning materials employed at
NAHS (which PM will confirm in a visit to the
kitchen), the Spectra Loc floor grout should remain.
CTI offered an extended warrantee; DDP stated a
bond may be required if this is accepted. Laticrete
must put their recommendation in writing.
(06/28/04) DDP received letter dated 04/23/04
directly from Laticrete on 06/28/04. DDP will
consider recommendations.

Department advising them that sprinkler heads in elevator

DDP 12/02/03 N89. Clothes Dryers - JTC noted no vents are shown on HVAC
F&T drawings for dryer in Study Skills#2050 and the kitchen.
DDP to review with F&T and advise.
DDP 05/04/04 N94.1 Press Box Colors - DDP forwarded masonry color
selection to JTC on 04/30/04. JTC requested
paint color selection.

P. SITE WORK
DDP 03/11/03 P11.3 Art Wing Storm Drainage -JTC submitted PCO
Clerk #066 to repipe roof drains to manhole 2B. DDP to
review with clerk.
JTC 09/30/03 P41. Art Plaza Pavement- DDP reviewed CRJA site visit report
of 09/26/03. JTC stated radius will be cut. The clerk
observed that the puddles noted disappeared in less than
an hour, and low spots are very shallow.
DDP 10/08/03 P41.1 Arts Plaza Pavement (Postscript) - CRJA
Clerk followed up, advising DDP that low spots would
be an icing concern, and requested that the
tolerances be checked per the paving spec (See
Spec 02500-3.01-19). DDP to advise clerk to
confirm.
JTC 03/16/04 P49. Catch Basins - SBNE noted that JTC has yet to install the
hoods on the catch basin structures as required.
NASD 04/20/04 P52. Commemorative Plantings- Paul Szymanski advised that
DDP the school will be planting some memorial trees, and is
looking into some way to incorporate future memorials in an
orderly, planned fashion. DDP and NASD will coordinate.
-- 04/27/04 P53. Athletic Fields - DDP, MBC, JTC, and Gale reviewed the
athletic field upgrade work, with the following noted.
DDP 04/27/04 P53.8 Scoreboards - JTC reiterated that their proposal
includes two scoreboards (one football, one track)
with conduits to a third "future" location (in
baseball field). DDP will confirm locations.
DDP 05/18/04 P53.8.1 Scoreboard - It was agreed that football
scoreboard will be relocated to the est end of the
football field, back-to-back with the "future"
baseball scoreboard. DDP will issue a field order
locating this scoreboard.
P&S 06/29/04 P56.3.3 Sand/Loam Mix Test- P&S will take samples of
stockpiled loam and will mix with samples of
Pitcherville Sand and will test for optimum
sand/loam/organic content.

P&S 06/29/04 P56.3.4 Mixing Sand and Loam -JTC noted that Section
02900 specifies mixing sand and loam by
harrowing, and at this time this is JTC's intent.
Bob Pine stated that more satisfactory results will
be achieved far more easily by mixing with loader
buckets after the loam is screened. P&S will come
to the site to monitor JTC's mixing of the sand and
loam to establish a procedure and consistant
result.
JTC 06/29/04 P56.3.6 Drainage - JTC and DDP described how the field
CRJA sub grade for drainage as shown on the contract
drawings no longer reflects the final grades due to
changes made necessary by the athletic field
upgrades. CRJA agreed that adjustments to
J-drain and collector pipe layout would be
acceptable as long as"downhill"criteria is met.
CRJA will inspect subdrainage when completed,
and before being covered with sand, to see that
this criteria is met.
JTC 06/08/04 P62. Field Grading -JTC stated will be ready for grading for Field
Gale Turf on 07/01/04, with grading expected to take two weeks,
with Field Turf on site for that period. Gale will inspect the
field subgrade whenever JTC notifies Gale that it is ready
for fine grading.
-- 06/22/04 P62.1 Field Sub Grade -JTC stated that the last section
of curb edge is being poured today. JTC is
beginning to lay out the filter fabric and place the
turf stone base. JTC expects Cape and Island
to start fine grading with Field Turf's participation
on 07/05/04. This operation is expected to take
two weeks with Gale's review of finish grade and
start of turf layout to begin Mid-July.
JTC 06/29/04 P62.2.1 Meeting -JTC stated C&I could not make the
Gale 06/29/04 meeting with Field Turf and Gale. JTC
Field Turf will reschedule.

Gale 06/22/04 P64. Emergency Access-JTC questioned whether ambulance
DDP access is possible to the football field, suggesting that the
stone dust paving behind the baseball backstop could be
used for access. DDP and Gale will review.
-- 06/22/04 P66. Netting -JTC noted a tall net may be advisable behind the
07/06/04 football field goal posts to keep field goals from reaching Rte.
125. Gale will review and advise. Record item.
JTC 07/06/04 P66.1 Netting - DDP has forwarded to JTC for pricing
Proposal Request#58 for installing netting behind
the west goal posts. Gale noted JTC may price
either steel or wood (utility) poles; whichever will
be more cost effective. Greg Roberts provided
JTC with the name of a local powerline contractor
who could provide and set 50' poles.

07/08/04 received by DDP 07/08/04. Record item.

-- 07/09/04 P66.3 Field Goal Netting (postscript) - DDP reviewed
Netting Proposal with School Building Committee
on 07/08/04. SBC authorized proceeding with the
netting provided poles are not wood and that the
net is retractable. DDP advised JTC field super of
decisions on 07/09/04. JTC reviewed with fence
subcontractor and advised DDP of
recommendation and pricing on 07/09104. Work
Order No. 104 issued to JTC on 07/09/04.
-- 06/29/04 P67.4.1 Change Proposal - JTC will price the work as
07/06/04 recommended by R. Weber (Postscript- Clerk in
a phone conversation with R. Weber on 06/30/04
clarified"gravel' material recommendation) so that
the SBC can act on the recommendations at the
07/08/04 meeting. It was agreed that proposal
shall include removing from the site the unuseable
fill, and not try to use it elsewhere on site.
Several options for disposing the material were
suggested to JTC.
(07/06/04) PCO 188 distributed and discussed at
job meeting 07/06/04. DDP and MBC will bring
proposal to SBC for authorization at 07/08/04
meeting.
-- 07/09/04 P67.4.2 Change Proposal (postscript) - PCO 188
authorized by SBC 07/08/04. JTC advised on
07/09/04 and Work Order#103 issued. Record
item.
JTC 06/29/04 P68.1 Leaning Trees - CRJA expained that they do not
Emanouil require trees to be guyed because studies have
shown that unguyed trees are stronger. However,
the spec does require trees to be plumbed if
required. JTC will advise Emanouil to reset the
trees.
JTC 06/29/04 P69. Crabapple Trees - CRJA reviewed problems observed
Emanouil with crabapple trees, noting that the trees planted were not
tagged by CRJA. In any case, die back has caused most
crabapples to no longer meet specifications and those must
be replaced. (Postscript: CRJA has flagged trees that must
be replaced). JTC will so advise Emanouil.
Emanouil 06/29/04 P70.1 Lawns at Arts Wing -JTC agreed that Emanouil
would repair the bare spots in the lawn between
the arts classrooms and the driveway.
Q. ARCHITECTURAL
Bass 05/27/03 Q30.8.03 Academic Wing Gravel Stop does not conform at
Stanley 08/19/03 south side, and is coming loose on north side (item
DDP 04/27/04 Q30.7.1)
JTC (08/19/03) - Gravel stop issues shared by JTC
(blocking), Bass (forming) and Stanley
(installation). DDP and JTC will inspect entire
roof perimeter and list areas be reworked.
(04/27/04) JTC advised that Bass will likely not
perform any of the repairs previously reviewed
without a formal punchlist.

DDP 02/27/04 Q75.2.1 Floor Waxing (Postscript); JTC forwarded to
DDP PCO 157 for burnishing all floors. DDP to
review.
DDP 04/13/04 Q91. Auditorium Chair Lift- DDP will make calls to clarify Mass.
Elevator Board's position on whether the chair lift is to be
inspected.

DDP 02/29/04 R04.1 Roof Rigging - NASD has forwarded cuts of roof
rigging to be purchased as F&E.

S. FIRE PROTECTION
Hampshire 05/25/04 S20. Fire Protection Leaks- Hampshire on site on 05/25/04 to
repair some leaks in the system, including at the reduced
pressure pump.
RWS 06/22/04 S20.1 System Pressure - It was reported that the
Hampshire technician on site feels the reported
180#pressure in the system is proper. DDP will
discuss with RWS.

T. PLUMBING
Grasseschi 05/18/04 T30.2.1 Gas Shutoff Valve - DDP has been informed by
RWS that all their conversations with Grasseschi
were with regard to Grasseschi's request to
substitute a standard access panel for a valve
cabinet (which was done). RWS at no time
instructed Grasseschi that the door should have a
lock. Grasseschi is to remove lock and install latch
per detail on 8.0.3.
Grasseschi 04/13/04 T33. Photo Dark Room - Greg Roberts advised that backflow
preventer has been installed, but exposed hot and cold
water piping must be insulated.
Grasseschi 05/04/04 T35. Push Button Faucets - Greg Roberts reported that kids have
been taking the push buttons off of the lavatory metering
faucets. Upon examination, NASD found that set screws
were not tightened. Grasseschi to tighten set screws, and
shall provide NASD with spare knobs.
Grasseschi 06/04/04 T36.1 Hot Water (Postscript) - RWS memo distributed
on 06/07/04. Grasseschi to advise on system
balancing.
DDP 06/08/04 T36.2 Hot Water - Greg Roberts questioned type of
RWS tanks and piping arrangement, wondering if this
would play some part in the hot water circulation
issue. JTC stated tanks were piped per RWS
directives in response to a question from
Grasseschi. DDP will review further with RWS.

