(b) "Request Amended Order of Conditions Under the Town of North Andover's Wetland Protection Bylaw and request for Amended Superseding Order of Conditions Under the DEP Wetland Protection Act, Lot 64A Haymeadow Road, North. Andover MA 01845" . Dated March 20, 1992 . Prepared for Methuen Cooperative Bank by Wetlands Preservation Inc. Condition #22 is revised as follows: Representatives of Wetlands Preservation Inc. (or a suitable alternative approved by the NACC) shall monitor the construction of the bridge and all associated work. : This monitoring will include, but may not be limited to, the monitoring schedule detailed in Section E of the applicant' s "Request for Amended Order of Conditions" submitted by Wetlands Preservation Inc. dated March 20, 1992 . The monitoring shall be done on a daily basis during those phases of construction detailed in the above document. The monitor will be in verbal contact with the Conservation Administrator on a daily basis during that time period. Verbal status reports will be given to the Conservation Commission at each regularly scheduled Commission meeting during the construction of the bridge and all associated restoration and/or restoration. Written progress reports will then be submitted to the Commission each month until the final grading is completed around the house. Condition #40 is revised as follows: A deposit of money in the amount of $25, 000 shall be posted as a performance guarantee by the applicant to ensure compliance with this Order of Conditions. This bond will, in every respect, be acceptable to the NACC and the Town Treasurer. A standard bond agreement, as found in the NACC' s regulations, will be submitted by the applicant. The applicant may propose a bond release schedule keyed to completion of specific portions of the project for NACC review and approval.
