INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to compliance
with the Orders of Conditions issued by the North Andover Conservation Commission and the
Special Permit issued by the North Andover Planning Board with respect to sediment and erosion
controls,limit of work compliance, and stormwater management issues. For reference,the
Orders of Conditions issued for the project include:
• DEP File No.242-1045: Issued 2/21/01 with Amendment 2/19/04 Extended 3/17/04
• Planning Board Notice of Decision: Issued 5/14/01 Modification Approval 2/17/04

During this inspection,it was raining lightly. Overall,the site has shown very little erosion and
appears to be relatively stable. Site construction work is concentrated on the athletic fields. The
contractor has concentrated on the installation of track and field structures on the northern athletic
fields. Spreading of loam and grading has been completed in the southern athletic fields. The
installation of the infield of the baseball field is begining. More of the southern athletic field area
has been hydro seeded.The site had some sediment deposits on the paved areas. Sweeping is
recommended. The stone interceptor sub drain (gravel drain)needs to be cleaned out and
brought to finished condition. The drainage outfalls to Cochichewick Brook were clear.

INSPECTION SUMMARY
1. Sediment control barriers around the perimeter of the current work area:
• 11/02/04—There is a gap in silt fence by the easterly stream outfall and the silt fence is
damaged in this area.This should be repaired.The silt fence on the east side on the
northern athletic field was damaged during the spreading of loam. This must be
repaired until the area is stabilized.This area has also been hydro-seeded. (Item carried
over since 10/26/04)
2. Loam stockpile area in the northern portion of the site:
• 11/02/04—The loam pile is being removed and there are no sedimentation or erosion issues
with the loam pile at this time.
3. Main entrance gate near trailers:
• 11/02/04—The entrance to the school needs sweeping and the stone entrance needs
maintenance.(Item carried over since 10126/04)
4. Catch Basins:
• 11/02/04—Catch Basin#3 and#45 are in need of maintenance.New fabric had been
installed on Catch Basin#45 but was set incorrectly.
5. Other erosion control items.
• 11/02/04—Nothing to report.
6. Other drainage structures:
• 11/02/04—New rock has been added to the western stone interceptor sub drain(gravel
drain) but the contractor stated that the gravel had not been cleaned out.The east stone
interceptor sub drain(gravel drain)has been covered with loam and hydro-seeded.The
contractor was informed that this drain was not to be covered with loam and that the
gravel in both drains needs to be cleaned out and reinstalled to bring it to finished
condition.(Item carried over since 10/26104)
7. Wetland replication areas:
• 04/20/04—Some newer construction trash was observed in the west side this was brought to
the attention of the contractor. The contractor stated that deer ate some of the wetland
plantings that need replacing and alternative plantings should be considered.
• 11/02/04—Same as previous report.It appears that no effort has been made to clean up '..
the trash.This has been an issue for more then 6 months now and this needs to be
addressed.
8. Impermeable barrier and easterly embankment:
• 06/01/04—No seepage observed. Area appears stable at this time.(Record Item)
9. Dewaterinjz:
• 11/02/04—No dewatering was observed. '..
10. Truck Washing Area:

® 11/02/04—The site was not dusty due to rain but should be swept.
12. Wetland Delineation:
® 11/02/04—Nothing to report at this time.
13. Inspection Items by Others:
® 04/27/04—(Postscript)A meeting was held on Thursday 4/29/04 with Paul Szymanski
(NASD),John Cyr(DPW) and Fred King(SBNE) to review the preliminary Snow Disposal
Plan. Suggestions were made for additions and modifications to the Plan. SBNE will
incorporate the recommendations into the plan for further review.
A plan was also reviewed that shows the locations for permanent markers along the 25-foot
no disturbance buffer zone. The plan showed that 96 markers would be necessary to comply
with the Condition 51 of the Order of Conditions. Due to the necessary locations, it was
suggested that this would be best done as one of the last items of site work. This will need to
be discussed with the Conservation Commission. In addition, it appears to be appropriate in
some areas that these same posts be used for"No Snow Disposal" signage to comply with
Condition 52 of the OOC.
Also discussed were the installation of 35 Bird Boxes and the"non-invasive trail system"per
Conditions 35 and 37 of the OOC.
® 05/04/04—Postscript:Inspection by Arrow Environmental. The Items that needs to be
completed are as follows:
I -Pre-construction requirements:
• Bird Boxes: This is a requirement for the Applicant to comply with prior to a
certificate of compliance.
• Snow Stockpiling Plan:A revised draft plan has been prepared for review.

During this inspection,it was cold and partly cloudy. Overall,the site appears to be relatively
stable, with no signs of significant erosion or impact to wetland resource areas. At this time,the
site construction work is concentrated on the athletic fields.The contractor is working on the
installation of track and field structures on the northern athletic fields and the baseball facility in
the southerly fields. The slope from the lower parking lot to the southerly athletic field is now
fairly well vegetated. The stone interceptor sub drain (gravel drain) needs to be cleaned out
and brought to finished condition. The drainage outfalls to Cochichewick Brook were clear.

