Construction of a 14' x 18' Deck plus stairs on Sono-tube Footings
Conservation Conditions of Approval,NACC #127
Pursuant to section 4.4.2 a) of the North Andover Wetlands Protection Regulations,Robert&
Wendy Wilson, filed for a small project for work proposed at 30 Campbell Road, North Andover.
The proposed work includes construction of a new 14' x 18' deck plus stairs (301 sq. ft. total) on
sono-tube. supports. The deck is approximately 71 feet from the edge of Bordering Vegetated
Wetland (BVW) as shown on the herein referenced plan.
During the May 28, 2014 public meeting, the NACC;voted unanimously to approve this project. All.
work shall conform to the following:
RECORD DOCUMENTS: Small Project Filing Including:
Application.Checklist and narrative;
MIMAP 2012 Aerial Map showing proposed deck and distance
to the BVW;
Construction detail sketch prepared by the contractor;
Filing dated: 5/20/14.
The following conditions are hereby mandated:
CONDITIONS:
1. Prior to the start of construction the applicant shall ensure that the site contractor has reviewed
the small project pernvt and is aware of the wetland resource area and the limits of the proposed
work.

2. Erosion controls in the form of a silt sock or straw wattle shall be installed at the, litnit of work.

3. Excess construction material shall be properly disposed of offsite and accepted engineering
and construction standards and procedures shall be followed in the completion of the
project.
4. The existing stone wall shall be made complete where stones are missing in the iniddle of the
wall, excess stones from the property or either end of the wall inay be used to complete the wall.
5. Upon completion of the approved project and site stabilization,please contact the Conservation
Department for a final inspection.
6. This permit shall...........expire six months from the date of issuance.

The delineated resource area boundaries have been confirmed by the Conservation Department as
accurate.

Plan Requirements(Section 4.4.3 B):
Certified and/or professionally prepared plans are not required. However,the North Andover Conservation
Commission or its designated agent shall have the final authority to determine whether the submitted
plans/drawings are adequate and include all pertinent information for review.
At a minimum,all plans submitted shall clearly and accurately show the present conditions and proposed
work and depict the location of all wetland resource areas within 7.00' of the proposed work. All plans must
be signed and dated by the person who prepared them.
The Commission may also require the following information on the proposed plans in addition to the
aforementioned criteria where applicable:
❑ Location of any subsurface sewage disposal systems.
❑ Locations of any drainage structures.
❑ Locations of any easements,including but not limited to, drainage and utility easements.
❑ Locations of any open space boundaries if(eg.if the lot is affected by the 50' open space boundary as
required in a Planned Residential Development(PRD)),

Location and nature of all proposed alterations to the Buffer Zone Resource Area.

❑ COMPLETE APPLICATION-The application meets the submittal requirements. Therefore, the
Conservation Department has prepared a written list of conditions necessary to protect any adjacent
wetlands for approval by the Conservation Commission.
This condition list shall be filed with the North Andover Conservation Department as NACC# 1-' 7
❑ INCOMPLETE APPLICATION-The following sections of the Wetland Protection Bylaw&/or
Regulations have not been complied with and as a result this Department is rejecting this income
a 1rp 'cation w/in two (2)business days from the date of receipt (Section 178.4 North Andover Wetland
Protection Bylaw). You may pick up this application and supporting plans at the Conservation Office and
re-file with the required supporting documents and information.
❑ NOTA SMALL PROJECT-The North Andover Conservation Commission has determined that the
proposed project as submitted does not meet the small project criteria and hereby denies the application.
As such,a Request for Determination of Applicability(RDA) or a Notice of Intent(NOI) must be filed.
