Remove and dispose of:

DRYWALL
1. Supply and install Y2" drywall to ceiling and walls.
KITCHEN CABINETS
1. Install Kemper maple cabinets, Wheat finish, Whitman style doors. Kitchen cabinet layout per design provided by East
Coast Lumber dated 11/29/2016.

KITCHEN COUNTERTOPS AND BACKSPLASH
Supply and install 1 %11 polished Giallo Fiorito granite:
• To all base cabinets, island and hutch.
• 4° backsplash.
• Edging to be full bullnose.
• Cutouts for one (1) apron sink and one (1) single hole faucet.
Note: Side splash is not included in this proposal.
PLUMBING
Supply and install the following:
1. One (1) Blanco Silgranit 30" apron front sink (SAP -NR -192201) in Cafe Brown.
2. One (1) Blanco Silgranit Disal.Stopper/Flange (13441099) in Cafd Brown.
3. One (1) Kohler Artifacts single hole faucet (K99-259-2BZ) single handle pull-out spray faucet with oil rubbed bronze
finish.
4. One (1) InSinkErator Evolution Garbage Disposal (IPRO750) in black/stainless finish.
5. Install water line connection for Customer supplied dishwasher.
HEATING
1. Remove and dispose of one (1) radiator.
2. Supply and install one (1) hydronic toe -kick heater.
ELECTRICAL
Install the following per national code:
1. One (1) 30 circuit main breaker panel.
2. Five (5) countertop outlets and two (2) island outlets on two (2) 20 amp GFI/arc fault circuits.
3. Relocation of one (1) range oven with new dedicated 50 amp circuit.

FLOORING
1. Supply and install engineered Coretec Plus Design vinyl multi -width and multi -length plank flooring in Accolade Oak.
INTERIOR TRIM
1. Install pre -primed baseboard trim to match existing as close as possible.
2. Install pre -primed casing to match existing as close as possible around the window and doors.
INTERIOR PAINTING
1. Apply one (1) coat of interior primer to all new drywall.
2. Apply two (2) coats of interior latex finish paint to the walls and ceiling. Customer to select color and sheen.
3. Apply two (2) coats of interior latex semi -gloss paint to all new casings, base trim, and doors. Customer to select
color.
FLOOR AND DUST PROTECTION
1. Customer to remove all belongings from the work area before work starts. Including furniture, curtains and pictures.
2. Crossroads Contracting will not move items left in the work area and cannot be responsible for damage to those
items.
We will control dust and. debris as well as possible, due to the nature of the work to be completed.
4. Separate areas of construction with poly sheathing using the Zip System.
5. Protect the existing flooring with plastic and drop cloths.
CLEAN UP
1. We will clean up and remove our construction debris.
a. Site to be broom cleaned daily.
b. Completed job site to be left vacuum clean and all construction debris will be removed from the site.
SITE ACCESS VNe 1 oC L bCiX,
1. Our normal work hours range from 7:00 am to 5:30 pm depending on the phase and daily tasks being performed.
2. Unrestricted access during these work hours will be required.
3. A key lock box and front door key is the preferred method of entry. This allows us to lock the door if we need to leave
the site and get back in when we return assuming the Customer is not home.
4. The Customers are responsible for their pets. While most of us have them and love them, it is not feasible to complete
the following scope of work and guarantee that cats and dogs left out will not get outside or into our work areas. If
your pet(s) getting out of the house is a concern the Customer will need to plan accordingly by setting up an area that
the pet(s) cannot get out of during work hours. Extra litter boxes and toys are highly recommended during
construction.
DUMPSTER
1. Dumpsters are large and heavy as well as hard to load and unload. Care will be taken when placing and removing the
dumpster. The majority of jobs are.unaffected but there is a small chance that the driveway or lawn could be affected.
2. Dumpsters to be placed in the Customers driveway for the use of Crossroads Contracting only.
3. Dumpster to be placed on blocks but this does not guaranty that the dumpster will not leave impressions in the
pavement. It is impossible to assess how the driveway was prepped prior to paving and how much weight it will hold
before settling.

sprinkler heads.
5. Repair of driveway and lawn is not included in this scope of work.
6. If the Customer and/or subs of the Customer put items in the dumpster during construction additional charges may
apply. When the job is complete and if there is room left in the dumpster, the Customer is welcome to use the
remaining space. Liquids, tires, electronics, appliances, florescent bulbs and any type of hazardous materials are not
to be disposed of in the dumpster.
LEAD SAFETY FOR RENOVATION, REPAIR, AND PAINTING
1. Determine if Lead -Based Paint is present using an EPA recognized test kit in all homes built before 1978:
2. If it is determined that Lead -Based Paint is present, all work will be performed according to EPA lead safe renovation
guidelines.
3. Pricing to be determined and reported within 48 hours of test results.
MISCELLANEOUS
1. Any changes needed due to building code issues that are not included in the scope of work outlined in this proposal,
will be billed accordingly.
2. Permits will be obtained by Crossroads Contracting and billed at face value to the customer.
3. The $1,000.00 Partner Agreement deposit has been credited toward the base cost of the project.
MATERIAL IS GUARANTEED TO BE AS SPECIFIED, AND TO BE COMPLETED IN A WORKWOMANLIKE MANNER FOR THE SUM OF:

1. Install 4-5/8" pre -primed crown molding.
2. Apply two (2) coats of interior latex finish paint to the walls and ceiling. Customer to select color and sheen.
3. Apply two (2) coats of interior latex semi -gloss paint to casings, base trim, and windows. Customer to select color.
This form satisfies all basic requirements of the state's Home Improvement Contractor Law (MGL chapter 142A), but does
not include standard language to protect homeowners. Seek legal advice if necessary. Any person planning home
improvements should first obtain a copy of a "Massachusetts Consumer Guide to Home Improvement" before agreeing to
any work on your residence. You may obtain a free copy by calling the Office of Consumer Affairs and Business
Regulation's Consumer Information Hotline at 617-973-8787 or 1-888-283-3757.
PERMIT NOTICE:
1. The following permits will be required to complete the agreed scope of work:
a. Building Permit
2. Permits will be obtained by Crossroads Contracting and the face value will be billed to the customer.
3. It is the obligation of the Contractor to obtain such permits as the Owner's Agent.
4. Owner's who secure their own construction -related permits or deal with unregistered Contractors shall be excluded
from access to the Guarantee Fund provisions of MGL Chapter 142A.

