Dear Board Members:
Enclosed, please find an as-built plan for the above-referenced project. This plan shows that the
work has been conducted in conformance with the Special Permit conditions for the project. The
only substantial difference which we have observed between the as-built conditions and the

area and drainage will function as designed and approved. The reduction in grading
associated with this change slightly reduced the amount of grading required near the wetland.
At the time of our inspection the disturbed areas outside of the pavement has been graded,
loamed, seeded and mulched with hay. Due to the season vegetation was not yet established.
Accordingly, we request that the as-built plan and the site conditions be reviewed by the
Planning Department and the Division of Public Works, and that a release of a portion of the
security be made. Only a partial release of the security can be made at this time as the vegetation
has not yet established as noted above. As the area that has been revegetated is small we
recommend that an amount of $500 or less of t f t he sec
v, urity be held. In addition, we anticipate that
the vegetation will be established be early summer and the final release should be able to be
made at that time.

Dear Ms. Colwell:
On September 20,1995,O'Leary/Vincunas Partnership#2 posted a bond of$3,000 to cover tree
planting and an as-built plan for the above referenced property.
I believe all the work has been completed in accordance with the special permit and an as-built
plan was delivered to the town by Ross Associates within the last few weeks.
Based on this,I am requesting a release of this bond. Please call me if you have any questions on
the above, and let me know if there is anything else I need to do regarding this matter.

Dear Mr. Carron,
The Planning Board reviewed the site plan application for the
expanded parking lot proposed at 94 Flagship Drive. The Board
.continued the public hearing to August 1, 1995. The Board had the
following comments:
1. Drainage issues must be resolved with Coler & Colantonio.
2 . Issues with the conservation commission must be addressed.
Given the direction from the Conservation Commission at their
meeting last night, it is doubtful that the Planning Board
will be able to close on August 1. I suggest continuing the
public hearing until August 15.
3 . The Board suggested looking at expanding the parking area at
the front of the building.
4 . ' A snow removal plan must be in place.
5. Additional screening may be necessary along all sides,
especially along the border with the Marion Drive neighbors.
Please let me know as soon as possible if you would like to
continue the public hearing until August 15 as I will be on
vacation from July 28 until August 7 . If you have any questions
please do not hesitate to call me at (508) 688-9535.

Dear Board and Commission Members:
Enclosed, please find a revised plan and drainage calculations for the
above-referenced project. The plans have been revised as discussed at the
Conservation Commission hearing. By reconfiguring the northern entrance to the site
we have been able to relocate enough parking spaces to avoid the alteration of
wetlands .
In addition, we believe that the revised plan addresses all of the issues raised by
your consultant in his initial review of the project.
We are submitting the revisions directly to your consultant to expedite the review.
At this time we only submitting two sets of the documents to each Board/Commission.
We will submit the number of copies required by the regulations prior to the
continued hearings, after having received your consultant's comments and having made
any additional changes that may be necessary. If you feel you need additional
copies at this time please inform us and we will mail them to you immediately.
Please note that I will be on vacation the week of July 24 th. Mr. Denis McLaughlin
of our office has worked closely on this project with me. Please direct any
questions which may arise to him while I am on vacation.

Dear Mike:
Thank you for meeting us today on the site to review our planned revisions to the approved
plan. Based on your comments during the meeting we are proceeding with the following:
• We will revise the plan to fully describe the changes along the right hand side and
rear of the property and submit two copies to your office as soon as possible. As we
discussed,you will bring the plan to the Commission and Planning Dept.for review.
• We will stake the limit of pavement and the limit of work for your review and for use
by the site contractor. This should happen by Wednesday,September 27th.
• The contractor will install silt fencing and hay bales as required in the order of
conditions along the edge of the proposed disturbance.
• The contractor will install a sign showing the DEP number of the project.
• The contractor will put together a"sequence of construction plan"for submission to your
office.
• I will forward to you the recording information from the Essex County Registry of
Deeds.

It is our understanding the modified plan will only go through an internal approval process
without the need for further public hearings. I look forward to getting together again with you
soon to make any further changes needed and hopefully get your approval to commence the
project. The weather is starting to become a factor and I would appreciate any help you can
give us in resolving the outstanding issues quickly.

Dear Mr. Howard:
Enclosed, please find the revised lan for the above-referenced project. The plan
has been revised as we discussed atp the site last Thursday.
The revisions which have been made include shortening the aisle of parking in the
rear to avoid having to relocate the swale, deletion of a portion of the interceptor
drain, the phasing of the construction at the front and the rear of the project, and
the relocation of the edge of pavement in some areas to maintain the five foot
offset required by the Order of Conditions.