ABP 06/08/04 030.1.2 Fume Hood Fans -ABP has ordered special
starter required for two fans.
Sagamore 06/15/04 U34.1.2 Mainstreet Fin Tube - DDP reviewed conditions
and found that fin tube cover has been
adequately screwed to support structure.
Sagamore shall consistently cope the end covers
tight to columns, concealing the piping.
F&T 03/16/04 U35. Kitchen ElevatorShaft - It was discussed that even with
keeping the boiler room doors closed, a strong down draft is
keeping the elevator shaft cold in the kitchen. DDP will
review with F&T.
-- 06/15/04 U37.3.1 ERU-2- Sagamore advised that the coil will be
06/22/04 replaced, which will necessitate using a crane.
Sagamore will advise on lead time.
(06/22/04) Sagamore reported new cooling coil will
be delivered these weeks from 06/21/04, and
installed the week of 07/19/04.
F&T 06/15/04 U37.3.2 ERU-2 - It was discussed that for cooling loop to
be balanced, a temporary flow must be created
until the coil in ERU-2 is replaced. Sagamore
requested that Fitzemeyer&Tocci review and
advise.
Sagamore 06/15/04 U37.6.2 Systems Balancing - F&T has returned the report
06/22/04 for revisions. At the 06/15/04 meeting, F&T noted
that information for pumps and several fans was
not provided. Furthermore, the water system
must be rebalanced following the recommissioning
of the control valves.
(06/22/04) Sagamore reported balancing will be
completed by 07/16/04.
Sagamore 04/20/04 U41. Pipe Insulation - It was noted that there is radiator piping
uncovered at the upper west end of the gym and at the shot
feeder system.
Sagamore 06/22/04 U41.2 Pipe Insulation - Sagamore stated
repair/completion of insulation will start on
06/28/04.
Sagamore 06/22/04 U43.1.1 Sagamore advised that the last of the filters are
due on site on 07/06/04 and will be installed on
delivery.
Sagamore 06/15/04 U44.2 BAS Training - It was discussed and agreed that
NASD JCI has fulfilled their specified training
requirements. NASD requested that Greg
Roberts obtain additional classroom training from
JCI; JCI will submit directly to NASD an
application.
-- 06/08/04 U45.1 Boiler Leak- Greg Roberts reported that
Sagamore has repaired the leak between
sections. However, the boiler has not since been
fired to determine if repair will hold.

Sagamore 06/15/04 U46.3 HVAC System Completion - Sagamore stated
that they are committed to seeing that the
outstanding issues are addressed and that the
system functions as designed. Sagamore could
not, at the 06/15/04 meeting, commit to a timetable
for completion because Sagamore needs input
from its subs and suppliers. However, Sagamore
will issue a written response to the issue including
a timeline no later than 06/18/04. It is agreed that
all work must be addressed by 09/01/04.
Sagamore 06/22/04 U46.5.1 Valves - Starting on 07/05/04, Sagamore, JCI and
JCI Thomas Young (balancer) will be on site to
TY recommission valves and rebalance the water
system. Sagamore will be on site to repair piping,
etc. found to be necessary during the
recommissioning. By 07/16/04 the balancing will
be completed, and RDK can retest a sampling of
equipment.
(06/29/04) Greg Roberts has been informed by
JCI that work will not start on 07/05/04, but has
been rescheduled for 07/12/04. JTC will request
that Sagamore provide an updated schedule.
Sagamore 06/22/04 U46.5.2 Punchlist- Sagamore will start work on F&T
JTC 06/29/04 punchlist on 06/28/04, and will work on punchlist
items during down time of the valve
recommissioning effort.
(06/29/04) Greg Roberts reported Sagamore was
on site on 06/28/04, but was not on site on
06/29/04. JTC will contact Sagamore.
Sagamore 06/22/04 U46.5.3 Dust Collector- Panel voltage will be corrected on
06/29/04, and fittings completed.
Sagamore 06/22/04 U46.5.4 Water Treatment System - Sagamore stated
water was retested last week and found to have
29% glycol content (specified range is 31-33%)
Sagamore is prepared to add more glycol with the
rust inhibitor and other chemical additives that are
on site, and startup the system. Greg Roberts
noted that the system only ran for 24 hours after
the last glycol was added, and recommended that
the system be run longer to more thoroughly mix
the glycol.
Sagamore 06/22/04 U46.5.5 As Built Drawings- Sagamore has piping `as
builts'on a cad disk, and has received a second
copy of ductwork `as builts'to be added to disk.
Sagamore 06/22/04 047.1 ERU-2-GR recommended that Sagamore check
the outside air damper operator at ERU-2, after
recently observed high cooling air discharge
temperatures.
Sagamore 06/22/04 U47.2 Control Booth Split System - GR advised that
Sagamore should check piping joints for leakage.

Sagamore 06/22/04 U47.3 Burners - GR advised that Power Flame should
check burners, noting odor associated with
combustion problems. Also, Boiler#1 has trouble
refiring.
Sagamore 06/22/04 U47.4 AHU-3 - Sagamore to replace pulley and furnish
GR with replacement belts.

V. ELECTRICAL
JTC 12/16/03 V56. Gym Lightning Protection -The clerk noted that the recent
DDP heavy snows have apparently loosened the lightning rods
on the low eave of the gym. Conditions will be monitored to
determine if a snow fence is needed over the entire roof
edge.
3Si 03/23/04 V59.3 Room 3011 -3Si has a recording meter on the
outlets in IDF 3011 due to three UPS units being
damages.
3Si 05/11/04 V59.3.1 Room 3044 - Paul Szymanski noted a power
TEC issue has also been observed at V/D room 3044.
DDP reported that 3Si and TEC are working to
isolate and identify the problem, but nothing has
been discovered that can be positively
addressed.
NAHS 05/18/04 V60.1 Knox Box (Postscript) - In a phone conversation
with Vinny Valentino, it was agreed that Vinny
would order the knox box.
ABP 03/16/04 V61. Gym Backboard Switches - NASD advised that the key
switches are breaking because the switch plate is
improperly punched, and the switch rotates with the turn of
the keys. ABP to correct.
Pappas 03/23/04 V61.1 Gym Backboard Switches - JTC advised that
switches have been temporarily repaired, with
new switches on order. NASD advised that west
backboard does not retract. JTC will review with
Pappas.
ABP 06/08/04 V64.4 Gym Lights (Postscript) - ABP advised JTC,
Lithonia DDP and TE on 06/10/04 that high/low gym
lighting system operates except for automatic
lock-out to prevent switching from high to low until
lamps have been at full power for 15 minutes.
Lithonia has been notified.
ABP 04/27/04 V68. Kitchen Door Bell - In response to Greg Roberts'question,
DDP stated the contract drawings specify a door bell at the
service entrance. ABP has not completed this work.

ABP 05/11/04 V69.3.1 Wiring Workmmanship -JTC reported that ABP
TE has examined the photos taken, and ABP has
noted that most of the photos were of security,
not fire alarm wiring. ABP is correcting above
ceiling wiring workmanship. DDP stated that once
ABP advises that conditions are corrected,
Thompson Engineering will re-inspect.
TE 05/04/04 V69.4 Gamewell Comments-TE will call Gamewell to
discuss questions as to system wiring, how
wiring can affect panel performance, and to confirm
Gamewell's recommendations and requirements.
ABP 05/18/04 V70. Duct Smoke Detector Testing - DDP advised JTC that RDK
RDK wishes to schedule the testing of the duct smoke detectors.
While NAFD tested the smoke detectors to see if they signal
an alarm, they have not been tested to see if they will shut
down the HVAC equipment. JTC will advise ABP to
schedule the testing.
ABP 06/01/04 V71. Outstanding Electrical Issues -JTC has written to ABP
detailing issues that must be addressed. ABP to respond.
JTC 06/22/04 V71.2 Outstanding Electrical Issues - JTC advised that
ABP was unable to attend today's meeting. JTC
stated that they will be addressing in a letter to
ABP their abandonment of the job, listing several
issues still not addressed.
DDP 06/22/04 V71.2.1 Monetized Electrical Punchlist- DDP will forward
to JTC the recently reinspected and monetized
punchlist from TE. The clerk is reviewing
completion of teacher punchlist electrical items.
JTC 06/29/04 V71.3 ABP Default - It was discussed that JTC has
been unable to reach anyone at ABP to get a
response on addressing outstanding issues and
that they may no longer be financially viable and
able to complete the project. JTC will be
contacting ABP's bonding company in order to
bring open electrical issues to closure, and will
report further.
-- 07/06/04 V71.4 ABP Default- JTC reported that they are entering
into discussions with ABP's bonding company
and have discussed with Ray Croteau the
possibility of retaining him for continuity. MBC
noted that since Systems Contracting is involved
with the Athletic Complex work, their participation
in the completion of the base electrical contract
work may be adventageous. Record item.
DDP 07/06/04 V72. Lamp List- Greg Roberts requested a list of lamps used on
TE the project. DDP noted this is to be part of the electrical
O&M manuals, but with ABP's default DDP will request a list
from Thompson Engineering.

vandal resistant indoor fixture. Record item.

JTC 04/27/04 W06.1 Existing Auditorium Soffit- DDP authorized
Clerk removal of asbestos containing soffit boards on a
time and materials basis.
DDP 06/22/04 W07. Gym Lights - DDP and JTC discussed additional information
JTC received from Lithonia. DDP and JTC will investigate further
to come to a resolution.

X. MISCELLANEOUS
Bass 08/26/03 X48. Sarnafil Roof Flashing - DDP, JTC and Stanley discussed
Stanley roof edge detail. DDP explained that Work Order required
JTC, Stanley and Bass to coordinate respective work. It
was discussed and agreed that Boss would provide
Stanley with stock so Stanley can form "snap on"trim. DDP
stated, in principle, this would be additional work by
Stanley, and less work (credit) by Bass (but no net change
in cost to Owner).
JTC 04/13/04 X61.2.1 Gym Curtain -JTC stated parts are on hand, and
ABP 05/04/04 JTC will coordinate installation with school use of
Papas 05/25/04 gym-
06/01/04 (04/13/04) JTC advised that installation is
ongoing.
(05/04/04) NASD reported that curtains drop, but
won't raise up. ABP and Papas have been
advised to correct.
(05/25/04) JTC reported that curtain
manufacturers data specify motor voltage to be
120v, however, motors require 240v. JTC is
coordinating with Pappas and ABP.
(06/01/04) JTC advised that Papas will correct
the voltage problem working with ABP. Curtains
will be raised for graduation.
JTC 04/27/04 X66.1 Lecture Hall Screen (Postscript) - Per Section
ABP 11130, switch and box are provided by screen
manufacturer, and wired by 16101-ELECTRICAL.
ABP 05/18/04 X66.2 Auditorium Screen - DDP advised JTC that the
JTC screen control switch is missing, and is to be
furnished under SECTION 11130 and installed
by ABP.

