CHANGE ORDER REQUEST
This is an agreement between the undersigned homeowner and contractor to
approve and carry out work that is in addition to the original project scope. Both
parties understand that the requested additions, as specified below.
Contractor will provide all labor and material for the following work:
1. Removed & install 16 Extra Windows
2. Replaced Front door
3. Rebuild main roof soffits
4. Replaced Heating Boiler
5. Rebuild kitchen floor with 2x6x8 and % cdx plywood
6. Demo kitchen chimney & downstairs bathroom
7. Demo downstairs plumbing
8. Demo Main kitchen cabinets
9. Demo Kitchen closet
10.Demo kitchen floor and hallway floor
11.Frame in new downstairs bathroom
12.Installed new LVLs on extra walls in kitchen
13.Removed existing floor between family room & kitchen existing 2x4 joist
sitting on ground kd wood
14.Rebuild with rebar & 6" concrete per engineer
15.Installed 2x12 Ivls between existing family room & kitchen
16.Removed main stairs & basement stairs facing kitchen area
17.Install new plumbing for kitchen sink, dishwasher, shower, sink &toilet for
down stairs bathroom. Installed new washer hookup in new Laundry
