This agreement is made on THURSDAY,MAY 16, 2013 by and between RICHARD A. FIORENZA
167 WASHINGTON ST, BOXFORD,MA 0 192 1.Mass. Home Improvement License number
126125 and Mass.Unrestricted Supervisor License number C867249 (hereinafter"Contractor")
and DARREN&FELICIA HOPKINS 11 HAMILTON RD, NORTH ANDOVER,MA 01845
(hereinafter "Homeowner").
WITNESSETH:
The Homeowner does hereby employ the Contractor to provide the labor,tools,machinery,
materials and supervision necessary for the construction of a HOME ADDITION located at
11 HAMILTON RD,NORTH ANDOVER,MA 01845 and based on architectural drawings
supplied to this contractor by W.A. HURLEY AND ASSOCIATES dated March 29, 2013 for the
amount detailed in the breakdown below.
THE WORK TO BE PERFORMED BY THE CONTRACTOR WILL CONSIST OF:

CHANGE ORDERS:
The Homeowner has the right to make changes to the project after the contract is signed.
The Contractor agrees not to make any changes in the schedule of work,design or
specifications outlined in the provided architectural drawings without written authorization
or confirmation email by the Homeowner(Change Order)and the Contractor. Any changes
requested will also require a written estimate or confirmation email of additional costs provided
by the Contractor with written authorization or confirmation email by the Homeowner.
WORK PERFORMANCE AND MISC NOTES:
• The Contractor shall not substitute any materials without written consent of Homeowner.
• The Contractor shall complete all work in a timely manner,time being of the essence
of this contract. Project time frame 3-4 weeks.Payment schedule will be broken into 5
drawers,with 1"drawer due upon issuance of permit and balance due upon completion.
All checks made payable to Richard Fiorenza.
0 The Contractor shall carry Workman's Compensation insurance with the limits of
$500.000 and shall provide certificate of proof to the North Andover Building Department
as required by the North Andover Building Department upon filing of permit application.
• The Contractor shall protect all work adjacent to the Contract site from any damage
resulting from the work of the Contractor. Homeowner is expected to remove all personal
belongings and breakables from the Contract site prior to project start.
• The Contractor shall replace and put in good condition any existing conditions damaged in
carrying out the contract.
• The Contractor shall take all precautions to protect persons from injury and unnecessary
interference or inconvenience.
• The Contractor shall conduct his activities in a businesslike manner and adhere to the
reasonable wishes of the Homeowner in relation to his working schedule.
CONDITION OF PREMISES:
• The Contractor agrees to keep the premises clean and orderly and to remove all debris as
needed during the hours of work in order to maintain work conditions which do not cause
health or safety hazards.
• The Homeowner shall have the right to inspect all work performed under this contract.
• The Contractor is responsible for all inspections required by the Town of North Andover
• Building Dept. at each necessary stage before further construction can continue.
