2. Might the project significantly affect vegetation, especially any rare or endangered species
of plant? (Prior consultation with the Massachusetts Natural Heritage Program is advised.)
(Estimate approximate number of mature trees to be removed: less than 10 )
Explanation and Source: Request for determination has been sent to NHP
3.Agricultural Land.Has any portion of the site been in agricultural use within the last 15 years?
If yes, specify use and acreage. yes
Explanation and Source: Current land use is mixed residential, and greenhouse
operation for retail. Approximately 5 ac. are utilized to grow flowers
and vegetables for sale.
Field observations of existing conditions.
D. Water Quality and Quantity
1. Might the project result in significant changes in drainage patterns? No
Explanation and Source: Pre and Post Development runoff will be
matched be design. Flow is to same point or area by design.
2.Might the project result in the introduction of any pollutants, including sediments, into marine
waters, surface fresh waters or ground water? No.
Explanation and Source: Designed storm drainage system with catch basin
with hoods and sumps to collect and trap oil/gas, floatables, and
sediment from project area.

cy or more, attach completed Standard Application Form for Water Quality Certification,
Part I (314 CMR 9.02(3), 9.90, DEQE Division of Water Pollution Control).
P.7
9. Does the operation of the project result in any increased consumption of water?
Approximate consumption R45 gallons per day. Likely water source(s)Town Water Mains
Explanation and Source: Flow is assumed to be equal to estimated sewage
flows from Mass Title IV regulations
E. Solid Waste and Hazardous Materials
1. Estimate types and approximate amounts of waste materials generated, e.g., industrial,
domestic, hospital, sewage sludge, construction debris from demolished structures. How/
where will such waste be disposed of?
Explanation and Source: Demolished greenhouse operation appoximately
1000 c.y. Disposal shall be at a permitted disposal facility. Domestic
waste of less than 10 c.y. will be generated, picked up and handled
by contracted permitted and licensed handler to permitted disposal
facility.
2. Might the project involve the generation, use, transportation, storage, release, or disposal
of potentially hazardous materials? No

Explanation and Source: Current use is residential and greenhouse operation
typical with use. Field observation.
F. Energy Use and Air Quality
1. Will space heating be provided for the project? If so, describe the type, energy source, and
approximate energy consumption. No
Explanation and Source:
P.9
2. Are there any sensitive receptors (e.g.,hospitals,schools, residential areas)which would be
affected by any noise caused by the project? No
Explanation and Source: Adjacent residential condominium development buffered
by existing vegitation and distance to project. Proposed development
proposes additional buffering by plantings and fencing.