JTC 04/27/04 X67.05.1 Boiler Room - JTC advised that they believe the
Sub 05/25/04 water is entering the boiler room at the
06/01/04 brick/foundation wall joint. JTC is installing a
06/08/04 french drain, to be tied into the vertical clean out
pipe at the corners. Wall penetrations still are to
be made watertight by the respective subs.
(05/04/04) JTC stated supplemental drainage has
been installed, with no further leaking at wall
observed. JTC advised that one irrigation line
penetrating the wall must still be made to be water
tight.
(05/25/04) JTC noted that some leaking was
observed at Stair#8 after yesterday's rains.
(06/01/04) JTC stated no leaks at Stiar#8 were
observed after the recent rain.
(06/08/04) JTC stated no leaks were observed
the past week.
JTC 05/04/04 X67.10 Roof Leak Status - In response to MBC's
questions, JTC stated that all leaks have been
permanently repaired, with the exception of
cupola doors, which are awaiting hardware and
weather stripping.
JTC 06/22/04 X67.11 Cafeteria Roof Leak- Greg Roberts reported that
a leak at the east end of the cafeteria has
reappeared. JTC will investigate.
Musco 05/11/04 X69.1.3 Lighting Submission -After discussion, it was
05/18/04 agreed that to expedite lighting installation, Musco
submission is "approved as noted - contingent on
satisfactory compliance with specified lighting
criteria". Musco shall re-submit for record.
(05/18/04) Musco to submit electrical controls per
TE comments.
-- 05/25/04 X70. Job Meetings - It was suggested by JTC that after the
06/01/04 meeting,job meetings could be held every other
week. It was agreed this will be considered.
NASD 07/06/04 X71. Garbage Disposals - Greg Roberts reported that an official
DDP request will be forth coming from school department for
installing garbage disposals in the Consumer Science
classroom. In advance of the formal request, DDP will initiate
proposal request.

Y. PROJECT CLOSEOUT
Consultants 09/10/02 Y12. As Built Drawings - DDP stated subcontractors should keep
as built drawings up to date. DDP will advise consultants to
view as builts each month.
Clerk 04/27/04 Y12.4 As Built Drawings -JTC has furnished clerk with
RWS as built mark ups of fire protection and plumbing
drawings. Clerk and RWS to review.
Clerk 03/02/04 Y13. Inventory - The clerk will begin an inventory of spare
materials and parts to be forwarded per specifications.

JTC 03/30/04 Y13.1 Inventory- Clerk distributed a list of closeout
Subs inventory to be furnished by JTC and respective
subcontractors.
JTC 04/13/04 Y13.2 Inventory - Clerk reported that stock has been left
NASD in Storage Room 1198, but no transmittals listing
Clerk stock has been provided. It was discussed that
much of the material on hand now is simply
leftover materials, not specified "attic stock."
NASD to have a secure room available to accept
stock. JTC shall see that transmittals
accompanying submittals, which the clerk shall
verify.
JTC 06/22/04 Y13.3 Auditorium Seat Inventory- JTC passed along to
S-D DDP a transmittal from Slay Dale stating seating
maintenance has been delivered. Clerk is
unaware of any such delivery. JTC will advise
Slay Dale to deliver materials to clerk's attention.

-- 07/06/04 ZZ1 20.Construction Progress The architect walked the site and

07/06/04 ZZ120.1 Lower Site - Finish course stone being placed and
spread for turf. Loam being placed for spreading in
future. JTC advised that subgrade must be
graded before loam is spread to assure uniform
depth of loam. Record item.
-- 07/06/04 ZZ120.2 Pressbox Building - Glazed block completed
except for upper part of ticket booths. Record
item.
-- 07/06/04 ZZ120.3 Upper Field - Loam screening and removal of loam
is ongoing, with approximately 14,000 CY
removed from site to date, according to the clerk.

2. Major Item Summary:
A) Lower Parking Stabilization
7/8/04
The extra work approved at the last committee meeting to stabilize
the full operation under the parking lot extension was not 100%
successful. The Geotech engineer determined that the approximate
last 2'-0 of fill be changed to gravel, or equilivent. P.C.O. to achieve is
included for review and action.
B) Existing Building Demolition and New Fields:
4/8/04
Portion of the existing structure(Field House and adjacent structure) has
been demolished with the remaining portion undergoing haz mat
abatement. Certain ballasts and light bulbs were found to be contaminated
and are being removed at a cost of approximately $12,000.00.
Erosion control at the lower fields, as approved by the Con Com, is being
constructed in conjunction with the temporary haul road.
Foundation work for the Toilet/Press box structure is in progress. Prelim
schedule for Fields and structure work is attached.
5/13/04
Building demolition and concrete wall work is nearing completion. Site
utility work, plumbing and filling is in progress. Turf details have been
reviewed and finalized.
7/8/04
Building demolition is complete. Site fill, drainage system and stone
fill work is in progress. Concrete walls are complete and masonry is
nearing completion. Rough under slab plumbing and slabs on grade
work is complete. Light pole bases are on site, field edge curb will be
cast in place concrete. The turf installer requires that all field
preparation work be completed by 8/1/04 for the lower fields to be
"ready to play" on Sept. 1, 2004. The G.C. projects that the
preparation work will be complete in mid July 2004, approximately
one month early.
C) HVAC and Commissioning:
4/8/04
A meeting to address final commissioning, chiller start up and open issues
was held on 4/6/04 with all team members. Action list is attached and is
targeted for completion by the end of April.

5/13/04
Start up of chiller, balancing and commissioning is in progress and due to
be finalized shortly. (See attached list). Several unit control valves have
been replaced with the cause being investigated.
7/7/04
Work is in progress. (See JTC letter to Sagamore—5/26/04).
D) Building Water Intrusion:
4/8/04
The unusual torrential 6 to 8" rainfall of 3/30 through 4/1/04 caused water
to enter the structure in certain locations. Inspections and repairs
commenced immediately on the morning of 4/2/04. The majority have
been addressed and those requiring intrusive testing and investigation will
be addressed during April vacation.
5/13/04
All leaks have been investigated, identified and corrections made.
Monitoring continues in all areas.
E) Electrical Systems
6/10/04
Miscellaneous items are being expedited (See JTC letter to Palumbo—
5/26/04).
7/7/04
We have been informed that Palumbo Electric is no longer doing
business. Replacement options are being reviewed by Calahan. No
project delay is anticipated at this time.
F) Punch list
5/13/04
The contractor is working a 2nd shift to minimize disruption in completing
the punch list. The contractor and Architect are updating the current status
of the list.
7/7/04
Work continues with updating in progress.
3. Change Proposals:
1/08/04
Proposed change orders (PLO's) continue to be solicited and reviewed. (See
attached change order log).
Athletic field up grade plans, Option#3, have been finalized and submitted to the
contractor for pricing.

Athletic field pricing was received from the contractor on 12/10/04 and is being
compared with the estimate received from Fogarty on 2/9/04.
4/8/04
Change Order#11 for the Athletic Field upgrades has been executed in the
amount of$4,099,808. Change Order 912 is attached for the committee's
approval.

Re: North Andover High School, Athletic Fields Site Visit(Test Section)
As discussed at the project site meeting on June 22,2004,the contractor Constructed a test section
within the roadway and parking section behind the bleachers In the lower athletic field area. The test
section consisted of approximately 12-inches of trap rock(minus Cinches)forced into the silty sand
material. The objective was to mitigate severe weaving in the underlying site fill used to bring the area
up to grade. We understand that the contractor selected an area where the weaving was visibly most
severe. This area was beyond of the area that had been treated with geotextile fabric and granular soil.
The test section was constructed on June 26,2004. When 1 arrived at the site on June 28,2G04 the
contractor began running over the approximately 30-ft by 70-ft test section with a loaded front-rend
loader. The middle section of the test section experienced significant weaving while the edges of the
test section appeared more stable. Next the contractor used a self-propelled vibratory drum compactor
to produce additional compaction over the test,section,particularly within the middle area where the
weaving was most severe.
After several passes,the area appeared to tighten although some general weaving was observed.
Rutting decreased markedly. It was then decided to place a 12-inch thick layer of gravel and sand over
the trap rock to simulate completing the fill using granular material. The gravel and sand layer was
rolled with the compactor to the extent that minor weaving was still visible but no significant rutting was
observed.
The area held up well when a pickup truck made several passes over the area. We concluded that:
1. The test section was successful in that one of the worst areas of the site had been stabilized.
Slight general weaving was noted below lightweight truck traffic.
2_ Since the loaders and rollers place more weight over the surface than the expected passenger
automobile traffic,the area is expected to be more stable under the lightweight traffic.
3. The area had been sufficiently stabilized using methods described above so that it could be paved.
4. There is little if any additional stabilization that can be done.
The contractor then used a loaded Euclid hauler to prootroll the remaining areas of the site. In general:
1. The area beyond the end of the fabric/gravel construction areas is where most sever weaving
occurs.

2. Within the fabric/gravel area there were two apparent areas where significant weaving was
observed. The remaining areas(most of the site) appeared stable although minor weaving was
observed.
We recommend that:
1° The entire area not underlain by fabric should be prepared using the trap rock method described
for the test section.
2. The two isolated areas above areas of fabric should also be prepared using the Trap rock method.
3. The entire remaining pavement area should be finished using gravel borrow up to the subgnade
(that is, below the base course layer). We do not recommend using on site material for the
remainder of this work.
The methods described herein are expected to stabilize the on-site fill to an extent where weaving and
severe rutting have been reduced. The area might still experience minor weaving but the intent is to
stabilize the area sufficiently so that the bituminous pavement can be successfully placed and
compacted. However,even though these procedures are undertaken, it is still possible that the
pavement could experience premature cracking or deterioration since minor weaving was still
observed.
The alternative to these procedures is to remove the entire on-site fill and re-fill using granular borrow.
We understand that this is not the Client's intent.

ABP 06/10/04 A48.4.06 Security System (Postscript) - JTC, DDP and TE
met with ABP and Simplex on 06/10/04. ABP
confirmed that in addition to the three gym area
doors that are not monitored due to wiring .
problems, another ten doors (mostly in the
cafeteria, tech ed and art areas) now have wiring
problems. ABP shall troubleshoot and correct the
wiring problems.
Simplex 06/10/04 A48.4.08 Security System (Postscript) - Simplex will
NAHS through Greg Roberts, obtain a printout from
American Alarm so that the "supervisory"trouble
can be traced. It was discussed that.Simplex
needs to advise Greg Roberts as to what has
been done with the system after each of their
service calls.

