Dear Mr. Skambas,
According to the North Andover Zoning Bylaw Section 8.3.2.c.i, Waiver of Site Plan Review,the change
of ownership at 133 Main St, will not require an application for Site Plan Review. The waiver request is
granted based on the following information:
• The property will remain in its current use as a restaurant, a use which is permitted in the
General Business District, according to the Town of North Andover Zoning Bylaw section
4.131(1). 4
• The footprint of the building will remain the same and modifications will be made to the interior.
• There will be no changes to the parking and landscaping areas. The number of parking spaces
available in the rear of the building are adequate for this use.
• New signage will require a sign permit from the Building Inspector.

The undersigned proposes along with and owner and other subcontractor to furnish all
materials, necessary equipment and perform all labor necessary to complete the following
work: Put up non-bearing walls, countertops, and install kitchen appliances for a casual
and take out restaurant. This includes installation of plumbing and electrical fixtures as
per code.
1 Rework counter outlets, 2 conveyer ovens, I pizza fan kook up, 1 walk-in
refridge/freezer combo and electrical permit plus insurance. Carpentry includes change
some floor underlayment sheets,new fllor tiles, add to existing counter, replace drop
ceiling tiles with commercial grade fire rated tie,put up new wall panel, install new
shelving and paint. Plumbing: Install three hand sinks, change location of the vegetable
sink and connect all gas appliances.
Owner will hire sub-contractors to perform electrical and plumbing works. Owner shall
and must request proof of insurance and and lincensing from sub-contractors.
MDJ Inc. Engineering& Construction will perform permitting process services and
construction supervision.
Total cost: $16,360
