General Scope of Work Description:
WE HEREBY SUBMIT SPECIFICATIONS AND CONTRACT FOR: Carol &
John Bianchie

Scope of Work:
1. William Bruinsma Construction confirms that it is licensed, insured as
required and ensures any subcontractors utilized on this site will have
the appropriate insurance coverage.
2. Meet with local building, electrical, and plumbing officials and apply for
permits.
3. All work will be in accordance with local building code regulations and
will be inspected by local officials prior to continuing with next phase.
4. William Bruinsma Carpentry will be responsible for the safe storage of all
its property and any materials to be used on the site. Any items,left on
site will be covered under William Bruinsma Carpentry insurances.
5. Owner is responsible for removal and return when necessary of all items
within the house and their safe storage prior to our work commencing.
6. Owner will then be responsible for returning all items to the home after
completion of the "Scope of Work".
7. William Bruinsma Carpentry will remove all trash associated with this
project into a dumpster and arrange for safe disposal in to a registered
site.
Demolition:
1. Take existing garage down to existing foundation.
2. Take down storage shed attached to garage.
Foundation:
1. 6' freeze wall tied into existing, measurements supplied by homeowner in approved
plans.
Framing:
1. Frame garage, mudroom & partitions according to blueprints supplied by homeowner to
building code.
Exterior:
1. To match existing, consisting of Hardie plank siding.
Trim:
1. Azek trim boards to match existing.
Carpentry "exterior":
1. Trim facia, corners, clapboards to match existing.
2. Trim Windows to match existing.
3. Bump out rear window to aprox. 4.5 inches.
Interior:
1. Insulate to code.
2. Blue board and plaster to smooth finish in second level & mudroom, garage plastered to
rough coat.
3. Trim to match existing, 6 panel hollow core doors.
Floors:
1. 7.00/sq.foot allotment,tile 7.00/sq. foot, labor only.
Kitchen:
1. Due to no definitive plans, allotted $3500.00 for cabinets (appliances supplied by
homeowner).
Bathroom:
1. Tile floor(Tile supplied by homeowner) consisting of toilet, fiberglass shower&sink (all
fixtures supplied by homeowner).
Laundry Room:
1. Plumbed for washing machine &wired for electric dryer (washer and dryer supplied by
homeowner).
Heat:
1. 2 wall mounted units.
Garage Doors:
1. 2 middle of the line insulated with glass, allotment $2,500.00
Total Cost: $141,500.00
Payments:
1/3 to start
1/3 when roofing is complete
1/3 at completion of job

A Special Permit for the R-3 Zoning District is needed. The Petitioner is requesting a Special Permit to construct a

Legal notices were sent to all the certified abutters provided by the Town of North Andover,Assessors Office,and
were published in the Eagle-Tribune,a newspaper of general circulation in the Town of North Andover,June 14,
2016,and June 21,2016.
The following regular voting members were present:Albert P.Manzi III,Ellen P.McIntyre,D.Paul Koch Jr.,Doug

The Board finds that this use, as developed by the building and site plans, will not adversely affect the
neighborhood. There will be no nuisance or serious hazard to vehicles or pedestrians since there are provisions for
the required off-street parking. Adequate and appropriate facilities are provided to the existing residential dwelling 1
and will be provided for the proper operation of a-Family Suite. The Board finds that the Family Suite will not be i
substantially more detrimental than the existing single family dwelling to the neighborhood and that this use, to
