Many of the items on your review letter, dated December 3, 1996, were discussed and resolved without requiring any revisions to the set of plans, However, the following items of concern have been addressed for further clarification, in an order consistent with your letter: . ITEM 2 The Abbott Street driveway access has been relocated to improve vehicle access. The drive-up teller has been relocated to allow for 3 lanes at 8.5 feet width for each lane. ITEM 3 The project sign has been relocated on the-plan and shall be set in place at the top of the.berm between the parking area and the detention pond. A detail of the sign has been added(see sheet#3) based on a design by John Daly, ' chitect. ITEM 6 Additional spot grades a-, -.dded to tl- : plans to enable the contractor to do .finish grading, as intended, to remove water from the pavement surfaces. i ITEM 9 Handicap ramp locations have been revised. ITEM 10 Footing drains have been eliminated. Only roof drainage will be collected and discharged into the detention basin. ITEM 14 The proposed detention basin has been designed to eliminate any potential increase in the pear rate of runoff from the site. In addition, we spoke with Mr. Hmurciak, who indicated that there are"no problems" with the drainage system on Salem Street; there are no "as-built" drainage system plans; and the drainage system is inaccessible, as the manhole covers are paved over, Mr. John'Chessia, P.E. All of the pavement areas, pavement markings, sidewalks, curbing, and drainage systems leave been completed. RE: Site Development Mathew Xenakis Salem Street m North Andover,MA Dear Ms. Colwell: Relative to my correspondence of May 18, 1998, which was relative to our meeting on May 14, 1998 with Mr. Daley and me,Mr. Rand of North Andover DPW, Mr. Xenakis, Mr. Matses, and Mr. Chongris the site contractor, the following is a summary of punch list items and items of site construction still to be implemented on the project site. Based on the site inspection, it was determined that some elements of site construction should be reconsidered by the Planning Board, and at the request of Mr. Xenakis, those changes implemented on the site, in order to allow the Planning Board to issue a Final Certificate of Compliance for the project. These items are as follows: FRONT OF SITE (DALE STREET) 1. Along the property line/street line, adjacent to the parking area located north of the driveway, the sidewalk shall be saw cut, so that it is 5' wide, rather than 6' as it currently is, and the pavement removed, and a loam and grass strip constructed between the curbing and the sidewalk. In the grass strip, a fence will be constructed, and terminate 1 space before the driveway entrance. As an alternative to the above, in lieu of a fence, Mr. Xenakis proposes construction of a woad guardrail which would be built in the loam and seed planting strip. 2. Along the street line/property line in the parking area south of the driveway entrance, the sidewalk will remain as is, but a fence will be constructed along the layout line beginning 1 space back from the driveway entrance, and will terminate near the hydrant. This will allow access to the hydrant located at the end of the parking area. Ms. Kathleen Colwell September 24, 1998 Page 2 In lieu of the fence, Mr. Xenakis proposes construction of a wood guardrail along the property line. 3, After the hydrants, Mr. Xenakis proposes the construction of a wood guardrail along the top of slope of the detention area, and around it, extending along the frontage on Abbott Street. The guardrail will also run along the pat-king lot which faces Abbott Street. SIDE OF SITE (ADJACENT TO ABBOTT STREET) I Mr. Xenakis will continue the construction of a retaining wall east of the utility pole and northwest of the transformer in accordance with Town Staff and DPW recommendations. 2. Mr. Xenakis will construct a landscape fence along the sidewalk on Abbott Street, west of the transformer, up to the Abbott Street driveway. In lieu of the fence, he will construct a guardrail and plantings as set forth in the specifications provided by Mr. Rand 3. The curbing adjacent to the building between the bump outs will remain in place but 2 bollards will be added to the site, near the doorway, offset the bump out in the building, so that vehicles are directed away from the building. 4"" The dumpster will be relocated to I of the rear parking spaces. 5. A separate plan will be prepared relating to the bank drive-up on the end oft e building. REAR OF SITE (ADJACENT TO RESIDENTIAL USE) 1.
