Dialysis Facility Dear Sir: On behalf of our client, Renex Dialysis Clinic of North Andover, Inc . , I respectfully request that the North Andover Planning Board waive the Site Plan Review process for this proposed Change in Use project. As outlined in Section 5 .3, 2, c, of the Town of North Andover Zoning By-Laws, this proposed Dialysis Treatment Facility will not have an adverse impact both within the site and in the immediate vicinity of this facility. Special Permit for Site Plan Review. Dear Sir: On behalf of our client Reny Dialysis Clinic of North Andover, Inc. , this correspondence is intended to outline the basis of their petition for Site Plan Approval for a Proposed Change In Use for an existing building located at 201 Sutton Street in North Andover, Ma. The building is owned by Dizzy Bridge Realty Trust and has supported light assembly, office and general tenants since its construction in 1987 . The building is currently partially occupied by a Dance Studio. It is the intention of Renex Dialysis Clinic of North Andover, Inc . , to renovate the interior of this structure and convert it to a dialysis treatment facility serving the needs of the Greater North Andover Community. The interior of the structure will be totally renovated to accommodate the needs of a dialysis facility. No work will be done on the exterior of the building with the exception of raising the grade locally at an existing loading dock, and the painting of parking space lines on the existing paved surfaces around the building. As outlined in the "Town of North Andover Planning Board Application Check List for Site Plan Review" , all required items have been included on the site plan with the exception of the following: Item is Building Elevation A building elevation is not necessary in our view, since we are not going to renovate the exterior of the structure. We have, however, enclosed photographs of the building for your use. Item o: Landscape Plan In our view, a landscape plan is not necessary since we are not altering the grounds around the exterior of the building. It is our proposal to leave the grounds in their current condition. Item q: Lighting Facilities The existing exterior lighting is sufficient for the proposed use. Item r: Drainage Basin Study Since there is no intention to renovate the site, nor create any new impervious area, the existing drainage system is adequate and a study is not warranted. Item s : Traffic Impact Study The traffic anticipated with the proposed Change in Use should not be significantly increased as compared to that generated by the previous tenants over the last ten years. We, therefore, feel that a traffic study is not warranted. Item t: Commonwealth Review The interior renovation of this building does not require a review by any State Agencies with the exception of a Permit from the Massachusetts Department of Health, Division of Health Care Quality, which requires an Architectural Plan Review. Licensing of a Dialysis Unit in Massachusetts is governed by the Department of Health. K A M I N S K I North Andover Planning Board January 17, 1997 Page 3 . Item u: Utilities The existing utilities are adequate to service the proposed facility. Item v: Fiscal Impact The fiscal impact to the Town of North Andover will be positive in that local jobs might be created by the employment of qualified technicians, as well as tax revenue generated by this facility. Item w: Community Impact The community impact will be positive in that this Dialysis Facility will provide a life saving service to those in need in the Greater North Andover Community. It should not cause an adverse strain on the services that the Town currently provides to this building, nor should it create traffic difficulties in the immediate neighborhood. The proposed facility will be a positive asset to the Community. It will generate jobs and provide an invaluable service to those in need. r Respectfully, RICHARD F.
