Re: #10 Stiles St. S SDS revision for Tim Willis Dear Sue, Please find attached, 3 stamped copies of the Sptic System Design Plan revised April 5, 2004 for the above referenced lot. Please note that the plan refers to 0 Stiles St. The DPW has issued a street number for this lot and it is now known as 10 Stiles. The extent of the revision was to replace the well with a municipal water supply. This addresses the comments from the town consultant, Mill River Engineering, as to title 5 compliance. Please review this change and issue an approval for construction plan. Per your comments on the form U, the as-built plan will show the water line location when submitted to your office. Inc. 160 Pleasant Street North Andover, MA 01845 Re: 0 Stiles Street Dear Mr. Willis, As you are aware, the plan submitted to the North Andover Board of Health, for the , property noted above, has previously been approved. In addition, a review of the plan was conducted after the approval that was granted by default. According to a conversation held with Mr. Tim Willis on March 29, 2004, the comments generated by this review were addressed as follows. Please note that the item numbers relate to the review letter dated November 21, 2003. 2. The wetland line has been confirmed by the North Andover Conservation Department 10. The installation of a well has been replaced with a connection to the municipal water system. The location of the water line must be shown on the final as built. The water line must be at least 10 feet from any portion of the septic system components. 4, 6, 7 and 8 are items that are needed to provide information to the licensed installer. A copy of the review letter will be provided to the installer upon issuance of the permit and will be discussed in the pre-construction meeting with the health department personnel. Upon the writing of this correspondence, your form"Y' for the building department has been signed. Enclosed is a copy for your records. Be advised that an application for the septic installation must be accompanied by a foundation as built, at a scale of 1"=20'. In closing, the final comment of the review will be repeated to note its importance. "Lastly, it should be noted that any future divisions of the current 7.29 acre parcel will need to assure compliance of the existing and proposed parcels with the Nitrogen loading limitations and division of facility requirements in Title 5". You had desired a review of the plan to determine compliance with state and local codes and forwarded the site plan to our office for comment. Please note the following items: I. The parcel appears to be a proposed subdivision of existing land which contains a dwelling located on Gray Street. We would encourage the submission of a recorded site plan which shows the property boundaries. The lot lines should match those shown on the septic design plan. 2. The wetland delineation on the septic design plan should be confirmed by the Conservation Commission. 3. The basement floor is not located 1' above groundwater elevation as required by North Andover Board of Health Regulation 5.04. 4. Details regarding the building sewer pipe should include specifications for it to be watertight and laid in a straight line. The design plan requires the use of cast iron piping which is permitted by code but common industry practice is the use of plastic pipe material. 5. The septic tank loading is not specified. 6. The distribution box should specify that the first 2' of piping are to be laid level, that the box is to be watertight, and that the soil beneath the stone base is to be mechanically compacted. 7. Notation should be provided to make it clear for the need to maintain a 2% slope of the final cover material over the soil absorption system. 8. The design plan does not specify the removal of 6" of natural soil pursuant to North Andover Board of Health Regulation 9.02. 9. A typographical error exists in the naming of Percolation Test 2. 10. This was originally submitted to Sandy for review on June 26, 2003. However, Sandy had put her review of it on hold until it had Planning Board approval for the lot. In the meantime,the plan has been constructively approved, but any comments that you have with regard to the best interests of the health of the Town are most welcome, and will certainly be taken into consideration. Heidi will work with the applicants (Willis)to the best of her ability to ensure that the plan meets all the Title 5 specifications, As always,thank you for your assistance. Pam p.s. —outside e-mail is still down,so I never did receive this week's e-mails from you. I did, however receive your fax regarding the 70 Oakes Drive and 292 Granville Lane plan reviews.
