RE: Conditional Operational Approval Boston Hill&Forest View Sewer Pumping Stations
Dear Mr. Grandstaff:
This letter grants CONDITIONAL operational approval to the Boston Hill and Forest View Sewer Pump
Stations for 60 days.This"conditional operational approval"expires,ceases,and is void if the following
conditions are not met or completed within 60 days of the date of this letter.
CONDITIONS
1. Attachment labeled"A"is a condition of this approval,with particular reference to the last
paragraph on the first page. Also, all the items listed under"Repair, alteration and
additional still need to be done"they are as follows:
A. Pump performance
B. Pump Rooms
C. Grounds
D. Lighting
E. Wet Wells
F. Electronics and Data Transmission
2. A revised"As-Built"plan must be submitted for review by Guertin and Associates and
Town personnel.
A. Must be submitted within 30 days to allow time for proper review.
B. If revisions and additional data are required this must be completed and
resubmitted for review with sufficient time within the 60 days.
C. A copy of the preliminary record drawing comment list is attached
3. Permanent easements must be provided to the Town of North Andover,Division of Public
Works to access and repair both the Boston Hill and Forest View sewer pump stations.

Also, your acceptance of this"Conditional Operational Approval"is also granting us full
access to the aforesaid pump stations for this 60-day period, The easements must be
delivered,within this 60-day period,and recorded at the Registry of Deeds,North Essex
County and proof of said recording is delivered to the Division of Public Works.
4. The length of sewer pipe between NH 208+54 and 206+17.40 after review by Guertin and
Associates is temporarily permitted to remain, based on the calculations submitted by
Mark Gross,P.E. dated May 12, 2000. The personnel of the Town of North Andover will
monitor this line. If unacceptable problems occur during the five-year maintenance period
of Mesiti Development Corporation,this and any other work needed to correct the
situation will be completed prior to the stations being accepted by the Town of North
Andover. All of this work will be completed at the expense of Mesiti Development
Corporation, or its successor in title. The Director of the Division of Public Works will be
the sole authority to determine the acceptability of this line. The Director's decision is final
and if work is required, and is not performed,band money will be utilized to complete this
work.
5. The"Preventive Maintenance Contract"is unacceptable as written. Comments follow:
A. AUTHORITIES Section,the first"C"If Roy's men refuse to enter the site,who
will enter the site?
B. AUTHORITIES Section,the second`B"who will do this work and who will pay?
C. AUTHORITIES Section, the second"C"who will be refused access?The Town
of North Andover personnel& other maintenance personnel cannot be denied
access at any time and need keys to the buildings.
D. AUTHORITIES Section,the second`W"this is UNACCEPTABLE the contract
cannot be canceled without 45 day written notice delivered to the Division of
Public Works by certified mail and the Mesiti Development corporation must have
a new firm under a new contract within 21 days of any notice of termination of
service. This new firm must be pre-approved by the DPW.
E. SERVICES section B), 4 how will services and parts over$100.00 be provided
for?
F. NON-PROVIDED SERVICES who will provide these services and who will pay
for them?
G. FREQUENCY OF VISITS 1 per month is unacceptable. The stations must be
visited once per week and Roy must submit copies of all reports to the Town of
North Andover weekly.
H. This contract is only for one year who will provide this service for the remaining 4
years and how will they be paid
I.
All purchasers of any property discharging into this sewer system shall be given a copy
this document by Mesiti Development Corp., or its successors in title prior to conveyance.

The Town of North Andover,Division of Public Works needs unlimited authority to
accomplish whatever is necessary to insure the proper operation and maintenance of the two
pumping stations. Any clause either written or omitted that could inhibit or limit this authority is
unacceptable and void.
All of these items must be addressed within the 60-day conditional operational period. A
cash bond will be accepted in the amount of$25,000.00 and a paper bond of$100,000.00 for
those items only which must extend out the 5-year maintenance period Mesiti is responsible. This
bond is for the purpose of providing repairs,maintenance, utility services and any other service
that may be required if Mesiti Development Corporation fails to provide them. All other items
must be completed within the 60-day life of the"Conditional Operational Approval.
It has been mentioned that Pulte Home Corporation may wish to bond or guarantee the
work to be completed. This is acceptable to the Division and the details on how this will be done
must be acceptable to the DPW and submitted prior to the 60-day approval expiring, All permits
issued to Pulte and work done based on this 60 day conditional approval is done at the applicants
risk.

