• A motion to accept the meeting minutes Of 4/26/17 & 6/14/2017 as drafted and reviewed, is made by
Mr. Manzi and seconded by Ms. Feltovic.

NACC#196,NACC#196,575 Osgood Street(Edgewood Retirement Community,Inc.)
• Robert Coppola from Edgewood Retirement Community presents the proposal to install a new
sewer force main for the Small Home project. There are existing utilities trenched in the drive.
Erosion control is already in place along the entire length of the gravel drive.
• Mr. Lynch asks about the location of the existing force main.
• Mr. Coppola reviews the plan. He states the pump station is outside jurisdiction and the line was
originally to be located in the drive but they are trying to avoid the disruption of tearing up the
pavement.
• Mr. Lynch asks if dewatering would be necessary.
• Mr. Coppola states he does not anticipate it.
• Mr. Napoli states depth appears to be 5'.
• The Administrator states the work is downgradient of the resource area.
• A motion to accept the small project under 4.4.2 D is made by Mr. Manzi and seconded by Mr.
Lynch.
• Vote: Unanimous
• A motion to approve the proposal as presented with conditions for no overnight stockpiling and a
post-construction inspection is made by Mr. Manzi, seconded by Ms. Feltovic.

• Philip Henry, P.E. with Civil Design Group reviews the proposal to repave the parking lot and
make improvements to make the parking ADA compliant. He states the lot will be milled with an
over of 2 1/2 -3" of new pavement. The grade will be brought up then binder and top coat. A
portion of this work is in the Riverfront Area. He states work will take one week and erosion
control will be installed. The existing drainage will remain. He states there are hoods in the
existing catch basins and the owners do maintenance.
• Mr. Lynch states the catch basins should be protected with silt sacks.
• Mr. Henry states there are two catch basins on the site.
• Mr. Manzi asks if there are any monitoring wells on the site.
• Mr. Henry states there are not.
• Mr. Napoli asks how deep the pavement will be milled.
• Mr. Henry states a few inches. He states the site is in disrepair.
• Mr. Lynch states the pavement cracking could be a sign of a bad sub-base. He asks if core
samples were done.
• There is discussion of the base material.
• Mr. Napoli asks why they do not just mill and grade.
• Mr. Henry states all pavement will be removed to the top of the gravel base.
• Mr. Napoli states it appears the site will be regraded with the reclaimed material.
• A motion to issue a negative #2 and negative #3 determination with conditions for pre and post
construction inspections.
• Vote unanimous.

242-1629, Outlet Structure Update, 1 High Street(East Mill)
• Tania Hartford of RCG, LLC reviews the plan to install a coffer dam before installing the new pond
outlet structure.
• Michael Clark of DCI states it will be a steel sheet coffer dam. A turbidity curtain will be used. A
perforated pipe will be installed in the excavation area to assist in the construction of a stable base
area. The pumps will be kept in place in case they are needed. Dewatering will occur to a frac tank.
• Mr. Lynch discusses his concerns about the pipes being undermined.
• Mr. Clark states there will be some leaking around the pipes but they will let the water table to come
back up before pulling the pilings out.
• Mr. Lynch has concerns about the integrity of the embankment around the new outlet structure.
• Mr. Clark states sands and gravel will be used.
• Mr. Lynch and Mr. Clark discuss water movement around the inlet structure and along the pipes.
• Mr. Manzi asks about the warning system to be installed in the pipe to detect problems early.
• Ms. Hartford states cameras will be installed.
• Mr. Manzi asks when they will be installed.
• Ms. Hartford states after the pipes are cleaned out.
• Mr. Clark presents the headwall plan and associated landscape features. He states the pipe will carry
100 year flow. Since erosion is a concern, I ton stone will be used for the wall. Stones will be 4'
thick at the base. He discusses the various elevations based on engineering studies. A new retaining
wall (precast block)will be constructed and a guardrail installed. Plantings will be proposed.
• Mr. Lynch makes a motion to accept the plan as part of the project record.
• Ms. Feltovic seconds the motion.
• Vote unanimous.