ABP 06/22/04 A48.4.09 Security System - Greg Roberts stated that the
Simplex security system has not been armed since the
06/10/04 meeting with Simplex, ABP, DDP and
JTC and neither Simplex nor ABP have been in
contact with Greg Roberts since that date. JTC
stated that this is in conflict with JTC's explicit
instructions to Simplex and ABP. JTC will
address this issue with ABP immediately.
Simplex 06/29/04 A48.4.10 Security System - Greg Roberts stated that
07/06/04 Simplex has not yet come to the site and the
07/13/04 system remains unarmed,which is more critical
now that school is over. Both JTC and NASD
will contact Simplex to come and block out the
doors which are a problem, so that the system
may be armed.
(07/06/04) Greg Roberts reported system still
cannot be armed, but Simplex was on site
07/01/04 and is to come back 07/06/04 to delete
16 to 17 problematic door contacts so that the
school may arm the system.
(07/13/04) Problematic door contacts have been
deleted and the alarm is operational.
Ingersall 07/13/04 A48.4.11 Coaches have been using the palm readers,
Rand however, training is needed to use the Palm Pilot
NAHS aspect of the system.

B. OWNER ISSUES
JTC 04/27/04 B33.2.4 Studio Door Bell - JTC stated that they may have
ABP 05/04/04 another electrical sub install the door bell, given
Clerk ABP's failure to do so.
(05/04/04) ABP stated doorbell will be installed,
stating that they had difficulty pricing the flashing
light required for the studio. Clerk shall monitor time
for ABP to install doorbell.
DDP 03/16/04 B47. Tech Ed Compressor- In response to NASD question, DDP
advised that the compressor for the shop air system is an
F&E item.
ABP 05/25/04 B47.1.1 Tech Ed Equipment- DDP confirmed that tech ed
equipment will be delivered and installed in June
(Postscript: vendor confirmed week of 06/07/04).
ABP to remove stored materials from the
Fabrication Shop.
DDP 06/01/04 B48.1.7 Bleacher Crawler- JTC will obtain from C-H the
NASD 06/22/04 quote for a crawler for the portable bleachers as a
condition for payment.

JTC 05/18/04 B49.2.1 Admin/Arts Windows -JTC reported that the
R&R replacement crank being installed by R&R only
allows the vent to open a few inches. DDP
stated this does not meet specification and is
unacceptable. JTC will so advise R&R.
DDP 03/30/04 B50. Gymnasium Switches - Greg Roberts requested that DDP
TE investigate substituting pushbutton switches for gym
lighting, curtain, and backboard counters. GR noted that the
key switches installed are all keyed differently and require a
good deal of time to operate individually. The pushbuttons
could be secured from unauthorized use by a surface
mounted locking cabinet. Also, GR requested that the gym
lighting only be operated by the center control point. DDP
and TE to review.
3S1 04/20/04 B52. Intercom - NASD advised that the intercoms cannot be heard
ESCOA in rooms 3004 and 3006. 3S1 has been advised, and it is
thought that this is a cabling issue to be addressed by
ESCOA.

C-H 06/08/04 C99.12.9 Bleachers - JTC stated `p' aisle section will be
06/29/04 installed after the end of the school year, due to
the welding required.
(06/29/04) Greg Roberts reported rails are in
place with some paint touch up required.

overtopping the swale on the north side of the
driveway during the heavy rains. This will be
monitored as the north site is completed.
JTC 07/06/04 C119. Erosion Control - SBNE advised that there are a number of
long standing issues that have yet to be addressed
(perimeter sediment fence at north field, CB's 38 &44, trash
in resource areas). In addition the haul road and Prescott
Street must be swept, and CB's 12, 20, &23 cleaned. JTC
stated these would be addressed this week.
JTC 07/13/04 C119.1 SBNE stated North Field fence has been repaired.
The silt fence at the southern outfall has been
temporarily fixes with haybales. This fix is being
watched. Trash still needs to be picked up in
resource area.

08/26/03 commissioning meeting.
(08/19/03) - HVAC manual (one copy) was
returned "approved as noted" by F&T. DDP
requested final three copies be submitted for
RDK/Owner's use and information.

DDP 03/11/03 P11.3 Art Wing Storm Drainage - JTC submitted PCO
Clerk #066 to repipe roof drains to manhole 2B. DDP to
review with clerk.
JTC 09/30/03 P41. Art Plaza Pavement- DDP reviewed CRJA site visit report
of 09/26/03. JTC stated radius will be cut. The clerk
observed that the puddles noted disappeared in less than
an hour, and low spots are very shallow.
DDP 10/08/03 P41.1 Arts Plaza Pavement (Postscript) - CRJA
Clerk followed up, advising DDP that low spots would
be an icing concern, and requested that the
tolerances be checked per the paving spec (See
Spec 02500-3.01-19). DDP to advise clerk to
confirm.
JTC 03/16/04 P49. Catch Basins - SBNE noted that JTC has yet to install the
hoods on the catch basin structures as required.
NASD 04/20/04 P52. Commemorative Plantings - Paul Szymanski advised that
DDP the school will be planting some memorial trees, and is
looking into some way to incorporate future memorials in an
orderly, planned fashion. DDP and NASD will coordinate.
-- 04/27/04 P53. Athletic Fields - DDP, MBC, JTC, and Gale reviewed the
athletic field upgrade work, with the following noted.
DDP 04/27/04 P53.8 Scoreboards - JTC reiterated that their proposal
includes two scoreboards (one football, one track)
with conduits to a third"future" location (in
baseball field). DDP will confirm locations.
DDP 05/18/04 P53.8.1 Scoreboard - It was agreed that football
scoreboard will be relocated to the est end of the
football field, back-to-back with the "future"
baseball scoreboard. DDP will issue a field order
locating this scoreboard.
P&S 06/29/04 P56.3.3 Sand/Loam Mix Test- P&S will take samples of
stockpiled loam and will mix with samples of
Pitcherville Sand and will test for optimum
sand/loam/organic content..
P&S 06/29/04 P56.3.4 Mixing Sand and Loam - JTC noted that Section
02900 specifies mixing sand and loam by
harrowing, and at this time this is JTC's intent.
Bob Pine stated that more satisfactory results will
be achieved far more easily by mixing with loader
buckets after the loam is screened. P&S will come
to the site to monitor JTC's mixing of the sand and
loam to establish a procedure and consistant
result.

JTC 06/29/04 P56.3.6 Drainage - JTC and DDP described how the field
CRJA sub grade for drainage as shown on the contract
drawings no longer reflects the final grades due to
changes made necessary by the athletic field
upgrades. CRJA agreed that adjustments to
J-drain and collector pipe layout would be
acceptable as long as"downhill" criteria is met.
CRJA will inspect subdrainage when completed,
and before being covered with sand, to see that
this criteria is met.
JTC 07/13/04 P56.3.7 Water and Sewer Extension - JTC stated that the
Grasseschi water and sewer extensions in the North Field will
begin 07/14/04. They requested direction on
whether to install the handicap access ramp as
shown on the CD's or in the proposed locker
plan.
JTC 06/08/04 P62. Field Grading -JTC stated will be ready for grading for Field
Gale Turf on 07/01/04, with grading expected to take two weeks,
with Field Turf on site for that period. Gale will inspect the
field subgrade whenever JTC notifies Gale that it is ready
for fine grading.
-- 06/22/04 P62.1 Field Sub Grade -JTC stated that the last section
of curb edge is being poured today. JTC is
beginning to lay out the filter fabric and place the
turf stone base. JTC expects Cape and Island
to start fine grading with Field Turf's participation
on 07/05/04. This operation is expected to take
two weeks with Gale's review of finish grade and
start of turf layout to begin Mid-July.
JTC 06/29/04 P62.2.1 Meeting - JTC stated C&I could not make the
Gale 06/29/04 meeting with Field Turf and Gale. JTC
Field Turf will reschedule.

Gale 06/22/04 P64. Emergency Access -JTC questioned whether ambulance
DDP access is possible to the football field, suggesting that the
stone dust paving behind the baseball backstop could be
used for access. DDP and Gale will review.
-- 06/29/04 P67.4.1 Change Proposal - JTC will price the work as
07/06/04 recommended by R. Weber (Postscript - Clerk in
07/13/04 a phone conversation with R. Weber on 06/30/04
clarified"gravel" material recommendation)so that
the SBC can act on the recommendations at the
07/08/04 meeting. It was agreed that proposal
shall include removing from the site the unuseable
fill, and not try to use it elsewhere on site.
Several options for disposing the material were
suggested to JTC.
(07/06/04) PCO 188 distributed and discussed at
job meeting 07/06/04. DDP and MBC will bring
proposal to SBC for authorization at 07/08/04
meeting.

JTC 06/29/04 P69. Crabapple Trees - CRJA reviewed problems observed
Emanouil with crabapple trees, noting that the trees planted were not
tagged by CRJA. In any case, die back has caused most
crabapples to no longer meet specifications and those must
be replaced. (Postscript: CRJA has flagged trees that must
be replaced). JTC will so advise Emanouil.
Emanouil 06/29/04 P70.1 Lawns at Arts Wing - JTC agreed that Emanouil
would repair the bare spots in the lawn between
the arts classrooms and the driveway.

Grasseschi 05/18/04 T30.2.1 Gas Shutoff Valve - DDP has been informed by
RWS that all their conversations with Grasseschi
were with regard to Grasseschi's request to
substitute a standard access panel for a valve
cabinet (which was done). RWS at no time
instructed Grasseschi that the door should have a
lock. Grasseschi is to remove lock and install latch
per detail on 8.0.3':
Grasseschi 04/13/04 T33. Photo Dark Room - Greg Roberts advised that backflow
preventer has been installed, but exposed hot and cold
water piping must be insulated.
Grasseschi 05/04/04 T35. Push Button Faucets - Greg Roberts reported that kids have
been taking the push buttons off of the lavatory metering
faucets. Upon examination, NASD found that set screws
were not tightened. Grasseschi to tighten set screws, and
shall provide NASD with spare knobs.
Grasseschi 06/04/04 T36.1 Hot Water (Postscript) - RWS memo distributed
on 06/07/04. Grasseschi to advise on system
balancing.
DDP 06/08/04 T36.2 Hot Water- Greg Roberts questioned type of
RWS tanks and piping arrangement, wondering if this
would play some part in the hot water circulation
issue. JTC stated tanks were piped per RWS
directives in response to a question from
Grasseschi. DDP will review further with RWS.

