Abandonment of existing septic tank, Installation of new septic pipe and
minor grading
Conservation Conditions of Approval,NACC #145
Pursuant to section 4.4.2 (1.,) of the North Andover Wetlands Protection Regulations,Jeffrey
Greene., haled for a small project for work proposed at 1353 Salem Street, North Andover. The
proposed work includes the purnping, crushing and filling of the existing septic tank which is to be
abandoned, the installation of approximately 17 feet of new 4" PVC septic line and minor grading.
All work is proposed more titian 50' from the Bordering Vegetated Wetland.
During the May 27, 2015 public meeting, the NACC;voted unanimously to approve this project. All
work shall conform to the following:

The following conditions are hereby mandated:

CONDITIONS:
1. Prior to the start of construction the contractor shall place a minimum 12" silt sock (or other
erosion control as approved by the Conservation Department) between the proposed work and
the wetland resource area and contact the Conservation Department fora pre-construction site
visit.

2. The existing tank is to be abandoned pursuant to the regulations under Title 5 of the State
Sanitary Code.
3. Excess construction material shall be properly disposed of offsite and accepted engineering and
construction standards and procedures shall be followed in the completion of the project.
4. Upon completion of the approved project and site stabilization,please contact the Conservation
Department for a final inspection.
5. This permit shall expire six months from the date of issuance.

❑ A. Open slotted decks,without roofs,equal to or less than 300 square feet surface area,inclusive
of stairs and ramps,which are to be attached to an existing home,where soil disruption during
construction is limited to less than 50 square feet,and where no permanent soil alterations are
planned under the footprint of the deck
❑ B. In Find replacement of existing decks,regardless of size.

❑ C. Sheds or other outbuildings of less than 100 square feet,which are to be placed on footings,
not permanent foundations.
D. Sewer system connections as required by the Town of North.Andover.

❑ Two (2) copies of a 1" =20' scale (or other as deemed appropriate by the Conservation Commission or
agent thereof) drawing showing existing and proposed conditions and the delineated wetland resource
areas (see plan requirements below for additional detail).

❑ A pre-construction/pre-approval meeting was held on site between the applicant and a representative of
the Conservation Commission. Date of Meeting:
❑ The delineated resource area boundaries have been confirmed by the Conservation Department as
accurate.
Plan Re uirements Section 4.4.3 B):
Certified and/or professionally prepared plans are not required. However,the North Andover Conservation
Commission or its designated agent shall have the final authority to determine whether the submitted
plans/drawings are adequate and include all pertinent information for review.
At a minimum,all plans submitted shall clearly and accurately show the present conditions and proposed
work and depict the location of all wetland resource areas within 100' of the proposed work. All plans must
be signed and dated by the person who prepared them.
The Commission may also require the following information on the proposed plans in addition to the
aforementioned criteria where applicable:
❑ Location of any subsurface sewage disposal systems.
❑ Locations of any drainage structures.
❑ Locations of any easements,including but not limited to,drainage and utility easements.
❑ Locations of any open space boundaries if(e.g.if the lot is affected by the 50' open space boundary as
required in a Planned Residential Development(PRD)).
❑ Location and nature of all proposed alterations to the Buffer Zone Resource Area.

E' Regulations have not been complied with and as a result this Department is rejecting this incomplete
application w/in two (2)business days from the date of receipt (Section 178.4 North Andover Wetland
Protection Bylaw). You may pick up this application and supporting plans at the Conservation Office and
re-file with the required supporting documents and information.
❑ NOT A SMALL PROJECT-The North Andover Conservation Commission has determined that the
proposed project as submitted does not meet the small project criteria and hereby denies the application.
As such,a Request for Determination of Applicability(RDA)or a Notice of Intent(NOI)must be filed.

Small Project Narrative
The locus property is 1353 Salem Street, North Andover, MA 01845
The owner/applicant proposes to abandon an existing septic tank which is located
approximately 85 feet from a BVW and construct 17 feet of a 4" PVC sewer line
which is located approximately 90 feet from a BVW.
The septic tank will be pumped, crushed and removed from the site and the
sewer line trench will be dug approximately 3 feet deep and 3 feet wide, both
excavations will be backfilled, compacted, loamed and seeded upon completion
of construction.
The work is shown on an Upgraded Sewage Disposal System Plan prepared by
Merrimack Engineering Services, dated 4-28-15 and submitted to the Board of
Health for approval.

"Phis letter is in regards to a proposed extension to the existing rear porch at 136 Old Farm Rd.As outlined in the attached
drawings we would like to extend a portion of the deck 4' toward the rear-of the property. Tile redesigned deck would have
a new set of stairs to a hinding platform then one more step to grade. The new stairs would intersect the existing chain link
fence around the existing in-ground pool. This would allow the residents to put a gate across the strep going toward the fool,
while slip allowing their children access to their swing-set on the other side of the fence, 1 have included a site plan tl�
wetland boundaries as they were staked out by Seekaunp Environmental Consultants oil May 6, 2015. The rrew deck
extension would require two new 12" sono tube footings and four 8" that would be located outside the 50' no build zone.
Tire new stairs would require two 4-6"'x10"footings that would also be outside the 50'no build zone. Tire deck is
Iapproximately 5'above finished grade. The new deck surd stair design would lreally improve the safety of the residents'
yard while still allowing the access to their play area.