Boston Hill and Forest View Sewer Pump Stations
On June 5a`, 2000 Inspection and operation of the Boston Hill and Forest View Sewer
Pump Stations were conducted. On site were Dennis Bedrosian,Jerome Welch, Brian Ford
and Youssef Issa from the North Andover Water/Sewer Dept. Also there was George Roy
of George Roy Mechanical Services,Dick Pelletier and Moe Harpin of Mesiti Construction,
Fred Wise of the Wise Company Inc, and Bill Stricht from Superior Concrete.
Below are some of the conditions we created in order to determine this station readiness to
operate.
The pumps were operated at each station in auto mode,based on level control set points.
The pumps were tested for alternation(order of operation).
The backup level float control system was tested in manual mode for proper operation in the
event of failure of the normal auto shutoff level control system.
The main power was shut off while a pump was in operation (building power drop test) to
determine if the Emergency Power Generator would operate and provide electricity
needed to the building without straining power to machinery.
The pumps and motors were felt for vibration.
Fifteen-second time delays were adjusted to the electrical start components of the motors in order
to reduce electrical demand and prevent potential damage to the pumps.
It is my opinion that the Boston Hill and Forest View SPS's are suffic_, ienth capable of
operation at this time.
The pumps started, stopped and alternated properly in automatic and manual control.
The Emergency power generators operated without problem and provided electricity to the
pumps without major strain.
The automatic and backup level control systems worked without failure.
Regardless of who the owner of Boston Hill and Forest View SPS's are just prior to the time of
transfer to the Town of North Andover, if any conditions mentioned on this report aren't
completed then the owner will be responsible for completing them as well as any necessary
repairs. This includes upgrading the station due to any unforeseen changes in development or
construction such as additional residential, commercial or industrial properties that places a
greater demand on these stations in comparison to what was originally designed.
Repair, alteration and additional work still need to bbe,done
The following work must be completed in good assurance to the Town of North Andover. It is
my understanding from conversations with Moe Harpin that arrangements are presently being
made for the continued troubleshooting and correction of the following.

Boston Hill and Forest View Sewer Pump Stations
Pump performance
Pump hydraulic performance testing according to the Hydraulic Institute of Standards.
Verify that each pump in the field can meet the certified curves based on manufacturers bench
line testing.
Insure efficiencies are met according to the Town of North Andover hydraulic requirements in
respects to the subject areas.
Each pump must be tested at least five points along the pump curve by a professionally, qualified
person using test equipment or instruments which are newly calibrated and certified for such
work.
Along with these tests I require pump-motor vibration testing and analysis work to be performed
by reputable professionals of that field of work. This must include temperature testing of the
machinery in respects to their bearing area.
The emergency power generator at Boston Hill took a little longer to fire up vs. Forest View.
Pump Rooms
Ventilation must be lowered closer to floor elevation
Change all pump discharge psi gauges to the correct psi range
Boston Hill#2 pump makes a squeak noise in flex coupling area.
Forest Hill pumps 1 & 2 making knocking noise in pump impeller case due to either pre rotation
or cavitations.,
Forest Hill ground water leaking through concrete wall next to pump#2.
Grounds
Ponding and poor drainage behind Forest View SPS
Require fencing around stations.

Replace fluorescent light fixtures in both stations(see submittal).
Repair emergency light pack at Forest View.
Install light fixtures above outside entrance doors at both stations.
Wet Wells
The Town of North Andover Water/Sewer Dept reserves the right to inspect the interior of the
both wet wells prior to going in service for proper design and construction.
All construction debris and foreign matter must be removed from the wet wells prior to going in
service
Install clear plastic shields over outside valves both stations(refer to Rodney Hunt Co).
Wet well wash down 1 1/2 " water line, must core through wail and elbow this line to the
outside.
Wet well vent pipe is should be schedule 80 PVC not schedule 40.
Wet well vent pipes need a support brackets.

