Dear Town Clerk:
Pursuant to Section &A of the Rules and Regulations of the Planning Board of the Town of North
Andover, you are hereby notified that on May 2, 1997, a Form A Application, filing fee, and the
original and five (5) copies of a plan entitled"Plan of Land in North Andover, Mass.", by Hayes
Engineering, Inc., Scale: 1" = 40', dated April 14, 1997; was submitted to the Planning Board for
endorsement. Attached hereto is a copy of the Form A Application and attachments.

The undersigned wishes to record. the accompanying plan and requests a
determination by said Board that approval by it under the Subdivision
Control Law is not required. The undersigned believes that such approval
