PLANNERS ,SURVEYORS North Andover Board of Health 120 Main Street T I North Andover, MA 01845 S RECEIVED Re: 60 Deer Meadow Road - Revised Sanitary Disposal System Upgrade Plan TOWN OF NORTH ANDOVER HEALTH DEPARTMENT Dear Brian, Please find attached revised plans based on your comments dated September 18, 2017. The following comments were addressed: 1. According to the manufacturer, Roth Multil:anti's can be buried with up to 72" of cover following the installation of pipe supports that are available through the manufacturer. It is unclear however if the Roth MultiTank is considered 1I-20 following the installation of the pipe supports. To simplify matters and avoid the need for a potential DEP variance we have revised the plan to show no more than 36 inches of cover over the proposed tank. 2. Since the cover over the tank was reduced to less than 36 inches the need for a Local Upgrade Approval, (LUA), is no longer needed. The LUA request has been removed from the plan. 3. The profile specified that all piping proposed was Schedule 40 PVC, however,to clarify we added an additional note to the Construction Notes on sheet 2 of 3. Note#28 states that all piping must be Schedule 40 PVC. 4. A "key map" showing the full legal boundaries of the parcel was added to sheet 1 of 3. 5. The setback distances from the proposed SAS to the wetlands are shown on sheet 1 of 3. 6. Indication that the sewer piping shall have watertight joints and be laid on a compacted firm base in a continuous grade was added as construction note 427. 7. A test pit was dug into the side of the hill during the soil testing effort on August 9, 2017 however it was not logged and is not shown on the plan set. The test pit confirmed that there was at least 10 feet of fill material on the slope. The proposed septic tank is not in the estimated seasonal high water table and buoyancy will not be an issue. 8. The septic tank detail was updated as requested. q. A Label A1800 effluent filter is proposed on the outlet tee of the septic tank. A note was added to sheet 1 of 3 to insure that the proper effluent filter is installed. 10. A note specifying that the tank shall be set on mechanically compacted layer of 6" of crushed stone was added to both the profile view on sheet 3 of 3 and the septic tank detail 1 on sheet 2 of 3. 1.1. Note#6 was added to the distribution box detail on sheet 2 of 3 to insure that the outlet inverts are all equal. 13. Note#7 was added to the distribution box detail to insure proper installation of the inlet tees. 14. Note #5 was added to the distribution box detail to insure that the distribution boxes are set on compacted soil and 6" of crushed stone. 15. Construction note #29 was added to insure that the leaching pipes have 318"-518" orifices. 16. The building sewer lengths were adjusted to match the design plan.
