Removal of Cement Walkway and Debris Pile & Installation of Crushed Stone
Conservation Conditions of Approval, NACC #108
Pursuant to sections 4.4.2 0) of the North Andover Wetlands Protection Regulations,Leonard Halloran,
applicant/property owner, filed a small project application for work proposed at 0 Innis Street (a.k.a.
Turnpike Street),North Andover. The work consists of the removal of an existing concrete walkway in its
entirety,removal of an existing debris pile and installation of clean crushed stone in two areas as shown on
the-record plan/sketch.All work will be conducted more than 50' from the Bordering Vegetated Wedand
(BVV/). Vehicles are not to be parked within the 100'Buffer Zone and No-Parking signs as well as
wetland markets will be placed on the existing fence.
During the June 12,2013 public meeting, the North Andover Conservation Commission (NACC) voted
unanimously to approve this project.The following conditions are hereby mandated:

CONDITIONS:
1. Prior to the start of work the owner shall ensure that the site contractor has reviewed the small project
permit and is aware of the wetland-resource areas and the limits of the proposed work.
2. No-Parking signs and wetland markets are to be installed along the existing fence. Wetland Markets (3
round for every 1 square-available at the Conservation Department) should be installed every 30 feet
along the fence. No-Parking signs should be installed every 50 feet.
3. Excess material shall be properly disposed of offsite. No material shall be stockpiled on site.

4. Accepted engineering and construction standards and procedures shall be followed in the
completion of the project.
5. Upon completion of the approved project and site stabilization,please contact the Conservation
Department for a final inspection.
6. This permit shall expire 6 months from the date of issuance.
