Griffin RE: Stor-U-Self Facility Route 125, North Andover, MA Dear Ms. Griffin: I have enclosed the original mylar for Planning Board endorsement at the up-coming meeting on October 17, 2000. The Massachusetts Highway Dept. District 4, in their permit review process (copy enclosed), have required that the entry radius on the South side of the site be increased from 10 to 30 feet. We have made the change on the mylar and have noted it as Change "G" dated 10/04/00. This minor change hopefully will not be considered significant by the Planning Board. If you have further questions regarding this matter, please call me. Respectfully, RICHARD F. KAMINS:I & ASSOC RTES, INC. Richard F. Kaminski, P.E. President RFK/lh rfk9828 Cc: R. J. We have enclosed the following revised drawings; 1, Drawing SP-2 - Proposed Site Plan (Revision Date 8/28/00) We have modified this drawing to show the proposed dumpster with wood fence enclosure in its new location in the rear of the building. 2. Drawing SP-5 - Construction Details (Revision Date 8/28/00) We have modified this drawing to show the location of the proposed 18" RCP pipe as it passes beneath Retaining Wall #1, and we have also shown a typical drainage manhole to be used on this project. Both of these items were requested by your review consultant. 3. Drawing LP-1 - Landscape Plan (Revision Date 8/04/00) This drawing reflects the additional landscaping requested by the Planning Board around the proposed Detention Pond in the front of the site. fSncinc;-rnin Kiurvoyii g Land flanr'ing Town of North Andover Community Development & Services August 29, 2000 Page 2. Additionally, we request that English Oak trees be substituted for the Silver Maple trees initially proposed for this project. We have been told by our client that the Silver Maples are not readily available and the English Oaks are readily available. The English Oaks will meet the Zoning Regulations requirements regarding the type of growth form that must be provided. 4. Drawing A-I and A-2 represent the proposed elevations of the building as requested by the Planning Board at the public hearing. We hope that these revisions meet with your and the members of the Planning Board approval, and we look forward to our continued public hearing on September 5, 2000. Respectfully, JRIW F. J. Kelly Company 55 Cambridge Street Burlington, MA 01803 Dear Sir: As required in Section 4a of the Notice of Decision, dated September 8, 2000, issued by the North Andover Planning Board, this correspondence is intended to inform you that this project generally complies with the Engineering Documents previously submitted and used as a basis for the Site Plan Special Permit with the following exceptions of which we are aware. Chan eg s from Definitive Plan 1. Landscaping is outside the landscaped buffer located at the rear zoning line, 2. Masonry blocks were used instead of reinforced concrete walls for the drainage headwalls and the project retaining walls. 3. The proposed re-grading, particularly in the side driveways and rear parking lot, has been changed. 4. At least three (3) parallel parking spaces along the detention pond do not meet the design dimensions. One was added to this area because it was eliminated from the adjacent parking area. 5. The office area has moved from the location proposed at the South West corner of the building to the North West corner. The accessible parking spaces are located at the South West corner of the building. A.rchileclurp FngtneeraT swveying Land Planning ' Town of North Andover Planning Board August 2, 2002 Page 2. Changes from Definitive Plan (Continued) 5. The sidewalk along the front of the building is one foot wider than that proposed. 6. The building is 351 Sq. Ft. larger per floor and 1053 Sq. Ft. larger in total. The proposed area per floor was 16,350 Sq. Ft. and 16,701 Sq. Ft. was constructed. The proposed total building area was 49,050 Sq. Ft. and 50,103 Sq. Ft. was constructed. 7. Landscaping and Re-grading was extended onto the abutter's property along the South property line. Because the building was slightly wider than that proposed, the 3 Foot landscaping strip along the South property lime-was reduced and the proposed vegetation in this area was installed over the property line. 8. Two parking and driveway lights were installed over the South property line. 9. DM-8 to SC-1 Backflows approximately 4 inches back to DM-8. 10. CB-11 to DM-11 Backflows approximately .06 back to CB-11. 12. The floor area ratio is 0.510:1 and the maximum allowed is 0.50:1. As you recall, our client, Richard J. Kelly, and I appeared before the Planning Board in order to discuss the various changes that our client wanted to make during the construction of this project. We were instructed by the Planning Board to incorporate the various changes on our As-Built Plan(copy enclosed) which would be in affect a Modified Definitive Site Plan and submit it to the Planning Board for their discussion and review with Mr. Kelly. In our view, the various changes do not adversely affect the performance of the site with the exception of the location of the accessible parking spaces relative to the office area and the backflow discussed in Item 10. If you have questions regarding our submission, please call me. Respectfully, RICHARD F. KAMINSKI & ASSO IATES, INC. Richard F. Kaminski, P.E. President RFK/lh Rfl,,10247 cc: R. J.