Payments will be made according to the following schedule:
20% UPON SIGNING CONTRACT (not to exceed 1/3 of the total contract price or the cost of special order items,
whichever is greater)
20% UPON START OF PROJECT (plus permit fee = TBD)
20% UPON START OF ELECTRICAL
20% UPON START OF CABINET INSTALL
15% UPON START OF FLOORING
5% UPON COMPLETION (Law forbids demanding full payment until contract is completed to both party's satisfaction.)

NOTE: Including all finance charges law requires that any deposit or down -payment required by the Contractor before
work begins may not exceed the greater of (a) one-third of the total contract price or (b) the actual cost of any special
equipment or custom made material which must be special ordered in advance to meet the completion schedule.
Proposed Start and Completion Schedule:
The following schedule will be adhered to unless circumstances beyond the contractor's control arise.

Contracted work will be substantially complete (At least 95%) on: MARCH, 2017
EXPRESS WARRANTY — Is an express warranty being provided by the Contractor? Yes (all terms of the warranty must
be attached to the contract.) Contractor warrants that Work will be of good quality, free from defects and in compliance
with the requirements of the Contract Documents, as well as applicable local, state and federal laws, ordinances, rules
and regulations. At the Owner's request Contractor shall promptly repair or replace defective work for a period of one
year after the date of Work is substantially complete. All other statutory or common law warranties are specially
disclaimed.
Subcontractors — The Contractor agrees to be solely responsible for completion of the work described regardless of the
actions of any third party/subcontractor utilized by the contractor. The contractor further agrees to be solely responsible
for all payments to all subcontractors for materials and labor under this agreement.
Contract Acceptance — Upon signing, this document becomes a binding contract under law. Unless otherwise noted
with this document, the contract shall not imply that any lien or other security interest has been placed on the residence.
Review the following cautions and notices carefully before signing this contract.
Don't be pressured into signing the contract. Take time to read and fully understand it. Ask questions if something is
unclear.
Make sure the Contractor has a valid Home Improvement Contractor Registration. The law requires most home
improvement Contractors and Subcontractors to be registered with the Director of Home Improvement Contractor
Registration. You may inquire about contractor registration by writing to the Director at One Ashburton Place, Room
1301, Boston, MA 02108 or by calling 617-727-3200 or 1-800-223-0933.
Does the Contractor have insurance? Check to see that your Contractor is properly insured.
Know your rights and responsibilities. Read the Important information that follows on this form and get a copy of the

You may cancel this agreement if it has been signed at a place other than the Contractor's normal place of business,
provided you notify the Contractor in writing at his/her main office or branch by ordinary mail posted, by telegram sent or
by delivery, not later than midnight of the third business day following the signing of this agreement. See the attached
notice of cancellation form for an explanation of this right.

Contractor Arbitration
The Home Improvement Contractor Law provides Homeowners with the right to initiate an arbitration action (as an
alternative to court action) if they have a dispute with a Contractor. The same right is not automatically afforded to a
contractor, however. The Contractor would have to resolve any dispute he/she has with a Homeowner in court unless
both parties agree to the optional clause provided below. This clause would give the Contractor the same right to
arbitration as is afforded to the Homeowner by the Home Improvement Contractor Law.
The Contractor and the Homeowner hereby mutually agree in advance that in the event the Contractor has a dispute
concerning this contract, the contractor may submit the dispute to a private arbitration firm which has been approved by
the Secretary of the E=xecutive Office of Consumer Affairs and Business regulation and the consumer shall be required to
submit to such arbitration as provided in Massachusetts General Law chapter1

Homeowner's Rights
A Homeowner's rights under the Home Improvement Contractor Law (MGL chapter 142A) and other consumer protection
laws (i.e. MGL chapter 93A) may not be waived in any way, even by agreement. However, Homeowners may be
excluded from certain rights if the Contractor they choose is not properly registered as prescribed by law. Homeowners
who secure their own building permits are automatically excluded from all Guaranty Fund provisions of the Home
Improvement Contractor Law. The Contractor is responsible for completing the work as described, in a timely and
workmanlike manner. Homeowners may be entitled to other specific legal rights if the Contractor guarantees or provides
an express warranty for workmanship or materials. In addition to guarantees or warranties provided by the Contractor, all
goods sold in Massachusetts cant' an implied warranty of merchantability and fitness for a particular purpose. An
enumeration of other matters on which the Homeowner and Contractor lawfully agree may be added to the terms of the
contract as long as they do not restrict a Homeowner's basic consumer rights. If you have questions about you

Execution of Contract
The contract must be executed in duplicate and should not be signed until a copy of all exhibits and referenced
documents have been attached. Parties are also advised not to sign the document until all blank sections have been filled
in or marked as void, deleted, or not applicable. One original signed copy of the contract with attachments is to be given
to the Owner and the other kept by the Contractor. Any modification to the original contract must be in writing and agreed
to by both parties. Contracted work may not begin until both parties have received a fully executed copy of the contract,
and the three day rescission period has expired.
