Dear Sir:
In conformance with Section 5 (b) of the "Special Conditions" of
the Special Permit/Site Plan Review issued to Renex Dialysis
Clinic of N.A. Inc . dated March 7, 1997 (copy enclosed) , I hereby
state that the landscaping, lighting and site layout
substantially comply with the site plans submitted to the North
Andover Planning Board as part of this application. This office
was not involved in the interior design or layout and therefore
cannot certify to its compliance with the design plan.
Additionally, I have enclosed two (2) copies of the as-built site
plan in accordance with "Special Conditions" : Section 2 (b) . I
also suggest that the $2, 000 Bond be released to the Contractor,
Ashland Engineering Corporation (after verification) , if they had
originally posted the bond as was required.
This company was the sub-contractor to Dialysis Constructors,
Inc . , 12118 North Loop Road, San Antonio, Texas 78216 . Our
office was not involved in the actual posting of the bond, and
obviously cannot certify who actually submitted it to the Town of
North Andover. I have been informed by representatives of
Ashland Engineering, Inc . that they posted the Bond. I suggest
that this be verified by your office.
Additionally, it is important to note that the Contractor,
Ashland Engineering, Inc . , has only recently completed the
parking space striping in general conformance with our site plan

dated January 17, 1997 . During the interim, a number of
evergreen plants that were installed as part of this project have
died or have been overrun by traffic . However, when I checked
their numbers on 4/13/98, they had conformed to our Landscape
plan dated 3/25/97 in numbers of plants.

Dear Ms. Stephenson
I am receipt of your letter of October 3, 1997 requesting the release of the bond money held for
the above referenced project. Prior to release of the funds I will need a letter from the architect or
engineer for the project certifying that the building, signs, landscaping, lighting and site layout
substantially comply with the plans approved by the Planning Board.
I will also need to review the site for compliance with the Planning Board decision. Please call
me to set up a time to meet on site. Once I have reviewed the site and have received a
certification letter I will place your request on the next available Planning Board agenda.

dated September 17, 1997. The proposed plan you have on file for this project is current. There
were no changes to the final as-built plans for this project.

I have reviewed the plan prepared by Richard F.Kaminski&Associates dated 1/17/97 regarding the
proposed Renex Dialysis Clinic with regard to fire department vehicle access.
The fire department has no objections to the proposed plan as presented and the sixteen and one half
foot wide access road width in front of the building. The plan allows for adequate access for fire
department vehicles into either parking lot at the sides of the building.
