The application to the North Andover Planning Board for a Definitive Subdivision located at
1679 Osgood St., applicant GMZ Realty Trust, was submitted on 7/16/2010. The application is
currently under review and is in the Public Hearing process. The application has been reviewed
by the town's outside consultants, Lisa Eggleston(Stormwater Analysis) and VHB (Civil
Engineering Review).
At the last Planning Board Public Hearing on February 15, 2011,the applicant and the Board
discussed the inclusion of the lot owned by Chris Adams/S.P.E.C. Realty Trust(Map 34, lot 31)
into the Definitive Plan. Jones & Beach provided a draft `yield"plan showing a proposed cul-
de-sac that would provide frontage for a new one acre parcel owned by Mr. Adams, as well as
frontage for lots 16-6 and 16-5, as depicted on the latest version of the Plan. The applicant is
requesting that the Board waive the construction of the roadway. The engineer also provided a
plan showing the "driveway"that would be constructed, with a hammerhead turn-around. The
Board instructed the applicant to perform the following:
File an Amended Definitive Subdivision Plan, showing the inclusion of the new lot. The
application should include the following:

• Defini-1, ; S, -jdivision Application with application Fee: of$325 ($325 per additional
lot)
• `Yield Plan', showing a full cul-de-sac and ROW
• Plan showing the construction of the proposed roadway
• Revised Stormwater Management Plan
• Certified list of abutters
• List of waivers
• Narrative describing the modification, including a description of a proposed
homeowner's association and their responsibilities regarding the driveways and the
drainage structures on lots.
• Materials required for mailing, per application instructions
• Form L, including signatures of department heads
• Lisa Eggleston: $3700. This will cover the outstanding invoice ($2,016) and will provide
for additional review of the project.
Re-advertise the Public Hearing for the June 21, 2011,which would include a notice in the
newspaper to be scheduled for June 7 and June 14. It would also include a notice to all abutters
to the Modified Definitive Subdivision.
File an application for Special Permit for a Common Driveway. This application should
include the following:
• Special Permit Application with application fee: $300.
• Narrative including a description of a proposed homeowner's association and their
responsibilities regarding the driveways.
The Public Hearing for both applications will take place on the same date (June 21, 2011), so
that only one notice to abutters and one newspaper advertisement will be necessary.
If any of these items are missing from the application, it would be cause for rejecting the
application filing. The application filing should be presented at the Planning Office no later
than Friday, May 20,2011.