3. Main entrance gate near trailers:
® 11/09/04—Only minor tracking of dirt onto Prescott Street at this time. Stone entrance
needs maintenance. Contractor needs to continue to monitor this area while the loam
removal continues,particularly during wet weather.
4. Catch Basins:
® 11/09/04—Catch Basin#3 and#45 are in need of maintenance.New fabric had been
installed on Catch Basin#45 but it has some sediment buildup. Frequent maintenance
of this fabric is recommended to minimize ponding during rain storms.
5. Other erosion control items.
® 11/09/04—Grass on the slopes is becoming quickly established.
6. Other drainage structures:
• 11/02/04—New rock has been added to the western stone interceptor sub drain(gravel
drain) but the contractor stated that the gravel had not been cleaned out.The east
portion of the stone interceptor sub drain(gravel drain)has been covered with loam
and hydro-seeded.The contractor was informed that this drain was not to be covered
with loam and that the gravel in both drains needs to be cleaned out and reinstalled to
bring it to finished condition.(Item carried over since 10126/04)
® 11/09/04—Same as previous report. We also noted a grading problem with the
driveway to the northerly athletic fields relative to the stone interceptor drain. The
driveway was supposed to drain to interceptor drain. However the as-built grades
appear to prevent this and the runoff drains to the main perimeter driveway,and a
portion drains to Chickering Road. This should be carefully looked at to determine
how this is to be corrected.
The issues in this section are very important and need to be addressed as soon as possible.

established at this time. We recommend a follow-up vegetation assessment in the spring to

® 11/09/04—Minor dusty condition was noted when construction vehicles used the northerly
access road. This should continue to be monitored.

the southerly work area and in the open area east of the northerly athletic field.

During this inspection,it was sunny and clear. This inspection occurred during a site walk that I
preformed with a town conservation agent,Pamela A. Merrill.The agent stressed the importance
of the installation of the No Disturbance Zone inarkers. These markers must be installed
A.S.A.P. Overall, the site appears to be relatively stable, with no signs of significant erosion or
impact to wetland resource areas. At this time, the site construction work is still concentrated on
the athletic fields. The contractors work continues on the installation of track and field structures
on the northern athletic fields and the baseball facility in the southerly fields. Repairs have been
made to the perimeter sediment barrier and to the catch basins,that required maintenance. The

driveway was supposed to drain to interceptor drain. However the as-built grades
appear to prevent this and the runoff drains to the main perimeter driveway,and a
portion drains to Chickering Road. This should be carefully looked at to determine
how this is to be corrected.
The issues in this section are very important and need to be addressed as soon as possible.

® 04/27/04—(Postscript)A meeting was held on Thursday 4/29/04 with Paul Szymanski
(NASD),John Cyr(DPW) and Fred King(SBNE)to review the preliminary Snow Disposal
Plan. Suggestions were made for additions and modifications to the Plan. SBNE will
incorporate the recommendations into the plan for further review.
A plan was also reviewed that shows the locations for permanent markers along the 25-foot
no disturbance buffer zone. The plan showed that 96 markers would be necessary to comply
with the Condition 51 of the Order of Conditions. Due to the necessary locations,it was
suggested that this would be best done as one of the last items of site work. This will need to
be discussed with the Conservation Commission. In addition, it appears to be appropriate in
some areas that these same posts be used for"No Snow Disposal"signage to comply with
Condition 52 of the OOC.
Also discussed were the installation of 35 Bird Boxes and the"non-invasive trail system"per
Conditions 35 and 37 of the OOC.

INTRODUCTION
The purpose of these inspections is to observe and review the project work relative to compliance
with the Orders of Conditions issued by the North Andover Conservation Commission and the
Special Permit issued by the North Andover Planning Board with respect to sediment and erosion
controls, limit of work compliance, and stormwater management issues. For reference,the
Orders of Conditions issued for the project include:
• DEP File No.242-1045: Issued 2/21/01 with Amendment 2/19/04 Extended 3/17/04
• Planning Board Notice of Decision: Issued 5/14/01 Modification Approval 2/17/04

any questions regarding the information contained in this report.
During this inspection,it was sunny and clear. The need for the installation of the No
Disturbance Zone markers was discussed with the contractor. These markers must be
installed A.S.A.P. Overall,the site appears to be relatively stable, with no signs of significant
erosion or impact to wetland resource areas. Silt fence damaged during the heavy rains of the past
week had been repaired. The contractor has maintained the sediment controls and erosion
controls. The stone interceptor sub drain (gravel drain) needs to be cleaned out and
brought to finished condition. The drainage outfalls to Cochichewick Brook were clear.

Schofield Brothers of New England,Inc.has been designated as the"Environmental/Erosion
Control Monitor"for the North Andover High School Project. The purpose of the Environmental/
Erosion Control Monitor is to perform periodic inspections of the project site to insure compliance
with the Order of Conditions issued by the Conservation Commission and the Special Permit
issued by the Planning Board with respect to sediment and erosion controls, limit of work
compliance,wetland replication and stormwater management issues.
This is the thirty-sixth monthly inspection report prepared and submitted in compliance with the
above referenced permits. It covers the period from October 27, 2004 through November 30, 2004.
Attached are copies of the Inspection Reports (#145,#146,#147,#148 and#149)relative to the
inspections performed on November 2nd,9d', 16d, 23rd, and 30`s, 2004.
The month of November was seasonally normal with no excessive rain events. Overall,the erosion
and sedimentation controls functioned properly during the past month. The site work is nearing
completion with work focused on both the northern and southern athletic fields. The work in the
southern athletic fields included spreading of loam, hydro seeding and construction of the baseball
field. The work in the northern athletic field consisted of the installation track and field equipment,
grading, and spreading loam on the site.

2. The site pavement was swept to minimize dust and sediment load,but sweeping must be
continued.
