an Application for Definitive Subdivision Approval for the above referenced parcels. The
existing parcel currently has 4 dwelling units located at 1679 Osgood Street. The intent of this
application is to subdivide the property into seven (7)residential lots with private individual
wells and on-site leach field areas. Two (2) new roads are proposed with c„1-de-sacs to provide
access to the seven residential homes.

5.1.5 Site Evaluation Statement
Environmental and Community Impact Analysis (5.4)
I. Project Scope:
The intent of this project is to conventionally subdivide Map 61, Lots 16 and 34 into eight
(8) single family residential lots with municipal water and onsite septic. Parcels A and B as
shown on our plans are not buildable lots. Lot 34 is already a building lot in the Town and
therefore, this is the creation of seven additional lots. The access will be from Bradford
Street and will require the construction of a 1,062' cul-de-sac. All of the proposed homes
will have driveways off of the proposed roadway. The lots will be serviced with Town
water and onsite septic systems.
The existing parcel currently has 4 dwelling units located at 1679 Osgood Street. These
units have existed for many years and there is no proposed change to this portion of the lot.
The remaining parcel has been commercially used over the years,however, most recently
this area has been remained wooded and undeveloped. You can see historical earth moving
was performed on the property many years ago and we are not sure for what purpose.
Section 5.4.4.1 Natural Environment
a. Air &Noise Pollution - The only air and noise pollution generated from the project
will be the temporary impacts associated with construction. We expect that this
roadway will only take 2-3 months to construct.
b. Water Pollution- The drainage design of the stormwater system requires multiple
small detention ponds and treatment systems to meet the Town and State
requirements. The stormwater impacts offsite will be mitigated by these structures.
We have a decrease in stormwater runoff exiting the property and therefore no
adverse impacts will occur downstream. This design will be reviewed by the Town
Engineer to ensure compliance with Town codes.
c. Land—This site consists mostly of uplands and the soils are suitable for construction.
We have a buffer to the downhill abutting properties which will not be affected by the
development.
d. Plants &Wildlife—Our Wetlands Consultant will be addressing the environmental
issues associated with this project in his Notice of Intent. This will be reviewed by
the Conservation Commission.
e. Water Supply— 8 additional 4 bedroom homes using 440 gallons per day adds 3,520
gallons per day. This is a small amount of usage and the actual usage is typically less
than the 110 gallons per bedroom. There are larger subdivisions up the hill from us
on Bradford Street,therefore,water pressure should not be a concern.
f. Sewage Disposal—This development will require 8 septic systems to be constructed.
The soils have been evaluated by Ben Osgood, P.E. and he has no concerns about the
placement of septic fields on this parcel.
Section 5.4.4.2 Man-Made Environment
a. Existing Neighborhood Land Use—The existing neighborhoods on Bradford Street
consists of old and new residential dwellings. The properties fronting on Bradford
Street in North Andover are approximately 50 years old. The properties fronting on
Bradford Street in Haverhill are less than 10 years old. On the south side of the
property,White Birch Lane Subdivision borders our property.
b. Zoning—Our property is split zoned, with Residential 2 and Corridor Development
District 3. The proposed subdivision is proposed within the Residential 2 zone and
we are utilizing 100' of the Corridor Development District 3 zone for a couple of the
lots. Our proposed use is compatible with the zoning.
Section 5.4.4.3—Public Services
Schools - The impact to the school system will be minimal as the project is small. Not
every house will have school age children at the same time. The location is convenient to
existing bus routes and the roadway is short enough for school children to walk to the end
for pickup. Buses would also be able to drive down the roadway and turnaround if desired.
Police—We do not anticipate the need for any above normal police presence because of
this proposed project.
Fire—We do not anticipate the need for any above normal fire department presence
because of this project. We have equipped the roadway with a watermain and hydrants.
The road meets the Town standards for design widths and safety.
Recreation—Onsite recreation will be provided on each individuals lot as they deem
necessary. Offsite recreation is available throughout the Town and Community and this
project will not overburden the public facilities.
Solid Waste Disposal—We expect the typical trash disposal of this project as every other
subdivision house lots within Town. The collection of this material will be provided by the
Town and paid for by the taxes collected.
Traffic- The additional impact will be from the 7 additional homes that will be constructed
from this proposed development. Remembering that one of the lots already exists.
Bradford Street is a two lane collector roadway and is residentially developed with frontage
lots and subdivisions. The addition of approximately 70 vehicle trips per day from this
subdivision will have no adverse impact on Bradford Street. The sight distance from the
proposed roadway is acceptable and larger subdivisions exist further to the southeast.
Highway—The proposed traffic will not require any changes to the highway system.
Future maintenance of Bradford Street will be required despite this development. We do
not anticipate that the roadway will have to repaired sooner than expected because of this
development. The construction vehicles will have to access Bradford Street during the
roadway construction. Typically a road bond is placed by the contractor and developer in
order to address any repairs that are necessary prior to road acceptance.
Section 5.4.4.4—Aesthetics
a. Lighting—At this time, we are not proposing any lighting within the subdivision other than
that provided by Homeowners on their houses and driveways. If the Town requires
lighting, we can certainly address that concern.
b. Landscaping—We are proposing the roadside trees between the sidewalk and the roadway
as required by the Town codes. All other landscaping will be dependent on Homeowner's
preference.
c. Visual—This property is currently wooded and we do not have any homes proposed for the
first 400' of the roadway. Therefore, no visual impacts will occur from vehicles on
Bradford Street. The residents along Bradford Street will have a couple homes behind their
existing dwellings. They have been able to enjoy this property as undeveloped land for
many years. Whoever builds the homes within this development has just as much interest
in not seeing the neighbors as vice versa.