starter required for two fans.
Sagamore 06/15/04 U34.1.2 Mainstreet Fin Tube - DDP reviewed conditions
and found that fin tube cover has been
adequately screwed to support structure.
Sagamore shall consistently cope the end covers
tight to columns, concealing the piping.
F&T 03/16/04 U35. Kitchen ElevatorShaft- It was discussed that even with
keeping the boiler room doors closed, a strong down draft is
keeping the elevator shaft cold in the kitchen. DDP will
review with F&T.
-- 06/15/04 U37.3.1 ERU-2- Sagamore advised that the coil will be
06/22/04 replaced, which will necessitate using a crane.
Sagamore will advise on lead time.
(06/22/04) Sagamore reported new cooling coil will
be delivered these weeks from 06/21/04, and

be balanced, a temporary flow must be created
until the coil in ERU-2 is replaced. Sagamore

Sagamore 06/15/04 U37.6.2 Systems Balancing - F&T has returned the report
06/22/04 for revisions. At the 06/15/04 meeting, F&T noted
that information for pumps and several fans was
not provided. Furthermore, the water system
must be rebalanced following the recommissioning
of the control valves.
(06/22/04) Sagamore reported balancing will be
completed by 07/16/04.
Sagamore 04/20/04 U41. Pipe Insulation - It was noted that there is radiator piping
uncovered at the upper west end of the gym and at the shot
feeder system.
Sagamore 06/22/04 U41.2 Pipe Insulation - Sagamore stated
07/13/04 repair/completion of insulation will start on

(07/13/04) Sagamore stated insulation will be

Sagamore has repaired the leak between
sections. However, the boiler has not since been
fired to determine if repair will hold.
Sagamore 06/1:5/04 U46.3 HVAC System Completion - Sagamore Stated
that they are committed to seeing that the
outstanding issues are addressed and that the
system functions as designed. Sagamore could
not, at the 06/15/04 meeting, commit to a timetable
for completion because Sagamore needs input
from its subs and suppliers. However, Sagamore
will issue a written response to the issue including
a timeline no later than 06/18/04. It is agreed that
all work must be addressed by 09/01/04.

RDK 07/13/04 U48. Commissioning - RDK will require JCI and Sagamore's

JTC 12/16/03 V56. Gym Lightning Protection -The clerk noted that the recent
DDP heavy snows have apparently loosened the lightning rods
on the low eave of the gym. Conditions will be monitored to
determine if a snow fence is needed over the entire roof
edge.
3Si 03/23/04 V59.3 Room 3011 - 3Si has a recording meter on the
outlets in IDF 3011 due to three UPS units being
damages.
3Si 05/11/04 V59.3.1 Room 3044 - Paul Szymanski noted a power
TEC issue has also been observed at V/D room 3044.
DDP reported that 3Si and TEC are working to
isolate and identify the problem, but nothing has
been discovered that can be positively
addressed.
NAHS 05/18/04 V60.1 Knox Box (Postscript) - In a phone conversation
with Vinny Valentino, it was agreed that Vinny
would order the knox box.
ABP 03/16/04 V61. Gym Backboard Switches - NASD advised that the key
switches are breaking because the switch plate is
improperly punched, and the switch rotates with the turn of
the keys. ABP to correct.
Pappas 03/23/04 V61.1 Gym Backboard Switches - JTC advised that
switches have been temporarily repaired, with
new switches on order. NASD advised that west
backboard does not retract. JTC will review with
Pappas.
ABP 06/08/04 V64.4 Gym Lights (Postscript) - ABP advised JTC,
Lithonia DDP and TE on 06/10/04 that high/low gym
lighting system operates except for automatic
lock-out to prevent switching from high to low until
lamps have been at full power for 15 minutes.
Lithonia has been notified.
ABP 04/27/04 V68. Kitchen Door Bell - In response to Greg Roberts' question,
DDP stated the contract drawings specify a door bell at the
service entrance. ABP has not completed this work.

ABP 05/11/04 V69.3.1 Wiring Worksmanship -JTC reported that ABP
TE has examined the photos taken, and ABP has
noted that most of the photos were of security,
not fire alarm wiring. ABP is correcting above
ceiling wiring workmanship. DDP stated that once
ABP advises that conditions are corrected,
Thompson Engineering will re-inspect.
TE 05/04/04 V69.4 Gamewell Comments -TE will call Gamewell to
discuss questions as to system wiring, how
wiring can affect panel performance, and to confirm
Gamewell's recommendations and requirements.
ABP 05/18/04 V70. Duct Smoke Detector Testing - DDP advised JTC that RDK
RDK wishes to schedule the testing of the duct smoke detectors.
While NAFD tested the smoke detectors to see if they signal
an alarm, they have not been tested to see if they will shut
down the HVAC equipment. JTC will advise ABP to
schedule the testing.
ABP 06/01/04 V71. Outstanding Electrical Issues - JTC has written to ABP
detailing issues that must be addressed. ABP to respond.
JTC 06/22/04 V71.2 Outstanding Electrical Issues - JTC advised that
ABP was unable to attend today's meeting. JTC
stated that they will be addressing in a letter to
ABP their abandonment of the job, listing several
issues still not addressed.
DDP 06/22/04 V71.2.1 Monetized Electrical Punchlist- DDP will forward
to JTC the recently reinspected and monetized
punchlist from TE. The clerk is reviewing
completion of teacher punchlist electrical items.
JTC 06/29/04 V71.3 ABP Default - It was discussed that JTC has
been unable to reach anyone at ABP to get a
response on addressing outstanding issues and
that they may no longer be financially viable and
able to complete the project. JTC will be
contacting ABP's bonding company in order to
bring open electrical issues to closure, and will
report further.
-- 07/13/04 V71.5 ABP Default- JTC is requesting immediate
workers from ABP's bonding company to
complete the punchlist items. JTC is expecting an
answer by the end of the week.

Bass 08/26/03 X48. Sarnafil Roof Flashing - DDP, JTC and Stanley discussed
Stanley roof edge detail. DDP explained that Work Order required
JTC, Stanley and Bass to coordinate respective work. It
was discussed and agreed that Boss would provide
Stanley with stock so Stanley can form "snap on"trim. DDP
stated, in principle, this would be additional work by
Stanley, and less work (credit) by Bass (but no net change
in cost to Owner).
JTC 04/13/04 X61.2.1 Gym Curtain -JTC stated parts are on hand, and
ABP 05/04/04 JTC will coordinate installation with school use of
Papas 05/25/04 gym.
06/01/04 (04/13/04) JTC advised that installation is
ongoing.
(05/04/04) NASD reported that curtains drop, but
won't raise up. ABP and Papas have been
advised to correct.
(05/25/04) JTC reported that curtain
manufacturers data specify motor voltage to be
120v, however, motors require 240v. JTC is
coordinating with Pappas and ABP.
(06/01/04) JTC advised that Papas will correct
the voltage problem working with ABP. Curtains
will be raised for graduation.
JTC 04/27/04 X66.1 Lecture Hall Screen (Postscript) - Per Section
ABP 11130, switch and box are provided by screen
manufacturer, and wired by 16101-ELECTRICAL.
ABP 05/18/04 X66.2 Auditorium Screen - DDP advised JTC that the
JTC screen control switch is missing, and is to be
furnished under SECTION 11130 and installed
by ABP.
JTC 04/27/04 X67.05.1 Boiler Room - JTC advised that they believe the
Sub 05/25/04 water is entering the boiler room at the
06/01/04 brick/foundation wall joint. JTC is installing a
06/08/04 french drain, to be tied into the vertical clean out
pipe at the corners. Wall penetrations still are to
be made watertight by the respective subs.
(05/04/04) JTC stated supplemental drainage has
been installed, with no further leaking at wall
observed. JTC advised that one irrigation line
penetrating the wall must still be made to be water
tight.
(05/25/04) JTC noted that some leaking was
observed at Stair#8 after yesterday's rains.
(06/01/04) JTC stated no leaks at Stiar#8 were
observed after the recent rain.
(06/08/04) JTC stated no leaks were observed
the past week.

questions, JTC stated that all leaks have been

a leak at the east end of the cafeteria has
reappeared. JTC will investigate.
Musco 05/11/04 X69.1.3 Lighting Submission - After discussion, it was
05/18/04 agreed that to expedite lighting installation, Musco
submission is,"approved as noted - contingent on
satisfactory compliance with specified lighting
criteria". Musco shall re-submit for record.

Planning Board Notice of Decision dated 5/14/01
Schofield Brothers of New England,Inc. has been designated as the"Environmental/Erosion Control
Monitor"for the North Andover High School Project. The purpose of the Environmental/Erosion Control
Monitor is to perform periodic inspections of the project site to insure compliance with the Order of
Conditions issued by the Conservation Commission and the Special Permit issued by the Planning Board with
respect to sediment and erosion controls,limit of work compliance,wetland replication and stormwater
management issues.
This is the thirty first monthly inspection report prepared and submitted in compliance with the above
referenced permits. It covers the period May 26,2004 through June 29,2004. Attached are copies of the
Inspection Reports(#123,#124,#125,#126,and#127)relative to the inspections performed on June ls` 8`h
15°i,22nd,and 29t' 2004.
The month of June was warm with normal amounts of rainfall. Overall,the erosion and sedimentation
controls functioned properly during the past month.Considering the amount of site work and exposed soils in
the southern work area,the site remained in good and stable condition. Construction of the athletic fields was
proceeding in the southerly and northern work area (athletic fields).
Section A. Items completed:
1. Sediment protection has been maintained on the catch basins along the west side haul road and on the
catch basins by the southeastern entrance to the southerly work area.
2. Drainage manhole#13 has been repaired.
3. The contractor has continued to move material from both the loam stockpile and the ordinary borrow
stockpile.
4. Sediment basins#2 and#3 have now been taken out of service and are being filled for the parking lot
construction as planned.
5. Sediment basin#5 has been filled as planned for the athletic field construction and is no longer necessary.
This temporary basin functioned well during the initial work in the athletic fields.

