CONSERVATION DEPARTMENT Community Development Division July 6,2009 Shari and Steve LoPilato 136 Bridle Path North Andover, MA 01845 136 Bridle Path, North Andover Sewer System Connection Conservation Conditions of Approval, NACC #54 Pursuant to section 4.4.2 D of the North Andover Wetlands Protection Regulations,Shari and Steve LoPilato,applicants/ homeowners,filed for a small project for work proposed at 136 Bridle Path, North Andover. The work consists of abandoning an existing septic system and connecting to Town of North Andover Sewer service. The sewer line will be installed from the street to a 1,000 gallon pump chamber within the 100-foot buffer zone to a Bordering Vegetated Wetland (BVW) which is located to the north of the existing drive. Work will be conducted outside of the 25-foot No-Disturb Zone from the edge of BVW. During the June 24,2009 public meeting,the North Andover Conservation Commission (NACC) voted unanimously to approve this project as described above and in the documents referenced below. CONDITIONS: 1. Excess material and construction debris shall be properly disposed of off site and all stockpile areas shall be located outside the 50-foot No-Build Zone. 3. The Plan of Record calls for 2" Ridged Insulation on the pipe. The NACC requires the use of specified insulation for this sewer connection (contractor estimate does not specify insulation in the project description). 4. All disturbed areas shall be loamed and seeded post construction. 5. Upon completion of the permitted work and stabilization of the disturbed areas, the applicant shall contact the North Andover Conservation Department for a post-construction inspection. 6.
