BOSTON • PROVIDENCE a NEWBURYPORT' 8 October 2014 M&K Recovery Group, Inc. 100 Willow Street North Andover, MA 01845 Via Email: mkrecoverygroup.com ATTENTION: Mr. Bill Rockett, Vice President REFERENCE:. FEMA Letter of Map Amendment for 100 Willow Street, North Andover, Mass. Dear Bill: As requested I have prepared the following summary of the process which resulted in a Letter of Map Amendment (LOMA) being issued by the Federal Emergency Management Administration for the above parcel of land. The current (2012) Flood Insurance Rate Map (FIRM) for North Andover shows the westerly part of your land parcel (rear of your building) to be in SPFA Zone A, but with no Base Flood Elevation (BFE) established. I submitted to the FEMA LOMC Clearinghouse on your behalf a FEMA Property Information Form MT 1 for the subject parcel along with supporting documentation, as is required by FEMA. Subsequently FEMA issued a "Letter of Map Amendment Determination Document (Removal)" for Case No. 14- 01-2559A, dated August 14, 2014. As stated therein under "Outcome — what is removed from the SFHA" the "Property," that is, the entire parcel, is removed from the Flood, Zone. If you have any questions about the above matter I can be reached at my direct line — 857-524-1405 - or at mclifford@dgtsurvey.com. t0 OF Very truly yours, oaf s',y DGT Survey=..Group MICHAEL cGi1 o A- CLIFFQRp m • 9 No. 35394 Q Michael A, Clifford, PLS AlLANS Principal -in -Charge Copy: Mr. Scott Patrowic2, PE - Email: scott.patrowicz@verizon.net PADGTSIS-10CM-1053 100 willow Sh=t\Rcpom\LOMA Summary dor t. _ 803 SUMMER STREET 1 — FLOOR • BOSTON.