• Mr. Manzi recuses himself as an abutter.
• TJ Melvin, P.E. of Christiansen & Sergi states there are limited options for relocating the failed
septic system based on a retaining wall and steep slopes on the site. He states the proposed system
if more than 50' from the resource area and will contain a breakout barrier. A waiver is required
from the Board of Health (BOH). He states there are only clerical issues to resolve with the BOH at
this time and those will not affect the location.
• Mr. Lynch states the plan shows the required changes but still needs procedural approval from the
BOH. He recommends closing and issuing at the next hearing.
• Mr. Napoli asks about construction access.
• Mr. Melvin states it will be over the drive and through the front yard. He states stockpiling will be
upgradient of the 50' No-Build Zone.
• Mr. Lynch states the existing system is located closer to the resource area.
• Ms. Feltovic states there should be no additional dumping of yard waste over the retaining wall.
• A motion to continue the public hearing to 8/9/17 is made by Mr. Lynch and seconded by Ms.
Feltovic.
• Vote: four in favor and one abstention

• The administrator presents the replacement septic system plan. She states she reviewed the wetland line but
her one flag change is not on the plan due to its location far back on the lot.

• Michael O'Neil, P.E. states the new system will be in the same location as the failed one. All of the
old material will be removed. He points out the location of the water and gas lines.
• Mr. Manzi asks about the debris in the wetland shown in presented photos. He states this should be
removed as a pre-construction requirement.
• Mr. O'Neil addresses comment letter from the BOH.
• Mr. Lynch states the matter should continue for a DEP file number.
• Mr. O'Neil states he tried to contact Mr. Freely at Mass DEP.
• Mr. Manzi states debris removal could start now under an Enforcement Order(EO).
• Mr. Lynch states probably cleaner to have it as part of the Order of Conditions (OOC).
• A motion to continue the public hearing to 8/9/17 is made by Ms. Feltovic and seconded by Mr.
Lynch.

• William Dufresne represents the applicant Arthur and Carol Resca who are also present.
• Mr. Dufresne states the subdivision plan has been recorded. He reviews the previous ANRAD filed
with the commission in Jan/Feb by Leah Basbanes. He states 2 single family homes are proposed.
He reviews portions of the homes within the buffer zone. He states there is no work within the 50'
No Build Zone (NBZ) with the exception of a foundation drain.
• Mr. Lynch asks about the No-Disturbance Zone (NDZ) demarcation.
• The Administrator recommends a permanent barrier at the 25' NDZ. She recommends a 2' x 2'
fieldstone wall.
• Ms. Feltovic asks about the need for a land disturbance permit and about dewatering.
• Mr. Dufresne states he can provide a detail for dewatering.
• The Administrator states the land disturbance permit is the jurisdiction of the Planning Board. She
also asks about construction entrances.
• Mr. Lynch asks about the dump that was noted on the site during the delineation.
• Mr. Dufresne states restoration of that area is part of the filing.
• Mr. Manzi and Mr. Lynch state the restoration should occur prior to construction.
• A motion to continue the public hearing to 8/9/17 is made by Mr. Lynch and seconded by Ms.
Feltovic.

• The Administrator states she met on site with the homeowner to discuss what is needed to correct the
violation. She states the homeowner has requested a three month extension to engage an engineer or
contractor to permit the work.
• Mr. Lynch asks how the violation was discovered and the concern about collapse.
• The Administrator states the homeowner is new and didn't know there was a violation until he filed a small
project to replace a rotted deck.
• A motion to continue the matter to the second October meeting is made by Ms. Feltovic, seconded
by Mr. Lynch.

be removed.

30 years.
• The Commission discusses the procedures and the decision required.
• The Commission decides to continue the matter to the next meeting to get more information.

• A motion to issue the Order of Conditions as drafted is made by Mr. Manzi and seconded by Mr.