Section B Items from previous reports and the most recent report that remain to be addressed:
1. The perimeter barriers in the northern athletic field area need to be refreshed,now that work has started
in northern field area.
2. Trapped sediment in Catch Basin#38 needs to be removed from the filter fabric to allow the basin to
function and sediment protection needs to be added to catch basin#44.
3. Sediment needs to be removed and the filter fabric reset at each of the protected catch basins in the west
side haul road.This will be an ongoing maintenance item while hauling operations occur.
4. West side Haul route needs to be swept.
5. Construction trash was observed and should be picked up.
6. Tracking of mud on Chickering Road and Prescott Street needs to be swept.This will be an ongoing
maintenance issue while hauling operations continue.
7. Drainage manhole#12 is in need of repair.
Based on our inspections,we have found that the work on the project has been performed in general
compliance with the Special Permit and Order of Conditions.We have found that the controls have functioned
properly and there have been no observed adverse affects to the resource areas. The contractor needs to
address some outstanding issues.
Over the next month,work is scheduled to move quickly in the southern athletic fields.The southerly drain
system and parking area will be proceeding. It is anticipated that most of the soil stockpiles will be completed
in July so that work will proceed on the northerly athletic fields.
The attached inspection reports provide detailed information as to our findings.Please let us know if there are

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to compliance
with the Orders of Conditions issued by the North Andover Conservation Commission and the
Special Permit issued by the North Andover Planning Board with respect to sediment and erosion
controls,limit of work compliance,and stormwater management issues. For reference,the
Orders of Conditions issued for the project include:
• DEP File No.242-1045: Issued 2/21/01 with Amendment 2/19/04 Extended 3/17/04
• Planning Board Notice of Decision: Issued 5/14/01 Modification Approval 2/17/04
Please contact Fred King or Matt Smith at Schofield Brothers of New England,Inc. if you have
any questions regarding the information contained in this report.
During this inspection, it was partly cloudy and warm. Construction of the southerly and
northerly athletic fields is proceeding along with the removal of excess loam from the site and
moving of ordinary borrow from the north stock pile to the southerly construction. The drainage
outfalls to Cochichewick Brook were clear.The sheen previously observed in the watercourse
along the tennis courts is gone. Overall,the erosion and sediment controls appear to be
functioning. Some maintenance and repairs of erosion controls in parts of the site are necessary
as listed below. The west side haul route needs to be swept and dust is beginning to become a
problem.Along with the sweeping of the west side haul route there is also a manhole along the
haul route that is in need of repair.Refurbishing of the perimeter siltation barrier to east of the
northerly athletic fields is now necessary.

INSPECTION SUMMARY
1. Sediment control barriers around the perimeter of the current work area:
• 06/01/04—Several sections of sedimentation barrier along the entrance off Osgood
Street have been damaged from snowplowing operations and will need repair(carried
over on report since 03/02/04). Sediment barrier east of the loam and ordinary stockpile
needs to be reestablished prior to the grading work planned for the northern athletic
fields.
• 06/08/04—Same as previous report
• 06/15/04- Same as previous report plus a short section if the haybale/silt fence barrier
needs to be repaired adjacent to the westerly side of the stone filter berm along the new
security fence in the southern work area. The filter berm is in good condition.
Also as noted in the introduction, large areas of the easterly side of the site are now vegetated
and in a stable condition. In those areas where no further earthwork is to be done,the
perimeter erosion controls will no longer need routine maintenance. Planning for removal of
those controls could proceed following inspection and approval by the Conservation
Commission and applicable officials.
• 06/22/04—Same as previous report.
• 06/29/04—Same as previous report.Now that work on the northerly athletic fields has
begun, the sediment barrier in the northern area needs to be addressed immediately.
2. Loam stockpile area in the northern portion of the site:
• 06/01/04—The loam pile is being removed. The loam pile is stable with vegetation at this
time. The contractor stated the removal of the loam should be increasing. This area will be

During the inspection it was overcast and had rained the night before. The recent weather conditions have
slowed progress on the loam removal and construction of the athletic fields.The drainage outfalls to
Cochichewick Brook were clear.The recently constructed erosion and sediment controls in the southerly
work area appeared to be fimetioning properly. Some maintenance and repair of erosion controls in other
parts of the site is necessary as listed below.There is a large amount of sediment that has accumulated in
the vicinity of catch basin 12 and drainage manhole 13 this needs to be cleaned up.Drainage manhole 13
has been damaged is in need of repair. Construction of the stormwater galley and sub-drain system is
continuing and is near completion.The west side haul route,the southeastern entrance to the southern work
area,and the mud tracked on to Prescot Street and Chickering Road needs to be swept.Back filling
operations continued in the southerly work area for the construction of the athletics fields.

proceeding along with the removal of excess loam from the site and moving of ordinary borrow
from the north stock pile to the southerly construction. The drainage outfalls to Cochichewick
Brook were clear. Overall,the erosion and sediment controls generally appear to be functioning
properly One exception is noted in the report that needs repair. Some maintenance and repairs of
erosion controls in other parts of the site are necessary as listed below. The west side haul route
needs to be swept and dust is beginning to become a problem as the site drys out.
Major portions of the easterly side of the site are now vegetated and stable. Continued

truck entrance,which has had a gravel berm installed. Sediment basin#5 was in good
condition and functional.
• 06/08/04—Sediment basin#5 has been filled along with the diversion berm itself as planned.
The contractor stated that fill/grading will continue over the upcoming weeks.
• 06/15/04—Sediment Basins 4 and 5,and the diversion berm are now eliminated to bring the
areas to finished grade. Perimeter erosion controls including the stone filter berm will now
be effective for the southern area. (Record Item).

06/15/04—Northerly Stone Interceptor Drain: Grading is taking place on the north side
of the interceptor drain installation without erosion controls to protect the drain
surface. The catch basin inlets are protected with fabric but Sediment has covered
portions of the stone drain. This will require replacement of the top stone and filter
fabric.
10. Wetland replication areas:
• 04/20/04—Some newer construction trash was observed in the west side this was brought to
the attention of the contractor. The contractor stated that deer ate some of the wetland
plantings that need replacing and alternative plantings should be considered.
• 06/01/04—Trash is not yet picked up along the area cited above. This should be attended
to. The contractor also stated that the dead plants in the replication areas would be replaced in

Plan. Suggestions were made for additions and modifications to the Plan. SBNE will
incorporate the recommendations into the plan for further review.
A plan was also reviewed that shows the locations for permanent markers along the 25-foot
no disturbance buffer zone. The plan showed that 96 markers would be necessary to comply
with the Condition 51 of the Order of Conditions. Due to the necessary locations, it was
suggested that this would be best done as one of the last items of site work. This will need to
be discussed with the Conservation Commission. In addition, it appears to be appropriate in
some areas that these same posts be used for"No Snow Disposal"signage to comply with
Condition 52 of the OOC.
Also discussed were the installation of 35 Bird Boxes and the "non-invasive trail system"per
Conditions 35 and 37 of the OOC.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to compliance
with the Orders of Conditions issued by the North Andover Conservation Commission and the
Special Permit issued by the North Andover Planning Board with respect to sediment and erosion
controls, limit of work compliance, and stormwater management issues. For reference,the
Orders of Conditions issued for the project include:
• DEP File No. 242-1045: Issued 2/21/01 with Amendment 2/19%04 Extended 3/17/04
• Planning Board Notice of Decision: Issued 5/14/01 Modification Approval 2/17/04

During this inspection, it was overcast and it was lightly raining. Construction of the southerly
athletic fields is proceeding along with the removal of excess loam from the site and moving of
ordinary borrow from the north stock pile to the southerly construction. The drainage outfalls to
Cochichewick Brook were clear. A small amount of surface sheen was observed on the surface of
the watercourse that flows by the tennis courts and merges with Cochichewick Brook. The source
of this could not be determined. Overall,the erosion and sediment controls generally appear to be
functioning. Some maintenance and repairs of erosion controls in other parts of the site are
necessary as listed below. The west side haul route needs to be swept and dust is beginning to
become a problem. Along with the sweeping of the west side haul route there is also a manhole
along the haul route that is in need of repair.

Major portions of the easterly side of the site are now vegetated and stable. Continued
maintenance of the erosion controls in these areas is no longer necessary. Maintenance would
still be required in any sections of this area where some finish work is still planned, such as near

Schofield Brothers of New England,Inc. has been designated as the`Environmental/Erosion
Control Monitor"for the North Andover High School Project. The put-pose of the Environmental/
Erosion Control Monitor is to perform periodic inspections of the project site to insure compliance
with the Order of Conditions issued by the Conservation Commission and the Special Permit issued
by the Planning Board with respect to sediment and erosion controls, limit of work compliance,
wetland replication and stormwater management issues.
This is the thirty-second monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period June 30, 2004 through July 27, 2004. Attached are
copies of the Inspection Reports (#128,#129,#130, and#131)relative to the inspections performed
on July 6`h, 13`h, 20`h, and 27`h,2004.
The month of July was warm with normal amounts of rainfall. Overall,the erosion and
sedimentation controls functioned properly during the past month. Considering the amount of site
work and exposed soils in the southern work area,the site remained in good and stable condition.
Construction of the athletic fields was proceeding in the southerly work area with the artificial turf
being installed. The work also began in the northern athletic field.
Section A. Items completed;
1. Sediment protection has been maintained on the catch basins along the west side haul road and
on the catch basins by the southeastern entrance to the southerly work area.
2. Drainage manhole#12 has been repaired.
3. The contractor has added protection to Catch Basin#44 and removed sediment from the
protection on Catch Basin#38.
4. Tracked mud was observed on Prescott Street, Chickering Road and the southeastern entrance
drive to the southern work. This has been swept. The stone construction entrance has been
maintained.
5. Some of the perimeter sedimentation barrier to the east of the northerly athletic fields has been
replaced.

7. According to the contractor,the dead plants in the buffer zone planting area have been
replaced.
Section B Items from previous reports and the most recent report that remain to be addressed:
1. A short section of the haybale/silt fence barrier needs to be repaired adjacent to the westerly
side of the stone filter berm along the new security fence by the entrance of the southern work
area.
2. It is recommended that protection be added to the freshly graded hillside facing Chickering Rd
by the southwest corner of the new school building to secure the new loam.
3. The southeastern entrance drive to the southern work area needs to be swept.
4. Sediment needs to be removed and the filter fabric reset at each of the protected catch basins in
the west side haul road. This will be an ongoing maintenance item while hauling operations
occur.
5. West side Haul route needs to be swept.
6. Construction trash was observed and should be picked up.
7. Tracking of mud on Chickering Road and Prescott Street needs to be swept. This will be an
ongoing maintenance issue while hauling operations continue.
Based on our inspections,we have found that the work on the project has been performed in general
compliance with the Special Permit and Order of Conditions. We have found that the controls have
functioned properly and there have been no observed adverse affects to the resource areas. The
contractor needs to address some outstanding issues.
Over the next month,work is scheduled to move quickly. The southerly athletic fields and parking
area will be continuing. It is anticipated that the paving of the track and finishing of the drainage
system for the northerly athletic fields will be completed in August.
The attached inspection reports provide detailed information as to our findings. Please let us know

Special Permit issued by the North Andover Planning Board with respect to sediment and erosion
controls, limit of work compliance, and stormwater management issues. For reference,the
Orders of Conditions issued for the project include:

During this inspection, it was sunny. Construction of the southerly and northerly athletic fields is
proceeding with some of the northern athletic field drainage system being installed. The drainage
outfalls to Cochichewick Brook were clear. Several outstanding erosion control maintenance
items from last week's report have been addressed. These are listed below. The west side haul
route and all entrances to the construction site have been swept but this must be maintained.
Some of the perimeter sedimentation barrier to the east of the northerly athletic fields requires
more work. The dead wetland plantings have been replaced.

