COOK & COMPANY INSURANCE SERVICES, INC.

COOK & COMPANY INSURANCE SERVICES, INC. agrees to perform services
relative to the Town of North Andover's Health Plans (hereinafter referred to as Town)
as are listed below:
• General consulting and advisory services related to the day-to-day finance,
operation and management of the Town's group employee health insurance
programs.
• Attendance at monthly Group Insurance Commission meetings. Will prepare
written report for submission to Town regarding all issues discussed at meeting.
• Communicate to Town benefit changes voted at February Commissioner's
meeting.
• Communicate to Town rate changes voted at March Commissioner's meeting.
• Act as a liaison between the Town and Group Insurance Commission, when
appropriate.
• Upon request, attend all meetings between Town management and the Public
Employee Committee.
• Attendance at appropriate budget meetings, when necessary, with the Town
officials to assist with the preparation of annual budget calculations.
• Advise the Town of changes in state and federal laws which may impact the cost
and/or administration of Town's health benefit program; provide interpretations
regarding compliance with these statutes.
Advise Town on issues subject to collective bargaining relevant to benefits -
related costs, proposals, or related matters.
Perform an enrollment audit on the Town's entire roster of enrolled subscribers
in the GIC plan to ensure that payroll/retiree deductions are accurate based on
date of hire, date of enrollment, and date of retirement.
COOK & COMPANY INSURANCE SERVICES, INC. agrees to perform these
services for the period November 16, 2015 through November 15, 2016. COOK &
COMPANY & COMPANY INSURANCE SERVICES, INC. shall receive as
compensation for its services under this contract the amount of eight thousand ($8,000)
dollars per year.
COOK & COMPANY INSURANCE SERVICES, INC. agrees that either party may
terminate this contract upon thirty (30) days written notice. COMPANY & COMPANY
INSURANCE SERVICES, INC. will receive payment for services performed up to the
termination of the contract on a prorated basis.
This contract, executed on the date first hereinabove mentioned, in duplicate, is
intended to take effect as a sealed instrument.