5.1.8 List of Waivers
On behalf of our client, GMZ Realty Trust, we respectfully request a waiver of the following
section of the Town of North Andover Subdivision Regulations be granted for the proposed
Subdivision at 1679 Osgood Street and Bradford Street.
Section 6.8.1 — Streets—Design Standards, Table IA Minimum Design Standards for
Local Streets—Maximum Length (roadway plus turnaround) = 600 feet
5.1.9 Construction Schedule
Start of Construction: March 1, 2011
Completion of Construction: December 31, 2013
Actual dates may vary based on contractor,market conditions and housing market demands.
5.1.10 Type of Performance Guarantee
The type of Performance Guarantee will be specified once the project is sold to a builder or
constructed by the landowner. We cannot predict what type of Guarantee they will decide to
utilize, but it will have to one suitable to the Town prior t0 project commencement. This wvuid
be a well suited topic for the pre-construction meeting.
5.1.11 Quitclaim Deed to streets subject to acceptance: and to open space areas subiect to
Town Purchase
This seems like it would be a suitable condition of approval. We will provide the legal deed to
the street at a later date.

Important:When A Stormwater Report must be submitted with the Notice of Intent permit application to document
filling out forms compliance with the Stormwater Management Standards. The following checklist is NOT a substitute for

The Stormwater Report must include:
• The Stormwater Checklist completed and stamped by a Registered Professional Engineer(see
page 2)that certifies that the Stormwater Report contains all required submittals.' This Checklist
is to be used as the cover for the completed Stormwater Report.
• Applicant/Project Name
• Project Address
• Name of Firm and Registered Professional Engineer that prepared the Report
• Long-Term Pollution Prevention Plan required by Standards 4-6
• Construction Period Pollution Prevention and Erosion and Sedimentation Control Plan required
by Standard 82
• Operation and Maintenance Plan required by Standard 9
In addition to all plans and supporting information, the Stormwater Report must include a brief narrative
describing stormwater management practices, including environmentally sensitive site design and LID
techniques, along with a diagram depicting runoff through the proposed BMP treatment train. Plans are
required to show existing and proposed conditions, identify all wetland resource areas, NRCS soil types,
critical areas, Land Uses with Higher Potential Pollutant Loads (LUHPPL), and any areas on the site
where infiltration rate is greater than 2.4 inches per hour. The Plans shall identify the drainage areas for
both existing and proposed conditions at a scale that enables verification of supporting calculations.
As noted in the Checklist, the Stormwater Management Report shall document compliance with each of
the Stormwater Management Standards as provided in the Massachusetts Stormwater Handbook. The
soils evaluation and calculations shall be done using the methodologies set forth in Volume 3 of the
Massachusetts Stormwater Handbook.
To ensure that the Stormwater Report is complete, applicants are required to fill in the Stormwater Report
Checklist by checking the box to indicate that the specified information has been included in the
Stormwater Report. If any of the information specified in the checklist has not been submitted, the
applicant must provide an explanation. The completed Stormwater Report Checklist and Certification

The Stormwater Report may also include the Illicit Discharge Compliance Statement required by Standard 10. If not included in
the Stormwater Report,the Illicit Discharge Compliance Statement must be submitted prior to the discharge of stormwater runoff to
the post-construction best management practices.
2 For some complex projects,it may not be possible to include the Construction Period Erosion and Sedimentation Control Plan in
the Stormwater Report. In that event,the issuing authority has the discretion to issue an Order of Conditions that approves the
project and includes a condition requiring the proponent to submit the Construction Period Erosion and Sedimentation Control Plan
before commencing any land disturbance activity on the site.

Checklist for Stormwater Report
The following checklist is intended to serve as a guide for applicants as to the elements that ordinarily
need to be addressed in a complete Stormwater Report. The checklist is also intended to provide
conservation commissions and other reviewing authorities with a summary of the components necessary
for a comprehensive Stormwater Report that addresses the ten Stormwater Standards.
Note:Because stormwater requirements vary from project to project, it is possible that a complete
Stormwater Report may not include information on some of the subjects specified in the Checklist. If it is
determined that a specific item does not apply to the project under review, please note that the item is not
applicable (N.A.)and provide the reasons for that determination.
A complete checklist must include the Certification set forth below signed by the Registered Professional
Engineer who prepared the Stormwater Report.
