The proposed work includes the pumping, crushing and filling of the existing septic tank which is to be abandoned (a new septic system has been proposed outside of the Buffer Zone), All work is proposed more than 50' from the.Bordering Vegetated Wetland. Removal of three trees in the rear yard (within existing lawn) was also proposed and approved. During the July 8, 2015 public meeting, the NACC voted unanimously to approve this project All work shall conform to the following: RECORD DOCUMENTS: Small Project Piling Including: Application Checklist and Plan Titles: Septic Upgrade Plan 976 Turnpike Street Dated:June 22, 20115 Email from Jack Sullivan dated June 29, 20115 Email from Peg Gravel ne dated July 8, 2015 Application Received:June 25,2015 The following conditions .are hereby mandated: CONDITIONS: i 1. prior to the start of construction the contractor shall place staked hay bales and trenched siltation fence between the proposed work and the wetland resource area. When the above is complete, please contact the Conservation Department for a pre-- construction site visit. 3. The existing tank is to be abandoned pursuant to the regulations under Title 5 of the State Sanitary Code. 4. Brush and debris dumped down the slope within the Buffer Zone shall be removed and disposed of properly off-site. 5. Upon completion of the approved project and site stabilization,please contact the Conservation Department for a final inspection. 6. This permit shad expire six months from the date of issuance. The owner/applicant is proposing to upgrade the entire septic system due to the existing system being in failure. The entire proposed upgraded system will be outside the 100 foot buffer zone to the wetlands, but the existing septic tank which needs to be decommissioned per Title 5 requirements falls within the 100 buffer zone. In order for the contractor to properly decommission the septic tank (pump contents, crush tank, and fill with clean sand) it is highly likely that some work will take place within 100 feet of the wetland resource area to either crush the tank or remove the tank (if it is in conflict with the new septic tank location). To address this concern the owner is proposing the installation of silt fencing and staked hay'bales as a perimeter defense to potential silt migration. The closest point of the haybale line to the wetland line is 88 feet. Additionally, this shouldbe a short term project (1 day) to decommission existing tank, set the new tank, and temporarily stabilize the disturbed area with haymulch. have attached my plan showing the proposed work and the proposed silt fence/haybale line. Norse Environmental Services had delineated the wetland line and the flags were surveyed located. The flagging is still present in the field. If you have any questions please feel free to contact me. Date of Meeting: The delineated resource area boundaries have been confirmed by the Conservation Department as accurate. Plan Requirements(Section 4.4.3 B): Certified and/or professionally prepared plans are not required. However,the North Andover Conservation Commission or its designated agent shall have the final authority to determine whether the submitted plans/drawings are adequate and include all pertinent information for review.