During this inspection, it was partly cloudy and warm. Construction of the southerly and
northerly athletic fields is proceeding along with the removal of excess loam from the site and
moving of ordinary borrow from the north stock pile. The drainage outfalls to Cochichewick
Brook were clear. All the erosion control maintenance items from last weeks report r6main to be
addressed. These are listed below. The west side haul route and all entrances to the construction
site need to be swept and dust is a problem at this time. Along with the sweeping there is also a
manhole along the haul route that is in need of repair. Refurbishing of the perimeter siltation
barrier to east of the northerly athletic fields is now necessary.

INSPECTION SUMMARY
1. Sediment control barriers around the perimeter of the current work area:
• 07/06/04—Several sections of sedimentation barrier along the entrance off Osgood
Street have been damaged from snowplowing operations and will need repair(carried
over on report since 03/02/04).Now that work on the northerly athletic fields has begun,
the sediment barrier in the northern area needs to be addressed immediately.In
addition, a short section of the haybale/silt fence barrier needs to be repaired adjacent
to the westerly side of the stone filter berm along the new security fence by the entrance
of the southern work area.
2. Loam stockpile area in the northern portion of the site:
• 07/06/04—The loam pile is being removed and there are no sedimentation or erosion issues
with the loam pile at this time.
3. Ordinary borrow stockpile area in the northern portion of the site:
• 07/06/04—The Ordinary Borrow pile is almost completely removed.
4. Main entrance gate near trailers:
• 07/06/04—Tracked mud was observed on Prescott Street, Chickering Road and the
southeastern entrance drive to the southern work.This needs to be swept.The stone
construction entrance needs maintained.
5. Catch basins:
• 07/06/04—The catch basins along the west haul route are in need of maintenance and
will need continued maintenance during the stockpile hauling operations. (See Item 9)
Catch basins#38 needs sediment removed from the filter fabric protection.It was
recommended that protection be added to Catch Basin#44 to prevent sediment from
entering it during the back filling operations in the southerly work area. Catch Basin
#45 also needs maintenance.
6. Sediment basins#2 and#3, and outlet structures:
• 06/15/04—Both of these sediment basins have now been taken out of service and are being
filled for the parking lot construction as planned. (Record Item).
7. Temporary Sediment Basins#5 outlet structures and diversion berm in south work area:
• 06/15/04—Sediment Basins 4 and 5, and the diversion berm are now eliminated to bring the
areas to finished grade. Perimeter erosion controls including the stone filter berm will now
be effective for the southern area. (Record Item).
8. Other erosion control items.
• 07/06/04—It is recommend that hay bales be installed along the chain lint:fence by the
southeast entrance to the southern fields it is also recommend that this area be swept
prior to the installation of these hay bales. It is recommended that protection be added
to the freshly graded hillside facing Chickering Rd by the southwest corner of the new
school building to secure the new loam.
9. Other drainage structures:
• 07/06/04—Drainage manhole 12 has been damaged during the hauling operations.
Repair is needed. (Item carried over since 6/22/04)

10. Wetland replication areas:
• 04/20/04—Some newer construction trash was observed in the west side this was brought to
the attention of the contractor. The contractor stated that deer ate some of the wetland
plantings that need replacing and alternative plantings should be considered.
• 06/01/04—Trash is not yet picked up along the area cited above. This should be attended
to. The contractor also stated that the dead plants in the replication areas would be replaced in
the next few weeks.

11. Impermeable barrier and easterly embankment:
• 06/01/04—No seepage observed. Area appears stable at this time.(Record Item)
12. Dewatering:
• 07/06/04—No dewatering was observed.
13. Truck Washing Area:
• 07/06/04—No truck washing was observed.
14. Dust Control:
• 07/06/04—Significant dust was observed during this inspection and is an issue at this time.
Measures need to be taken to minimize this. The west side haul route is in needs of
sweeping along with the entrances to the site to help control dust.
15. Wetland Delineation:
• 07/06/04—Nothing to report at this time.
16. Inspection Items by Others:
• 04/27/04—(Postscript)A meeting was held on Thursday 4/29/04 with Paul Szymanski
(NASD),John Cyr(DPW)and Fred King(SBNE)to review the preliminary Snow Disposal
Plan. Suggestions were made for additions and modifications to the Plan. SBNE will
incorporate the recommendations into the plan for further review.
A plan was also reviewed that shows the locations for permanent markers along the 25-foot
no disturbance buffer zone. The plan showed that 96 markers would be necessary to comply
with the Condition 51 of the Order of Conditions. Due to the necessary locations,it was
suggested that this would be best done as one of the last items of site work. This will need to
be discussed with the Conservation Commission. In addition, it appears to be appropriate in
some areas that these same posts be used for"No Snow Disposal'signage to comply with
Condition 52 of the OOC.
Also discussed were the installation of 35 Bird Boxes and the"non-invasive trail system"per
Conditions 35 and 37 of the OOC.
• 05/04/04—Postscript:Inspection by Arrmv Environmental. The Items that needs to be
completed are as follows:
I -Pre-construction requirements:
• Bird Boxes: This is a requirement for the Applicant to comply with prior to a
certificate of compliance.
• Snow Stockpiling Plan: A draft plan has been prepared for review.
• No Disturbance Zone markers:Not completed at this time. A plan has been

questioned why "scissors" type used at the
admin wing are being replaced with shorter arms

DDP 03/30/04 B50. Gymnasium Switches - Greg Roberts requested that DDP
TE investigate substituting pushbutton switches for gym
lighting, curtain, and backboard counters. GR noted that the
key switches installed are all keyed differently and require a
good deal of time to operate individually. The pushbuttons
could be secured from unauthorized use by a surface
mounted locking cabinet. Also, GR requested that the gym
lighting only be operated by the center control point. DDP
and TE to review.
3S1 04/20/04 B52. Intercom - NASD advised that the intercoms cannot be heard
ESCOA in rooms 3004 and 3006. 3SI has been advised, and it is
thought that this is a cabling issue to be addressed by
ESCOA.

overtopping the swale on the north side of the
driveway during the heavy rains. This will be

contract work must pull a permit. JTC will
coordinate.
JTC 07/27/04 C122. Erosion Control - SBNE reported that maintenance work has
been performed in the last week, and no severe conditions
exist at this time. SBNE advised JTC of some CB
maintenance items, and noted that some erosion controls
need to be addressed at the southwest corner of the site at
Chickering Road.
JTC 07/27/04 C123. Auditorium Electrical Inspection - It was discussed that a final
inspection by the town inspector is needed. JTC will
schedule with the electrical inspector.

DDP 03/11/03 P11.3 Art Wing Storm Drainage - JTC submitted PCO
Clerk #066 to repipe roof drains to manhole 2B. DDP to
review with clerk.
JTC 09/30/03 P41. Art Plaza Pavement- DDP reviewed CRJA site visit report
of 09/26/03. JTC stated radius will be cut. The clerk
observed that the puddles noted disappeared in less than
an hour, and low spots are very shallow.
DDP 10/08/03 P41.1 Arts Plaza Pavement (Postscript) - CRJA
Clerk followed up, advising DDP that low spots would
be an icing concern, and requested that the
tolerances be checked per the paving spec (See
Spec 02500-3.01-19). DDP to advise clerk to
confirm.
JTC 03/16/04 P49. Catch Basins - SBNE noted that JTC has yet to install the
hoods on the catch basin structures as required.
NASD 04/20/04 P52. Commemorative Plantings - Paul Szymanski advised that
DDP the school will be planting some memorial trees, and is
looking into some way to incorporate future memorials in an
orderly, planned fashion. DDP and NASD will coordinate.
-- 04/27/04 P53. Athletic Fields - DDP, MBC, JTC, and Gale reviewed the
athletic field upgrade work, with the following noted.
DDP 04/27/04 P53.8 Scoreboards - JTC reiterated that their proposal
includes two scoreboards (one football, one track)
with conduits to a third "future" location (in
baseball field). DDP will confirm locations.
DDP 05118/04 P53.8.1 Scoreboard - It was agreed that football
scoreboard will be relocated to the est end of the
football field, back-to-back with the "future"
baseball scoreboard. DDP will issue a field order
locating this scoreboard.
NASD 07/20/04 P53.8.2 Track/Softball Scoreboard - It was discussed that
07/27/04 the track scoreboard could be fed from the
construction trailer service on Presscott. DDP will
consult with Thompson Engineering to provide
Paul Szymanski with the information needed to
obtain a permanent service from this location from
Mass Electric.

recommended sand mix ratio. Follow up with P&S

P&S 06/29/04 P56.3.4 Mixing Sand and Loam - JTC noted that Section
02900 specifies mixing sand and loam by
harrowing, and at this time this is JTC's intent.
Bob Pine stated that more satisfactory results will
be achieved far more easily by mixing with loader
buckets after the loam is screened. P&S will come
to the site to monitor JTC's mixing of the sand and
loam to establish a procedure and consistant
result.
JTC 06/29/04 P56.3.6 Drainage - JTC and DDP described how the field
CRJA sub grade for drainage as shown on the contract
drawings no longer reflects the final grades due to
changes made necessary by the athletic field
upgrades. CRJA agreed that adjustments to
J-drain and collector pipe layout would be
acceptable as long as"downhill" criteria is met.
CRJA will inspect subdrainage when completed,
and before being covered with sand, to see that
this criteria is met.
Gale 06/22/04 P64. Emergency Access - JTC questioned whether ambulance
DDP access is possible to the football field, suggesting that the
stone dust paving behind the baseball backstop could be
used for access. DDP and Gale will review.
JTC 06/29/04 P68.1 Leaning Trees - CRJA expained that they do not
Emanouil- require trees to be guyed because studies have
shown that unguyed trees are stronger. However,
the spec does require trees to be plumbed if
required. JTC will advise Emanouil to reset the
trees.
-- 06/29/04 P69. Crabapple Trees - CRJA reviewed problems observed
with crabapple trees, noting that the trees planted were not
tagged by CRJA. In any case, die back has caused most
crabapples to no longer meet specifications and those must
be replaced. (Postscript: CRJA has flagged trees that must
be replaced). JTC will so advise Emanouil.
CRJA 07/30/04 P69.1 Crabapple Trees (Postscript) - Response from
Emanouil received by DDP and forwarded to
CRJA on 07/30/04. CRJA to respond:
Emanouil 06/29/04 P70.1 Lawns at Arts Wing -JTC agreed that Emanouil
would repair the bare spots in the lawn between
the arts classrooms and the driveway.

