Brown, Building Official FINAL AFFIDAVIT - STRUCTURAL CERTIFICATION The referenced building project has been substantially completed for the structural portion of the work, and this final report is written to the Building Official, in accordance with the terms of the Construction Control document, testifying to the satisfactory completion and readiness of the project for occupancy. I have been present at the construction site on a regular and periodic basis to determine that the work proceeded in accordance with the construction documents approved for the building permit. I have prepared and issued Engineer's Field Reports Nos. 1 thru 12 during the period of April thru December, 2008 with comments regarding my construction site observations. I have reviewed, for conformance to the design concept, shop drawings and submittals for the structural portion of the work submitted in accordance with the requirements by the Owner. I have reviewed and approved the quality control procedures in the form of field testing reports by others regarding cast in place concrete quality and soil testing and soil compaction results. This is written to certify that I have been present at the site at intervals appropriate to the stage of construction during the construction period, and that I generally became familiar with the progress and quality of the work, and that I determined, to the best of my knowledge and belief, that the structural portion of the work was performed in a manner consistent with the construction documents, the applicable provisions of the Mass. Building Code- 780 CMR and good engineering and construction practices. I have made the usual and normal field observations for the purposes of evaluating the referenced building construction, and, based upon my findings, it is my opinion that the building structure is sufficiently sound and adequate to accommodate the proposed use and occupancy, and that no hazard to the public safety will result. It is my determination that the structural portion of this project has been satisfactorily completed and the building is ready for the intended occupancy.
