A:The above quotation is for structural/estechtic repairs to the rear deck&stairs at Total $4,832.50
Westgate Condominium.Project scope will be as follows
1:Remove/salvage existing balustrade for re-use
2:Remove existing 1 x4 mahogany decking for structural repairs/rebuild.Mahogany
decking will be replaced with new decking as it will not be salvageable.
3:Remove existing stair stringers for structural repairs/footings/deck posts.
3:Careful inspection of existing framing members have concluded that they will not
be adequate for jacking/shoring the existing structure into place. Therefore,opting
to rebuild the rather small second floor deck,so proper flashing of ledger up against
existing masonry building,and an all around safer structure can be built.
B:Contractor proposes to try to salvage/remove existing mahogany balustrade
railings,top cap,&balusters,and selective newel posts for re-use.
Bi:Any portion of existing balustrade not salvageable will be considered a change
order&billed on a time&material basis,at a cost of$45 per man hour for labor,and 1 of 2

A:Contractor has been informed that the structure had been hit by a plow truck in the last snow storm at the property.After careful inspection,
the entire structure has clearly been hit in a manor that has done enough lateral structural,load/footing/,flashing,and the amount of exterior
trim that will need to be dismantled to do the structural work,that it would be more cost effective do rebuild most of the structure.There is also
a liability of safety at hand after repairs are complete that deems a rebuild appropriate.Efforts can be made to salvage as much material as
possible,but job costs would need to be billed on a time&material basis not knowing what can be slavaged and what cant.This quotation
would include salvaging balustrades only.

ARTICLE 1. SCOPE OF WORK
1.1 Contractor agrees to furnish all labor; materials and equipment to perform all work
described below on above stated project.Allotments where applicable

THIS AGREEMENT, made this 16"' Day Of Sept in the year 2014. By and between
Edward Rayner hereinafter called the Owner, and Shawn Woodburn hereinafter called
the Contractor.
For the consideration hereinafter named, the said Owner covenants and agrees with
said Contractor, as follows:
FIRST: The Contractor agrees to furnish all material and perform all work necessary to
complete the project called "Phase 1" at the owner's residence on 74 Elmcrest Rd North
Andover, Massachusetts, 01845.for the above named structure, according to the
proposal marked#53 and to the full satisfaction of the Owner.
SECOND The Contractor-agrees to promptly begin said work as soon as notified by
said Owner, and to complete the work as follows: Remove existing deck structure in rear
of home into onsite dumpster. Remove existing bulkhead and owners belongings in
basement as directed/needed.
THIRD All work will be done at a rate of job direct costs plus 15% of all labor,
material, and equipment, etc. All labor will be billed at at rate of$40 per man hour, per
man.Any unforeseen issues will be brought to Owners attention immediately.
FOURTH. This contract shall not be assigned by the Contractor without first obtaining
permission in writing from Owner. .
IN CONSIDERATION WHEREOF, the said Owner agrees that he will pay to the said
Contractor, a deposit of$750 for said materials and permits. Billing to be done weekly
going forward. All direct job cost reports will be provided to contractor. Estimated job
costs are$1,9200.00 ( One Thousand Nine Hundred)
The contractor and Owner for themselves, their successors, executors, administrators
and assigns, hereby agree to the full performance of the covenants of this agreement.
