Safe Load: a load determined by using a safety factor. Safety Factor: a factor that engineers use to allow for the failure stress or stresses assumed to exist in a structure or a member. It provide&a margin of error in the strength, rigidity, deformation, and endurance of a structure or its component parts to compensate for irregularities in structural materials and workmanship, uncertainties involved in mathematical analysis and stress distribution, service deterioration, and other unevaluated conditions. Dead Load: a static load due to the weight of the structure. Live Load: a dynamic load (such as traffic) that is applied to a structure suddenly or that is accompanied by vibration, oscillation, or other conditions that affects its intensity. The design has been approved for use in the construction of an onsite wastewater system. The proval included a local variance to allow a dwelling be constructed with a cellar floor elevation below the estimated seasonal high groundwater elevation. At a regularly scheduled Board of Health meeting, held on January 18, 2007, the Board of Health members unanimously approved this variance. This approval is valid for three years from the date of this letter and during this time a licensed septic system installer must obtain a permit and complete this work, and a Certificate of Compliance must be endorsed by the installer, designer and the Town of North Andover. This approval is subject to the following conditions: 1. If site conditions are found in the field to be different from those indicated on the design plan and/or soil evaluation, the originally issued Disposal System Construction Permit is void, installation shall stop, and the applicant shall reapply for a new Disposal Systems Construction Permit (3 10 CMR 15.020(1)). 2. It is the responsibility of the applicant and/or the applicant's designer, installer or other representative to ensure that all other state and municipal requirements are met. These may include review by the Conservation Commission,Zoning Board, Planning Board, Building Inspector, Plumbing Inspector and/or Electrical Inspector. The issuance of a Disposal System Construction Permit shall not construe and/or imply compliance with any of the aforementioned requirements. _T. . ........... _....... rc.. , ......._..2 _ ._.._... ,.. 1600 Osgood Street HEALTH DEPARTMENT Page 1 of 1 Building 20;Suite 2-36 E-Mail:healthdept@townofnorthandover.com North Andover,MA 01845 Phone:978.688.9540 Fax:978.688:8476 I I Your effort to provide a properly functioning onsite wastewater treatment and dispersal system for your property is greatly appreciated. The applicant has submitted a revised plan dated April 6,2007 that has addressed my previous comments which were: 1. The fill on the lot pitches runoff to the right of way on the west side of Salem St. The applicant should show grading in the shoulder for this runoff to reach the existing catch basins to the north and south. 2. The grading referenced in#1 above should create a negative slope from edge of pavement in Salem Street. 3. The existing asphalt apron should be removed and disposed of properly. 4. The roof runoff should be infiltrated 5. The paved portion of the driveway apron should be graded to create a negative pitch off Salem St. applicant licant should address the volume of water flowing g from the foundation drain discharging to the right of way and its impact on Salem St. I would ask that, if approved, the decision require specific attention be paid the grading of the driveway entrance with regards to providing enough pitch as to preclude runoff from entering onto the paved portion of Salem St. Also,'the revised plan does not appear to call for roof gutters. I a would ask that the decision require gutters be installed and tied into the infiltration system shown _ on the plan. fEune ely, 16.P. Willis, P.E.