AUTHORITIES:
The signing of this contract provides George Roy Mechanical Services with the following authorities:
A) Entrance to all locations owned by the customer and serviced by George Roy Mechanical
Services as indicated below under"SITES."
B) Performance of preventive maintenance including adjustments,changes,minor repairs,and
the like to the customer's equipment.
C) Refusal to enter any of the customers's sites which may present any unusual hazard to the
company's personnel.
The signing of this contract allows the customer the following authorities:
A) Maintaining the status quo at any site.
B) Rejection of any service work over and above preventive maintenance.
C) Refusal of access to any site outside normal business hours.
D) Termination of contract prior to completion upon written request of George Roy Mechanical
Services.
SITES:
The following customer sites will be covered by this contract:
A) 2 pump stations(North Andover,Route 114)
Identified as Route 114/Palomino Drive and Route 1 I4/Boston Hill pumping stations
Ree approved plans designed by Camp,Dresser,&MacKee(CDM)
SERVICES:
George Roy Mechanical Services will provide the following services as a function of this contract:
A) Checklist to include:
1) pump(s)operation manual and automatic;
2) controls condition and operation;
3) wet-well inspection;
4) building/structure inspection;and
5) emergency generator or emergency pump inspection.
B) Service to include:
1) oil and filter change on engine driven equipment;
2) packing changes on pump(s)—where applicable;
3) test with report on emergency generator and related apparatus;and
4) minor services including parts totaling no more than$100.00 over the term of this
coytract.
NON-PROVIDED SERVICES:
Georg:.Roy Mechanical Services will not provide the following services as a function of this contract:
A) Major overhaul—retrofitting existing equipment due to imminent or existing failure.
B) Major service—services which require extended down time.
C) Secondary services—bypass pumping,vacuum pumping,wet-well cleaning and the like.
D) Cleaning,painting,and surface preparation for any coatings.
E) Trucking—services to move equipment or materials on or off site.
F) Emergency response.(see comments on Special Conditions)
HOLD HARMLESS:
The customer shall hold harmless George Roy Mechanical Services and any of its personnel from any and
all liabilities arriving from any activities of the personnel providing services to the customer under the
terms of this contract.
George Roy Mechanical Services shall hold harmless the customer from any and all liabilities resulting
from injuries and damages to the Company's personnel or equipment during the performance of the
services of this contract at the customer's site.
FREQUENCY OF VISITS:
This contract allows for 1 visit(s)to each site(monthly or as required by manufacturer).
COST AND PAYMENTS:
Payment in the amount of$200.00 monthly($100.00 per site)shall be made to George Roy Mechanical
Services upon completion of each visit. Terms are net 30 days with 2%interest per month for accounts
over thirty days.
COMMENTS OR SPECIAL PROVISIONS: (list as required)
(1)Emergency Service/Response-$100.00 per hour for emergency calls from monitoring
company and/or Town of North Andover.
SIGNATURES:
The customer agrees to allow George Roy Mechanical Services to provide preventive maintenance to the site(s)
listed above for a period of one year,commencing from the date of signing this contract
George Roy Mechanical Services agrees to provide the services as outlined above for the customer at the site(s)
listed above and under the conditions and terms of this contract

Dear Mr. Pelletier,
As requested by Moe Harpin,Masterson Construction has had an engineering analysis completed
to determine if the slope of the 8"diameter sewer pipe installed between MH 208+54 and MH
206+17.40 was sufficient.
Based on the attached engineering calculations completed by MBF Design Consultants,Inc.the
slope of 0.0036 ft/ft is sufficient to provide the required minimum of velocity of 2 to 3 feet per
second.

Dear Mr.Dion:
Per your request, this letter serves to address the issue regarding the as-built slope on a section of the sewer
line constructed on Route 114 in the Town of North Andover,Massachusetts. Based on our as-built
survey,the slope on this particular section of 8"sewer line was calculated to be 0.0036 8/$. Attached are
calculations which determine the velocity in this pipe based on full flow conditions within this section of
sewer pipe. A velocity of 2.58 fps was calculated based on full flow of this 8"pipe. This well within the
range of 2 to 3 fps minimum velocity required for scouring under full flow conditions

A. According to the drawings the new SMH 40+30.3 is higher than the SMH
at Hillside Road. ??
B. There are no manhole ties on this drawing. (This is true of all drawings)
C. There are no ties to the service connections.(#607&623)
D. Note 3 says force main has 3.5'min. cover. Profile says 4 feet.
E. Drawings should be stamped and signed.
F. Drawing title should show stationing on that drawing.
2. Drawing No. 2
A. Detail sheets need to be attached. (What does FM cleanout manhole look
like?) (Does it require periodic maintenance: If so,how often and what
maintenance?)
B. Sta. 59+00 There is a 10"FM in a 12"sleeve. (The sleeve is not large
enough to remove and replace the main.)
C. There should be periodic ties to a force main of this length. i.e. How far
off the streetline etc.

A. Bends on force main should be tied down.

A. Note 2. This kind of statement is not acceptable on a record drawing.
What good is the drawing?
