Heating and Cooling Equipment Sizing: Cj Additional requirements for equipment sizing are included by an inspection for compliance with the International Residential Code. For systems serving multiple dwelling units documentation has been submitted demonstrating compliance with 2009 IECC Commercial Building Mechanical and/or Service Water Heating(Sections 503 and 504). Circulating Service Hot Water Systems: E] Circulating service hot water pipes are insulated to R-2. Circulating service hot water systems include an automatic or accessible manual switch to tum off the circulating pump when the system is not in use. Heating and Cooling Piping Insulation: HVAC piping conveying fluids above 105 degrees F or chilled fluids below 55 degrees F are insulated to R-3. Swimming Pools: El Heated swimming pools have an on/off heater switch. Pool heaters operating on natural gas or LPG have an electronic pilot light. Timer switches on pool heaters and pumps are present. Exceptions: Where public health standards require continuous pump operation., Where pumps operate within solar-and/or waste-heat-recovery systems. Heated swimming pools have a cover on or at the water surface.For pools.heated over 90 degrees F(32 degrees C)the cover has a minimum insulation value of R-12. Exceptions: Covers are not required when 60%of the heating energy is from site-recovered energy or solar energy source. Deleading Portions of the Work Write Ups: Likewise, where the abatement of such materials is required to obtain compliance with state and federal lead laws/regulations, the contractor is entirely responsible for any leaded materials noted in the lead paint inspection report but not mentioned in the work write up. Similarly, if any of the work specified is insufficient to obtain a Federal Interim Control Compliance and/or Letter of Full Deleading Compliance, the contractor shall be responsible for any revisions to the work needed in order to obtain this letter. Contractor shall also be responsible for notifying the owner and/or tenant and the Salisbury Housing Rehab. Program Coordinator about the results of the re-inspection, so the owner and/or tenant may re-occupy their unit or extend the time period of their relocation. B. CHANGES IN THE WORK: once the contract between the owner(s)and contractor is executed, the owner, contractor, Rehabilitation Specialist, and the Program Manager must agree upon any subsequent changes to the scope of work, budget, or time schedule in writing. There shall be no changes in the scope of work without a written change order, signed by all parties, which officially revises the Work Write-Up. The cost or credit from a change order shall be determined by mutual agreement. C. CONTRACTOR RESPONSIBILITIES REGARDING DELEADING (if applicable): Contracts that involve extensive lead hazard reduction/abatement may require the relocation of occupants and, on occasion, their belongings. Once the structure is turned over to the control of the contractor, the contractor shall remain fully responsible for security at the property. As the relocation of occupants carries significant daily costs, the contractor must use his/her utmost effort to complete the lead hazard reduction/abatement activities as indicated by a successful clearance examination within an agreed-upon time period. The contractor shall be responsible for all additional relocation and related expenses incurred beyond this time period, including the occupants' per diem expenses and the cost of any additional lead clearance examinations required(beyond the first clearance exam performed on each unit). Such costs will be deducted from the total contract amount. D. LICENSES: The contractor shall hold a Mass. General Contractors or Supervisors License, a Mass. Home Improvement License, and, if applicable, a MA Certified De-leaders License. All work performed to eliminate lead- based paint hazards must be performed by a Massachusetts-licensed/certified abatement contractor in accordance with the Massachusetts Lead Law and associated regulations. E. PERMITS: The contractor is responsible for obtaining and submitting to the Housing Rehab Program all necessary permits PRIOR TO STARTING WORK. The contractor shall include in his/her bid the cost of all permits, inspections, certificates, etc., necessary for completion of the project as outlined in the Work Write Up. F. INSURANCE: The contractor shall hold Property, Liability, and Worker's Compensation Insurance of such amounts as to hold the owner harmless from all claims. Specifically, contractors must maintain the following minimum coverage: liability insurance of$500,000.00 per occurrence and $1,000,000.00 aggregate, property damage coverage of$500,000.00, and adequate worker's compensation insurance for all employees. Certificate of Insurance must be presented to the Housing Rehab Program as proof of coverage prior to the signing of contract documents. GENERAL WORK STANDARDS/CONDITIONS Page 2 of 17 Initials A. BUILDING CODES: All construction, repairs, alterations, demolition, and installation performed under the Salisbury Housing Rehabilitation Program, and materials associated with this work, shall conform to the requirements of the MA State Building Code, as amended. All repairs must meet the MA State Sanitary Code, Chapter 2. B. INTERRUPTION OF SERVICES: The work of the project shall be scheduled and performed by the contractor(s) to afford minimum interruption to the normal and continuous operation of the Owner's Business Procedures and Related Utilities. The owner or his agents shall not permit shut down or interruption of any utilities, services of facilities without approval and authorization. Contractor(s) shall submit the Rehabilitation Specialist a proposed schedule for the contract work for review and approval. C. PROTECTION: The contractor(s) shall at all times take care in the performance of this project so as not to constitute a source of danger or nuisance to the public or adjacent properties. D. CLEANUP: The contractor(s)shall clean up the site on a daily basis and shall not leave materials, equipment or debris in such a manner as to obstruct walks or roadways or in any way constitute a source of danger or nuisance. E. OCCUPANCY: The owner expects a completed and fully usable structure that completely satisfies the intent of the contract documents and any certificates that may be required by the Salisbury Building Inspector's Department. F. QUALITY&WORKMANSHIP: All work shall be performed in a neat and workman-like manner. The contractor(s) shall guarantee his installation against defects of materials and workmanship for a period of one year after the receipt of final payments. All installation shall be in keeping with the manufacturer's recommendations and the standards of this trade. All work performed by the contractor/subs must include all aspects of construction that will produce a functional and attractive finished product, whether or not specifically stated in the work write-up. Likewise, the contractor is responsible for the reinstallation of any interior or exterior item (light fixtures, for example)that have to be temporarily removed in order for work to be completed. For example, if new flooring is called for in a bathroom, the contractor/sub must remove and re-install the toilet. All materials shall be new and comply with the correct standards, specification, and tests of the American Society for Testing Materials. Materials shall be of stock size unless otherwise noted and shall be installed in accordance with the approved recommendations of the manufacturer. No seconds or inferior materials will be permitted. All existing materials to be repaired shall have all deteriorated parts removed until sound construction is reached and replaced with new materials to provide sound construction and match or beat the quality and design of original work. When the opening of walls or ceilings is necessary to facilitate the fishing or installation of electrical lines or circuits, the removal of obsolete piping, or for any other purpose,the areas which area involved must be patched and refinished to their former condition. The contractor shall perform all demolition related to his work in a safe and orderly manner. Unless otherwise noted, all materials removed shall become the property of the contractor and shall be promptly removed from the premises. The contractor is responsible for obtaining dumpsters and any necessary street permits from the City of Salisbury. The contractor is fully responsible for the labor and material cost to repair any damage caused during work performed by the contractor and/or subs, with no cost to the owner or to the City of Salisbury. New 42"wide stairs would be installed to the left. B. New wall may require substantial framing in the 31d. floor joists to obtain proper headroom clearance. This may also include solid posting down. However, this new wall would only be approximately 6" off the main carrying support wall in the cellar. 27. Floors 2nd. Floor: A. Contractor shall remove all rugs in the front bed room.
