Dear Mr. Murphy
As you requested I visited thb above site October 16, 2008 to review the LVL
beam used in the construction of the above project. This consist of 2-1.75" * 7.25"LVLs
supporting the front roof of the entrance area as shown on the Typical Cross Section
sheet prepared by Steve Foster dated 8/8/08.
These plans indicate 2 additional LVLs on the Typical Cross Section sheet. There
are actually existing wood beams at these locations consisting of 2-2*1Os with 1/2"
plywood with a 2*12 new wood rim board above. Theses are sufficient to support the
loads and therefore these LVLs are not required. As we discussed ssed in the field a 2 4
on the �
flat should be added to the right rear post to insure proper support of the wood beam.
Based on this site visits I can certify that to the best of my knowledge the LVL
members and the wood beams mentioned above are acceptable and meet the loading
conditions required by the 7t'Edition of the Massachusetts State Building Code.

Section 1-Work Schedule
Contractor will begin the work or order the materials before the third day following the signing of this agreement, unless specified here in
writing contractor will begin work on or about 11/1/08.
Barring Delay caused by circumstances beyond Contactors control,the work will be completed by 12/24/08.The owner hereby acknowledges
and agrees that the scheduling dates are approximate and that such delays that are not avoidable by the Contractor shall no be considered as
violations of this agreement.
Section 11-Wanan

The Contractor warrants that the work furnished hereunder shall be free from defects in materials and workmanship for a period of 1 year
following completion and shall comply with the requirements of this Agreement. In the event any defect in workmanship or materials, or
damage caused by the Contractor, his subcontractors, employees or agents, is discovered within one year after completion of any job,
including cleanup,the Contractor shall,at his own expense,forthwith remedy,repair correct,replace,or cause to be remedied repaired,or

replaced, such damage or such defect in materials or workmanship.The foregoing warranties shall survive any inspection performed in
connection with the agreed-upon work.

General
Proposal is to build dormer over existing breezeway for new master closet, renovate / expand existing
masterbath, and replace decking and railings on existing deck. No allowance has been made to change
footprint of existing deck. Building permit wit be provided by contractor. No allowances have been made for any
title v inspections,variances, or conservation approvals if required by town. Plans to be provided by owner.
Building
All frame, roof, and siding materials will be supplied to match existing / meet code. Exterior walls will be 2x4,
floor joists and roof rafters will be sized to meet code. All floor, wall, and roof sheathing will be fir plywood ( 3/4
on floor, 1/2 on walls, 5/8 on roof) . Ice&water sheild will be installed at all roof edges, roof shingles to match
existing. Siding will be pre-primed cedar clapboards over Tyvek or equivalent. Anderson windows will be
supplied as shown on plan. Decking will be a standard Trex color, or equivalent. Railings will be Endurance
white vinyl) .
Plumbing
Plumbing required to add five fixture master bath will be provided.An allowance of$2000 has been included for
plumbing fixtures.
Electrical
Electrical work required to wire master bath and closet area to meet code will be provided. Fan/light unit will be
supplied. Surface mounted fixtures to be provided by owner(vanity light).
Heating/Air Conditioning
Existing zone of heat on second floor, will be extended to properly heat added closet area. No allowance has
been made for any air conditioning.
Insulation
All added/renovated areas will be insulated to meet or exceed code. Floors and ceilings will be R-30, exterior
walls will be R-13.
Plaster
Added / renovated areas will be blueboarded and skimcoat plastered. Walls will be smooth, ceilings to match
existing.
Interior Trim/Doors
Interior trim will be supplied and installed to match existing.Two new interior doors have been included.
Painting
Interior and exterior painting will be provided. One coat of primer, and two coats of finish will be applied. No
allowance has been made for any wallpapering.
