Williams & Sparages 189 forth Main Street Middleton, MA 01949 Re: Subsurface Sewage Disposal System Plan for 60 Deer Meadow Road, Map 104B,Lot 118 Dear Mr. Hochmuth: The proposed wastewater system design plan for the above site dated September 11, 2017 and received on September 13, 2017 has been reviewed. Unfortunately,the plan cannot be approved until the following items are corrected. The specific section in Title 5: 310 CMR 15.000, or North Andover regulation that is not met by this design follows each item, 1. The MassDEP approval for the brand and model tank being proposed(Sep-Tech by Fralo, also known as a Roth Multi-Tank) indicates that>36" of cover is not allowed. You may wish to seek a variance for this usage and if so it is suggested you provide background reference material from the manufacturer 2. The Local Upgrade Approval request for>36" cover over the septic tank can only be approved for tanks which have 11-20 loading. You may wish to modify the tank design to provide such loading, change its cover material amount and retract the Local Upgrade Approval request, or retract the LUA request and submit a Title 5 variance request (15.405(1)(b)) 3. Indicate that all piping must be Schedule 40 PVC (NA 3.2) 4. Show the full legal boundaries of the parcel (15.220(4)(a)) 5. Show the distance from the SAS to wetlands (NA 3.2) 6. Building sewer piping needs to have watertight joints specified, to be laid on a compact firm base and in a continuous grade (222(3)(4)(5)&(7)) 7. Tank details need to depict 3' air space above the tees, 9" above the flow line, tees extending 6" above the flow line, inlet tee 10" below the flow line, outlet tee 14" below the flow line and a gas baffle (or as may be altered in approval of poly tank being proposed) (227(1)(4)&(6)) 9. The design plan provides notes indicating an effluent filter is proposed inside the tank but no brand and model is specified 10. Soil compaction and 6" of stone beneath the tank is required (or as may be altered in approval of poly tank being proposed) (221(2)) 11. The tank is to be noted as watertight(221(1)) 12. The final distribution box needs to be notated to have all outlets at the same elevation (232(3)(b)) 13. Provide details and dimensions of distribution box inlet tees 14. All distribution boxes must have soil compaction and 6" of stone beneath them specified and to be watertight (221(1)&(2)) 15. Leaching pipes must have 3/8"-5/8" orifices specified(215(8)) 16.
