Fee of$425.0O per lot for new construction. This covers the mioimoun two deep holes and
two percolation tests required for each disposal area. Fee of$360.00 per lot for repairs or upgrades.
GENERAL INFORMATION

Only Certified Soil Evaluators may perform deep hole inspections.
�
Only Mass,Registered$noitu,iany and Professional Engineers can design septic plans.
�
At least two deep holes and two percolation tests are required for each septic systern disposal area.
�
Repairs require at least two deep holes and at least one percolation test,at the discretion of the BOH -
representative.
�
Full payment will be required for all additional tests within two weeks oftesting.
�
Within 45 days of testing,a scaled plan(no smaller than I"-100')shall be submitted to the Board of Health
showing the location of all tests(including aborted temu).

Within 60(lays oY testing soil evaluation forms shall bmsubmitted.

Please find attached, the soil testing results for the above referenced property.
As you already know from Jennifer, there are wetlands behind the house. The test pits were all approximately 100' from
the wetland boundary. The bottom of Perc 2 had damp soil so we attempted the perc test but it was influenced by the
groundwater. Most likely Luke will not be using this area for the upgrade.

4/28/09—Site checked by Jennifer Hughes in Conservation. There is a pond very far in the back away from the proposed
test pits. Distance appears to be okay. You can go ahead and schedule an appt.for soil testing.
