The attached letter was sent to the 2nd floor tenant on July 23, 2004. Please be
advised that although that letter specifically mentions the second floor tenant, all other
tenants are expected to comply with the sanitary code regulations. If you are identified
as a violator in the future, you may be subject to the penalties as set forth in the Human
Habitation code.

North Andover Health Department personnel made an authorized inspection of
your property at the above referenced address on July 21, 2004, in response to a
complaint regarding trash.
This inspection revealed violations of certain regulations of the State Sanitary
Code, Chapter II, as listed on the attached Violation Form. You as the tenant of #11
Waverly have been identified as a violator to the code and are hereby ORDERED to
correct these violations immediately and cease from causing unsanitary conditions to
accumulate on the property. Failure to comply may result in further action by the
North Andover Board of Health If you have additional information or feel that you
have been wrongly identified in this matter, please contact the health office as soon as
possible.

ORDER LETTER
An authorized inspection of the exterior area of the multi unit dwelling, at 11 Waverly,
was performed by Board of Health staff on July 21, 2004 at which violations of 105
CMR 410.000 Chapter 11 of the State Sanitary Code, Minimum Standards of Fitness for
Human Habitation were found. As a tenant you are responsible for certain aspects of
the sanitary code and the landlord/owner of this property.
All violations must be corrected immediately.

- Garbage shall be stored in watertight receptacles with tight fitting covers.
- The occupant of each dwelling unit shall be responsible for the proper placement
of his garbage and rubbish in the receptacles
Clean up trash in yard area and ensure that all rubbish is stored inside the trashcans
between pick up days. If the receptacles do not have lids, please notify the landlord
who is responsible for providing trashcans with lids.
Also, please be advised that the trash day on your street is Monday. Please remove the
trashcan from the street by the end of that day.