(installation). DDP and JTC will inspect entire
roof perimeter and list areas be reworked.
(04/27/04) JTC advised that Bass will likely not
perform any of the repairs previously reviewed
without a formal punchlist.
(07/20/04) Bass, JTC, DDP and clerk met to
review outstanding cladding items. DDP
reviewed with Bass cladding issues from exterior
punchlist inspection. Bass will be on site the
week of 07/26/04 to address outstanding items.
Bass will forward to DDP a drawing of proposed
cladding between clerestory and curtainwall on
north side of arts wing. It was agreed that brake
metal backed by polystyrene insulation would be
used in this area.

Hampshire technician on site feels the reported

Grasseschi 05/18/04 T30.2.1 Gas Shutoff Valve - DDP has been informed by
RWS that all their conversations with Grasseschi
were with regard to Grasseschi's request to
substitute a standard access panel for a valve
cabinet (which was done). RWS at no time
instructed Grasseschi that the door should have a
lock. Grasseschi is to remove lock and install latch
per detail on 8.0.3.
Grasseschi 04/13/04 T33. Photo Dark Room - Greg Roberts advised that backflow
preventer has been installed, but exposed hot and cold
water piping must be insulated.

Grasseschi 05/04/04 T35. Push Button Faucets - Greg Roberts reported that kids have
been taking the push buttons off of the lavatory metering
faucets. Upon examination, NASD found that set screws
were not tightened. Grasseschi to tighten set screws, and
shall provide NASD with spare knobs.
Grasseschi 06/04/04 T36.1 Hot Water (Postscript) - RWS memo distributed

tight to columns, concealing the piping.
F&T 03/16/04 U35. Kitchen ElevatorShaft - It was discussed that even with
keeping the boiler room doors closed, a strong down draft is
keeping the elevator shaft cold in the kitchen. DDP will
review with F&T.
-- 06/15/04 037.3.1 ERU-2- Sagamore advised that the coil will be
06/22/04 replaced, which will necessitate using a crane.
07/20/04 Sagamore will advise on lead time.

Sagamore 06/15/04 037.6.2 Systems Balancing - F&T has returned the report
06/22/04 for revisions. At the 06/15/04 meeting, F&T noted
07/20/04 that information for pumps and several fans was
not provided. Furthermore, the water system
must be rebalanced following the recommissioning
of the control valves.
(06/22/04) Sagamore reported balancing will be
completed by 07/16/04.
(07/20/04) JTC advised that the last of the
balancing will be completed following the
replacement of ERU-2's coil this week.

uncovered at the upper west end of the gym and at the shot

Sagamore 06/15/04 046.3 HVAC System Completion Sagamore stated
that they are committed to seeing that the
outstanding issues are addressed and that the
system functions as designed. Sagamore could
not, at the 06/15/04 meeting, commit to a timetable
for completion because Sagamore needs input
from its subs and suppliers. However, Sagamore
will issue a written response to the issue including
a timeline no later than 06/18/04. It is agreed that
all work must be addressed by 09/01/04.

06/22/04 U46.5.1 Valves - Starting on 07/05/04, Sagamore, JCI and
07/13/04 Thomas Young (balancer) will be on site to
07/20/04 recommission valves and rebalance the water
07/27/04 system. Sagamore will be on site to repair piping,
etc. found to be necessary during the
recommissioning. By 07/16/04 the balancing will
be completed, and RDK can retest a sampling of
equipment.
(06/29/04) Greg Roberts has been informed by
JCI that work will not start on 07/05/04, but has
been rescheduled for 07/12/04. JTC will request
that Sagamore provide an updated schedule.
(07/13/04) Johnson is on site this week
recommissioning control valves. Thomas Young
will be balancing valves beginning 07/14/04.
(07/20/04) JTC reported that the last of the
valves will be completed this week.
(07/27/04) Sagamore stated all valves have been
replaced and RDK may begin recommissioning .

(06/2p/04) Greg Roberts reported Sagamore was
on site on 06/28/04, but was not on site on
06/29/04. JTC will contact Sagamore.
(07/13/04) Sagamore punchlist is completed to
date with the exception of the fin tube covers not
matching the fan coil units. Sagamore will be
replacing the fin tube covers in the weeks to
come.
(07/20/04) JTC reported Sagamore is still
investigating mismatched cover color issue.
(07/27/04) Sagamore advised that both the fin
tube covers and fan coil units on the first floor
were the colors selected by DDP during shop
drawing review. Following the job meeting, DDP
and JTC confirmed that the fin tube cover color
was the off-white color selected by DDP.
However, DDP and JTC agreed that the fan coil
units were not the off-white color selected by
DDP, but were insted the manufacturer's
"standard beige." As perviously stated by DDP,
the fin tube covers on the first floor must match the
adjacent FCU's. JTC will so advise Sagamore.
Sagamore 06/22/04 U46.5.3 Dust Collector- Panel voltage will be corrected on
07/13/04 06/29/04, and fittings completed.
(07/13/04) Dust collector controller is on site and
all that is left for completion is final electrical
connections.

Sagamore 06/22/04 U46.5.4 Water Treatment System - Sagamore stated
07/27/04 water was retested last week and found to have
29% glycol content(specified range is 31-33%)
Sagamore is prepared to add more glycol with the
rust inhibitor and other chemical additives that are
on site, and startup the system. Greg Roberts
noted that the system only ran for 24 hours after
the last glycol was added, and recommended that
the system be run longer to more thoroughly mix
the glycol.
(07/27/04) Sagamore reported water treatment
system work is completed, and that subcontractor
will be called back to site to retest after coil in
ERU-2 is completed.
Sagamore 06/22/04 U46.5.5 As Built Drawings - Sagamore has piping `as
builts' on a cad disk, and has received a second
copy of ductwork 'as builts' to be added to disk.
Sagamore 06/22/04 U47.1 ERU-2-GR recommended that Sagamore check
the outside air damper operator at ERU-2, after
recently observed high cooling air discharge
temperatures.
Sagamore 06/22/04 U47.2 Control Booth Split System - GR advised that
Sagamore should check piping joints for leakage.

07/27/04 check burners, noting odor associated with

Consumer Science classroom. DDP will review

Sagamore 07/27/04 U49.1.1 Condensation on Piping - DDP reviewed
observations that condensation is forming on
uninsulated strainers, etc. with F&T. F&T stated
that HVAC specification requires that the entire
piping system be insulated, and any valves,
strainers, etc. that are not now insulated shall be.

F&T that the specified 55°discharge air
temperature is a typo, and the temperature should
be 650-700. JTC advised Sagamore to confirm

DDP 07/27/04 U49.3.2 Pumps It was questioned whether pumps will
F&T meet the GPH requirements of the cooling coils in
the ERU's. DDP will review with F&T.
DDP 07/20/04 U49.4 Unoccupied Schedule - Greg Roberts questioned
F&T if unoccupied control sequence is contributing to
condensation. DDP will review with F&T.

3Si 03/23/04 V593 Room 3011 -3Si has a recording meter on the
outlets in IDF 3011 due to three UPS units being
damages.
3Si 05/11/04 V59.3.1 Room 3044- Paul Szymanski noted a power
TEC issue has also been observed at V/D room 3044.
DDP reported that 3Si and TEC are working to
isolate and identify the problem, but nothing has
been discovered that can be positively
addressed.
TE 05/04/04 V69.4 Gamewell Comments - TE will call Gamewell to
discuss questions as to system wiring, how
wiring can affect panel performance, and to confirm
Gamewell's recommendations and requirements.

down the HVAC equipment. JTC will advise ABP to
schedule the testing. Record item.

Maiuri that JTC's priority list is being addressed.
A meeting with the bonding company may be
necessary in the near future.
DDP 07/06/04 V72. Lamp List Greg Roberts requested a list of lamps used on
TE the project. DDP noted this is to be part of the electrical
O&M manuals, but with ABP's default DDP will request a list
from Thompson Engineering.
DDP 07/27/04 V74. Consession Stand Data/Power - In response to DDP's
3Si question, JTC reported that Systems Contracting has not
yet pulled the fire alarm cable to the concession stand.
Before they do; DDP`'stated that a communications cable
should be pulled with it. DDP will consult with 3Si and
advise as to the type of cable needed.

Bass 08/26/03 X48. Sarnafil Roof Flashing - DDP, JTC and Stanley discussed
Stanley roof edge detail. DDP explained that Work Order required
JTC, Stanley and Bass to coordinate respective work. It
was discussed and agreed that Boss would provide
Stanley with stock so Stanley can form "snap on"trim. DDP
stated, in principle, this would be additional work by
Stanley, and less work (credit) by Bass (but no net change
in cost to Owner).
JTC 05/25/04 X67.05.1 Boiler Room - JTC advised that they believe the
Sub 06/01/04 water is entering the boiler room at the
06/08/04 brick/foundation wall joint. JTC is installing a
07/20/04 french drain, to be tied into the vertical clean out
pipe at the corners. Wall penetrations still are to
be made watertight by the respective subs.
(05/04/04) JTC stated supplemental drainage has
been installed, with no further leaking at wall
observed. JTC advised that one irrigation line
penetrating the wall must still be made to be water
tight.
(05/25/04) JTC noted that some leaking was
observed at Stair#8 after yesterday's rains.
(06/01/04) JTC stated no leaks at Stiar#8 were
observed after the recent rain.
(06/08/04) JTC stated no leaks were observed
the past week.

JTC 05/04/04 X67.10 Roof Leak Status - In response to MBC's
questions, JTC stated that all leaks have been
permanently repaired, with the exception of
cupola doors, which are awaiting hardware and

JTC 06/22/04 X67.11 Cafeteria Roof Leak- Greg Roberts reported that
a leak at the east end of the cafeteria has
reappeared. JTC Will investigate.
Musco 05/11/04 X69.1.3 Lighting Submission -After discussion, it was
05/18/04 agreed that to expedite lighting installation, Musco
submission is "approved as noted - contingent on
satisfactory compliance with specified lighting
criteria". Musco shall re-submit for record.
(05/18/04) Musco to submit electrical controls per
TE comments.
