• Carefully reviewing each insurance form for omissions, corrections—following up with those individuals. Copying
each form, mailing to reimbursement company, and entering each vaccine given into the MILS database.

• MAVEN-Communicable disease-calls and follow ups. (Salmonella x3, Shiga Toxin, Campylobacter, LTBI).
• Sharps-change of service on their end-at length updating information, packaging of sharps for pickup
• Fielding phone calls and drop-ins from residents and staff for education, flu vaccine. Multiple sharps drop offs.
• Cholesterol--Proficiency Testing passed-starting first clinic by appointment on Wednesday 11/22-Will split BP
clinic timeframe (9-10 BP Clinic 10-11:30 Cholesterol screening)
• Library Series—first speaker will be a North Andover nutritionist from Nutrition in Motion.Talk is scheduled to
take place at the Library Wednesday January 10th from 6-8pm. Calls/emails to set up. Working on advertisement.
(future topics to include: opioids, mental health, dementia, heart disease, sun safety, tick borne diseases,
vaccines, smoking etc.)

Hey,
So, I'm only halfway through placing the"unused"needles she dropped off into sharps conatiners.
I am not sure if it on the webpage or not, but I think moving forward we should make it clear that we will not accept needles,used or unused,
unless they are in an approved sharps container.(I did make her take back her cardboard milk carton of needles so she could bring them back
in a sharps container). But her so called unused needles that were in bags did in fact have a few used needles in them.They were capped, but
this still poses a safety risk to me as I am going through them.
I also think moving forward we should have sharps containers that people can buy from us at cost value to make it easier for them to take care
of their sharps the way they are supposed to. I think sometimes people get lazy and don't want to go the extra step of looking for where to buy
something.If we have it available for whatever price we buy it for...that at least makes it a little more convenient for them.

family dwelling meets the-following-additional requirements:

area shall not exceed 50% of the original building's gross floor area. up to a
maximum of 1500 s.f, The size of the second dwelling unit can never exceed 1500

h. 1f a.conversion involves razing an existing staucture, the gross floor area of the
new residential structtvo shall not exceed 150% of the gross floor area of the
of ginal building, nor shall the new structure be. more.than 1,000 square feet of
gross floor area-larger than the 'original structure, Whichever is less. The size of
the second dwelling unit may never exceed 1,500 square feet, if an e�dsting lot is
srabdivided to form two or more new lots, and the existing structore Iies within
more than one of the new lots, and if the existing structure is to be. razed in
connection with the conversion, then new buildings on any lot formerly covered
by the existing structure hiust comply with all the provisions of 4,122.14.B of the
Bylaw. Newly created lots not f'ormexly, covered by the existing structure must
meet the requitements of 4.122.14,) of the Bylaw,
o. There must be two parldng spaces for each dwelling unit:
d. No paikingldriveway shall be permitted within 1-0 feet of any lot line,
e. No garage or cat-port shall face the street ui�css it is located of least 10 feet behind
the front fagade of the principal structure and in accordance with the dimensional
setbacks outlined in Table 2 of this Bylaw.
E The converted structure shall meet all of the dimensional requirements of the R-4
District identified in Table 2 of-this Bylaw.
g. Stairways leading to the second or atry higher floor shall be enclosed.
h. The principal building in a conversion to a two-family dwelling shall share a
connected common wall (or floor) for at least '75% of the wall's (or floor's)
surface. No unheated stiuetarc., no stiacturc without foundation, and no stractttre
that is entirely of pattially a garage shall be considered as meeting the 75%
requirement,
L The conversion of a one-family dwelling to a two-family dwelling;
1. Must not result in any portion of the post-conversion xoofline height
exceeding the pre-conversion roofline height by chore than five(5)feet,and
2. Must not significantly increase or decrease the pitch of any additional post-
conversion roof area
C, The conversion of an existing dwelling to aworninodate not more than five (5)
residential units, by special permit from. the Zoning Board of Appeals ih accordance
with gQaiow 1:0,3 and 4,. 22,14.33 of this Bylaw. The.conversion of a single family
dwelling to a two-fancily dwelling must comply with the provisions of Sections 10.3,
4,12.2.14.B,and 4122,14. D.
D. Special Permit Grarttizig Criteria for Two-Fainily Dwelling and One-Family to Two-
Family or Multi-Family Conversions,
a. The Zoning Board of Appeals may approve a special permit for a proposed use of
a building, dwelling of strgatixe provided by Section 4.122.14.A.b.,4.122.14,B or
4.122.14.0 ulion. finding that the application complies with the purposes of this
Bylaw, and is consistent with the use of the site. for the purpose permitted within

the Residential 4 District. In malting its decision,the Zoning.Board shall consider
the f6116Ning criteria in addition b'those listed in Section 10,31 i

21. Cpngregate Housing for Elders—Special Permit
a. In the R-4 zone the Plarming Board may grant a Special Permit for congregate
housing consistent with Special Permit criteria and procedures set forth in Section '
1.0.3 of this Bylaw.
b. The maxi num allowable FAR for congregate houshig shall be 00 in the R-4 District
seven(7)units.
c. In no instance shall any new or pre-existing building used for congregate elderly
housing,,have no more than fourteen(14) dwelling units,
d. All dhnensionM criteria established in Section 7, Table 2 Summary of Dirnehsional
Regulations shall apply to ail strac",es used for-congtegate lousing put-poses.
22. Family Suite—a separate dwelling unit within or attached to a dwelling for a member of
a household is allowable by Special Permit provided;
a. The dwelling unit is not. occupied :by anyone except brothers, sisters, maternal and
paternal parents and gratidparonts, or children of the residing owners of the dwelling
unit;
b. That the premises axe inspected annually by the Building Inspector for conformance
to this section ofthe.Bylaw;
c. The Special Permit shall be recorded.at the North Essex Registry of Deeds!
