Signature_Llm '�VA& J , I , G wpaform 8b.doc•rev.7/13104 Page 2 of 4 OVJOJJV —L. rrHaC Cly OPERATIONAND MAINTENANCE PROCEDURES FOR FOREST VIEW SUBDIVISION ROUTE 114 NORTHANDOVER., MASSACHUSETTS Operation and Maintenance Plan An Effective Site Operation and Maintenance Plan is essential for the proper operation of the stormwater management systems that are designed to provide for water quality and quantity measures. The stormwater management system owner will be the developer of the subdivision until such time as the Town takes ownership of the roads and associated utility systems including the drainage facilities. The developer of the subdivision wall be Mesiti Development.Corporation, 11 Old Boston Road, Tewksbury,Massachusetts, 01876. Routine and non-routine maintenance tasks which are part of these procedures are defined and listed for the Operation and Maintenance Plan and the schedule for the _ proposed maintenance plan is outlined in this report. It is also suggested that the Operation and Maintenance Procedures for this site be made part of the conditions-of approval.for the Order of Conditions that may be issued for this project and a copy be put on file with the Town of North Andover. In addition, maintenance and inspection reports prepared for the owner as part of this project during the initial construction can also be submitted to the Town of North Andover, if required. I. STREET SWEEPING Sweeping of the streets shall be done monthly during construction of the subdivision and then will be done seasonally upon completion of the remaining portions of the subdivision roads. Street sweeping in the spring will remove much of the sand and other debris that is often left as a result of snow plowing. Snow removal from the site or placement of snow outside of paved areas will also prevent sand and debris accumulation in tate paved areas and reduce the amount of materials that could potentially enter the storm drainage system. Accumulated sediment in non-paved areas from snow storage shall be removed as well. In addition, sweeping:of the roadways shall be done in the fall after the majority of the leaf drop occurs but prior to the first snowfall. i rL CATCH BASIN CLEANING The removal of sediments and associated pollutants and trash occurs only when inlets or sumps are cleaned out;therefore regular maintenance of the catch basin sumps is essential to the longevity_ofthe storrawater drainage system. The more frequent the cleaning, the less likely sediments and trash will be resuspended and discharged downstream to the drainage system.Frequent cleaning also provides more volume for future storms and enhances overall performance. Minimum requirements for cleaning of inlets and sumps for catch basins as recommended under the Stormwater Management policy are four tunes a year. IIL WATER QUAI.I Y SWALE Water quality swales should be inspected at least semi-annually, and maintenance and repairs nude as necesmy. Additional inspections should be made during the first few months after construction-to make sure that the vegetation has been adequately established. Repairs and reseedingshould be done as required. Water quality swales s6uld be mowed ohm a year.and grass clippings be removed. Grass must not be cut shorter than 4 inches and eitcessive mowing is discouraged. Sediment and debris removal should be done xnanually,at least once a year. Water quality swale planting specifications are outlined on the plans and are as listed below: WATER QUALffY SWALE PLANTING SPECIFICATIONS Tall fescue ` 201bs/acre or 0.45 lbs/10,000 sf Creeping red fescue 20 lbs/acre or 0.45 lbs/10,000 sf Birdsfoot trefoil 81bs/acre or 0.20 lbs/10,000 sf Une and fertilizer should be applied prior to or at time of seeding and incorporatedinto the soil. The following rates are recommended: Agricultural limestone 2 tons/acre or 100 lbs/1,000 sf Nitrogen(N) 50 lbs/acre or 1.1 1bsIM000 sf Phosphate(P205) 100 lbs/acre or 2.2 lbs/10,000 sf Potash(K2O) 100 lbs/acre or 2.2 lbs/10,000 sf (This is;equivalent to 500 lbs/acre of 10-20-20 fertilizer or 1,000-lbs/acre of 5-1010). J I9flf LGJ1 Vv� IV. SEDIMENT FOREBAYS/DETENTION BASINS There are six detention basins/sediment forebays proposed for the site. Pretreatment through the use ofdeep*sump hooded catch basins and sediment forebaycoflection systems. and the proper maintenance of these pretreatment systems as described above will eliminate sediment and litter accumulation within the proposed detention basins. It is, essential to any detention system to perforin a proper inspection and maintenance program. The proposed detention systems shall be inspected biannually or after a heavy rainfall and cleaned-is needed,depending on the amount of sediment accuandation.in the systems. The outfall structures as well as the emergency overflow structures shall be checked as well and debris cleaned from the trash racks, if required. V. ROOF INFILTRATOR SYSTEMS - The responsibility of the maintenance of the rooftop infiltrator systems will be by the individual homeowner. The developer will notify the buyer of the requirements of this system and it's maintenance and provide the homeowner a.copy of this manual as well as a copy of the specified matudhaure r's.mamud for these systems. The roof infiltrator systems will be checked yearly for proper operation or after a major storm event. Each infiltrator system shall be provided with a winin=of two inspection ports located at each end of the system. Should a system start to experience failure,upon inspection;if siltation has occurred within the system,.there are two options available for remediating the system. Option one is to clean.the system using high pressure water injected into the system and. having the water borne sediment removed by a licensed septic hauler. Each infiltrator chamber is provide with a"knockout"in the center of the unit. This may be used to Provide an inspection Part,at! de level. A 6"PVC pipe is placed through the access port as a riser to grade and then capped with a cast iron cover. Inspections shall be conducted following major storm events(25 year storm or greater) so that potential sediment levels and draw down times can be determined. Various types of equipment are available commercially for clean out of drainage systema. The most commonly used equipment and techniques.used for cleaning systems are vacuum pump and water jet spray; Both systems are generally mounted on a self contained vehicle and can effectively remove stones,leavesand sediment deposits from sumps and chambers.. Should cleaning of the system not be effective,then replacement of the system and stone bed would be required. .
