This plan and application has been approved with the changes as shown on the attached drawing. . Looking forward towards pre -opening; prior to receiving your permit to operate, you must, at minimum, have two Health Department inspections; a construction inspection and a final inspection. When all equipment is in place, a construction inspection should be requested. Please call, at minimum of a few days ahead to avoid delays. At that time a complete punch list will be provided. Call the Health Department again when the list is completed. The Building permit will be signed off by the Health Inspector when the list is satisfied. Once all other departments are satisfied with the construction, the building department will grant you occupancy approval. As it is difficult to anticipate details at the time of this letter of approval, the next steps toward opening will be based on the specifics that exist at that time. The Health Inspector will instruct you on the next step in the process and you will discuss together when you may begin bringing in food and when food preparation may begin. Just prior to issuing the Food Establishment Permit to Operate, the inspector expects to view food properly stored on shelves. Each establishment opening is unique, feel free to contact the Health Department at any point in the process. **The annual fee for your establishment is $185. Your dumpster fee will be $60. Note that a final inspection will not be scheduled until all permit fees are paid. You may submit the applications with the fees at any point in the process. * * Dunkin Donuts 1018 Osgood Street June 28, 2013 Some of the items needed to receive the permit to operate are: 1) The establishment will be clean of all construction materials; floors and surfaces all cleaned 2) The hand sink(s) and bathroom(s) will have immediate access to wall mounted paper towel and soap dispensers and they must be stocked. 3) The ladies room will have a covered trash can for feminine item disposal 4) Bathroom(s) must have "employee must wash hands before returning to work" signage 5) Sanitizer bucket should be made up and test strips available. 6) Label grease trap per plumbing code. If you have one or more interior grease traps please note the plumbing code 248 CMR 10.09 (m): 7) Signage for allergens and disclaimers placed as required by law 8) Directions on mixing the sanitizer should be available. 9) Gloves must be on site. Please note that the state does not recommend the use of latex gloves due to some person's sensitivity to latex that may cause them illness. 10) At minimum, employees should be trained on the sick policy and sanitation basics. 11) The Food code must be on site or shown to be easily accessible by the person in charge. As we previously discussed, you have the right to file an appeal and request a hearing before the Board of Health in regards to this approval if you wish to do so. A request for said hearing must be made in writing and received by the Health Department at least 10 days prior to a regularly scheduled Board of Health meeting. Meetings are generally scheduled on the fourth Thursday of every month. At said hearing you will be given an opportunity to be heard and to present witnesses and documentary evidence to support your request. Thank you for your cooperation in this matter. In your letter you outlined 7 areas of concern. Below in bold I will address your concerns in order: 1. "Page 10 Referring to finish schedule in the form and finish sched, on the plan. There is no key relating to the code on the plan's finish schedule. It is noted that it is not clear if the quarry the coving is curved in required areas.". . a. I added another hand washing sink (3 in total, see attached sketch) to the backline, causing me to move my ice coffee brewer to a location that slows speed of service. Regarding the "dump sink", we have trough drains built into the frontline coffee station. These drains are there for the specific use of discarding unwanted liquid. Situating a "dump sink" on the backline creates an unsafe condition with employees walking around with hot pots of coffee. Additionally, there is no available room for another sink in the already crowded back and frontlines. Respectfully, it is redundant and unnecessary. Furthermore, after adding another hand washing sink per your request, I have eliminated any room for future, equipment on my backline. If the Brand comes out with.a new product that requires additional equipment in the service area, I am already maxed out. Present in the meeting with us was Michele Grant, the N. Andover Health Inspector. This meeting was set up to discuss the food establishment renovation proposal for the above noted property that was received on May 27, 2010 and the subsequent denial letter by the Health Department. The meeting began with discussion over non -contentious items such as; rear of the house repairs, outside safety issues, providing cut sheets, the equipment identification on the plan, repair of floors, walls and coving as needed etc. The delivery area safety issues, although highly important, will require landlord inclusion in finding and implementing a real solution to this long time issue. Please forward the violation regarding food safety and personal worker safety to the landlord and advise this office with their response in writing, within fourteen (14) days of this date, concerning their intentions to comply. Discussion over the requirement of the sinks in the newly designed service area was the main issue. The Health Department request for a total of four (4) sinks in the front of the house was based on numerous factors. 1) A major key to serving safe food is segregation of use. Each hand sink is designated for hand wash only and cannot be used for other needs. 2) You can never have enough hand sinks. 3) Each food service, ware washing and preparation area must have a hand sink within reasonable access without obstruction. After a long and contentious discussion the meeting ended, however, during a post meeting phone discussion a compromise was achieved. The final decision was the installation of the following; a single hand sink and a side by side set of two sinks, one of which will be labeled "Hand Washing Only" and the other will be of deeper design to be used for alternatives needs than hand washing and properly marked for its use. The sinks shall also be separated by a non porous divider to eliminate splashing. Please submit a complete plan showing the changes. Having received all cut sheets, the draft changes and a commitment to correct all violations noted on the previous letter, the Health Department has approved the renovation plan for 129 Main Street.
