December 4, 2015 Initial Project Estimate #215.26.099 Mr. Walter Amigo 623 Turnpike Street North Andover, MA 01845 Re: Excavation of Contaminated Soil Beneath Basement Concrete Slab 623 Turnpike Street North Andover, MA 01845 Dear Mr. Amigo, TMC Environmental (hereinafter referred to as "ENPRO"), is a full service company covering all of New England and New York. ENPRO prides itself on project consistency and its logistics expertise in: environmental remediation, abatement services, emergency response, environmental construction, and industrial cleaning. ENPRO is pleased to provide this proposal to you (hereinafter referred to as the "Client"). PROJECT OBJECTIVE We understand that you are requesting ENPRO to perform remediation services associated with a petroleum release of a home heating tank in the basement of a house at the above referenced address. It is known that the oil penetrated the slab and permeated the soil and ground water beneath the concrete floor. ENPRO is pleased to provide the estimated costs necessary to perform this service at the above referenced site based on the assumptions and exclusions listed below. The following outlines ENPRO's proposed scope, assumptions and project costs. SCOPE OF WORK Preconstruction Activities Phase 1 • Contact Dig-Safe at least 72 hours prior to performing excavation activities to obtain an approval number. • ENPRO will coordinate operations and scheduling with the Client and LSP prior to mobilization • Coordinate the mobilization of resources (crew and equipment) that may include a 2k +/ gallon Frac tank for round m contaminated soil a 9 vacuum truck to vacuum ( ) 9 water, excavator to dig soil and place it into 25 cy roll-off containers and other large equipment. • Prepare site specific health and safety plan prior to start of project. • Prior to undermining the structure, conduct a video site walk of the area and the inside of the house to record current conditions prior to excavation of the basement (unless waived by the client) Initials TMC Client Page 1 of 6 24 HOUR EMERGENCY RESPONSE 1.800.223.8865 19 National Drive • Franklin,Massachusetts 02038 - tel 508.966.6000 • fax 508.966.4861 • tmcenvironmental.com ENPRO TMC ENVIRONMENTAL "Keeping a check on our Environment" Quote Number Excavation and Management of Impacted Soil Phase II: Site Preparation • Mobilize crew and equipment to the site to prepare the basement by placing poly sheeting on the walls to limit migration of odors and dust from the work area. • ENPRO crew will saw cut the concrete slab into small manageable pieces and manually remove them from the basement along the staining lines of from the oil release. Stained concrete will be disposed with the impacted soil and clean non- stained concrete will be disposed of at a local asphalt batch recycling facility. Phase III A: Excavation of Impacted Soil • The impacted material that is required to be excavated, hard clay will be removed using pneumatic hammers and electrical chipping guns in conjunction with a high volume air moving vacuum loader. • The entire designated area has been divided into two area: the front and back Petroleum contaminated soil will be removed from the areas sequentially. Based on the provided Engineers Plan to a depth around 4' below slab grade. . • The excavation will be under the guidance of the LSP using a PID meter to delineate limits of excavation. The assumption is that the excavation will go down in depth two to three feet (2-3") below the current slab level with no undermining of the current house's foundation and footings. • It is expected that ground water will be between 30"-48" below the current concrete slab level. ENPRO will install pumps and hoses to manage the ground water by pumping it out into a frac tank staged in the back alley. The ground water will be samples by the LSP for disposal determination at an approved licensed facility. • ENPRO will transport the contaminated soil in the vacuum loader and or roll-off container to Brox Industries, Dracut, MA, a state approved asphalt recycling facility. • LSP will provide all testing, laboratory analytical characterization data and waste profiling and disposal facility acceptance package • Assumptions: A favorable characterization results will allow this material to be approved at an instate asphalt batch facility. Phase 111 13: Underpinning House's Foundation/Footings • If the situation creates undermining of the foundation and or footings then ENPRO will follow the structural engineer plan provided. • If the foundation requires underpinning, the soil will be removed in accordance with the Structural Engineer's Plan using helical and or atlas pressure piers supports. Once the material has been removed from one area then it will be backfilled prior to excavating the remaining area. If this cannot be done then the task will take longer to complete using sectional underpinning techniques. • Once the entire section has been determined by the LSP to meet standards or limits of excavation based on safety, each section will be backfilled with concrete. The Initials TMC Client Quote No. 215.26.099 Page 2 of 6 ENPRO TMC ENVIRONMENTAL "Keepinga Check on our Environment" Quote Number concrete will be pumped into the space and aggressively vibrated to ensure it fills and compact the entire area. • Once a section has been completed ENPRO will repeat the operation for the remaining section until the entire section has been excavated. Phase IV: Restoration • At this time it is assumed that the excavation will be backfilled with flow fill cement or concrete unless ground water flow is determined to be impacted by such material. If ground water impacts the excavation and flow fill will increase pressure and divert the water to the lower living spaces, the excavation will be backfilled with 3/a" stone. The stone will be manually moved to the basement and placed in the excavation. To limit damage to the yard and demerge charges, Once a section has been completed ENPRO will repeat the operation for the remaining section until the entire section has been excavated. • Once a section has been completed ENPRO will repeat the operation for the remaining section until the entire section has been excavated. • Once the excavation has been backfilled, a poly barrier will be installed and concrete pumped into the basement to restore the slab floor to the same level. ASSUMPTION AND/OR EXCLUSIONS • ENPRO will have full and clear access to the work locations and space to stage equipment and/or materials. Work area must be clean and free of debris prior to the commencement of excavation activities. The parking spots in the back alley will be available to storage of the frac tank and roll-off container. Coordination of additional space maybe required which the client is assumed to coordinate and finalize any agreement. • This proposal is based on 8 hours on site from 0800 to 1600 weekdays. • All analytical testing, discharge permitting, regulatory notifications (if required) and associated engineering tasks shall be performed by others at no cost to ENPRO. • ' This proposal assumes that the client will contract an oil company to permit and remove the AST, install a temp AST in a location outside the designated work zone and to re-install the tank upon completion. • Cost of the building permit is only an estimate. The cost will be the actual cost of the permit plus 15% and any labor and equipment necessary to obtain such permit. • All disposal pricing contained herein are based on facility acceptance. • The Client will authorize and sign for disposal documentation. • Impacted material must be physically and chemically suitable for the intended facility. • LSP will conduct all analytical testing required by disposal facility for disposal acceptance. • LSP will be on-site at regular occasions when required by ENPRO to direct the excavation activities. • This proposal does not include any costs associated with excavation dewatering, management of contaminated groundwater, or contaminated soil other than those directly specified herein. Client must understand that heavy equipment will be used to undertake this project and some damage to the yard may occur, such as removal of shrubs, plants, the damage to grass from repeated uses/walking and possible damage to the driveway as a result of heavy equipment parking on it for the day during working hours. ENPRO will take precautions to limit and or prevent damages, but some may still occur which TMC will not be held responsible. • All engineering, testing, and reporting associated with contamination remediation and geotechnical quality control issues shall be conducted by others at no cost to ENPRO Concrete pricing is based on Graziano Concrete in Braintree. Should they not be available then another vendor may be used at cost plus 15%. • ENPRO will not be responsible for damage, associated losses, and repair costs to unmarked or inaccurately marked structures/utilities. • ENPRO has not included the cost for excavation and/or utility support/bracing in this proposal., If required, additional costs will apply. • The proposed project approach is based upon information provided by the Client (LSP) during a site walk. • Verification of any and all presumed conditions and exclusions stated herein are the sole responsibility of the Client.
